This work began early in 2017 with market and focus group research involving staff, referral sources, patients, board and community members to determine public perception of our organization. We received overwhelming confirmation that Androscoggin is a tremendously valuable resource to the community, often described as “capable, compassionate, kind, and trusted” according to the hundreds of people we spoke with. We used the research to guide our areas of strategic focus moving forward. This effort of reintroducing our organization will help us to remain an important and relevant part of the fiber of the communities we serve. We meet patients where they are in life, helping them to achieve their personal health goals. From infancy to end of life, we offer all of our care services to individuals regardless of ability to pay. It’s healthcare as it should be, and we’re proud to be leading the way.

REMEMBRANCE ORNAMENT Each year, a holiday remembrance ornament can be purchased and personalized in memory of a loved one. The event raises needed funds to support home healthcare and hospice services. The 2017 Remembrance Ornament raised over $12,000. This release is a symbolic gesture of life’s transformation, and all loved ones names were read at this moving and beautiful remembrance ceremony. 3510_AHCH_Community Report_2017_FINAL - Copy.indd 7 10/22/18 7:01 PM Addilyn’s Story When Jamie and Kyle Davis learned they were expecting their first child they went through the gamut of emotions all first-time parents do: excitement, nervousness, uncertainty, but mostly, joy. Both Jamie and Kyle are young and healthy and Jamie’s pregnancy was normal by all accounts. “Plenty of morning sickness,” recalls Jamie, “but nothing alarming.” On August 23, 2011, Jamie and Kyle welcomed their daughter Addilyn Sophia Davis into the world. The doctors ruled Addilyn a healthy baby girl but Jamie knew something wasn’t right. She calls it a “mother’s unspeakable instinct.” Her parents knew their child was in pain and suffering but without knowing what was wrong with their precious child, they were at a loss. It wasn’t until six months later that Addilyn was diagnosed with Krabbe Disease, a form of Leukodystrophy. Having never heard of the disorder, Jamie and Kyle were thrown into a whirlwind of questions and fear. After asking what could be done to treat the illness, they received the answer no parent ever wants to hear: “You can’t, it’s fatal.” Krabbe Disease is an inherited genetic disorder that damages the protective coating of cells in the nervous system and the brain. Babies diagnosed with Krabbe Disease are generally given a life expectancy of two years or less. There is no cure for Krabbe Disease and treatments are focused on support and comfort. “I never thought my baby or my would face such devastation on what was supposed to be a joyous occasion. Babies bring joy, not heartache and pain,” Jamie recalled. Despite such a bleak diagnosis, Jamie and Kyle devoted themselves to providing Addilyn with the very best life, however long it might be. They learned everything they could about her disease and how to keep her as comfortable as possible. Some days were good while others were much more difficult, but they always focused on Addilyn. They took family vacations, even a trip to Disney, and when Addilyn was two years old she became a big sister to baby sister Rilyn. The Davis family learned about screenings that can be administered at birth to test for life-threatening diseases, like Krabbe, and treatments which, when administered early, can impact the outcome and quality of life for sick children. “I’m a huge advocate for Krabbe screening at birth. We’re only left to wonder what early treatment could have meant for Addilyn and the difference it could’ve made in her life if we had been given the opportunity to test early,” Jamie said. Addilyn and her family very quickly made it their mission to advocate for early screening. “Early detection is key and could result in life or death,” shared Jamie. Throughout the battle, Jamie documented ’s struggles, joys, and daily life with the community through a Facebook page: “Addilyn’s Journey of Hope.” Addilyn’s hometown of New Sharon named her their “Hometown Hero.” Her story continued to spread well beyond Maine. More than 100,000 social media followers, better known as “Team Addilyn,” followed her and learned more about this rare and little- known disease.

The impact technology makes in their daily lives has proven to be remarkable and propels our mission to ensure patients are cared for in their home, where they want to be. No piece of equipment embodies this sentiment better than our telehealth monitors that utilize state-of-the-art, remote technology combined with nursing assessments to manage the care of acute and chronic conditions. The devices are easy to use and monitor weight, blood pressure, oxygen levels, and blood sugars, all from the comfort of home. This information is automatically submitted to our nursing team for review and if medical intervention is needed, we contact the patient and their physician. In addition, our team can reach out via phone to with the patient and assess their needs. I wanted to express Shane Levasseur, RN, heads Androscoggin’s Telehealth Program and has spent his fair share of how thankful we are time on the phone making these calls to check in with our patients over the years. He recalls the story that your program of one patient who requested a daily phone call from a nurse, yet when nursing staff reached out there allowed Mom to spend was never an emergency or change in status. “She was a widow and in her late eighties” Shane said, her sixtieth wedding “I think she just needed reassurance that we were anniversary at home. looking out for her.” During one of their daily conversations the patient was upset because she was no longer able to maintain her old farmhouse after the death of her husband and had reluctantly agreed to leave her home to move in with her daughter. She reminisced that she had shared this home with her husband for nearly six decades years and their 60th anniversary was coming right up. It was the place where they had raised their children and shared so many memories. She expressed that she just wished to remain at home until her anniversary so she could celebrate what would have been such a joyous occasion. “I told her we would do our best to support her in reaching that goal,” shared Shane. Shortly after, the patient’s daughter sent the following note of thanks: I want to express my gratitude. It was extremely important to my mother that she celebrate her 60th wedding anniversary in the home that my parents shared for all of those years. Mom was able to get all dressed up and set two dinner plates, one for her and one for dad. She enjoyed her dinner by candlelight. I want to express how thankful we are that your program allowed Mom to spend their wedding anniversary at home one last time. She has agreed to move in with me so that I can help care for her. Last night was a special evening for her and you helped her achieve her greatest wish.

3510_AHCH_Community Report_2017_FINAL - Copy.indd 29 10/22/18 7:01 PM Hospice House Renovations The expansion and renovations to the Hospice House are complete, and thanks to people like you, the entire cost was funded by donations made over the years to the Hospice House Fund. The showpieces of the new construction are the drive-under canopy, covered walkway, and vestibule. This attractive and functional addition makes for a more comfortable arrival for patients, particularly during the harsh Maine winters. Other parts of the completed project include an additional treatment room, making it easier for more outpatient services to be delivered. There’s also more staff space which allows for more privacy. Additional storage space for supplies and equipment allows for easy accessibility for patient comfort and care. Harriman and Associates designed the project and H.E. Callahan Construction, the original builder of the Hospice House, completed the work. Thank you to all who have donated to the Hospice House Fund in the past. You were integral in ensuring the completion of this project.
