Dear Parents and/or Guardians,
As part of the Kindergarten registration process and in advance of the start of the next school year, we want to be sure you are aware of the following health services requirements:
• Your child's/children’s immunizations must be up-to-date in order for him/her to start school. As required by the New Jersey Department of Health and Senior Services, documentation of immunizations must be submitted prior to the first day of school. A student will be refused entry until immunization requirements are met. Exceptions may be granted only for religious beliefs or medical recommendations.
• All students entering Kindergarten in the State of New Jersey must have documentation of a completed physical examination. The physical should include hearing and vision exams, which can be completed by your physician.
Please keep your school nurse informed of changes in immunizations or health status during the course of the school year.
If you have any questions, please contact your physician or your elementary school nurse. We appreciate your support in helping us establish a safe and healthy school environment for your child.
