This report, which is the fourth and final to document OIG’s findings in these areas,3 addresses efforts undertaken by the Department of State (Department) to preserve and secure electronic records and communications involving the Office of the Secretary. Specifically, this report (1) provides an overview of laws, regulations, and policies related to the management of email records; (2) assesses the effectiveness of electronic records management practices involving the Office of the Secretary; (3) evaluates staff compliance with records management requirements; and (4) examines information security requirements related to the use of non-Departmental systems. As part of the current evaluation, OIG reviewed laws, policies, and practices from (and, in some cases, prior to) 1997 through the present, covering the tenures of five Secretaries: Madeleine Albright (January 23, 1997–January 20, 2001); Colin Powell (January 20, 2001–January 26, 2005); Condoleezza Rice (January 26, 2005–January 20, 2009); Hillary Clinton (January 21, 2009– February 1, 2013); and John Kerry (February 1, 2013–Present). OIG reviewed the requirements of the Federal Records Act4 and the Federal Information Security Management Act (FISMA)5 and related regulations; circulars and directives issued by the President, the National Archives and Records Administration (NARA), the National Institute of Standards and Technology (NIST), and the Office of Management and Budget (OMB); applicable 1 OIG has identified the following issues: inconsistencies across the Department in identifying and preserving records, hacking incidents and other issues affecting the security of Department electronic communication, delays and other processing problems related to FOIA requests, and concerns about an Ambassador’s use of private email to conduct official business.     OFS REPORT TO CONGRESS FRONT MATTER OPERATION FREEDOM’S SENTINEL LEAD INSPECTOR GENERAL REPORT TO THE UNITED STATES CONGRESS APRIL 1, 2021–JUNE 30, 2021 FRONT MATTER ABOUT THIS REPORT A 2013 amendment to the Inspector General Act established the Lead Inspector General (Lead IG) framework for oversight of overseas contingency operations and requires that the Lead IG submit quarterly reports to Congress on each active operation. The Chair of the Council of Inspectors General for Integrity and Efficiency designated the DoD Inspector General (IG) as the Lead IG for Operation Freedom’s Sentinel (OFS). The DoS IG is the Associate IG for the operation. The USAID IG participates in oversight of the operation. The Offices of Inspector General (OIG) of the DoD, the DoS, and USAID are referred to in this report as the Lead IG agencies. Other partner agencies also contribute to oversight of OFS. The Lead IG agencies collectively carry out the Lead IG statutory responsibilities to: • Develop a joint strategic plan to conduct comprehensive oversight of the operation. • Ensure independent and effective oversight of programs and operations of the U.S. Government in support of the operation through either joint or individual audits, inspections, investigations, and evaluations. • Report quarterly to Congress and the public on the operation and activities of the Lead IG agencies. METHODOLOGY To produce this quarterly report, the Lead IG agencies submit requests for information to the DoD, the DoS, USAID, and other Federal agencies about OFS and related programs. The Lead IG agencies also gather data and information from other sources, including official documents, congressional testimony, policy research organizations, press conferences, think tanks, and media reports.

In this inaugural publication, we highlight some of the success stories we discovered while conducting the inspections of 43 embassies and bureaus over the past two fiscal years. Part of our mission is to increase collaboration to maximize the benefits of OIG’s work. In highlighting successful projects and processes throughout the world, we encourage others to adopt practices that have already been Matthew S. Klimow tested and have enjoyed positive results. We began to highlight innovative practices in 2014 but it was not until FY 2017 that we formally started calling them “Spotlights on Success” and included them in more inspection reports on a consistent basis. By highlighting those things that have worked well in the Department, we seek to increase good governance, efficiency and encourage positive change. This publication includes 38 Spotlights on Success from five different regional bureaus, and four domestic operations. Several embassies had multiple entries, with Embassy Nairobi being credited with six Spotlights on Success. The spotlights are categorized by topics reviewed and presented in OIG’s inspection reports. Additionally, several of the success stories fall under management challenge categories that are identified in a companion publication, Inspector General Statement on the Department of State’s Major Management and Performance Standards Fiscal Year 2020.
