Sample Student Information Sheet for Letters of Recommendation In order to write a recommendation letter for you, counselors and teachers need information that will be used solely and confidentially for that purpose. You benefit when they are able to provide a comprehensive academic and personal report. You'll need to have this same information for your college applications, so it will help you later as well. Please print neatly or type your responses. You may attach separate sheets as needed.

5. Attach a copy of your draft college personal essay if it is completed, or answer, "What sets you apart as an individual?"
6. Ask your parent/guardian or a friend to write an anecdote that describes your character and attach it to this form. This should be about one paragraph long.
