Dear Instructor, Welcome to Trillium Health Partners! We look forward to having you and your students join the team for your clinical rotation. Orientation: Instructors are responsible for giving orientation to their students. It is strongly advised that you meet with the Clinical Educator before your start date so you can discuss unit policies/procedures and orient yourself with the unit. Documents: Instructors are expected to collect all documents from students. 1) Code of Conduct 2) Workplace Violence 3) Confidentiality 4) Require Student Checklist 5) Flu documentation – when made available Please drop off the forms to one of the following locations: Credit Valley Hospital – Valley House (located outside the hospital near the parking exit) 2V226 Mississauga Hospital – Staffing Office on the Main Floor beside the pharmacy (there should be a box with my name on it) Queensway Health Centre – Please put in an envelope and submit through interoffice mail. Dress Code: School scrubs are acceptable for your placement. Hair: i) Must be clean, neat and well groomed. ii) Beards and mustaches must be clean, short and well-trimmed. Jewelry: i) To be kept to a minimum. Cosmetics: i) Make-up may be worn in moderation. ii) Nails must be clean, short and well-trimmed. Artificial nails are NOT permitted. Footwear: As a requirement of the Occupational Health and Safety Act, we are required to wear appropriate shoes when working in clinical areas. A policy is in place to reduce potential foot injury due to inappropriate footwear. This policy applies to all employees, professional staff, volunteers, students, contractors and any other individuals who perform work at Trillium Health Partners. Please make sure you read the Footwear Policy attached. Students not wearing proper footwear will result in being sent home. Scent Free Policy: The hospital is also a scent-free environment. Individuals with environmental sensitivities experience a variety of adverse reactions to environmental agents ... those living with environmental sensitivities are entitled to the protection of the Canadian Human Rights Act, which prohibits discrimination on the basis of disability. It is important to recognize the need for accommodating persons with scent sensitivities in order to minimize or eliminate exposure to triggers. Access Cards Credit Valley Hospital: Instructors can receive a swipe card for the unit- individual cards for each student will not be given. Cards can be obtained at the security office located near the main ER entrance (left hand side – blue door). Access cards will open locked doors and staff room on your unit. Please ensure you return your swipe card to security at the end of your placement. Mississauga Hospital: Instructors do not need swipe cards for access on the units. Flu Shot Policy Getting the vaccination is strongly encouraged. Please collect and submit a copy of the flu shot documents as soon as they have received them. These numbers must be reported, so it is important to remember to do this. If a student refuses to get a flu shot – please make a note when submitting the forms and the reason why. If a student does not receive a flu shot, they may be required to wear a mask. Lockers: Credit Valley Hospital: Lockers on the units are for Staff ONLY. Please do not have your students utilize these lockers. The female locker room is located on the first floor near the Environmental Services and the male locker room is located on the first floor near the maintenance office. If you and your students require lockers please visit the security office located beside the Tim Horton’s on the main floor. You will need to provide your name, school, and list of students who require lockers. You and your students will be provided with a blank access card. At the end of the term, students are to surrender their cards to their instructor, who will hand them over to Security. Failure to receive the access cards will result in a $25.00 fee to the school for that student. Students do sign a sheet acknowledging the agreement and responsibility of the locker, locker room and access card. Mississauga Hospital: PLEASE DO NOT USE THESE LOCKERS IF YOUR PLACEMENT IS IN THE J-WING. WE NEED TO GIVE PRIORITY TO THE UNITS THAT HAVE VERY LITTLE SPACE. The password for the combination lock is 1529 The lockers are located in the staff locker room, ground floor across from the cafeteria main entrance. Bring your lock for the day only. There are a limited amount of lockers available. Please let me know of any issues with conditions in the room. There are only female lockers available at this point. Computer Access/Training & Infection Prevention and Control Credit Valley Hospital: Instructors can sign up for computer training through the link provided on HSPnet. Once you have completed your training, you will be given a date to train your students. Our computer training room is located on the 4th floor room 4F139 or in our lab seminar room ( 2H150). To open the training room door, instructors can call security at 2525 or go to the security office. Please manually lock the door when you are finished. In order to receive your computer access password for you and your students, please bring your signed hospital forms to Allison Shin. All forms must be submitted before receiving your computer access. Instructors must log students onto the system within a week of their start date or passwords will expire. *Please note that computer training can still be done without your password. Temporary log in and password information is located in the room on the board. Clinical instructors who are familiar with the system do not have to attend a computer training session. Please note that instructors are responsible for training their students. Med Room/Lounge Credit Valley Hospital: Clinical instructors can go to the security office to obtain an access card to the med room. Students are not given access cards. OMNICELL Credit Valley Hospital: Instructors are not given OMNICELL access. This can be access through the patient’s primary nurse. Infection Prevention and Control Credit Valley Hospital: Infection Prevention and Control (IPAC) practices are important for the safety of our patients, staff, physicians, and you. Before your clinical placement begins, please ensure that you take the time to review CVH site IPAC practices available through the following links: http://trilliumhealthpartners.ca/patientrelations/pages/infection.aspx http://cvh-ishare/departments/infectioncontrol/Pages/default.aspx To attend an IPAC in-service specific for student healthcare providers, please contact the CVH IPAC main office at 905-813-1100, ext. 5865. Mississauga Hospital: IPAC is provided during your computer training session. It is important that you attend this part as well so you can go over this information with your students. Transportation Parking Office: Parking can be arranged at the parking office located in the front of the main parking lot. A monthly parking pass can be obtained with a student discount rate at $40.00. Please show your student ID card. This applies to instructors as well. Shuttle: Trillium Health Partners provides a shuttle bus to all sites. The shuttle bus runs from 07:30am to 17:15. It comes every 40 minutes, between the Mississauga Hospital site and the Queensway Health Centre site. It runs every 30 minutes between the Mississauga Hospital site and Credit Valley Hospital site. Credit Valley Hospital – 2200 Eglinton Ave. W: If you are going to drive to the hospital, please park in the hospital’s main parking lot above level 4. Should you be using public transit, the hospital is serviced by both GO Transit and Mississauga Transit. Mississauga Hospital – 100 Queensway W: The Main Entrance, Parking, Patient Discharge Door, Family Care Centre and Emergency Care Centre entrance are all at the first set of lights. Queensway Health Centre: 150 Sherway Drive: The Queensway Health Centre is located at 150 Sherway Drive, off the West Mall, south of The Queensway, and across the street from Sherway Gardens Shopping Centre.
