The study identified a new equipping and maintenance strategy that has the potential to enable the Army to reduce the cost of equipping and maintaining its CSHs at fully modernized levels, providing them with equipment that is newer and in better condition on average than what they have now. Because the proposed strategy represents radical change in several respects, the study recommends a phased imple- mentation, beginning with the fielding of new equipment set designs to several active and reserve CSHs at home station. Evaluation of the performance and effects of these sets will permit fine tuning of the proposed strategy. This research was sponsored by the Commanding General, U.S. Army Medical Command/The Surgeon General. It was conducted within RAND Arroyo Center.     Acta Theologica Supplementum 7 2005 THE EVOLUTION OF HOSPITALS FROM ANTIQUITY TO THE RENAISSANCE ABSTRACT There is some evidence that a kind of hospital already existed towards the end of the 2nd millennium BC in ancient Mesopotamia. In India the monastic system created by the Buddhist religion led to institutionalised health care facilities as early as the 5th century BC, and with the spread of Buddhism to the east, nursing facilities, the nature and function of which are not known to us, also appeared in Sri Lanka, China and South East Asia. One would expect to find the origin of the hospital in the modern sense of the word in Greece, the birthplace of rational medicine in the 4th century BC, but the Hippocratic doctors paid house-calls, and the temples of Asclepius were vi- sited for incubation sleep and magico-religious treatment. In Roman times the military and slave hospitals were built for a specialised group and not for the public, and were therefore not precursors of the modern hospital. It is to the Christians that one must turn for the origin of the modern hospital. Hospices, originally called xenodochia, ini- tially built to shelter pilgrims and messengers between various bishops, were under Christian control developed into hospitals in the modern sense of the word. In Rome itself, the first hospital was built in the 4th century AD by a wealthy penitent widow, Fabiola. In the early Middle Ages (6th to 10th century), under the influence of the Be- nedictine Order, an infirmary became an established part of every monastery.

Fort Sam Houston and San Antonio Joint Medical Command were alerted on 19 December to activate their contingency plans for support and prepare to receive large numbers of casualties. BAMC received 43 casualties during the conflict and, fortunately, all injuries were diagnosed as minor. All of FSH played a major role in ensuring soldiers in combat support readiness roles were prepared to respond in whatever capacity necessary. (“Panama: FSH Responds to Major Crisis,” News Leader, 5 Jan 1990) A new “tool” traveled with the 41st Combat Support Hospital during a week of training at Camp Bullis. The new tool was a computer program that became part of the Theater Army Medical Management Informa- tion System (TAMMIS). Abstract: To give a representative overview of medical professionals working in today’s remote and extreme environments, we consider the following teams and organizations: • Expeditions on 8000m peaks led by accredited guides of the American Mountain Guides Association (AMGA) or International Federation of Mountain Guides Association (IFMGA) • Everest Base Camp Medical Clinic (Everest ER) in Nepal • US Antarctic Program (USAP) • Medical Departments aboard US Navy ships and submarines • US Special Operations Command (SOCOM) Corpsmen and Medics • US Navy Diving Medical Officers • Crew Medical Officers (CMO’s) aboard the International Space Station After reviewing the practice environments and medical training of the various teams, we consider the medical qualifications of a hypothetical future team embarking on a mission to a particularly remote and extreme environment, a human spaceflight mission to the surface of Mars. Introduction: This article explores the qualifications and training of current practitioners of medicine operating in remote and extreme environments. Remote environments have limited access to rescue and hospital care. Extreme environments have exposure to temperature and pressure extremes, radiation, as well as exposure to significant mechanical hazards, including from enemy action during military operations. Several environments combine these descriptions. A prospective human spaceflight to Mars would operate within remote and extreme environments through the entirety of the mission. As observed by Dr. HD Backer, MD, “Wilderness medicine not only overlaps with sports medicine, but also emergency medicine, military, occupational, and travel medicine and international health” and “It shares with military medicine an intense interest in environmental stresses and hazards, field management and mobile response” (Backer, 1995).
