299 12 Retina Gabriele K. Lang and Gerhard K. Lang 12.1 Basic Knowledge The retina is the innermost of three successive layers of the globe. It comprises two parts: ❖ A photoreceptive part (pars optica retinae), comprising the first nine of the 10 layers listed below. ❖ A nonreceptive part (pars caeca retinae) forming the epithelium of the cil- iary body and iris. The pars optica retinae merges with the pars ceca retinae at the ora serrata. Embryology: The retina develops from a diverticulum of the forebrain (proen- cephalon). Optic vesicles develop which then invaginate to form a double- walled bowl, the optic cup. The outer wall becomes the pigment epithelium, and the inner wall later differentiates into the nine layers of the retina. The retina remains linked to the forebrain throughout life through a structure known as the retinohypothalamic tract. Thickness of the retina (Fig. 12.1) Layers of the retina: Moving inward along the path of incident light, the individual layers of the retina are as follows (Fig. 12.2): 1. Inner limiting membrane (glial cell fibers separating the retina from the vitreous body). 2. Layer of optic nerve fibers (axons of the third neuron). 3. Layer of ganglion cells (cell nuclei of the multipolar ganglion cells of the third neuron; “data acquisition system”). 4. Inner plexiform layer (synapses between the axons of the second neuron and dendrites of the third neuron). 5. Inner nuclear layer (cell nuclei of the bipolar nerve cells of the second neuron, horizontal cells, and amacrine cells). 6. Outer plexiform layer (synapses between the axons of the first neuron and dendrites of the second neuron). -

Older adults are at a higher risk of developing dry eye disease as well as needing for ophthalmic surgery. Anterior segment surgery may induce or worsen existing dry eye symptoms usually for a short-term period. Despite good visual outcomes, ocular surface dysfunction can significantly affect quality of life and, therefore, lower a patient’s satisfaction with ophthalmic surgery. Preoperative dry eye disease, factors during surgery and postoperative treatment may all contribute to ocular surface dysfunction and its severity. We reviewed relevant articles from 2010 through to 2021 using keywords “cataract surgery”, ”phacoemulsification”, ”refractive surgery”, ”trabeculectomy”, ”vitrectomy” in combina- tion with ”ocular surface dysfunction”, “dry eye disease”, and analyzed studies on dry eye disease pathophysiology and the impact of anterior segment surgery on the ocular surface. Anesthesia for Eye Surgery
Anesthesia for Eye Surgery Having a surgery can be stressful. We would like to provide you the following information to help you prepare for your eye surgery. Eye surgeries are typically done under topical or local anesthesia with or without mild sedation. Topical Anesthesia This is administered via special eye drops by a preoperative nurse. Local Anesthesia This is administered via injection by an ophthalmologist. Your ophthalmologist will inject local anesthesia to the eye that is to undergo the operation. During the injection, you will be lightly sedated. In the operating room During the procedure it is very important that you remain still. This is a very delicate surgery; any abrupt movement can hinder a surgeon’s performance. If you have any concerns during the surgery, such as pain, urge to cough or itching, please let us know immediately. Typically, patients are not heavily sedated for this type of procedure. Therefore, it is normal for patients to feel pressure around their eyes, but not pain. In order to maintain surgical sterility, your face and body will be covered with a sterile drape. You will be given supplemental oxygen to breathe. An anesthesia clinician will continue to monitor your vital signs for the duration of the procedure. The length of the procedure usually ranges from 10 minutes to 30 minutes. In the recovery room After surgery, you will be brought to the recovery room. A recovery room nurse will continue to monitor your vital signs for the next 20 to 30 minutes. It is important that you do not drive or operate any machinery for the next 24 hours. -
Chalazion Treatment
Chalazion Treatment This material will help you understand treatments for chalazion. What is a chalazion? A chalazion is a red, tender lump in the eyelid. It is also known as a stye. The swelling occurs because one of the oil glands that is next to each eyelash can get backed up and become inflamed. This is very similar to a pimple. How is a chalazion treated? In many cases, chalazia resolve on their own without treatment. Applying a warm compress over your eye for 5- 10 minutes two to four times a day can soften the oil that is backed up. This helps the chalazion heal. If the chalazion does not heal after one month of using warm compresses, your doctor may suggest surgical removal or injection with medications to help it heal faster. How is a chalazion surgically removed? Surgical removal of a chalazion is an outpatient procedure. Before the procedure, your doctor will give you a local anesthetic to numb the area around the chalazion. Next, your doctor will place a clamp to help hold your eyelid in place for the procedure. That way, you will not need to worry about keeping your eyelid open for the procedure. The doctor will then make a small incision in the eyelid and remove the chalazion with a special instrument. The location of the incision (front or back of the eyelid) depends on the size of the chalazion. Small chalazia can be removed by making an incision on the inside of the eyelid. If your chalazion is large, the doctor may make an incision on the front of the eyelid and close it with dissolvable stitches. -

BOSTON TERRIER EYE DISEASE Corneal Ulcers and Prevention Corneal Ulcers are the single largest eye problem in Boston Terriers. Perhaps 1 dog in 10 will experience a corneal ulcer sometime during its life based on the l903 dogs surveyed in the 2000 Boston Terrier Health Survey. The Boston Terrier Standard for the Breed calls for eyes to be “wide apart, large and round and dark in color. The eyes are set square in the skull and the outside corners are on a line with the cheeks as viewed from the front". The ideal Boston Terrier eye does not protrude but is "set square in the skull". Unfortunately the Boston eye is fairly prone to eye injury because of its large size and prominence. Corneal ulcers are caused initially by injury to the eyes. The common practice of removing Boston Terrier whiskers may be a reason that eyes become injured due to lack of sensory feelers. Some breeders do not trim whiskers once a dog's show career is finished because they know that whiskers can prevent injury to the eye. There are a number of external reasons why an injured eye doesn't heal. These may include irritation from eyelashes or from facial hairs, infection, and lack of moisture in the eye. Some of these reasons are hereditary. Internal reasons for an eye not healing include glaucoma and infection. Corneal ulcers can be difficult and expensive to treat and often result in the loss of the eye. This is a case where an "ounce of prevention is worth a pound of cure".
