PURPOSE OF THE POSITION The Nurse in Charge, Regional Home Care Services, is responsible for the provision of community home care services in accordance with established Continuing Care Standards, the standards of nursing practice and the philosophy and objectives of the Northwest Territories Health and Social Services Authority to ensure the provision of safe standardized care to the residents of the Sahtu Region.

January 19, 2021 GNWT Job Description Page 1 of 9 The NTHSSA administers all regional health and social services (HSS) delivered to 4 Community Health Centres and 1 Community Health Station, in the Sahtu Region of the NWT. The NTHSSA provides and supports the delivery of health care services to adults and children on a short term observation, outpatient and outreach basis, in order to enhance healthy communities’ well-being through excellence, accountability and respect for regional diversity.

Located in Norman Wells, the Nurse in Charge, Regional Home Care Services (NIC-RHCS) reports directly to the Regional Manager, Continuing Care (RMCCS) and is responsible for the coordination and delivery of community home care services to the client population in all 5 communities within the Sahtu region. The NIC-RHCS is responsible for planning, implementing, supervising and evaluating the community home care services as part of an integrated care team. In addition to managing the community home care services, the NIC- RHCS will provide services and direct client care. Typically, the above qualifications would be attained by: A Baccalaureate Degree in Nursing, and three (3) years of recent nursing experience with at least one (1) year in continuing care and one (1) year of supervisory experience.
