Strength Training Course Contract

Directions: Read through this contract with your parent, guardian, or supervising adult. Fill in the appropriate information, “sign” your name by typing it where indicated, have your parent/guardian “sign” as well, and submit to your instructor.

I, ______(first and last name), enter into this contract on the ______day of ______(month) ______(year). In taking Strength Training, I agree to the following conditions.

1. I must complete three hours of exercise per week for at least twelve weeks.

2. I must have a minimum of 90 minutes of strength training, accumulated over at least 2 different days.

3. I must include 60 minutes of cardio as well as stretching or flexibility training in my weekly workouts, as outlined under the exercise requirements.

4. I understand that with any physical or medical limitations, I should follow the recommendations of my doctor, and discuss these recommendations with my instructor.

5. I understand that I am encouraged to seek medical approval before beginning any type of exercise program.

6. I understand that if I submit my log more than one week late, I will not receive credit for it!

7. I have viewed and printed a copy of the schedule. I will refer to the schedule often so that I can be prepared to submit my assignments on time.

8. I will not falsify the time or activities posted on my Strength and Fitness Log in any way, as this is cheating. I understand that if I miss a week of activity, I can make it up with an additional week if there is time before the end date of the course.

9. I will not copy or plagiarize information from the internet, textbooks, other students, or any other resources in order to complete my assignments.

10. I understand that cheating and plagiarism are serious offenses that can have serious consequences.

11. I will be courteous and respectful to my instructor and other class members, particularly when participating in class discussions.

12. I understand that every week I must submit the following items for a grade:  Team Huddle (Class Discussion)  Assignment  Quiz  Strength & Fitness Log

1 © Carone Fitness Strength Training 13. I understand that every few weeks, in accordance with the posted schedule, my parent or guardian must submit a fitness verification to verify my Strength & Fitness Logs.

14. I understand that my school or instructor may contact my parent or guardian at any time if there are any concerns about my progress or to further verify my physical activity.

15. I understand that I will receive zeros for any assignments that are not completed throughout the semester.

16. I understand that it is my responsibility to seek clarification. If I do not fully understand a lesson, assignment, or directions, I should contact my instructor for further information.
