A la suite des recommandations de l'OMS dans les annees 70s, des efforts ont ete deployes partout dans le monde pour encourager la population a utiliser preferen- tiellement les services de premiere ligne plutot que les urgences des hopitaux et les ser­ vices specialises. Plus particulierement pour certaines minorites visibles et les person- nes Sgees, la porte d'entree dans le systeme de sante est encore bien souvent celle des urgences des hopitaux. Depuis une decennie, des recherches se multiplient au Canada, plus particulierement au Quebec, sur cette problematique de l'acces aux soins de sante. Les utilisateurs de dro­ gues injectables (UDIs) constituent notamment une categorie a risque dont le mode de recours aux soins de sante est mal connu. Selon les ecrits anterieurs, les UDIs utilise- raient les services d'urgence dans une proportion plus elevee par rapport au reste de la population. L'objectif de cette etude sera done d'examiner le profil d'utilisation des services de soins par les UDIs de Montreal et ressortir d'une part les facteurs qui les predisposent a une utilisation preferentielle des services d'urgence, et d'autre part les barrieres a leur utilisation des services de soins de premiere ligne. La tentative de repondre a ces deux objectifs a souleve Pimperatif de decrire minutieusement le profil des UDIs et d'etudier au prealable les facteurs associes a leur morbidite elevee. Cette etude est de type transversal. La population cible se compose des UDIs de Mon- iv treal. Pour etre inclus dans l'&ude, le sujet devait s'Stre injecte une drogue illicite au moins une fois dans les six dernier mois, 6tre age de 18 ans ou plus et habiter la ville de Montreal depuis au moins 12 mois. L'&hantillonnage s'est deroule entre fevrier et sep- tembre 2005. La methode de recrutement a consists" a contacter les UDIs sur les rues de Montreal, les informer du deroulement et des objectifs de l'&ude et distribuer des car­ tes d'affaires contenant un numero de telephone qu'ils pouvaient composer au cas ou ils voulaient participer a l'etude. L'instrument de mesure etait un questionnaire auto- administre. Au total 678 sujets ont &e interroges, et parmi eux 666 ont ete retenus pour les analyses finales. Les analyses effectuees ont ressorti d'une part les liens entre les conditions de vie des UDIs et leur etat de sante, et d'autre part les facteurs associes a Putilisation des services d'urgence pendant un episode de maladie, de meme que, pour ceux qui visitent habi- tuellement les services de premiere ligne, les facteurs qui determinent leur choix entre les Centres Locaux des Services Communautaires (CLSC) et les cabinets medicaux. Nos resultats montrent que la sante des UDIS est principalement influencee par les ma­ ladies mentales [rapport de cote (RC) : 2,09; intervalle de confiance (IC) a 95% : 1,15- 3,81], les infections a virus de Wmmunodeficience humaine (VIH) (RC : 2,03; IC a 95% : 1,00-4,14) et les infections a virus de l'hepatite C (VHC) (RC : 1,84; IC a 95% : 1,07-3,15) qui affectent nombre d'entre eux. Les autres facteurs qui expliquent la mor­ bidity chez les UDIs sont les comportements a risque (types de drogues consommees par injection, partage de materiel d'injection), les facteurs de marginalite (homosexualite, bisexualit^, mendicite, prostitution), les contraintes financieres, le sexe et l'age. Les UDIs sans-abris (RC : 4,81; IC a 95% : 1,18-19,60), de meme que ceux V qui abusent d'alcool (RC : 2,79; IC a 95% : 1,08-7,22) et de drogues non injectables (RC : 2,54; IC a 95% : 0,95-6,80), auront plus tendance a consulter dans les services d'urgence des hopitaux. Parmi ceux qui vont aux services de premiere ligne, les plus demunis, notamment les sans-abri, preierent les CLSC (RC : 6,23; IC a 95% : 1,53- 25,32) tandis que ceux qui sont plus integres, notamment les UDIs plus ages (RC : 0,89; IC a 95% : 0,82-0,97), preferent les cabinets medicaux. Les services preventifs et sociaux ofFerts par le systeme de sante de Montreal semblent bien cibler les personnes a risque. Une proportion de 74% des participants a utilise un service de soins preventifs quelconque, et 49% des sujets recourent a un centre communautaire pour recevoir de l'aide. En conclusion, la morbidity elevee des UDIs de Montreal semble expliquee principale- ment par leurs conditions de vie difficiles. Globalement, ils utilisent les services d'ur­ gence plus frequemment que la population generale, etant donne les nombreux facteurs de risque de maladies aiguSs auxquels ils sont confronted. Une exception a cette vue generale est celle des UDIs plus socialement integres, qui semblent utiliser plutot les services de premiere ligne, comme la population generale. Les resultats de cette etude nous amenent a recommander une plus grande integration du systeme socio-sanitaire du Quebec, et une consolidation des services sociaux en fa- veur des UDIs. Following rec­ ommendations from the WHO in 70s, many efforts have been deployed everywhere in the world to invite the population at preferentially using front line services instead of hospital emergency rooms and specialized services. For certain minorities and elderly in particular, hospital emergency rooms are still, most of the time, the main entry point to the health care system. During the last decade, many studies have been carried out in Canada, more specifi­ cally in the Quebec, on the access to health care services for minority populations. Among them, injection drug users (IDUs) represent a category at risk for diverse dis­ eases and their pattern of health service use is quite unexplored. The purpose of this study is to describe the health care service utilization profile in Montreal IDUs and assess barriers to their primary health care service use, along with other factors that could explain their preferential utilization of hospital emergency ser­ vices. Our attempts to address those two objectives have raised the requirement to cau­ tiously describe IDIs' profile and, as a preliminary step, to explore factors associated with their high morbidity. This work is a cross-sectional study. The targeted population is composed by IDUs in Montreal. Inclusion criteria are: having injected illicit drugs within the 6 months before interview, being 18 years old or over, and living in Montreal since 12 months or more before interview. The sampling was carried out from February to September 2005.

Jensen School of Education, University of California, Berkeley, Calif. 94720 Abstract: Although the black and white populations in the United States differ, on average, by about one standard deviation (equivalent to 15 IQ points) on current IQ tests, they differ by various amounts on different tests. The present study examines the nature of the highly variable black-white difference across diverse tests and indicates the major systematic source of this between- population variation, namely, Spearman's g. Charles Spearman originally suggested in 1927 that the varying magnitude of the mean difference between black and white populations on a variety of mental tests is directly related to the size of the test's loading on g, the general factor common to all complex tests of mental ability. Eleven large-scale studies, each comprising anywhere from 6 to 13 diverse tests, show a significant and substantial correlation between tests' g loadings and the mean black-white difference (expressed in standard score units) on the various tests. Hence, in accord with Spearman's hypothesis, the average black-white difference on diverse mental tests may be interpreted as chiefly a difference in g, rather than as a difference in the more specific sources of test spore variance associated with any particular informational content, scholastic knowledge, specific acquired skill, or type of test. The results of recent chronometric studies of relatively simple cognitive tasks suggest that the g factor is related, at least in part, to the speed and efficiency of certain basic information-processing capacities.
