Half of all women in Canada report that they experienced at least one incident of physical or sexual violence since the age of 16. In the Hamilton area, Good Shepherd Women’s Services is often the first place they turn. We provide a sanctuary for women and their children dealing with abuse, violence, homelessness, poverty and mental illness. They also receive counselling, safety planning, health care and housing services. In the past year, 110 women and 174 children fleeing violence and abuse were welcomed to Martha House. Mary’s Place opened its doors to 251 women who are homeless or street-involved and in need of emergency shelter and advocacy. But demand exceeds our space and we turn away women who are living in vulnerable circumstances on hundreds of occasions every month. When we invest in the health and well-being of women and children, our entire community wins. We’re honoured to offer services that give them hope and restore their dignity. MARTHA HOUSE Martha House is a uniquely styled emergency shelter for women and women and their children. 15 apartments are designed to accommodate families of different sizes - safe, short-term accommodation for women and their children in private family-style units. Safety planning and emergency transportation to the shelter, if needed, is available. SECOND STAGE SERVICES Good Shepherd Second Stage Services offers furnished, affordable housing to both single women and women with children for up to one year. Programs and supports are geared to creating greater physical, emotional and financial stability in the aftermath of violence and abuse. TRANSITIONAL AND HOUSING SUPPORT PROGRAM Transitional and Housing Support Workers will help women and their children access supports and services to establish violence and abuse-free lives in their community. NOTRE DAME HOUSE Committed to helping youth who are street-involved or homeless due to family violence, conflict, abuse, neglect, poverty and exposure to mental health and addiction issues. Our goal is to ensure that they have the support they need to overcome obstacles and break down barriers to become active and contributing members of our community. Notre Dame House is a 24-hour emergency shelter providing 21 beds for homeless and street involved youth between 16 and 21 years of age. The program uses a unique team approach with community collaboration to help youth reach their personal goals using a strength based and client centred approach.

COMMUNITY OUTREACH SERVICES The Notre Dame Community Resource Centre, is a multi-agency resource centre for at-risk youth residing within the community who are between 16 and 21 years of age. Youth have access to a number of outreach support services and programs to assist them in remaining housed; diverting them from homelessness and street involvement. A case coordinator assists the youth in navigating community and internal services based on their identified needs. Resources available include, but are not limited to: Help with getting back into school or finding employment Assistance obtaining or replacing ID Access to Housing Workers Help with applying for or maintaining Social Assistance Hot Meal Program, 7 days a week (Lunch and Dinner) Ontario Works Youth Trustee Program Connections to community supports Free laundry facilities and clothing/household donations Access to all on-site Mental Health, Medical Care, Addiction/Substance UseR Services Recreational and Life skills based programming Early Diversion and Family Mediation In partnership with the Hamilton Regional Indian Centre we provide opportunities for youth to engage in cultural activities and to reconnect with their traditional teachings In partnership with Centre 3, we work with the NU deal program Notre Dame House School offers a free studio based course for youth in transition In this professional 12 week course youth learn how to design, print, film, market and sell their work. NOTRE DAME HOUSE SCHOOL Provides an alternative learning environment, breaking barriers for students who have been affected by homelessness, poverty, family breakdown and mental illness. Students may attend the school for a brief period and then reintegrate into a main stream high school or they may complete their high school credits and graduate from Notre Dame House School. All community referrals can be forwarded through the Community Resource Centre.

BRENNAN HOUSE 15 bed, co-ed residential mental health program for youth 16 to 20 years of age, with identified social, emotional, behavioural, mental health and addictions issues. Staff develop individualized treatment plans to support or enhance youth with their integration into the community. Youth have access to the Mental Health Clinician as well as individualized and clinically informed care. Youth support workers offer coaching to help manage mental health symptoms. Staff provide 24-hour support with the management of medication schedule and mental health concerns. Staff offer unconditional acceptance in a warm, supportive home-like atmosphere. Residents are expected to assume responsibility for setting and following house rules and routines, and establishing personal goals and completing their education or undergoing job skill training or employment. Services offered: 24 hours staff support / Access to Mental Health clinician and consulting psychiatrist / Assignment to a youth support worker to encourage residents’ strength and aid in working out personal goals / Life and social skills training / Access to after care / Access to the Youth Trustee Program helping with financial aid for educational purposes. BRENNAN HOUSE A.C.T.S Aftercare Transitional Support (A.C.T.S) housing is available to youth who require minimal support for up to two years after living at Brennan House. The A.C.T.S. consists of six furnished bachelor units supported by Brennan House youth liaison worker with weekly resident meetings.

