DISEASES OF THE TONSIL Dr. Amer salih aljibori Acute Tonsillitis: acute inflammatory condition of the faucial tonsil which may involve the mucosa, crypts,follicles and /or tonsillor parenchyma. Causatve agents; -Viral:Initially starts with viral infection then followed by secondary bacterial infection.Common viruses are influenza,parainfluenza.adenovirus and rhinovirus. -Bacterial:Streptococcus hemolyticus,Hemophilus influenza,pneumococcus,M.catarrahalis. Pathology and pathogenesis: Usually it starts in the childhood when there is low immune status.Depending on the progress of the disease,this can be classified further into the following types. •Catarrhal tonsillitis:It occurs due to viral infection of the upper respiratory tract involving the mucosa of the tonsil •Cryptic tonsillitis: Following viral infection ,secondary bacterial infection supervenes and gets entrapped within the crypts leading to localized form of infection •Acute follicular tonsillitis. It is sever form of tonsillitis caused by virulent organisms like streptococcus hemolyticus and Hemophilus. It causes spread of inflammation from tonsillar crypts to the surrounding tosillar follicles. Acute parenchymal tonsillitis: The secondary bacterial infection will invade to the crypts and it is rapidly spreads into the tonsillar parenchyma.. Cryptic tonsillitis 1- Symptoms -Fever -Generalized malaise and bodyache. -Odynophagia. -Dry cough. -Sorethroat. 2- Signs -Congested and oedematous tonsils -Tonsils may be diffusely swollen in parenchymatous tonsillitis. -Crypts filled with pus in follicular tonsillitis. -Membrane cover the tonsil and termed as membranous tonsillitis. -Tender enlarged jugulodigastric lymph nodes. -Signs of upper respiratory tract infection and adenoiditis. Investigations. -Throat swab for culture and sensitivity. -Blood smear to rule out hemopoeitic disorders like leukemia,agranulocytosis. -Paul-Bunnel test may be required if membrane seen to rulr out infectious mononucleosis. -

This new Framework is grounded in a comprehensive body of research regarding what young children should know and be able to do during these formative years. Shaker* Department of Anatomy and Embryology, Faculty of Veterinary Medicine, Cairo University, Egypt *Corresponding author: [email protected] Abstract The present study provides detailed anatomical description of the parotid and mandibular salivary glands of the one humped camel with their segmentation based on arterial blood supply and salivary ducts; to facilitate partial removal of the pathologic gland. The shape, position, relations and blood supply of both salivary glands with their ducts were studied on six cadaveric heads. The mandibular and parotid ducts were injected with Urographin® as contrast medium; through inserting the catheter into their openings in the oral cavity; then applying lateral radiography immediately after the injection. The common carotid arteries were injected with red Latex Neoprene and dissected. The parotid gland was irregular rectangular and had five processes while the mandibular gland was irregular triangular with rounded proximal and pointed distal extremity. The parotid duct enters the oral cavity on the cheek opposite the upper 4th molar tooth. The mandibular duct opens in the oral cavity at the sublingual caruncles on the sublingual floor, just about 2cm cranial to frenulum linguae. Both The parotid and the mandibular salivary glands could be divided into four segments. -

Tongue -Tie (Ankyloglossia) and Lip -Tie (Lip Adhesion) What is Tongue-Tie? Most of us think of tongue -tie as a situation we find ourselves in when we are too excited to speak. Actually, tongue- tie is the non medical term for a relatively common physical condition that limits the use of the tongue, ankyloglossia. Lip -tie is a condition where the upper lip cannot be curled or moved normally. Before we are born, a strong cord of tissue that guides development of mouth structures is positioned in the center of the mouth. It is called a frenulum. As we develop, this frenulum recedes and thins. The lingual (tongue) or labial (lip) frenulum is visible and easily felt if you look in the mirror under your tongue and lip. In some children, the frenulum is especially tight or fails to recede and may cause tongue/lip mobility problems. The tongue and lip are a very complex group of muscles and are important for all oral function. For this reason having tongue tie can lead to nursing, eating, dental, or speech problems, which may be serious in some individuals. When Is Tongue and Lip- Tie a Problem That Needs Treatment? Infants A new baby with a too tight tongue and/or lip frenulum can have trouble sucking and may have poor weight gain. If they cannot make a good seal on the nipple, they may swallow air causing gas and stomach problems. Such feeding problems should be discussed with Dr. Sierra. Nursing mothers who experience significant pain while nursing or whose baby has trouble latching on should have their child evaluated for tongue and lip tie. -

