HODA ABDELAZIZ, NP DSRIP Nurse Practitioner NewYork-Presbyterian Hoda Abdelaziz is the DSRIP Palliative Nurse Practitioner in the NYP Queens PMO. In her role, she collaborates with internal and external stakeholders to drive education initiatives with the goal of improving palliative care services across the network. Hoda has been a Registered Nurse since 2007 and her breadth of experience covers various clinical specialties including Vent, Neuro and ED. And as a nurse practitioner since 2012 Hoda has experience in pain management, Weill Cornel’s surgical department, and primary care at Nursing Homes for United Health and Health First. Hoda’s skill set and expertise enabled the NYP Queens PMO to establish her as a point for contact, reference source for provider education and chart auditor for patients discharged from to SNF. Hoda has been at the forefront of the PPS efforts to create an environment with shared goals, shared knowledge and mutual respect between the hospital and staff. Hoda holds a bachelor degree in Science from Cairo University and MSN in nursing from Lehman College.

RICHARD AKOBI Assistant Director, Care Transitions NYU Langone PPS Richard Akobi, RN, MBA joined NYU in 2017 after working at Health and Hospital Corporations as the Senior Director of Transitional Care. Richard has spent more than 20 years developing and implementing care service models at HHC and New York Presbyterian via VNS. Currently, Richard is the Assistant Director of Care Transitions and oversees the Emergency Department Care Triage program. The program aims to reduce ED and inpatient utilization by improving connectivity to medical care, behavioral health services, substance use, and addressing social determinants of health.

WHITNEY ALE Program Manager, Center for Community Health Navigation NewYork-Presbyterian Hospital Whitney Ale has worked for New York-Presbyterian Hospital for the past nine years, spending two years in her current role as Program Manager of the Center for Community Health Navigation. Prior to that, Whitney spent seven years in pediatric psychiatry providing case management support to families with HIV and mental health needs while gaining her Master’s in at Columbia University. In addition, Whitney worked at the AIDS Service Center as an assistant COBRA Case Manager.

BRITTANY ALTIERI Project Manager Alliance for Better Health Brittany Altieri is with Alliance because she believes that through inspiration, collaboration, and action (backed by data) we can empower people to make significant and revolutionary change. In Brittany’s role at Alliance, she translates ideas and work into actionable project plans, bringing together groups of super smart people who can collaborate and make decisions, and set timelines to achieve their goals.

GEORGE ALVARADO, MD Medical Director, Health Home; Associate Director, Ambulatory Behavioral Health Solutions Dr. Alvarado is Medical Director of the Northwell Health Home Program serving adults. He is also Associate Director of Integrated Behavioral Health at Northwell Health Solutions. He has worked on several DSRIP related projects, including adults and pediatric behavioral health integration in primary care, as well as the Behavioral Health Urgent Care at Cohen Children’s Medical Center. As a child psychiatrist, he has a particular interest in children’s systems of care, including mental health in schools.

JAMES ANDERSON, PHD LCHP Medical Director; Clinical Psychologist, Bassett Healthcare Network Leatherstocking Collaborative Healthcare Partners; Bassett Healthcare Network Dr. Anderson is a clinical psychologist with a specialty in primary care integrated behavioral health. He completed his PhD at Western Michigan University in 2010 and Postdoctoral Fellowship at the University of Massachusetts Medical School in 2012. He has served a dual role as the LCHP Medical Director and clinical psychologist on the Bassett Healthcare Network Medical Staff since 2015.

AMY ANDERSON-WINCHELL Co-Chair Coordinated Behavioral Health Services Amy Anderson-Winchell is the President and CEO of Access: Supports for Living. Under her leadership, more than 10,000 people’s lives are touched across the Hudson Valley through behavioral health programs, services for people with intellectual and developmental disabilities, community outreach, child welfare services, and industrial operations. She is a Licensed Clinical Social Worker in the State of New York having received her Master’s in Social Work from the University of Pennsylvania in 1981. Prior to her graduate education, Amy was conferred a Bachelor’s Degree from Brown University after completing her final semester of study at Yale University.

