GCSE/GCE Exam application Form: Requirements: 1) Copy of passport/ drivers licence (for identification). 2) 4 large stamped, self-addressed envelopes (to despatch your examination documentation, statement of entry, statement of results and your certificate). 3) A copy of your most recent statement of entry or results showing your UCI.
