To get a complete description of your prescription coverage, including how to fill your prescriptions, please review the Evidence of Coverage and Aetna Medicare Rx plan’s formulary. We call the pharmacies on this list our “network pharmacies” because we have made arrangements with them to provide prescription drugs to Plan members. A network pharmacy is a pharmacy where beneficiaries obtain prescription drug benefits provided by Aetna Medicare Rx plan. In most cases, your prescriptions are covered under Aetna Medicare Rx plan only if they are filled at a network pharmacy or through our mail order pharmacy service. Once you go to one, you are not required to continue going to the same pharmacy to fill your prescription. You can go to any of our network pharmacies. We will fill prescriptions at non-network pharmacies under certain circumstances as described in your Evidence of Coverage. For Aetna Medicare Rx individual plans shown in the table below (does not apply to employer group plans): Preferred pharmacies are pharmacies in our network in which our plan has negotiated lower cost-sharing for its plan members for covered prescription drugs than at other network pharmacies. However, you will still have access to lower drug prices at these other network pharmacies than at out-of- network pharmacies. You may go to either of these types of network pharmacies to receive your covered prescription drugs. If you are an Aetna Medicare Rx individual plan member who resides in one of the states listed below, please refer to the table for your preferred pharmacy benefit.
Connecticut Food Bank ic of ou ting the fabr r communi onnec ty C 2013–2014 ANNUAL REPORT Message from our former President Dear Friends, 2014 was a milestone year in the 32-year history of Connecticut Food Bank. In June we broke ground for a new distribution center in Wallingford, CT, and shortly after I announced my decision to retire at the end of the year. Many of you have been working alongside me in the fight against hunger for many years. When we first started, a few hundred thousand pounds of food were being distributed to 70 food pantries, soup kitchens and shelters annually. With your help, Connecticut Food Bank has grown to distribute more than 20 million pounds of food last year to nearly 700 food-assistance programs throughout Connecticut. I want you to know that Connecticut Food Bank is positioned well into the future. Once the new distribution center is open and operational, it will enable Connecticut Food Bank to better serve hungry families. With greater efficiencies, the larger facility and increased refrigerated space, Connecticut Food Bank can accept and distribute more fresh food to people in need, add new programs and enhance current ones, increase partnerships with local growers and food manufacturers, and accommodate more volunteers. The number of meals distributed will double over the next 10 years. Food banking has been my passion for the last 30 years, and my decision to retire at the end of the year was not an easy one. But you can be assured that my successor, Rob Levine, a very experienced and knowledgeable leader, will ensure the continued success of Connecticut Food Bank’s mission as the organization prepares to move into a new distribution center in 2015.

This directory is for: CONNECTICUT, MASSACHUSETTS, RHODE ISLAND, VERMONT 2009 Pharmacy Directory United UA Medicare Part D American UA Medicare Part D Silver Insurance Company UA Medicare Group Part D This booklet provides a list of network pharmacies. All network pharmacies may not be listed in this directory. Pharmacies may have been added or removed from the list after this directory was printed. To get current information about network pharmacies in your area, or if you need further information, contact Customer Service at the number listed on the back of your Part D member ID card. Introduction To get a complete description of your prescription coverage, including how to fill your prescriptions, please review the Evidence of Coverage and your plan Formulary. If you have additional questions, please call Customer Service at the number listed on the back of your Part D member ID card. We call the pharmacies on this list our “network pharmacies” because we have made arrangements with them to provide prescription drugs to Plan members. A network pharmacy is a pharmacy where beneficiaries obtain prescription drug benefits provided by your Medicare Prescription Drug Plan. In most cases, your prescriptions are covered under your Plan only if they are filled at a network pharmacy or through our mail order pharmacy service. Once you go to one, you are not required to continue going to the same pharmacy to fill your prescription. You can go to any of our network pharmacies. We will fill prescriptions at non- network pharmacies under certain circumstances as described in your Evidence of Coverage.
