I am applying for and accepting a Blackberry phone through Local 110, on condition that, if I have been unemployed for three months or longer at the time of application, the union will assess me an amount of money equal to what the union is billed per month thereafter by Telus, for so long as I have agreed to have that phone, unless and until I return to work and union dues are deducted for any work done in a given month, which will then prompt the union to commence paying for the monthly cost of the Blackberry phone for that month and ongoing. 3. If so you are eligible for new Blackberry handset upgrade and the new rate plan, even if you are in an existing TELUS contract. Existing TELUS subscriber must comply with or provide the following: • No out standing balance and the account is in good standing • The name on the airtime account must be in the Union members name, no spousal, business or parental account • The account can not have any other subscriber on the account, only the union member can be part on the account • Complete a Blackberry Request Form • Completed and signed Order Form • Upon delivery, sign a contract offer redemption Forms are available at the union office, union website or from TELUS. Please ensure that your account is updated to fit the above eligibility criteria prior to requesting to take part in the Union 110 deal. After a member is transferred to the Union 110 offering an enhanced billing support team will handle all billing inquiries. Delivery Delivery of your chosen device will be made on pre-determined days to the union office or other predetermined location (union office, Fort McMurray, Calgary). TELUS and the Union need to complete a number of activities with every member to provide you with a working device, message delivery service and training. When you complete and submit your paperwork TELUS will contact you to select a day to deliver your new Blackberry. TELUS requires 3 to 8 business days to prepare your order. All paperwork must be completed and submitted to TELUS prior to scheduling your delivery date.
