SUMMARY Introduction: Labyrinthitis ossificans is a pathology characterized by sensorioneural hearing loss; secondary to infectious process, which produces irreversible injury to inner ear. Objective: To report a labyrinthitis ossificans case and review the literature. Case Report: A seven-year-old male patient, with profound hearing loss in tonal audiometry; no response from brainstem audiometry and compatible CT findings. Conclusion: Labyrinthitis ossificans results ossification on the inner ear structures. Pacient presents profound irreversible hearing loss, followed or not by disequilibrium, that can have important implication on educational socio-development. He had noticed signs of a possible upper respiratory infection, including a sore throat and headache, the day before his symptoms started. His medical history was unremarkable. He denied any history of otologic surgery, trauma, or vision problems, and he was not taking any medications. The patient was afebrile on physical examination with a heart rate of 48 beats/min and blood pressure of 117/68 mm Hg. A Weber test performed using a 512-Hz tuning fork lateral- ized to the right ear. A Rinne test showed air conduction was louder than bone conduction in the affected left ear—a normal finding. Tympanometry and otoscopic examination showed the bilateral tympanic membranes were normal. THE DIAGNOSIS Pure tone audiometry showed severe sensorineural hearing loss in the left ear and a poor speech discrimination score. The Weber test confirmed the hearing loss was sensorineu- ral and not conductive, ruling out a middle ear effusion. -

Vestibular neuritis, labyrinthitis, and a few comments regarding sudden sensorineural hearing loss Marcello Cherchi §1: What are these diseases, how are they related, and what is their cause? §1.1: What is vestibular neuritis? Vestibular neuritis, also called vestibular neuronitis, was originally described by Margaret Ruth Dix and Charles Skinner Hallpike in 1952 (Dix and Hallpike 1952). It is currently suspected to be an inflammatory-mediated insult (damage) to the balance-related nerve (vestibular nerve) between the ear and the brain that manifests with abrupt-onset, severe dizziness that lasts days to weeks, and occasionally recurs. Although vestibular neuritis is usually regarded as a process affecting the vestibular nerve itself, damage restricted to the vestibule (balance components of the inner ear) would manifest clinically in a similar way, and might be termed “vestibulitis,” although that term is seldom applied (Izraeli, Rachmel et al. 1989). Thus, distinguishing between “vestibular neuritis” (inflammation of the vestibular nerve) and “vestibulitis” (inflammation of the balance-related components of the inner ear) would be difficult. §1.2: What is labyrinthitis? Labyrinthitis is currently suspected to be due to an inflammatory-mediated insult (damage) to both the “hearing component” (the cochlea) and the “balance component” (the semicircular canals and otolith organs) of the inner ear (labyrinth) itself. Labyrinthitis is sometimes also termed “vertigo with sudden hearing loss” (Pogson, Taylor et al. 2016, Kim, Choi et al. 2018) – and we will discuss sudden hearing loss further in a moment. Labyrinthitis usually manifests with severe dizziness (similar to vestibular neuritis) accompanied by ear symptoms on one side (typically hearing loss and tinnitus). -

Mikolai Jennifer Duffey, MS, CCC-A David Adlin Select Picture Audiometry The level at which a patient can understand spoken language can be a valuable screening tool, especially with young children. Select Picture Audiometry is a unique approach that can screen the hearing of children as young as three years old. Select Picture Audiometry can be used to determine the “speech reception level” of children and has been an accepted screening procedure in the clinical and school settings for over 20 years. Hearing disorders in children Hearing disorders entail different effects on children and adults. An adult may have sustained a mild hearing loss of 35 dBHL without being conscious of the disorder. That is because an adult has more experience with the redundancy (i.e., information abundance) of speech and is able to add non-heard parts of words or even sentences automatically and unconsciously. With children, particularly at the preschool age, a similarly mild hearing loss can be critical for further speech and language development. The capability of realizing the complicated rules of speech and transferring them to the child’s own development of speech can be highly restricted. 1 Reasons for a hearing loss can be: • malfunction of the outer or middle ear (conductive loss) • malfunction of the inner ear (sensory loss) • malfunction of the neural pathway (neural loss) Sensory and neural losses can be caused by many different factors, including congenital disorders, ototoxic medications, disease or infection, and exposure to excessively loud sounds. With children, the most widespread reason for problems with hearing is a loss caused by disorders of sound conduction. -

