Office of the President of the Philippines PRESIDENTIAL MANAGEMENT STAFF Malacañang PMS-BIDS AND AWARDS COMMITTEE REQUEST FOR QUOTATION Date: August 10, 2021 The Office of the President-Presidential Management Staff (OP-PMS), through its Bids and Awards Committee (BAC), hereby invites all interested suppliers to submit their quotations for the SUPPLY AND DELIVERY OF DISASTER AND MEDICAL EQUIPMENT FOR THE INTEGRATED OPERATIONS AND SERVICE CONTINUITY SYSTEM SUBCENTRAL OFFICES IN THE CITIES OF PUERTO PRINCESA, LEGAZPI, AND ILOILO, subject to the General Conditions stated herein, and to submit said quotations, duly signed by their representatives, not later than August 17, 2021 (Tuesday). PhilGEPS Permit, and notarized Omnibus Sworn Statement (accompanied by a notarized Resolution, whichever is applicable), shall be attached to the quotation.1 3. The prospective bidders may submit an offer that provides for superior quality and/or better terms and conditions at no extra cost to the PMS. 4. All quotations shall be considered as fixed prices and are, therefore, not subject to price escalation during contract implementation. 5. All transactions are subject to withholding of appropriate taxes and fees. The PMS reserves the right to reject any or all Quotations/Bids, to waive any minor defects therein, to annul the bidding process, to reject all Quotations/Bids at any time prior to contract award, without thereby incurring any liability to the affected Bidder(s), and to accept only the offer that is most advantageous to the Government. To operationalize the IOSC sites, there is a need to procure disaster, medical equipment and supplies taking into consideration the value for money. II. IV. Timelines or Schedule of Deliveries The winning bidder shall deliver the said items within 30 days upon completion of respective IOSC sites. V. Terms of Payment Payment shall be made upon completion and acceptance of delivery of the required items, subject to deduction of applicable taxes by the PMS and 5% 2 retention fee. Late delivery shall be subject to penalty equivalent to 1/10 of 1% of the cost of the unperformed portion for every day of delay. All bid prices shall be considered as fixed prices (including shipping/delivery cost), are not subject to price escalation during contract implementation. VI. Approved Budget for the Contract The approved budget for the Supply and Delivery of Disaster and Medical Equipment for the Integrated Operations and Service Continuity Site (IOSC) in the Cities of Puerto Princesa, Legazpi, and Iloilo amounts to Two Hundred Twenty-Three Thousand and One Hundred Eighty Pesos (₱223,180.00), inclusive of all applicable taxes and fees. 3 Checklist of Eligibility Requirements (for Small Value Procurements with Approved Budget for the Contract (ABC) below PhP500,000.00) 1.
