Over 17,000 people over 65 years are living with dementia in . Support is provided by both the NHS, voluntary and statutory sectors that have developed a network of Dementia Action Alliances and Dementia Hubs: • Defying Dementia • Medical professionals • Community Services: Memory Assessment, Community Mental Health and Rapid Intervention Teams • Age UK Lancashire • Alzheimer’s Society • N-Compass/Carers Link/ Carers/Carers Support • Admiral Nurses • Fire Service and – Guardian Angels • Citizens Advice • Private companies, theatres, local charities and organisations Dementia Hubs

Following on from Dementia Action Alliances a series of Hubs have been established in Lancashire and South Lakes:

• Bay Dementia Hub and Network – Lancaster/ • Fylde Coast Hub – Various locations Fylde and Wyre • South Lakes – Brewery Arts Centre, Kendal • Pendle – Age UKL Walton Lane Centre, Nelson • West Lancs – Brookside, , • – Your Support Your Choice, Blackburn • – Clayton le Moors Freshers Young Onset Cafes Memory Assessment Services (MAS)

Across Lancashire the MAS aim to: • Deliver quick and timely diagnosis to people whose symptoms suggest that they may have dementia • Provide all patients who meet the referral criteria with a person-centred service • Empower people with dementia and their carers to make informed decisions about their care to help maximise quality of life • Help to reduce the risk of crisis later in the illness and enable the patient to be cared for at home for as long as is possible, whilst this is their preferred place of care

Referral: GP

Centres: Blackpool, Preston, , , Lytham, Morecambe and Ormskirk Community Mental Health Team (CMHT)

Supports people over the age of 65 with: • Severe and enduring mental health needs • Those under 65 with a diagnosis of early onset dementia

Team provides: Assessment, Diagnosis, Treatment, Education and creates individualised care planning.

Care Co-ordinator is primary point of contact for the service user and their carer. The team works closely with the Rapid Intervention Treatment Team (RITT) and Memory assessment service (MAS). The team also has close links with Age UK and with the Private and Voluntary sector.

Referral: Via GP or other Health/Social Care professionals Rapid Intervention and Treatment Team (RITT)

RITT sees patients with acute mental health needs at home or in care homes with the aim of: • Preventing admission to hospital or care home, and; • To facilitate early discharge from the inpatient wards

It provides: • Time-limited input to patients referred who have higher needs or risks requiring more intensive input than can be provided by the CMHT Age UK Lancashire

Provide services for older people across Lancashire, Blackpool and .

• Support for Hubs and membership of Dementia Action Alliances (DAAs) • Dementia Community Links – and • Day Time Support Services – 8 centres. Local Authority and Private places, varying levels of support available • Advice and Information – face to face and telephone support with benefits checks, attendance allowance form completion • Hospital Aftercare Service – All Lancashire, 6 weeks support • Integrated Care Service – East Lancashire, multiple conditions • Veterans Service Alzheimer’s Society

Practical and Emotional Support for people with dementia and their families. They provide: • Dementia Advisers – provide information, support, signposting • Dementia Support Service – information and support for carers • Carer Information and Support programme – carer workshops • Singing for the Brain – structured group using music therapy principles in Blackpool, Fylde and Wyre, Lancaster and Morecambe • Dementia Cafes – socialising, speakers, peer support in Fylde, Wyre and Morecambe • Carers Peer Support Group – information exchange, support in Morecambe • National Dementia Helpline and leaflets • Support Dementia Alliances and Hubs N-Compass North West

Help people regain control of their lives, providing hope and a sense of purpose, through the provision of Carers support, Advocacy, Wellbeing and Counselling services: • Peace of Mind for Carers contingency plan • Sitting in Service – short break from your caring role • Regular coffee and chat meetings with other Carers • Access to a 24 hour CHAT Line which is manned by carers • Carers Assessment and reviews • Support DAA and Hubs Local Charities and Groups

• Dementia Cafes – many different districts including Morecambe, Burnley, Hundburn, Fylde, Rossendale and West Lancashire

• Lyrics and Lunch – Lancaster, hosted by St Chads Church

• Memory Matters – Homewise, . Advice and support to individuals and carers to stay independent in own home.

• Dementia Friendship and Support Network – Empowerment Charity, Blackpool provides advocacy and support for people with a diagnosis of dementia Contact details
