OPLENARIA, CESO V, ~ hoofs iv, ion Superintendent / SUBJ ECT CONDUCT OF THE EVALUATION AND FEEDBACK ONLINE SURVEY ON TH E SCHOOL- BASED FEEDING PROGRAM (SBFP) DATE May 7, 2021 1. The Bu reau of Learning Support .Services through the Sch ool Health Section of the Divis ion of Lanae del Norte shall conduct an Evalu ation And Fe edback Survey on the School-Based Feeding Program (SBFP) which aims to gather feedback from the parents and or guardians of the beneficiaries and the field implementers on the implementation of SBFP for the SY 2020-202 1 and their perceived benefits to the program recepients. 2. Forty two SBFP recepient schools from poth regions I and II will be required to participate in the su rvey and two sets of questionnair~s will be administered, one intended for the parents and/ or guardians, and another one for the school feeding coordinators of recipient schools . These surveys are thru Google Forms and can be accessed th ru the following links: i. For parents/ guardians: ii. For the School Feeding Coordinators 3. In line with this, there will be a virtual conference that will be held on May n. 2021 via Google Meet from 8:30AM to 3:00PM. The participants to this activity are the District Nurses and School Feeding Coordinators to: a. Present the Evaluation and Feedback Survey Questionnaires b.
