2020 SHS PROM FASHION SHOW MODEL APPLICATION DUE: MONDAY JANUARY 27, 2020

The Schaumburg High School VIP Club will be sponsoring the 2020 Prom Fashion Show on ​ ​ Thursday, February 20, 2020 at 7:00 p.m. in SHS Auditorium. Prom Fashion Show models are open ​ to all junior and senior students. By signing below, you confirm that you are a junior or senior and will be available and available for the Prom Fashion Show preparation and activities, which include fittings, practices, preparation and the show.. If you are in any activity that may conflict with these dates, please confirm with your sponsors/coaches so that you can be excused for February 20 from 3:00 p.m. - 8:30 ​ p.m., before applying: IMPORTANT DATES

February 20, 2020 Prom Fashion Show 7:00 p.m. Practice 3:00 - 7:00 p.m.

February 8 - 18, 2020 Prom Dress Fittings The Ladies Room, 410 S Main St, Lombard

Students must have their own transportation to have your prom dress fitting completed during store th hours any day between February 8 –​ 18th, 2020. All dresses will be brought to the Prom Fashion ​ ​ ​ Show by The Ladies Room staff the day of the event and returned to them after the show.

By February 15, 2020 Tuxedo Fittings Formally Modern Tuxedo, 510 E Golf Rd, Schaumburg

Students will need to schedule and complete fitting appointments by Saturday, February 15th, 2020, ​ no exceptions. Students will need to pick up the Tuxedos on Wednesday February 19th and return ​ ​ ​ ​ tuxedos to Formerly Modern Tuxedo by Friday, Feb 28th after the show. There will be charges for ​ the tuxedos if returned damaged or missing pieces.

No deposit will be collected for the dresses or tuxedos.

Completed Prom Fashion show applications and photo release forms are due back to Mrs. Napier’s office no later than 3:00 p.m. on MONDAY, JANUARY 27, 2020. Your applications will be reviewed. You will be notified by your school email if you have been selected as a model. ​
