Who Receives Help 2014 Highlights

Waukesha County  A total of 834 on-site services were provided including medications, clinic community members whose appointments, and coordination of healthcare access household income meets  Individuals served reside in a wide geographic area of Waukesha County criteria. For example, the  547 clinic appointments serving 393 unduplicated patients guideline for a family of four  79 medical referrals to network providers is at $5,058 dollars per  469 lab tests were completed on clinic site month (250% of Federal  436 lab and diagnostic tests provided off site by network providers Poverty Limits) or less.  927 prescriptions were provided to 394 patients  Medical Case Management for homeless families and individuals served 19 county residents  Through the Diabetes Waukesha Wellness Working Together Grant SJMC served 10 Spanish speaking patients  102 patients were supported by the Waukesha County Community Block Development Grant to obtain their medications  361 patients received medication assistance through support of clinic  42 classes were provided to the public and the community on topics such as foot care, diabetic self-care, breast health and dental care  898 phone or walk in referrals were received to help residents access and navigate needed community resources An individual receiving a blood  169 individuals were provided assistance in enrolling in health insurance pressure check at one of five through BadgerCare or private insurance on Healthcare.gov. community outreach events in  which we participated in the 68 enrolled in Badger Care and received further assistance accessing care Waukesha County community in 2014.