JILLIAN ANNUNZIATA Sr. Project Manager, Strategy HRHCare, Community Health Jillian has a diverse set of experiences in her early career as a LEAN PMP-certified Public Health Program Manager, from Bilingual Health Educator, to developing County health policy, to her current role as Senior Project Manager of Strategy, implementing NYS Medicaid Redesign initiatives including DSRIP and Health Home programs. Jillian is a change agent, committed to managing quality improvement projects that aim to design systems and communities that promote health. She holds an MPH from New York Medical College in Health Policy & Management and received her bachelor’s degree from Binghamton University in Biological Anthropology and Spanish. Connecting the dots between the social, political, and racial determinants of health and implementing interventions to improve population health outcomes is what keeps Jillian up at night and gets her out of bed in the morning.

CHRISTOPHER ARNOLD, CASAC Coordinator of Crisis Management/Mobile Crisis Services Behavioral Health Services North Christopher Arnold, a certified alcoholism and substance-abuse counselor, is Coordinator of Crisis Management and Mobile Crisis Services at Behavioral Health Services North in Plattsburgh, NY.

ANI ARJUNE Project Manager Northwell Health

KEARYANN AUSTIN, LMHC Program Director, Short-Term Crisis Respite and Transitional Step-Down Housing ACMH, Inc. Kearyann Austin is the Program Director for Short-Term Crisis Respite and Transitional Step-Down Housing with ACMH. She received her Bachelor’s Degree in Psychology from CUNY York College and went on to receive her Master’s Degree in Mental Health Counseling at LIU Brooklyn Campus. Kearyann started her career at ACMH as a Case Manager in April 2013, focusing on getting her clients acclimated into the community as well as helping them to transition into permanent supportive housing. In January 2015, Kearyann was promoted to Program Coordinator where she spearheaded the OMH Crisis Respite Pilot program in the East Village NY. In February 2018, Kearyann became the Program Director with the expansion of Short-Term Crisis Respite in Washington Heights NY, overseeing both programs. Kearyann specializes in crisis intervention, conflict resolution, community integration and outreach, and life skills training. Kearyann is dedicated to helping Crisis Respite participants focus on their recovery goals and empower them to learn how to manage future crises to avoid hospitalization.

LYNSEY AVALONE, MSW, MPH Associate Director, Office of Behavioral Health NYC Health + Lynsey Avalone, MSW, MPH, is an Associate Director in the Office of Behavioral Health at Health + Hospitals. Ms. Avalone is currently focused on implementation of addiction intervention programs in the ED and inpatient settings. Prior to holding this position, Ms. Avalone was the Project Manager for the Boston HealthNet, an integrated healthcare delivery network between Boston Medical Center, Boston University Medical School, and 14 community health center partners. Ms. Avalone received her Master’s from Boston University, where she completed internships at the Boston Public Health Commission and Boston Alliance for Community Health. Ms. Avalone has also presented at the American Public Health Association and the Association of Schools & Programs of Public Health.

MARTINE BARON Director of Care Management SOMOS Community Care Martine Baron is a Licensed Clinical Social Worker (LCSW-R) with over 15 years experience in the field of social work in the managed care sector, providing case management with a specialization in behavioral health integration. She is currently working as the Director of Care Management at SOMOS Community Care. She creates new care management models and uses a patient-centered approach in her approach to care models. Ms. Baron has an extensive knowledge of program development and medical social work. Ms. Baron is a graduate of the Columbia School of Social Work. She co-chairs the National Association of Social Work Healthcare Committee.

JASKANWAR BATRA, MD, MHA Medical Director, Ambulatory Behavioral Health NYU Langone Brooklyn PPS Jaskanwar Batra is a board-certified psychiatrist currently serving as Medical Director of Ambulatory Behavioral Health at the Family Health Centers at NYU Langone in Brooklyn, NY. He completed his residency at the University of Vermont and his Master’s in Health Administration at George Washington University. He has had significant leadership roles including Medical Director of the Vermont State Hospital, Medical Director for Vermont Department of Mental Health and Chief Medical Officer of the Vermont Health Coop. His clinical interests include, improving access to mental health services in urban, diverse communities, and healthcare integration of mental health with primary care and suicide prevention.

TAMMY BENDER Regional Assistant Arms Acres and Conifer Park Tammy Bender has been working in the field of addiction for more than 15 years and with Arms Acres and Conifer Park Regional Team for over four years. Tammy is also the Alumni Coordinator for Arms Acres and manages several of the support services that are offered to our Alumni such as the “Connections” Recovery App and the Recovery Coach Services for the Hudson Valley. Tammy is a Certified Addiction Recovery Coach and a Certified Recovery Peer Advocate. She currently supervises two Recovery Coach Positions that were hired with the Innovation Fund Pilot Project with Montefiore Hudson Valley Collaborative.

