The major role of the Office of Student and Diversity Affairs (OSDA) is to assist you in the curricular and extracurricular aspects of your edu- ca on. As you all know, Au- gust 23, 2021 (the first official day of D1 classes) is just around the corner, and the beginning of your professional educa on and career will be here before you know it!

Financial Aid
Please be reminded that if you are planning on financing your educa on with loans, you should file a 2021‐2022 (Free Applica on for Federal Student Aid (FAFSA) form. This should be done as soon as possible as it opened in Octo‐ ber. If you have not yet done this already, please do so immediately! UIC's Title IV Code is 001776. Orienta on and Virtual Summer Mee ng
Orienta on will be held from Monday, August 16– Friday, August 20, 2021; due to the ongo- ing COVID pandemic, it has not yet been determined if orienta on will be held in person or virtually. Please note that orienta on is mandatory for all incoming DMD students; there are no excep ons to this requirement. If a student fails to be present for the first day of orien- ta on, they will be automa cally withdrawn from the program. White Coat Ceremony
The White Coat Ceremony is an annual tradi on marking the matricula ng class of students entering the dental profession. It celebrates the choice to pursue a life in service to the health of society. During the Ceremony, University of Illinois at Chicago College of Den stry deans and faculty members cloak students in their first white coat as a symbol of the trust bestowed upon them to carry on the healing profession on den stry. Due to COVID‐19 re‐ stric ons, the University is not allowing large events such as COD’s White Coat Ceremony to take place. Our office will no fy you once we are allowed to host the ceremony for your class.

Transcripts, Registra on, and Supplemental
Applica on
Transcripts As you are aware, our dental school requires proof of a US earned Bachelor’s and/or Mas‐ ter’s degree as reported on your AADSAS applica on. All newly admi ed D1 students should submit an official copy of all transcripts . We cannot obtain copies from AADSAS. The transcripts must have the degree posted. Students should also submit master’s degree tran‐ scripts and any other college or university transcript that posts prerequisite coursework or science coursework for dental school. All transcripts with degrees posted should be in our office no later than Wednesday, June 30, 2021. HOUSING — Off-Campus Living
Off-Campus Housing Based on past dental student classes, we have found that many students choose to live off‐ campus. If you decide to live off‐campus, we recommend that you research the neighborhood in which you are interested in. Another great resource is the UIC Off‐campus Housing database system. This site is for the use of UIC students, staff and faculty, as well as landlords who wish to lease property to UIC stu‐ dents. h ps://offcampushousing.uic.edu/

City Living—If you are interested in living close to the college consider doing a search for these areas: Li le Italy, University Village, UIC/Medical District/Tri-Taylor, West Loop and South Loop. A word of advice: If finances are a concern, we recommend that you do not rush into signing a housing contract (lease) un l you are aware of all your housing op ons. Calculate carefully what you can realis cally afford before making your decision. Be aware that hea ng costs may not be included in your rent and may be a separate expense. HOUSING — On-Campus Living
On-Campus Housing UIC has both an on‐campus and off‐campus Housing office located on the second floor of the Student Residence Commons, 700 S. Halsted Street, Room 2026. You may visit the UIC Hous‐ ing department website h p://www.housing.uic.edu, to obtain addi onal informa on.
The office provides housing informa on and offers many services that you may find helpful in making your living arrangements. The College of Den stry is located on the west‐side of UIC’s campus. The residence hall for graduate health professional students is located on the medi‐ cal campus, 3 blocks west of the dental school. This facility is called Single Student Residence (SSR);
Single Student Residence (SSR)—This building is mainly for graduate and professional stu‐ dents, as well as for undergraduate students 24 years of age or older. This building is apart‐ ment style, with each student having an individual bedroom and sharing a kitchene e and bathroom within each unit. Each bedroom has a high speed and cable tv connec on (must be ac vated; addi onal fees may apply). As this building is for graduate and professional stu‐ dents, the en re building is intensive study.

Background Check, Immuniza on Forms, and Bloodborne Pathogen Policy
Background Check
The University of Illinois at Chicago (UIC) College of Den stry requires that all students com‐ plete a background check prior to matricula on into the Doctor of Dental Medicine (DMD) Program. Informa on regarding the College’s policy on background checks has been a ached to this email. To facilitate this process, the College has iden fied an independent vendor, Castlebranch, to provide the required background screening. A document providing direc on on how to proceed with the background check accompanies this newsle er. The deadline to complete the background check is Friday, March 26, 2021.
Immuniza on Forms
Included in the website resources, you will find informa on about the University of Illinois at Chicago College of Den stry Immuniza on policy, Frequently Asked Ques ons and a Castle‐ branch instruc on sheet (Instruc ons include both background check and immuniza on infor‐ ma on). Please have your healthcare provider complete the Immuniza on form NOW and ob‐ tain the appropriate lab tests, as some vaccina on series take months to complete. Once your forms and labs are complete, you will upload them into a secure web‐based program through the Castlebranch Immuniza on Tracker. This will review your records and inform you if any‐ thing is missing. The deadline to upload all forms and reach compliance is Monday, July 5, 2021. Informa on will be updated by Clinical Affairs as provided.
Bloodborne Pathogen Policy The University of Illinois College of Den stry is commi ed to providing a safe and healthful clinic environment. In pursuit of this goal, an exposure control plan (ECP) is implemented to eliminate or minimize occupa onal exposure to bloodborne pathogens in accordance with OSHA standard 29 CFR 1910.1030, “Occupa onal Exposure to Bloodborne Pathogens.” The University for Illinois at Chicago College of Den stry has a Blood Pathogen Policy found on the website under Student Resources.