Salivary Gland Infections and Salivary Stones (Sialadentis and Sialithiasis) What is Sialadenitis and Sialithiasis? Sialdenitis is an infection of the salivary glands that causes painful swelling of the glands that produce saliva, or spit. Bacterial infections, diabetes, tumors or stones in the salivary glands, and tooth problems (poor oral hygiene) may cause a salivary gland infection. The symptoms include pain, swelling, pus in the mouth, neck skin infection. These infections and affect the submandibular gland (below the jaw) or the parotid glands (in front of the ears). The symptoms can be minor and just be a small swelling after meals (symptoms tend to be worse after times of high saliva flow). Rarely, the swelling in the mouth will progress and can cut off your airway and cause you to stop breathing. What Causes Sialadenitis and Sialithiasis When the flow of saliva is blocked by a small stone (salilithiasis) in a salivary gland or when a person is dehydrated, bacteria can build up and cause an infection. A viral infection, such as the mumps, also can cause a salivary gland to get infected and swell. These infections can also be caused by a spread from rotten or decaying teeth. Sometimes there can be a buildup of calcium in the saliva ducts that form into stones. These can easily stop the flow of saliva and cause problems How are these infections and stones treated? Treatment depends on what caused your salivary gland infection. If the infection is caused by bacteria, your doctor may prescribe antibiotics. Home treatment such as drinking fluids, applying warm compresses, and sucking on lemon wedges or sour candy to increase saliva may help to clear the infection quicker. -

Patient Information Maxillo-facial Angina Bullosa Haemorrhagica (Oral Blood Blister) What is Angina Bullosa Haemorrhagica? Angina Bullosa Hemorrhagica (ABH) is a condition where an often painful, but benign blood-filled blister suddenly develops in the mouth. The blisters are generally not due to a blood clotting disorder or any other medical disorder. It is a fairly common, sudden onset and benign blood blistering oral (mouth) disorder. It mainly affects people over 45 years and both males and females are equally affected. Usually there is no family history of the condition. It may be associated with Type 2 Diabetes, a family history of diabetes or Hyperglycaemia. What are the signs and symptoms of ABH? The first indication is a stinging pain or burning sensation just before the appearance of a blood blister The blisters last only a few minutes and then spontaneously rupture (burst), leaving a shallow ulcer that heals without scarring, discomfort or pain They can reach an average size of one to three centimetres in diameter The Soft Palate (back of the mouth) is the most affected site If they occur on the palate and are relatively big, they may need to be de-roofed (cut and drained) to ease the sensation of choking Patient Information Occasionally blisters can occur in the buccal mucosa (cheek) and tongue Approximately one third of the patients have blood blisters in more than one location. 899 MEDIDAS ANTROPOMÉTRICAS OROFACIAIS EM CRIANÇAS DE TRÊS A CINCO ANOS DE IDADE Anthropometrical orofacial measurement in children from three to five years old Raquel Bossle(1), Mônica Carminatti(1), Bárbara de Lavra-Pinto(1), Renata Franzon (2), Fernando de Borba Araújo (3), Erissandra Gomes(3) RESUMO Objetivo: obter as medidas antropométricas orofaciais em crianças pré-escolares de três a cinco anos e realizar a correlação com idade cronológica, gênero, raça e hábitos orais. Métodos: estudo transversal com 93 crianças selecionadas por meio de amostra de conveniência consecutiva. Os responsáveis responderam a um questionário sobre os hábitos orais e as crianças foram submetidas a uma avaliação odontológica e antropométrica da face. O nível de significância utilizado foi p<0,05. Resultados: as médias das medidas antropométricas orofaciais foram descritas. Houve diferença estatística nas medidas de altura da face (p<0,001), terço médio da face (p<0,001), canto externo do olho até a comissura labial esquerda/direita (p<0,001) e lábio inferior (p=0,015) nas faixas etárias. O gênero masculino apresentou medidas superiores na altura de face (p=0,003), terço inferior da face (p<0,001), lábio superior (p=0,001) e lábio inferior (p<0,001). Não houve diferença estatisticamente significante na altura do lábio superior em sujeitos não brancos (p=0,03). A presença de hábitos orais não influenciou os resultados. O aleitamento materno exclusivo por seis meses influenciou o aumento da medida de terço médio (p=0,022) e da altura da face (p=0,037). Conclusão: as médias descritas neste estudo foram superiores aos padrões encontrados em outros estudos.
