FY17 Designated IT Professionals Software Licensing Acceptance Form

The University of Alaska Anchorage has a variety of Licensing Agreements (e.g. campus, site, co-op, or volume) with several software vendors for a variety of software products. This acceptance form permits designated IT Professionals access to the network file share containing the digital distributions of software products available under either a campus site license or co-operative licensing agreements. Not all departments are participating in all possible software licensing agreements available to the University. Accordingly designated IT Professionals will only be granted access to the sub-set of software for which their department is eligible for. Furthermore, software installations may only be done in accordance with the relevant licensing agreement for each product. Some software may only be installed on University owned computers, where as others can be installed on personal computers, if authorized by department heads and/or employee supervisors.

A short summary of each relevant licensing program is provided below.

Apple The Apple Education Licensing Program (AELP) program is no longer active; however, those departments that have purchased licensing can still access their original versions purchased via the network file share. Other Apple software that is offered for free from Apple is available on the network file share in order to reduce network congestion and provide a consistent means of access to installation media by designated IT Professionals. Examples include the latest releases of Mac OS (e.g. 10.9, 10.10, 10.11, etc.) Autodesk Autodesk offers free licensing, within certain parameters, for academic use. Microsoft Enrollment for Education Solutions (EES), formerly known as the Microsoft Campus Agreement (MCA): This allows for the installation of the licensed software on university owned workstations. Designated IT Professionals are required to keep all Volume License Keys secure and are prohibited from sharing the codes with other employees. Departments will be held responsible for any unauthorized use by the designated IT Professional of volume licensed software and product codes. Work at Home Use (WAH): The use of the university’s volume licensed software media and licensing codes for installation on personally owned computers for work at home use may only be done by the designated IT Professional; provided that the employee requesting the software has written approval from their department head or supervisor authorizing the work at home use. The designated IT Professional must maintain procedures for all software dispensed and ensure that volume license media and license codes are not released to employees. Use of installed software of this nature is restricted to work-related purposes only. Home Use Program (HUP): Volume licensed software and Volume License Keys may not be used under any circumstances in association with the Home Use Program. Individuals interested in the Home Use Program must purchase the software themselves through the Microsoft Home Use Program e-commerce store (www.uaa.alaska.edu/mshomeuse/). Office 365: As an alternative to the Home Use Program (HUP) individuals may login to their UA Office 365 account to download and install a copy of Office 365 Pro Plus on up to five personally owned workstations. SAS SAS licensing agreement allows University of Alaska departments to purchase SAS at a discounted price. IBM IBM licensing agreement allows University of Alaska departments to purchase certain SPSS products at a discounted price. Symantec There are two products covered under the Symantec licensing: Symantec EndPoint Protection (SEP) and Symantec Ghost. The licensing agreement for SEP allows for installation on all university owned equipment. Additionally, students, staff, and faculty are licensed to use a copy of SEP on their personal computers. The licensing agreement for Ghost allows University of Alaska departments to purchase this software at a discounted price.

Only software licenses/licensing purchased through IT Services will be available for electronic distribution via this method.

Designated IT Professional (please initial each statement): _____ I will read and abide by the license agreement(s) associated with this software. _____ I will abide by the university’s distribution requirement(s) associated with this software. _____ I may make duplicates of some vendor media provided they are true and complete copies, including copyright and trademark notices, and that the specific media this is authorized for will be noted in a ReadMe file located in the software’s directory. _____ I understand that I will document all installations of all software covered by any of the licensing agreements on the appropriate spreadsheet, and that this documentation may be requested by either UAA IT Services, and/or software vendors in the event of a licensing audit. _____ I understand that I will be granted read-only access to the Software file-share, which will provide disc images and/or installers and labels of the licensed software, as well as documentation regarding installation policies and procedures. _____ I understand that I will only have access to the software on the Software file-share for which my department is authorized to use. _____ I understand that under no circumstances may I provide/make available the University’s license keys, product codes, activation keys, or other similar serial numbers to any individual. _____ I understand that under no circumstances may I provide/make available digital, or physical copies of any of the installation files to any individual and that the installation media/files are for use solely by authorized designated IT professionals. _____ I understand that if using the Microsoft campus volume license media and license keys to install software for Work At Home purposes on an employee’s personal computer I will ensure that the proper WAH authorization form is signed by both the employee desiring the software and their supervisor and keep on file the completed and signed form. _____ I understand the minimum specifications to run the software as listed on software vendor’s product web pages. _____ I understand that I am not licensed to use the software for personal purposes. _____ I understand that I may not install more copies of the software than my Department is licensed for. _____ I understand that I will be subscribed to the UAA Information Technology Group listserv.

Last Revised: 7/1/2016 _____ I understand that this form must be renewed annually, typically during September of each year. Failure to renew will result in a lost of access to the Software file-share.
