8 9 The information that all interns should know Some things are pretty standard no matter where in Australia you chose to work....

How much will I be expected to work? For all states across Australia a standard week is 38 hours, plus overtime. What you need to look out for is your particular state and hospital’s regulations about overtime. Generally expect to have at least one overtime shift each fortnight, if not every week. These include evening shifts, night shifts and weekend shifts. You also need to look out for when you have a sick relief shift. The doctor on sick relief can be called at any time of the day or night to step in. This includes being called to step into a night shift at 11.45pm when you are about to go to sleep. Each state has a different policy on ‘unrostered overtime’. Check out your state and hospital policies on this prior to commencing work. What is the structure of the year? Most hospitals operate with five rotation blocks throughout the year for interns, ranging between 10 and 12 weeks in length. What rotations will I do? In general, interns need to complete at least one medicine, one surgery and one emergency term within the year as part of their core rotations. A few hospitals consider emergency an elective, so the best bet is to check with them. The remaining two terms are electives. Each hospital offers a different set of rotations, so check the information in the IRG and get in contact with them Resident’s tend not to have core terms. Can I make a couple application with somebody else? Most states accept applications for couples or joint-tickets. It is best to check with the websites listed under each state’s applicarion information section. Can I get special consideration for my application? Each state has a separate policy for special consideration. We didn’t have enough room in these pages to go into much detail, but what we can say is that again each state has a different policy. Special consideration is generally reserved for applicants with extenuating circumstances such as dependant children or a serious illness. Check their websites.

Across all of the states the order of allocating intern places is the same in accordance with national guidelines. The order of allocation is as follows:

1. Doctors who graduated in the state and are citizens of Australia/New Zealand; or completed year 12 in the state 2. Graduates of other medical facilities in Australia/New Zealand who are permanent residents of Australia/New Zealand 3. Graduates of the state who are not permanent residents of Australia/New Zealand but are permitted to remain in Australia for their internship (international students who studied in the state) 4. Graduates of other medical faculties in Australia/New Zealand who are not permanent residents of Australia/New Zealand but are permitted to remain in Australia for their internship (international students who studied interstate) 5. Where positions are vacant after considering the above allocation process, offers will be made to international medical graduates from the Australian Medical Council or Junior Medical Officer databases.

The Postgraduate Medical Council of South Australia (PMCSA) will send out the application form / information pack for Medical Intern Appointment to all SA medical students. On-line application and hard copy application for internship in 2010 will be available to all applicants at www.pmcsa.org.au from 23 May 2009, the closing date for application is 19 June 2009. First round matches (for South Australian graduates) become available on the 20th of July, with subsequent matches available from the 5th of August onwards. Applications for hospital networks in Victoria are coordinated by the Postgraduate Medical Council of Victoria (PMCV). The first step is to register with the PMCV website, at which time you will be issued with a username and password. This service is already open for registrations. To become eligible to be matched to a position in Victoria, you need to provide the PMCV with a proof of Australian citizenship or permanent residency (e.g. birth certificate). In addition, interstate and AMC graduates need a letter from their medical faculty or AMC certificate. Two referees can be nominated to complete the online referee form. Referee Assessment Forms should be completed by a person who knows your clinical abilities and has been involved in directly supervising your clinical practice whether in a hospital or General Practice or Community Setting, such as consultants or medical practitioners or General Practitioners who are Fellows of an Australian Medical College or clinical Deans/clinical sub-deans (or equivalent) of medical faculties. In addition to the computer matching process, most hospital networks have a separate application process. Most application processes happen online and require you to send some documents e.g a cover letter and a CV. Although we have tried to provide the best contact details for each hospital, make sure that you check the website and the details provided prior to lodging your application. When selecting preferences via the PMCV website, it is important to only preference hospitals that you actually want to work at. If you preference a hospital you don’t want to work for at number five, and none of your top four hospitals offer you a place but hospital X does, you will not be offered another place in Victoria. Both the PCMV and your hospital preferences must be contacted before the date that applications close.

The PMCWA works closely with the primary allocation centres to support and facilitate medical students applying for intern training positions in WA. There are three primary allocation centres in WA – , Royal Hospital and Sir Charles Gairdner Hospital. The Medical Board of Western Australia (Medical Board) is an independent statutory authority responsible for overseeing the Medical Act 1894 (WA). The Medical Board is responsible for the registration of doctors and maintenance of standards of conduct and practice, including the registration of interns. Interns are required to complete 48 weeks of clinical training (exclusive of any time off for sick or annual leave). Ten days of study leave is available within the 48 weeks of clinical training. New Zealand intern/house surgeon positions are offered to doctors by the 21 District Health Boards that administer the hospitals. In general, it is one Health Board per city, plus surrounding province. All of the health boards normally employ doctors under the same contract, so work conditions are comparable across the country. Wages are based on a pay scale that takes in to account the hours expected to work. Job offers are allocated to graduates through a system called ACE (Advanced Choice of Employment) which ranks applicants preferences for where they want to work, against DHB preferences for who they want to employ, and matches you to your highest ranked DHB that will give you a position.
