I agree to abide by the specific terms and conditions of Connect Toy and iPad loan scheme as follows:

1. I confirm that I am a member of Connect and am a parent/carer of a child/young person (0-25 years) who is on SEN support and/or who has an EHC plan and/or a disability.

2. All items borrowed remain the property of Connect.

3. I agree to treat the items borrowed with due care and keep them in good condition.

4. Should any faults occur, I agree to return the item as soon as possible so that any necessary repairs can be undertaken.

5. Items may be borrowed for up to half a school term, though an extension may be granted by the library facilitator.

6. I will return the equipment/toy to the Library Facilitator at either The Bridge Centre or Carmel College.

7. Connect reserves the right to charge parents/carers for overdue, lost or damaged items.
