Methods of Payment Cash or Check; Credit Cards: Visa, MasterCard, Discover, American Express; Insurance Policies. Full payment is required 24 Hours prior to services rendered. GENERAL PRICE LIST These prices are effective August 30, 2019 and are subject to change without notice. The goods and services shown below are those we provide to our customers. You may choose only those items you desire. You may require additional items based on your specific needs or circumstances. However, any funeral arrangements you select will include a charge for our basic services and overhead described under Professional Services. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Prior to drafting any contract for goods or services, the responsible party or the decedent’s survivor who is handling the funeral arrangements is entitled to receive a copy of any preneed agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent. Professional Services 1. Basic Services of Funeral Director and Staff: This fee for our basic services constitutes a pro rata share of the business expenses required to operate the mortuary, such as property taxes, licenses, utilities, insurance, facilities maintenance, motor equipment and staff readiness to serve the public regardless of the type of disposition, and will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charge for direct cremations, immediate burials, forwarding or receiving remains). The fee includes the recording of statistical information for the death certificate, preparation of the legal documents/permits, certification of death by a local area physician for filing with appropriate county agencies; procurement of the permit for disposition as required by cemeteries, common carriers, and/or crematories; record retention and standard accounting procedures. Consultation and assistance in the planning and coordination of the funeral or memorial ceremony, recording of surviving relatives for obituary and clergy records, placement of newspaper notices (does not include the newspaper charge); consultation with clergy, florists, selected cemetery, common carrier, and/or crematory; completion of details for the services and merchandise selected, and advice concerning union, insurance, or other eligible benefits to which the decedent may have been entitled.............................................$ 1,595.00 Preparation We require the use of licensed personnel to supervise all technical aspects of our services. The utmost care is taken in all steps of our preparation and we take our role very seriously and it shows in the dedication to our clients. 2. Professional Embalming: A. Embalming: Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing.