Collection of Teeth
As part of the first year fall and spring semester dental curriculum, you will take courses en ‐ tled Den stry‐Applied Oral and Behavioral Sciences (DAOB). In these courses, you will learn about the dental anatomy and prac ce preparing teeth for restora on. The laboratory and pre‐clinical por on of these and other subsequent courses depend heavily on a collec on of extracted human teeth.
Again, in your D‐2 year you will learn root canal therapy mainly by performing root canal pro‐ cedures on extracted human teeth that you collect. You will need many average size, intact, hydrated, (not dry/bri le) teeth of all types: maxillary and mandibular molars, premolars and anterior teeth (restored teeth are acceptable).
It is important that you begin collec ng teeth now! Start with your family den st. Ask your orthodon st about other den sts in your area who extract teeth (oral surgeons can be help‐ ful). It is very important that you have a wide variety of teeth. All these den sts were once students too and learned their skills using extracted teeth they collected from prac cing den‐ sts, so they fully understand. The extracted teeth may contain restora ons but should other‐ wise be of good quality (not severely broken down by decay or damaged during extrac on), and must be hydrated. You should plan to collect teeth from several offices in order to obtain enough teeth for your dental educa on. You should collect at least 100 usable teeth in disin‐ fec on solu on (and many more teeth are strongly recommended) by the me you matric‐ ulate. You should con nue to collect teeth a er matricula on to be prepared for future courses. Non‐third molars are especially in demand. A ached you will find a le er you may copy and provide to den sts you approach with storage instruc ons.

A personal laptop computer and an iPad ARE REQUIRED for your dental school educa on. For general communica on, homework and network access, you will need a personal com‐ puter in order to be able to review the assigned weekly course requirements, using special course so ware. UIC I-Card & Campus Care
MyPhotoSubmit Upload for UID Card In order to remain compliant with COVID‐19 distancing regula ons, students and employees are now allowed to upload their own photo for their I‐Card using the MyPhotoSubmit portal. To submit photos, please visit h ps://www.icardnet.uillinois.edu/public/photo-submit.cfm.
ID Cards Typically, once a student registers for classes, they can obtain an “I‐card” which is the free UIC ID card. Please wait 48– 72 hours for eligibility following registra on. Once a student submits their photo for their I‐card via MyPhotoSubmit, COD will collect their ID from the UIC ID office and provide with equipment distribu on during orienta on week. UIC and UIUC students: If you will be exchanging your I-Card, please be sure to do so via the East campus office or a re- placement fee of $20 will be charged. I‐Cards are not printed on demand, likewise, same day service using MyPhotoSubmit is not available. Please refer to the website for hours and ad‐ dress informa on. Campus Care The University of Illinois at Chicago requires that all eligible students be covered by health in‐ surance and provides a plan for which the fee is automa cally assessed along with other tui‐ on and fees. Eligible students include all registered Undergraduate, Graduate, and Health Professional students. There is no enrollment form to complete. Students with comparable health insurance coverage, for example through a legal guardian or an employer, can waive out of the program by submi ng an online waiver form. Lockers During orienta on, students will be assigned lockers that will be used for different ac vi es at different mes during the program. It is required that you bring locks for these lockers that are easy to use. Combina on locks or with easy to use numbers, or master locks with a key are recommended. Please bring two (2) locks with you at orienta on. (No mini locks please).

UIC Col‐ lege of Den stry DMD/AS Policy: Students who have matriculated in August of 2018 and beyond are required to own one pair of navy scrubs with the embroidered UIC COD logo; these scrubs will be referred to as your navy dress scrubs. These scrubs are available for purchase at the UIC Medical Bookstore. Cohorts may be requested to wear them on oc‐ casion. Unless no fied in advance, students are permi ed to wear navy scrubs from the vendor of their choice, the UIC COD logo is op onal for these scrubs (not required).
All students are required to wear scrubs while in the building. Scrubs should be clean; top and bo om should be of a matching color. To buy your UIC COD navy dress scrubs you have to visit the UIC Bookstore located at
