INSTRUCTIONS FOR COMPLETING THE APPLICATION

1. Complete the attached cover sheet. 2. Applicant must complete, date, and sign the official Zonta application form. Applications and statements must be typed and signed in all instances. 3. Recommendation Form must be completed. In addition, each reference must provide a letter of reference. The reference letter must be on one side of an 8.5 x 11 paper, dated, and signed by the author. 4. An official transcript, certified by the school, of the applicant’s school records from that institution from the beginning of their freshman year to the date of the application. 5. The materials MUST be arranged in the order described below: A. Official Zonta Cover Sheet B. Official Zonta Application, Carefully plan a response before typing final draft. C. Statement of goals essay. The applicant shall prepare a statement of more than 250 and less than 300 words setting forth her vocational or professional goals. The essay will reflect on how past, present and future activities make the accomplishment of these goals probable. The applicant, by deed and circumstances, must demonstrate her worthiness. Excess verbiage will detract from the statement. D. Recommendation forms and letters of reference. Include current letters of recommendation from two persons in authority from the high school attended by the applicant. The letters must be on official school letterhead, signed and dated by the author, and include the title of the author. The letters may cover the applicant’s ability, work habits, leadership, personality, and integrity. E. Recommendation forms and letters of reference. Include current letters of recommendation from two responsible non-school persons. These references shall not be related to the applicant and should be capable of reporting the applicant’s participation in non-school activities in terms of work, service, leadership, notable skills, and outstanding recognition. It is helpful to know how the writer came to know the applicant. F. Documentation of Student involvement and extracurricular activities. Provide documentation of social involvement for the betterment of the community. Provide documentation of involvement in academic clubs and extracurricular activities. 6. The decision to grant or deny an award by Zonta Club of Roscommon County Area is final. All application materials remain the property of the Zonta Club of Roscommon County Area 7.
