Case History: A 30 year old female had a routine CBC performed as part of a physical examination. Her CBC results are as follows: WBC=5.9 x 109/L, RBC=4.53 x 1012/L, Hgb=13.6 g/dL, Hct=40%, MCV=88.3 fL, MCH=29.7 pg, MCHC=32.9 g/dL, Platelet=184 x 109/L. Introduction The images presented in this testing event represent normal white blood cells as well as several types of neutrophils that may be seen in the peripheral blood when a patient has the Pelger-Huët anomaly. -

Abstract: Background: Neonatal sepsis is a clinical syndrome characterized by signs and symptoms of infection with or without accompanying bacteraemia in the first month of life. Incidence differs among hospitals depending on variety of factors. Blood culture is considered gold standard for the diagnosis, but does not give a rapid result. Hence, there is a need to look for a surrogate marker for diagnosing neonatal septicaemia. Material & Methods: 335 neonates were studied for clinically suspected septicaemia over a period of one year. Blood was cultured and organism identified biochemically. Parameters of subjects like EOS, LOS and Band cell counts were recorded. Results analysed statistically. Results: Male preponderance was observed. Majority of the cases had a normal vaginal delivery. 47.46% cases had early onset septicaemia. Meconium stained liquor was the predominant risk factor .Culture positivity was found to be 32.24% and 87.96% of them also had band cells percentage ranging from 0 to >25. Conclusion: Band cell count can be used as a surrogate marker for neonatal septicaemia. An upsurge of Candida species as a causative agent in Neonatal septicaemia has been observed. -

His laboratory values are as follows: Hemoglobin 6.0 g/dL (normal: 12–15 g/dL) Hematocrit 18% (normal: 36%–46%) RBC count 2 million/L (normal: 4–5.2 million/L) Reticulocyte count 3% (normal: 0.5%–1.5%) Which of the following caused this man’s anemia? A. Decreased red cell production B. Increased red cell destruction C. Acute blood loss (hemorrhage) D. There is insufficient information to make a determination Answer: A. This man presents with anemia and an elevated reticulocyte count which seems to suggest a hemolytic process. His reticulocyte count, however, has not been corrected for the degree of anemia he displays. This can be done by calculating his corrected reticulocyte count ([3% × (18%/45%)] = 1.2%), which is less than 2 and thus suggestive of a hypoproliferative process (decreased red cell production). Q.2. A 25-year-old man with pancytopenia undergoes bone marrow aspiration and biopsy, which reveals profound hypocellularity and virtual absence of hematopoietic cells. Cytogenetic analysis of the bone marrow does not reveal any abnormalities. Despite red blood cell and platelet transfusions, his pancytopenia worsens. Eighty two consecutive patients (34% DVT, 66% non-DVT) Point-of-care testing; with suspected DVT of a lower limb were tested with the D-Dimer assay with a Chemiluminescent PATHFAST analyzer. The diagnostic value of the PATHFAST D-Dimer assay (which is immunoassay based on the principle of a chemiluminescent enzyme immunoassay) for DVT was evaluated with pre-test clinical probability, compression ultrasonography (CUS). Furthermore, each patient underwent contrast venography and computed tomogra- phy, if necessary. The sensitivity and specificity of the D-Dimer assay using 0.570 μg/ mL FEU as a clinical cut-off value was found to be 100% and 63.2%, respectively, for the diagnosis of DVT, with a positive predictive value (PPV) and negative predictive value (NPV) of 66.7% and 100%, respectively. -

Overview The most common form of nutritional deficiency is “iron deficiency”. It is observed more frequently among children and women of childbearing age (particularly pregnant women). Iron deficiency can result in developmental delays and behavioral disturbance in children, as well as increased risk for a preterm delivery in pregnant women. Iron status can be determined using several different types of laboratory tests. The two tests most commonly used to screen for iron deficiency are hemoglobin (Hgb) concentration and hematocrit (Hct). Proper screening for iron deficiency requires sound laboratory methods and procedures. Often, CPAs will hear the following questions: “Do you have to stick my finger? What does this have to do with my WIC foods anyway? Will it hurt?” For most of us, the thought of having blood taken, even from a finger, is not pleasant. However, evaluating the results of a blood test is a part of screening for nutritional risk. Why Does WIC Require Hematological Assessment? WIC requires that each applicant be screened for risk of a medical condition known as iron deficiency anemia. Anemia is a condition of the blood in which the amount of hemoglobin falls below a level considered desirable for good health. -

Photograph BCI-01 shows a reactive lymphocyte. The term “variant” is also used to describe these cells that display morphologic characteristics different from what is considered normal lymphocyte appearance. Reactive lymphocytes demonstrate a wide variety of morphologic features. They are most often associated with viral illnesses, so it is expected that some of these cells would be present in the peripheral blood of this patient. This patient had infectious mononucleosis that was confirmed with a positive mononucleosis screening test. An increased number of reactive lymphocytes is a morphologic hallmark of infectious mononucleosis. Some generalizations regarding the morphology of reactive lymphocytes can be made. These cells are often large with abundant cytoplasm. Cytoplasmic vacuoles and/or azurophilic granules may also be present. Reactive lymphocytes have an increased amount of RNA in the cytoplasm, which is reflected by an associated increase in cytoplasmic basophilia. The cytoplasm may stain gray, pale-blue, or a very deep blue and appear patchy. The cytoplasmic margins may be indented by surrounding red blood cells and appear a darker blue than the rest of the cytoplasm. Likewise, the nuclei in reactive lymphocytes are variably shaped and may be round, oval, indented, or lobulated. -

Methods: This hospital based study was conducted in the year 2008 on 1,012 children aged 6 months to 14 years who attended the Pediatric Department at Prince Rashed Bin Al-Hassan Military Hospital in North of Jordan using fully automated blood count of the mean corpuscular volume, serum ferritin level and high performance liquid chromography, or genotyping. None of the subjects included in the study had been on any hematinic in the previous six months, had infection in the past one month or had a chronic disease. The diagnosis of iron deficiency anemia was defined as mean corpuscular volume ≤ mean – 1 standard deviation corrected for age, with a ferritin level < 7 ng/ml of the serum (normal reference range 7 – 140 ng/ml). The diagnosis of thalassemia trait, for subjects with normal or high serum ferritin and those whose mean corpuscular volume was non-compliant to iron therapy, was obtained by high performance liquid chromography or polymerase chain reaction, which was performed at Princess Eman Research and Laboratory Science Center. Results: The frequency of iron deficiency anemia and thalassemia trait was 13.3% and 5.8% respectively. They were equally frequent among males and females. -

The epidemiological studies suggested that one-third of the world’s population is affected with anemia. Anemia is not a disease, but it is instead the sign of an underlying basic pathological process. However, the sign may function as a compass in the search for the cause. Therefore, the prediag- nosis revealed by thorough investigation of this sign should be supported by laboratory parameters according to the underlying pathological process. We expect that this review will provide guidance to clinicians with findings and laboratory tests that can be followed from the initial stage in the anemia search.