ANGELA’S PLACE A home to young moms and their children. Mothers aged 21 and younger, who are confronted with enormous responsibilities as they raise their babies, live in one of 15 fully furnished, subsidized transitional housing apartments. Residents of Angela's Place receive on-site support and programming including life skills, goal setting and child development training designed to break barriers for multiple generations. Young moms at Angela's Place learn to provide a better life for their family and discover faith in themselves. Supportive housing allows young moms to have a safe and stable living environment, while benefiting from support services that improve long-term outcomes for both mother and child, such as job attainment and retention, academic success, improved parenting skills and healthy child development. In addition to the alternative education school, tenants attend programs, based on their personal needs and strengths. These programs help young moms to continue to build life skills and increase parenting abilities. Programs include: Cooking / Community Gardening / Health & Nutrition / Infant bonding and attachment / Budgeting Pre-employment Counselling / Problem Solving / Public Health Support / Ontario Early Years Hamilton LEAP (Learning, Earning and Parenting) / Mental Health and addiction support JEANNE SCOTT PARENT & CHILD RESOURCE CENTRE Located next door to Angela’s Place is where young mothers earn a higher education to break the cycle of poverty and better their chances of employment and independence. The Hamilton-Wentworth Catholic District School Board, in partnership with Good Shepherd, operates a school for the mothers, while on-site child minding is provided for their babies. The Jeanne Scott School is for young moms who live at Angela's Place or in the east end of Hamilton. In addition to the alternative education school, tenants attend programs, based on their personal needs and strengths. These programs help young moms to continue to build life skills and increase parenting abilities.

COMMUNITY MENTAL HEALTH PROGRAM Based out of Notre Dame House, the Community Mental Health Program serves street-involved youth ages 12-21 in Hamilton, meeting youth at various residential and non- residential programs within Good Shepherd Youth Services and the Street Youth Planning Collaborative (SYPC). Youth are supported through the provision of youth focused, evidence informed mental health services. Consultation and liaison is also provided to staff and mental health and medical services in the community. Through referrals from staff at youth agencies, the community mental health program provides services and linkages to meet the needs of the youth, many of whom have serious and complex mental health issues. Program offers: Crisis Intervention for suicidal and seriously ill youth who need hospitalization / Mental health assessments / Consultation with Child and Adolescent Psychiatrists / Evidence informed Individual Therapy / Skills for Life group / Staff training at Good Shepherd Youth Services and the SYPC agencies to increase the mental health knowledge and skills of front line workers JEB’S PLACE A residential mental health treatment centre for males, 13 to 17 years of age, who are unable to live with their families due to complex mental health needs, family crisis, parent-teen conflict, trauma or loss. The program at JEB's Place teaches youth and their families Skills for Life to reduce individual and family stress, and to develop the tools necessary to successfully reintegrate back into the family living situation or community. Available support includes: Youth Support Workers Mental Health Clinician / Therapeutic Recreationist / Cook Housekeeper / Program Manager / 24-hour staffing Case Manager assigned to each family /On-site Child and Youth Therapeutic Recreationist responsible for planning and implementing therapeutic activities based on treatment goals and Skills for Life, with support from the Mental Health Clinician / Daily programming focused on teaching distress tolerance, emotional regulation and activities of daily living / Programming also utilizes community resources e.g. Rec centres, YMCA, Conservation areas, libraries

