Preface to the Fifth Edition This concise review of medical pharmacology is designed for medical students, dental stu- dents, and others in the health care professions. It is intended primarily to help students prepare for licensing examinations, such as the United States Medical Licensing Exami- nation Step 1 (USMLE) or other similar examinations. This book presents condensed and succinct descriptions of relevant and current Board-driven information pertaining to phar- macology without the usual associated details. It is not meant to be a substitute for the comprehensive presentation of information and difficult concepts found in standard phar- macology texts. ORGANIZATION The fifth edition begins with a chapter devoted to the general principles of drug action, fol- lowed by chapters concerned with drugs acting on the major body systems. Other chapters discuss autocoids, ergots, anti-inflammatory and immunosuppressive agents, drugs used to treat anemias and disorders of hemostasis, infectious diseases, cancer, and toxicology. Each chapter includes a presentation of specific drugs with a discussion of their gen- eral properties, mechanism of action, pharmacologic effects, therapeutic uses, and adverse effects. A drug list, tables, and figures summarize essential drug information included in all chapters. Clinically oriented, USMLE-style review questions and answers with explanations fol- low each chapter to help students assess their understanding of the information. Similarly, a comprehensive examination consisting of USMLE-style questions is included at the end of the book. Drug effects are produced by altering the normal functions of cells and tissues in the body via one of four general mechanisms: 1. To learn a little bit about what each hormone does, the FPP is providing the following summary: Estrogen: provides endometrial stability = menstrual cycle control. A higher estrogen dose increases the venous thromboembolism (VTE) or clot risk but OCP clot risk is still less harmful than the clot risk related to pregnancy and giving birth. Progestin: provides most of the contraceptive effect by ‐Preventing luteinizing hormone (LH) surge /ovulation ‐Thickening the cervical mucus to prevent sperm entry. Two major OCP formations are available. The time to onset of paralysis decreases and the duration of maximum effect increases with increasing Vecuronium Bromide for Injection doses. The use of a peripheral nerve stimulator is of benefit in assessing the degree of muscular relaxation. The ED90 (dose required to produce 90% suppression of the muscle twitch response with balanced anesthesia) has averaged 0.057 mg/kg (0.049 to 0.062 mg/kg in various studies). An initial Vecuronium Bromide for Injection dose of 0.08 to 0.10 mg/kg generally produces first depression of twitch in approximately 1 minute, good or excellent intubation conditions within 2.5 to 3 minutes, and maximum neuromuscular blockade within 3 to 5 minutes of injection in Vecuronium Bromide for Injection, Product Monograph Page 2 of 28 most patients.

NOT FOR USE IN NEONATES CONTAINS BENZYL ALCOHOL DESCRIPTION NUROMAX (doxacurium chloride) is a long-acting, nondepolarizing skeletal muscle relaxant for intravenous administration. Doxacurium chloride is [1α,2β(1'S*,2'R*)]-2,2' -[(1,4-dioxo-1,4- butanediyl)bis(oxy-3,1-propanediyl)]bis[1,2,3,4-tetrahydro-6,7,8-trimethoxy-2- methyl-1-[(3,4,5- trimethoxyphenyl)methyl]isoquinolinium] dichloride (meso form). The molecular formula is C56H78CI2N2O16 and the molecular weight is 1106.14. The compound does not partition into the 1- octanol phase of a distilled water/ 1-octanol system, i.e., the n-octanol:water partition coefficient is 0. Doxacurium chloride is a mixture of three trans, trans stereoisomers, a dl pair [(1R,1'R ,2S,2'S ) and (1S,1'S ,2R,2'R )] and a meso form (1R,1'S,2S,2'R). The meso form is illustrated below: NUROMAX Injection is a sterile, nonpyrogenic aqueous solution (pH 3.9 to 5.0) containing doxacurium chloride equivalent to 1 mg/mL doxacurium in Water for Injection. Hydrochloric acid may have been added to adjust pH. NUROMAX Injection contains 0.9% w/v benzyl alcohol. Reference ID: 2867706 CLINICAL PHARMACOLOGY NUROMAX binds competitively to cholinergic receptors on the motor end-plate to antagonize the action of acetylcholine, resulting in a block of neuromuscular transmission. This action is antagonized by acetylcholinesterase inhibitors, such as neostigmine. Pharmacodynamics NUROMAX is approximately 2.5 to 3 times more potent than pancuronium and 10 to 12 times more potent than metocurine.