BARBARA BERNSTEIN, PHD, MPH Chief Planning Officer Mental Health Association of Westchester As Chief Planning Officer for MHA of Westchester, Barbara Bernstein, PhD partners with the Leadership Team at MHA to plan and execute strategic objectives. Her specific focus is on external relations, funding sources and agency-wide transformations implementing Trauma Informed Care and a “Zero Suicide” framework. Dr. Bernstein is a licensed Psychologist with a PhD in Psychology from CUNY and an MPH from Columbia University. She is certified as a NYSDOH MAX Train-the-Trainer and is Lead Co-Facilitator for a MAXny Team, sponsored by WMCHealth PPS, addressing high utilizer patients who present to the ED seeking opioids for pain control.

DON BERWICK, M.D. President Emeritus and Senior Fellow Institute for Healthcare Improvement Don Berwick is the ’ leading advocate for high-quality healthcare, and one of the top thinkers in healthcare today. Don sees tremendous unrealized potential in American medicine. The perfect blend of content and inspiration, Dr. Berwick helps everyone pursue better, more affordable care. In his talks, he addresses the sources of excellence, and of defect, in health care settings today, explains which approaches are not effective, and provides real insight on where improvement can be attained. From July 2010, to December 2011, Dr. Berwick served as Administrator of the Centers for Medicare and Medicaid Services. For 22 years prior to that, he was the founding CEO of the Institute for Healthcare Improvement, a nonprofit dedicated to improving healthcare around the world. A pediatrician by background, he has also served on the faculties of the Harvard Medical School and the Harvard School of Public Health. In 2005, he was appointed “Honourary Knight Commander of the British Empire” by Her Majesty, Queen Elizabeth II, in recognition of his work with the British National Health Service. He is the recipient of many awards, including the Ernest A. Codman Award, the American Hospital Association’s Award of Honor, the Heinz award for Public Policy and the Institute of Medicine’s Gustav O. Lienhard Award. He has been named as a Fellow of the Royal College of Physicians (London) and a Fellow of the Royal College of General Practice. His over 160 articles have appeared in professional journals on such topics as healthcare policy and healthcare quality management. His books include Promising Healthcare: How We Can Rescue Health Care by Improving It, Curing Health Care, and New Rules: Regulation, Markets and the Quality of American Health Care. A powerful motivator, Don encourages institutions and individuals to do their work better.

ASHLEY BESIGNANO-LONG Academic Pharmacy Supervisor, Ambulatory Care University Hospital - Northwell Health Ashley Besignano-Long, BS, PharmD, BCACP is the academic pharmacy supervisor at Staten Island University Hospital. Ashley received her bachelor degree of science in pharmaceutical sciences from Northeastern University (2013) and her doctorate of pharmacy from Northeastern University (2014). She has completed a PGY-1 pharmacotherapy resident program at The in New York, followed by a PGY-2 at The Brooklyn Hospital Center specializing in ambulatory care pharmacy. Ashley is currently responsible for the implementation of pharmacy service in the ambulatory care clinic at Staten Island University Hospital and is a preceptor for student pharmacists from Long Island University.

LISA BIELAMOWICZ, MD Co-Founder and President Gist Healthcare Lisa Bielamowicz, MD is the Co-Founder and President of Gist Healthcare, a strategic advisory service that provides guidance to health care industry leaders. A national expert on health care strategy, Lisa is known for being able to distill actionable insight from complex and conflicting market forces. Whether speaking to an audience of hundreds or working with an executive team, her passion is increasing the pace of transformation in health care. Lisa’s work combines national-level insight with a deep understanding of local market dynamics and system strength. A trusted advisor to dozens of health systems and medical groups, executives call on Lisa to provide critical guidance on system strategy and investments, and to serve as a partner in developing consensus toward a common vision. Prior to founding Gist, Lisa was Chief Medical Officer and Senior Vice President of the Advisory Board, where she led the firm’s strategy and clinical research for over a decade and was the primary author of many publications on and physician strategy, population health and care transformation. As a physician Lisa brings a clinical lens to her work and has a deep understanding of the dynamics of physician practice. She trained in diagnostic at Johns Hopkins Hospital. A Howard Hughes fellow, she researched memory and learning using functional MRI at the National Institutes of Mental Health. She holds a bachelor’s degree from Rice University and received her MD with highest honors from Baylor College of Medicine, graduating first in her class.