BARRETT CENTRE FOR CRISIS SUPPORT Provides a safe environment in the community and responds to the needs of individuals, 16 years of age or older, who experience a mental health crisis and do not require a hospital stay. We provide confidential and free services 24 hours a day, 365 days a year. Services provided: Crisis telephone line / In-person crisis counselling / Short-term crisis bed stay / Beyond Self Harm support group. We support individuals in developing effective solutions to their crises through an immediate assessment, care and support. We also link individuals with resources for on- going support. Our crisis and peer support workers have a broad range of experience in the fields of health, mental health, addictions, recovery and crisis counselling. HEALTH CLINICS Good Shepherd Health Clinics are part of the Shelter Health Network (SHN), which is a multi-disciplinary team of healthcare providers operating within the shelters and community agencies in the downtown core of Hamilton. SHN was established in 2005 to reach out to a high-risk population who do not have stable housing and who have complex health problems, which often have a secondary presentation of a mental health and addiction issues. SHN has thirteen clinic sites, which includes five sites operated by Good Shepherd. Partners include; Good Shepherd Centres, Salvation Army, Wesley Urban Ministries, Mission Services, Wayside House, Men’s Addiction Service Hamilton (formerly Men’s Withdrawal Management Centre) and CMHA. Each clinic is supported by the host partner agency, while the SHN Board provides non-administrative leadership. All referrals are centralized and processed through the administrative office located at 128 Emerald Street South, in Hamilton. Good Shepherd provides an oversight for the administrative functions and support. The clinics are served by a number of physicians who work part-time, a nurse practitioner, registered nurses and a Foot Care technician. The primary criteria for individuals is that they do not currently have a family doctor, are homeless or at risk of being homeless, and may have challenges maintaining a relationship with a physician in the community. EMMANUEL HOUSE & PALLIATIVE CARE Emmanuel House is more than a hospice; it is a place for people to experience their end-of-life journey with dignity and comfort in a place that feels like home. The Residential Hospice is where we provide 24-hour care for people with a life-limiting illness and require comfort and support for themselves and their families. Our palliative care team provides 24-hour nursing care including pain and symptom management, a palliative care physician, social, psychological, spiritual and practical support. Experienced and compassionate staff assist families with their emotional needs and with understanding what is happening to their loved one. All meals and laundry service are provided for residents. Each resident has a private room with hospital bed, nursing call bell, telephone, cable television, small fridge, lounge chair, and private bathroom. We encourage people to include hospice in their end-of- life plan. PALLIATIVE CARE SHARED CARE OUTREACH TEAM (PC TEAM) Good Shepherd, in partnership with community palliative care providers, offers in home services to individuals with advanced progressive illnesses, and their families. We work with individuals to provide palliative care that is appropriate, timely and pertinent to their personal goals of care. The family physician remains the primary health care provider. The outreach team is funded through the Hamilton Niagara Haldimand Brant Local Health Integration Network. Our clinical experts include: Clinical Navigator - a palliative care nurse, who can help individuals navigate the complex system of health and community services required at end of life. Provides coordination of community resources (including liaising with CCAC, pharmacies, and family physicians) to help keep individuals in their homes and their caregivers informed and supported. Palliative Care Physician works with family physicians, providing home visits, and telephone consultation/assistance regarding pain and symptom control prescriptions. Spiritual /Bereavement Clinician supports individuals/families with emotional, social, spiritual issues. They offer grief/bereavement counselling and assist with end-of-life planning. Advanced Practice Nurses from HNHB CCAC work closely with the team to provide expert nursing assessment and care.

SENIORS ASSISTED LIVING This program supports seniors who require services at a greater frequency or intensity than home care but without the medical monitoring or supervision that would be provided in a long-term care home (LTCH). The program aims to promote wellness and improve the health of clients by providing a level of services that enables them, despite their illnesses or conditions, to live in the community with a high degree of independence, and to be integrated into community life as much as possible. The focus of assistance in the Assisted Living Program is to promote and strengthen the individual’s health and social environment. We work with clients and families to find the best possible ways to maintain independent living while respecting the dignity and personal choices of the individual. The staff is available 24 hours a day, seven days a week to help clients on both a scheduled and unscheduled basis. Services Include: personal care such as bathing / personal support such as dressing and taking medications / homemaking services including light housekeeping, laundry, meal preparation & care coordination. SENIORS ACTIVATION AND MAINTENANCE PROGRAM The SAM Program is a day program for seniors and adults with diminished abilities, and is offered in three community-based-centers. Activities are planned for the participants on a daily basis. Each day consists of a variety of activities that have been created to maintain and enhance the well-being of the participants/clients. Refreshments and a noon meal are also provided on a daily basis.

Provides employment experiences to individuals who face barriers to employment and require supports to be successful in working. Barriers include mental health issues; lack of recent employment experiences; past trauma, abuse or violence; receiving social assistance; or newcomer status. Recognizing that our participants come to us with reservations as well as varied backgrounds and abilities, we provide on-the-job coaching and mentoring to ensure that they have a positive work experience and can take pride and ownership in a job well done.