Summary There is great concern over the long-term influence of oral contraceptives on the development of breast cancer in women. Oestrogens are known to stimulate the growth of human breast cancer cells, and this laboratory has previously reported (Jeng & Jordan, 1991) that the 19-norprogestin norethindrone could stimulate the proliferation of MCF-7 human breast cancer cells. We studied the influence of the 19-norprogestins norgestrel and gestodene compared to a 'non' 19- norprogestin medroxyprogesterone acetate (MPA) on MCF-7 cell proliferation. The 19-norprogestins stimulated proliferation at a concentration of 10-8 M, while MPA could not stimulate proliferation at concentrations as great as 3 x 10-6 M. The stimulatory activity of the 19-norprogestins could be blocked by the antioestrogen ICI 164,384, but not by the antiprogestin RU486. Transfection studies with the reporter plasmids containing an oestrogen response element or progesterone response element (vitERE-CAT, pS2ERE-CAT, and PRE15-CAT) were performed to determine the intracel- lular action of norgestrel and gestodene. The 19-norprogestins stimulated the vitERE-CAT activity maximally at 10-6 M, and this stimulation was inhibited by the addition of ICI 164,384. MPA did not stimulate vitERE-CAT activity. A single base pair alteration in the palindromic sequence of vitERE (resulting in the pS2ERE) led to a dramatic decrease in CAT expression by the 19-norprogestins, suggesting that the progestin activity required specific response element base sequencing.

    TITLE 16. CRIMES AND OFFENSES CHAPTER 13. CONTROLLED SUBSTANCES ARTICLE 1. GENERAL PROVISIONS § 16-13-1. Drug related objects (a) As used in this Code section, the term: (1) "Controlled substance" shall have the same meaning as defined in Article 2 of this chapter, relating to controlled substances. For the purposes of this Code section, the term "controlled substance" shall include marijuana as defined by paragraph (16) of Code Section 16-13-21. (2) "Dangerous drug" shall have the same meaning as defined in Article 3 of this chapter, relating to dangerous drugs. (3) "Drug related object" means any machine, instrument, tool, equipment, contrivance, or device which an average person would reasonably conclude is intended to be used for one or more of the following purposes: (A) To introduce into the human body any dangerous drug or controlled substance under circumstances in violation of the laws of this state; (B) To enhance the effect on the human body of any dangerous drug or controlled substance under circumstances in violation of the laws of this state; (C) To conceal any quantity of any dangerous drug or controlled substance under circumstances in violation of the laws of this state; or (D) To test the strength, effectiveness, or purity of any dangerous drug or controlled substance under circumstances in violation of the laws of this state. (4) "Knowingly" means having general knowledge that a machine, instrument, tool, item of equipment, contrivance, or device is a drug related object or having reasonable grounds to believe that any such object is or may, to an average person, appear to be a drug related object.

Date of Revision: March 25, 2013 145 Jules-Léger Boucherville, QC, Canada J4B 7K8 Control no.: 161881 Atracurium Besylate Injection Page 1 of 19 PrATRACURIUM BESYLATE INJECTION 10 mg/mL THERAPEUTIC CLASSIFICATION Skeletal Neuromuscular Blocking Agent ACTIONS AND CLINICAL PHARMACOLOGY Atracurium besylate is a nondepolarizing, intermediate-duration, skeletal neuromuscular blocking agent. Nondepolarizing agents antagonize the neurotransmitter action of acetylcholine by binding competitively to cholinergic receptor sites on the motor end-plate. This antagonism is inhibited, and neuromuscular block reversed, by acetylcholinesterase inhibitors such as neostigmine, edrophonium and pyridostigmine. The duration of neuromuscular blockade produced by atracurium besylate is approximately one-third to one-half the duration seen with d-tubocurarine, metocurine and pancuronium at equipotent doses. As with other nondepolarizing neuromuscular blockers, the time to onset of paralysis decreases and the duration of maximum effect increases with increasing atracurium besylate doses. The pharmacokinetics of atracurium besylate in man are essentially linear within the 0.3 to 0.6 mg/kg dose range. The elimination half-life is approximately 20 minutes. The duration of neuromuscular blockade does not correlate with plasma pseudocholinesterase levels and is not altered by the absence of renal function. INDICATIONS AND CLINICAL USE Atracurium Besylate Injection is indicated, as an adjunct to general anaesthesia, to facilitate endotracheal intubation and to provide skeletal muscle relaxation during surgery or mechanical ventilation. It can be used most advantageously if muscle twitch response to peripheral nerve stimulation is monitored. CONTRAINDICATIONS Atracurium besylate is contraindicated in patients known to have a hypersensitivity to it.
