See the rate plan for more details • A future friendly Blackberry device at a competitive price, see the handset information hand-out for more details • A $250 airtime air-time credit at the point of subscription to the program as well as an additional $60 air-time credit monthly. • A cost effective electronic mail delivery service for distribution of Union information using a specific new email address The monthly payment by the Union shall be limited to Journey-workers and fourth year apprentices who are paying working dues and have not been unemployed for longer than three months. A member who remains unemployed after three months will be assessed the monthly amount covered by the Union until the member returns to covered employment. An active member who is on the employment list is eligible to join the plan but will be responsible for paying the amount the Union pays for those paying working dues. Existing TELUS Subscriber Do you have an existing TELUS number? If so you are eligible for new a Blackberry handset upgrade and the new rate plan, even if you are in an existing TELUS contract. Existing TELUS subscriber will need to complete the following steps: • Ensure no out standing balance exist and the account is in good standing • Complete the Service Agreement/Transfer of Ownership Document (See additional Documents) • Complete a Blackberry Request Form (See additional Documents) • Completed and signed Order Form (See additional Documents) • FAX all paperwork to TELUS at 1-866-750-1654 Or send them by mail to: TELUS Communications Attn: Garett Grant – Union 110 Project 411 1 St SE 8th Floor, Calgary AB T2G 4Y5 Or scan and email the signed documents to [email protected] Please ensure that your account is in good standing prior to requesting to take part in the Union 110 deal. If you are unsure about your current accounts status please contact TELUS client care for assistance at 1-866-558-2273 toll - free. TELUS is checking all existing subscriber for compliance to the above criteria. This check will be completed prior to the order being completed. Service will not be interrupted to your existing device. Important: Please note that client care will not have information on the Union 110 offering. After a member is transferred to the Union 110 offering an enhanced billing support team will handle all billing inquiries. The enhanced billing support team will have all information on this special offering. Delivery You have two delivery options; to your residence or to the union office. Union Office - Delivery of your chosen device will be made on pre-determined days to the union office. When you complete and submit your paperwork TELUS will contact you to select a day to deliver your new Blackberry. When delivered, TELUS will complete a number of activities with every member to provide you with a working device, message delivery service and training. Residence – TELUS can now ship to Post Office Boxes, Range Road addresses, and camp sites; please make sure you include these details in the ship-to information of the sales order form. Keep in mind you will miss the valuable orientation that comes with a scheduled delivery at the Union Office. Important: You must contact TELUS immediately after you have received your device to set up services; directions will be included with your package. Terms other than COD will be determined upon presentation of credit references satisfactory to us. Any other arrangement must be confirmed in writing. Acceptable payment methods include approved credit card or company cheque. Interest maybe charged on past due accounts. Return Policy & Procedures All returns to TELUS require a Return Authorization number (RA#) and must be returned in their original packaging. For return authorization please call 1-877-657-7512. All freight charges must be prepaid. Products returned are subject to our valuation without recourse. All returning product must bear the RA# on outside of the package. Returns will not be allowed under the following: Returned by other than the original purchaser Not in the original packaging Damaged in transit due to inappropriate packing and use of shipping material Items expressly sold on a non returnable basis Damaged, Shortage, Incorrect Shipment or Loss in Transit Claims for the above must be made within 5 business days to TELUS. Pricing & Taxes All prices are located in the Price and Availability List and are subject to change without notice. Orders will be charged at the prices in effect the day order is entered. Quoted or contract prices supersede the prices in the then current Price and Availability List. Provided the quotation or the contract is current at the order is placed.
