The Purpose The local St Albert Cosmopolitan Club has been active in the Capital Region for over 35 years, raising much needed funding for countless community groups with our overriding principle effort, the “Fight to End Diabetes”. As many of you know, the University of Alberta has received generous donations over the years from Cosmopolitan International directly associated with the development of Islet Transplants to provide groundbreaking diabetic treatment. The proceeds from this charity hockey pool will help fund this import- ant research, improving the lives of thousands of !

How to Enter

Fully complete the following steps:

1. Clearly print your full name and email address.

2. Complete the form attached: You must ONLY select 1 player from each of the 21 groups of boxes.

3. Form and a $25 payment, cash or cheque, must be submitted together by October 15th at 6 p.m.

4. Confirmation of your team being accepted will be:

An email informing each individual how to access the daily results of the Hockey Pool. You will receive a username and password to access Officepools.ca. You can then follow your team and the pool results, though the , at your leisure.

5. Mail to St Albert Cosmopolitan Club, Box 111, St Albert, Alberta, AB, T8N 1N2, postmarked no later than October 15 OR Give to any Cosmopolitan member. Payment MUST be included!

The contest will coincide with the NHL 2018-2019 regular season.

Eligibility The contest is open only to people who have reached the age of 18. You can enter as many times as you wish by paying $25 for each entry. The Cosmopolitan Charity Hockey Pool will be open to everyone with the exception of the following; the chairperson(s) of this event, the Cosmopolitan treasurer and Cosmopolitan President.

How to Enter To enter the Cosmopolitan Charity Hockey Pool you must register by correctly completing and submitting the form to a Cosmopolitan member prior to October 15th, 2018 at 6:00 pm. Cash or cheque must accompany the form, with cheques payable to St Albert Cosmopolitan Club.

How to Play The objective of Cosmopolitan Charity Hockey Pool is to accumulate the highest of Pool Points based upon the

Scoring System All Forwards and Defense: 1 = 1 Pool Point. 1 Assist = 1 Pool Point. 1 Shootout Goal = 1 Pool Point. All Goalies: 1 Win = 1 Pool Point. 1 Shutout = +2 Pool Points (3 total including a win). 1 Goal = 1 Pool Point. 1 Assist = 1 Pool Point.

Standings and Updates Contest standings are updated daily on officepools.com The web site will include player and entrant point totals. All statistics are updated daily, seven days a week. A confirmation email will be sent to each contestant with the username and password for the hockey pool access to view your team and results.

IMPORTANT: standings are UNOFFICIAL for one week following the conclusion of the 2018-2019 regular season. In the event there is a discrepancy please notify Mike Tod within the one week of the Unofficial Standings Period, otherwise the standings will become final and . Player points as published by officepools.com are considered to be official results for the purposes of this pool.

Prizes 1st Place - $300 ***If we exceed base entries, we will pay out a minimum of 20% of all entries received to the 2nd Place - $200 winners. With the ratio equaling 300/200/100. 3rd Place - $100 Last place is always $25*** Last Place - $25 (In the event of a tie, for last place, the winner will be drawn in a random draw of all those in the tie) Winners of prizes will be notified by email, if winners can not be located within 30 days after the prizes have been awarded, proceeds of the prize will be randomly drawing from the remaining contestants/entries. No refunds will be given to contestants/entries that withdraw, cancel or are terminated from the Cosmopolitan Charity Hockey Pool. All payments are final and non-refundable

Ties Between the pool teams that are tied, the pool team with the most goals will prevail. If still tied, the two prizes will be added together and shared equally.
