Page 1/3 LAZADA CITI PLATINUM CARD ONLINE APPLICATION PROGRAM RECEIVE LAZADA E-VOUCHER TERMS AND CONDITIONS 1. Program Scope: The program is applicable at Citibank, N.A., Ha Noi Branch and Citibank, N.A., Ho Chi Minh Branch for primary cardholders of Lazada Citi Platinum Card issued by Citibank Vietnam. 2. Program Period: All application documents must be submitted from 10 December 2020 until 31 December 2020 (both days inclusive) in Ho Chi Minh City and Ha Noi. 3. Eligible Products: Primary Lazada Citi Platinum Card issued by Citibank in Vietnam. 4. Promotion details: New customers when satify all terms and conditions of Eligible customers as defined in below Article 5 will receive: Maxinum gift Condition Gift Gift value quantiy Completing all steps of online application process and submitting all Lazada e-voucher VND 1,000,000 95 necessary, eligible documents to Citibank’s system Each customer can enjoy promotion once (01) during the promotion period. 5. Eligible Customers: Retail customers must meet all of the following conditions: Customers who apply for a primary Lazada Citi Platinum Card via Lazada and Citi’s channels by completing the credit card application and submitting all necessary documents during the Program Period; and Customer must not be an existing Citibank primary cardmembers of Citi PremierMiles, Citi Cash Back, Citi Rewards, Citi Simplicity+, Lazada Citi Platinum for a period of 365 days before the date customers re-apply for the new credit card for this promotion; and Customers must not be Citibank official staff; and Customers must not EU, EEA, Switzerland, Guernsey, Jersey, Monaco, San Marino, Vatican, and The Isle of Man citizens/residents; and Customers must not be US citizens or US residents. The Promotion runs while Gift lasts, maximum of 95 Lazada e-voucher. The order of Customers that redeem for the Gift will be based on the relevant order of Customers who first meet the Condition in order to qualify for the Gift. Hereinafter call “Eligible customer” Page 2/3 Important notice: Customer is ONLY eligible for promotion when meets all the Terms & Conditions for the promotion. 6. Eligible application: Complete all steps of online application process: - Step 1: Contact Details - Step 2: Personal Details - Step 3: Employment - Step 4: Card Options - Step 5: Submit - Step 6: Confirmation Upload all necessary and eligible documents to Citibank's system: - Proof of Identity (Passport, National ID, Citizen ID) - Proof of Employment (Labor contract/ Appendix of labor contract/ Appointment letter/ Regulation document...) - Proof of Income (Bank statements) - Proof of Residence (Family registration book, Temporary residence certificate, KT3, Utility bills, Postpaid mobile bill...) After Customer has completed all the above requirements, Citibank will send the confirmation receipt via email provided in Step 1. Citibank has the sole and exclusive right to review, approve or decline customer's application. 7. Gift: Each eligible customer will receive maximum one (01) Gift during the program period. Eligible customer will receive Gift while stocks last. 8. Gift redemption Eligible customer will receive Gift notification within 30 to 60 days once completing all steps of online application process and submitting all necessary, eligible documents to Citibank’s system. Redemption period is 60 days from the day the customer first receive Gift notification via SMS or email. Citibank will not handle any dispute related to the gift redemption after the above mentioned Redemption period or/and after customer already received the gifts. Any complaint related to the Gifts will not be accepted after 90 days from the date the online application is uploaded successfully. 9. Citibank is not the supplier or delivery service provider of the Gift. The products are sold and services are provided solely by the relevant vendors, under such terms and conditions as determined by such vendors, and Citibank accepts no liability whatsoever in connection with such products and services. The products and services have not been certified by Citibank and under no circumstances shall the inclusion of any product or service in this promotion be construed as an endorsement or recommendation of such product or service by Citibank. Eligible customer will be bound by the Terms and Conditions of the supplier of these products/services. In case of any inquires or disputes related to the products/services, eligible customer must contact the suppliers directly. 10. Citibank reserves the sole and exclusive right to refuse giving the Gift to any Eligible Customer considering he/she does not or refuses to provide Citibank with clear and complete supporting documents or violates any part of these Terms and Conditions of the Program. 11. Citibank may contact the eligible customer via phone number/email registered with Citibank for further information/clarification or additional documents. Page 3/3 12. Citibank accepts no liability for undelivered SMS/email notification due to incorrect or unupdated contact phone number/email address registered with Citibank or blocking SMS from mobile operator. 13. In case of our reasonable discretion, Citibank reserves the sole and exclusive right to change the Gift to any other promotional items of similar value, but should not exceed the value indicated in Article 4 of these Terms and Conditions in any case. 14. Unless otherwise provided in this Terms & Conditions, Citibank is not liable for any clear or implied agreement or guarantee about quality and conformity of the Gift . Citibank holds no responsibility in case the Gift is not redeemed successfully during Program Period. 15. Citibank, its affiliates, Board of Directors, staff and representatives of Citibank and its affiliates will be not liable for any loss or damage (including but not limited to indirect and derivative loss) or any personal incidents incurred from the participation of this Program or from the receipts or usage of the Gift unless liabilities specified under applicable laws. 16. By receiving or accepting the Gift, Eligible Customer or receiver agrees to ensure and hold Citibank, its affiliates, Board of Directors, staff and representatives of Citibank and its affiliates harmless against claims, actions, sues, judges, damages, loss, cost which may incur or any liability for Citibank, its affiliates, Board of Directors, staff and representatives of Citibank and its affiliates in connection with the participation of this Program or receiving and using the Gift. 17. Citibank holds no responsibility in relation to any tax incurred from the Gift. Any tax or tax return for Tax Agency incurred from the receiving of the Gift is the responsibility of the Eligible Customer. Eligible Customer himself/herself is liable for consulting with the experts about possibly incurred tax. 18. These Terms & Conditions have been written in both Vietnamese and English versions. The Vietnamese version shall prevail in case of discrepancies and/or inconsistencies. In case of dispute, Citibank’s decision is final. By participating in the program, Customer by default accepts all Terms and Conditions of the Program as listed above. .