Description Central auditory processing disorder (CAPD), also known as auditory processing disorder (APD), refers to the efficiency and effectiveness by which the central nervous system (CNS) utilizes auditory information in the perceptual processing of auditory information. The diagnosis, management, and even the existence of an auditory-specific perceptual deficit are controversial. Policy/Criteria I. It is the policy of Health Net of California that diagnostic testing and therapy for the management of central auditory processing disorder are considered investigational due to lack of scientific evidence to support the validity of any diagnostic tests and treatment. Background According to the American Speech-Language Hearing Association (ASHA), central auditory processing disorder (CAPD), also known as auditory processing disorder (APD), refers to difficulties in the perceptual processing of auditory information in the CNS as demonstrated by poor performance in one or more of the skills noted above. CAPD It is a complex and heterogeneous group of auditory-specific disorders usually associated with a range of listening and learning deficits. Children or adults suspected of CAPD may exhibit a variety of listening and related complaints such as difficulty understanding speech in noisy environments, following directions, and discriminating (or telling the difference between) similar-sounding speech sounds. The child may have difficulty with spelling, reading, and understanding information presented verbally in a classroom. Some individuals may also have behavioral, emotional or social difficulties. -

DBA welcomes the opportunity to comment on the inquiry into Hearing Health and Wellbeing to enable Government to engage with deafblind people needs and their supports to ensure they are provided with the care necessary to support their health and wellbeing. ABOUT DEAFBLIND AUSTRALIA (DBA) DBA was established in 1993 at the National Deafblind Conference in Melbourne, Victoria. This council was established to provide: security; sense of belonging; freedom of speech; and represent the Australian deafblind community and their supporting networks. At present, ADBC represents an estimated 300,000 deafblind people including those with multi disabilities, their families and organisations working the deafblind field. SUBMISSION INTO HEARING HEALTH AND WELLBING AFFECTING DEAFBLIND PEOPLE Throughout this submission, the terms deafblind, combined vision and hearing impairment and dual sensory impairment will be used interchangeably as all three are used to describe people with deafblindness. Deafblindness is described by Deafblind Australia as: “a unique and isolating sensory disability resulting from the combination of both a hearing and vision loss or impairment which significantly affects communication, socialisation mobility and daily living. People with deafblindness form a very diverse group due to the varying degrees of their vision and hearing impairments plus possible additional disabilities. Deaf-Blindness; Rubella, CHARGE Syndrome, Usher’S Syndrome, Genetic Disorders, Accident and / Or Illness Are Some of the More Common Ones
Deafblindness Deafblindness is a combination of vision and hearing loss. Deafblindness encompasses a spectrum from mildly hard of hearing plus mildly visually impaired to totally deaf and blind. It is rare that an individual with deafblindness would be completely blind and completely deaf. Individuals who have a combined vision and hearing loss have unique communication, learning, and mobility challenges due to their dual sensory loss. Deafblindness is a unique and diverse condition due to the wide range of sensory capabilities, possible presence of additional disabilities, and the age of onset for the vision and hearing loss. A child with deafblindness would include the infant who has a diagnosis of Retinopathy of Prematurity (a retinal condition that is associated with premature birth) and has an acquired hearing loss due to meningitis at age two. Another person with deafblindness may have been born with a profound hearing loss and developed a later vision loss due to a genetic condition called retinitis pigmentosa. There are many causes of deaf-blindness; Rubella, CHARGE Syndrome, Usher’s Syndrome, genetic disorders, accident and / or illness are some of the more common ones. Deafblindness occurs in three of 100,000 births. In Colorado, just over 140 children and youth (ages birth through 21 years) have been identified as having both a vision and hearing loss. These individuals are eligible for free technical assistance through the Colorado Services for Children and Youth with Combined Vision and Hearing Loss Project, located at the Colorado Department of Education. This project provides technical assistance which supports Colorado children and youth, birth to 21 years, who have BOTH a vision and hearing impairment.
