SMCHS Mothers Club Fashion Show 2021 Model REVISED Application & Contract

To participate as a model in the SMCHS Mothers Club Fashion Show on Sunday, May 2, 2021, a parent and model are expected to commit to the following:

Parent Expectations: • Purchase at least (1) Fashion Show Ticket. • Pay by check a non-refundable Model Fee of $50.00. (Space is limited to 40 contracts and payments received). • Complete and sign the model contract and return with payment made payable to SMCHS. • Email 3-5 pictures of your student and with other SM students for the photo montage. Please be sure to include one baby picture to Nancy Manix @ [email protected] with “Fashion Show Photos” in the subject line and your contact number.

Model Expectations: MOST IMPORTANT- HAVE FUN!! • Submit this contract to the Advancement Office on Tuesday, March 23rd. Contracts will be accepted from 7:30 a.m. to 3:30 p.m. Model fee must be included with the contract at time of submission – no exceptions. (Space is limited to 40 contracts and payments received). Applications will only be accepted from the student/model. Models will receive an email notification of acceptance and participation in the show by Friday, March 26th.

• Attend MANDATORY Model Orientation on Wednesday, March 31st, 4:30 PM – 6:30 PM

• Participate in all practices Wednesday, March 31st; Monday, Wednesday, April 21st, April 26th and Friday, April 29th 4:30 – 6:30 PM. Location will be at SM, campus area TBD.

• Model and parents agree that show producer and Fashion Show Chair have the final say on the garment to be worn in the show. Due to the large number of student participants prevents negotiation on this subject. The student agrees to take direction from the producer.

• On Sunday, May 2nd, arrive on time (Approx. 9:00 AM) for final rehearsal in the gym. • Models will not leave campus after arrival until the show has ended and all clothes and accessories are accounted for and returned. • You are representing SMCHS. Exemplary behavior is expected from all of our models. Listen and follow instructions by Fashion Show personnel.

• Treat all clothing items with respect. The clothes are on loan to us. Hang your garments on the garment rack if you change outfits. Clothes are not to be left on the floor and should be treated as though they DO NOT BELONG TO YOU AND NEED TO BE RETURNED TO THE VENDOR!

• Eating or drinking while wearing your fashion show outfit is NOT allowed. Keep all food and drink outside the dressing area and away from all garments. IF ANY ITEM IS DAMAGED YOU WILL BE PURCHASING IT!

Do you feel comfortable having your make-up and hair done the day of the show by a licensed stylist? (yes or no)______

Participation in the Fashion Show is a privilege. It is understood that the student will be held to the highest SM standards. Violation of the above may warrant dismissal from participation in show.

Parents of models – This is Mothers Club’s most significant fundraising event for the year, with all proceeds going to programs that directly benefit our students. APPLICATIONS MUST BE TURNED IN ON TUESDAY, MARCH 23rd by student/model. Applications will not be accepted from a parent The show is limited 40 models Additional models can participate in the Parade of Colleges-this will be a pre-recorded event, students will not be present at the Fashion Show on Sunday, May 2nd.
