These notes supplement the Perth and Common Housing Register application form. If you have any difficulty filling in the application form, or would like more information and advice, please speak to a member of staff at your local area housing office (contact details are in the back of these notes).

Guidance Notes

Section 1 - About You

Anyone named in this section of the form will be considered to be applying for a house from one of the registered social landlords of the Common Housing Register (Perth & Kinross Council, Housing Association or Hillcrest Housing Association).

You should provide full names (including any middle names) along with dates of birth and contact telephone numbers.

Documents we may need to see ID Verification ❏ Proof of age for every person on the application (this can include birth certificate, marriage certificate, driving licence, passport, National Health Card).

Address Verification ❏ If you own your current home please provide two proofs of address (this can include official letters from a local authority, public agency, government department, bank or building society or utility bills for gas, electric or telephone).

❏ If you rent your current home please provide a copy of your lease and one other proof of address (this can include official letters from a local authority, public agency, government department, bank or building society or utility bills for gas, electric or telephone).

❏ If you live care of or are using the address only for mailing purposes please provide a letter from the owner/tenant of the property confirming this and stating if you have a bedroom available for your own use, and if so for how long.

Section 2 - Who Will Be Moving In With You?

Documents we may need to see ❏ A letter from every person over 16 named on the application confirming their intention to live with you permanently. Applicant, joint applicant and their spouses/partners don’t need to do this.

❏ If you have included children in your foster care, you will need a confirmation letter from the responsible local authority.

Section 3 - Main and Joint Applicants’ Previous Address Details

We need to know where you have lived for the last three years. You should also provide your landlord's details where applicable.

If you have lived at the address given in Section 1 for more than three years you don’t need to fill in this section.

If the joint applicant’s previous addresses are the same as the main applicant’s you can note “see main applicant’s addresses” here. Guidance Notes

Section 4 - Where Do You Live Now?

If you are not sure what type of accommodation you live in, please contact your local area housing office for further information (contact details are in the back of these notes).

Q1. A single bedroom is between 6.5 to 10.1 sq metres. A room is not classed as a bedroom if it measures less than 6.5 sq metres.

Q2. A double bedroom is over 10.1 sq metres.

Q4. Answer Yes to this question if you have been asked to leave within the next six months, or you feel it is unreasonable for you to continue to stay there.

Documents we will need to see ❏ A copy of any notice issued by your current landlord.

❏ A letter from you confirming the reasons why you have given notice.

❏ Any documentation confirming the reason you gave us.

Q5. If you have answered No, and you are renting your current home (unless your landlord is a Housing Association or the Council), then a member of Environment Services may inspect your accommodation.

Section 5 - Health, Support and Welfare

Q1. If you want to move to take care of a family member, that person must live in the area. Because your request will be based on that person’s needs they may be asked to complete a Self- Assessment Medical application. Areas you will be able to select for housing will be restricted if you are awarded priority on this basis.

Q2. If you want to move because you need help from a family member, that person must confirm their willingness to provide such support. You may also be asked to complete a Self-Assessment Medical application. Areas you will be able to select for housing will be restricted if you are awarded priority on this basis.

Q3. We may need to do additional assessments to award priority on this basis.

Q4. Everyone detailed in this section must provide written confirmation of the information you have given. Priority will only be awarded on one ground.

Section 6 - What Kind of Property Would You Like?

Q1 and Q2. Please detail the type of accommodation you would like to apply for. You should note that not all sizes and types are available in all areas. If you have been awarded medical priority you may be restricted in what types of property we can consider you for.

Q3. This will depend on how many people are in your household and their ages. Full details of how household size/number of bedrooms are determined are available in the Perth & Kinross Council Common Allocations Policy which is available at www.pkc.gov.uk/housing, on request from any Council office or by phoning 01738 475000. Q5. You must have written permission from the Council to keep any pet before moving into any Council house you may be offered. If you wish to bring a pet into your home after accepting a tenancy you will need to seek permission from your local area housing office.

Section 7 - Where Would You Like to Live?

You should note that not all sizes and types are available in all areas (further details can be obtained from your local area housing office - contact details are in the back of these notes). You can also detail any streets/areas of your preferred neighbourhoods where you do not wish to be considered for housing.

Section 8 - Other Housing Options

Q1. We may send you additional information if you indicate you are interested. This information will come from the Council, Hillcrest Housing or Perthshire Housing Association. Your details will not be passed on to any third party.

Q2. If you answer Yes we will send you a further short application form to complete to join the Council’s Mutual Exchange Register.