ELBA BIZONI Operations Manager, Hudson Valley HRHCare, Community Health

ASHLEY BLAUVELT Managing Director, Strategic Initiatives & Operations Staten Island PPS Ashley has been with the Staten Island PPS since 2016. In her current role as Managing Director of Strategic Initiatives and Operations, Ashley oversees DSRIP project implementation and represents the PPS on externally led population health and diversion initiatives including the Richmond County District Attorney’s Office HOPE Program, Staten Island 911 Diversion Pilot, and the Bridge Home Program. Previously, she has worked on behavioral health crisis projects, outpatient operations management, and led community wellness events for CBOs. Ashley holds a Master of Public Health from the Mailman School of Public Health at Columbia University.

ANTONELLA BOJANICH Project Manager Northwell Health Antonella Bojanich is a Project Manager who works with Northwell Health Physician Partners as a leader in the transformation of healthcare in primary-care settings. Ms. Bojanich holds a bachelor’s degree in Business Administration specializing in Project Management, that has qualified her to run multiple projects, all focusing on the improvement of care for the patients. She continues to advance her education, with the purpose to acquire a deeper insight of project management in the healthcare industry. One of her personal goals is to help make an impactful difference in the way healthcare is rendered. During her time in the health system, over ten years, she has been able to work in different roles, allowing her to gain institutional knowledge and expand her perspectives in projects as well as operations. Utilization of project management life cycle methodology enables her to track project resources and schedules.

SUSAN BRETT Chief Operating Officer People Care, Inc. Susan Brett holds a key leadership role, as Chief Operating Officer at People Care, Inc., a NYS Licensed Home Care Service Agency. Susan has over 32 years of experience in the home health care industry with expertise in local, state and federal regulations, legislative matters, operations, workforce development, business strategy, contract procurement and value-based service models. Susan is directly involved with NYS’s DSRIP initiative on behalf of her corporation with the development of quality activities, and training and measurement systems to achieve desired patient measurement outcomes related to value-based care. Susan participates on various committees including the Governance Committee for NYS DOH funded Behavioral Healthcare Collaborative for Value Based Payment, and the Workforce Committee for Partners for Healthy Communities.

NAT BROWN Program Director, DSRIP Northwell Health Nathaniel is an IT Program Director at Northwell Health (formerly North Shore LIJ). He provides oversight to all technical and information service initiatives for the Delivery System Reform Incentive Payment (DSRIP) program, an overhaul of health delivery systems across Long Island and New York City. Nathaniel has over 10 years of experience in the healthcare industry, with a special focus on Medicaid redesign projects, population health management, Health Information Exchanges, and Electronic Health Record implementations. He is a certified Project Management Professional (PMP) and is trained in Lean Process Improvement. He has a Master of Arts degree from Tufts University, and also teaches Philosophy at Molloy College.

GARY BUTCHEN Executive Director Bridge Back to Life Center, Inc.

JESSICA BYRNE Program Manager Northwell Health Jessica Byrne received her Bachelor’s degree in Health Promotion from the University of Delaware and her Master’s in Public Health from George Washington University. Jessica worked for ten years at the National Institute in Bethesda, MD before moving abroad and working in medical higher education in Ireland before coming to New York. Jessica is currently a Program Manager with the Community Health Department at Northwell Health. Jessica works on DSRIP initiatives focusing on relationships with community based organizations, community engagement, and social determinants of health.

DAPENG CAO, PHD Manager, Health Care Analytics Community Partners of WNY PPS Dapeng Cao, PhD is the Manager of Healthcare Analytics for the CPWNY Project Management Office. He received Master’s and PhD degree in Industrial & Systems Engineering at the State University of New York at Buffalo, and he is certified as a Lean Six Sigma Black Belt project management professional. Dapeng has been working for CPWNY since its formation in 2014. He oversees performance analytics and reporting, financial modeling, and pay for performance incentive programs. Prior to joining Catholic Medical Partners, the PMO for CPWNY, Dapeng’s research has focused on process optimization and decision-making in healthcare settings.

JOAN CHAYA Director, Workforce Development and Management Montefiore Hudson Valley Collaborative Joan has been working in healthcare management for over 25 years specializing in organizational effectiveness, change management, future directed process improvement, compassionate strategic workforce planning, competency development, staff engagement, and joy-in-work. In her role of director of workforce development and management for Montefiore Medical Center, Joan is responsible for developing and leading a comprehensive workforce strategy in support of the Hudson Valley Collaborative Performing Provider program. Her work has been focused on the people side of change and the linkages between joy in work and supporting and developing staff to provide patient centered care. Joan is a certified Prosci change practitioner.

JIHOON CHOI MS, BA Medical Student Albany Medical College/Better Health for Northeast New York (BHNNY PPS) Jihoon Choi completed his undergraduate degree in Public Health Studies at the Johns Hopkins University, and received a Master’s degree in Biochemistry and Molecular Biology at the Johns Hopkins Bloomberg School of Public Health. He is currently a 4th year medical student at Albany Medical College and is pursuing a residency training in Internal Medicine.

SADIA CHOUDHURY, MPA Director of Ambulatory Care Initiatives Staten Island Performing Provider System At Staten Island PPS, Sadia oversees Model 1 for Primary Care and Behavioral Health Integration project, along with PCMH, Patient Activation, Diabetes Management, and other QI initiatives. Sadia also served in a leadership role at NYP Queens PPS and NQP PPS. In addition to her expertise in program and project management, Sadia also has extensive experience in Quality Improvements/Analytics. Sadia is a certified lean methodology trainer for NYSDOH sponsored Medicaid Accelerated Exchange (MAX) Program, focused to reduce inpatient and ED high utilization. Sadia attended LIU Brooklyn where she earned a Master’s Degree in Public Administration, with concentration in Health Policy and Management.

CAITLIN COLLINS Solutions Specialist Alliance for Better Health Caitlin Collins began her journey in the human services field five years ago working with children with developmental disabilities. After moving to Albany, NY, the journey continued evolving working with different populations that brings a unique perspective in various DSRIP initiatives. Caitlin’s passion has flourished with Alliance for Better Health in bridging the healthcare gap for underserved populations. Caitlin is a graduate of the University at Albany and holds a double major in Sociology and Women’s, Gender, and Sexuality Studies.

KATY COOK Project Manager, Telehealth Adirondack Health Institute PPS Katy Cook has been with the Adirondack Health Institute (AHI) as the Telehealth Project Manager since late 2016. She, alongside Fort Drum Regional Health Planning Organization, oversees the North Country Telehealth Partnership, Northern New York’s leading agency focused on increasing access to health care through the innovative use of telehealth and telemedicine technology. Miss Cook helps to oversee the day-to-day operations and strategic development of this state funded partnership, by providing expert assistance to health care providers and entities seeking information on telehealth as they relate to implementation, technology, reimbursement, clinical standards of practice, and more.

NANCY COPPERMAN Vice President, Community Health Northwell Health Nancy Copperman is a Vice President of Community Health at Northwell Health. She is senior contributor to the Northwell Health DSRIP in the areas of social determinants of health, community-based organization relationships, chronic disease self-management, cultural competency and health literacy. She has a MS in Clinical Nutrition from NYU, is a graduate of the Northwell Health High Potential Program, a recipient of the Academy of Nutrition and Dietetics Excellence in Community Nutrition and team lead for the CDC National Academy for the Leadership in Public Health. Nancy is an Assistant Professor of Population Health at the Zucker School of Medicine at Hofstra/Northwell.

MARGUERITE COSTICH, MD Primary Care Research Fellow in Community Columbia University Medical Center Marguerite Costich is a Primary Care Research Fellow and Assistant in Clinical Pediatrics at Columbia University Medical Center.

ALICE CRONIN Assistant Vice President/Chief Information Officer Montefiore Nyack Hospital Alice Cronin is the AVP/CIO at Montefiore Nyack Hospital. Alice is a RN who began her career at NYU where she worked clinically for over 20 years. She moved to Our Lady of Mercy Medical Center as the ED Clinical Director. While at OLM she became the PM for the new HIS project and transitioned to IT. At Montefiore Nyack Hospital she has had responsibilities for multiple areas including Registration, Laboratory, Medical Records, Information Technology, CDI and the Medical Library. She has used her skills in lean healthcare and as a Six Sigma Green Belt for process redesign projects.

ROXANNE CUEBAS Project Administrator Community Partners of Western New York Roxanne Cuebas is a Project Administrator at Community Partners of Western New York. She oversees the Behavioral Health/Primary Care Integration, Telemedicine, and Palliative Care projects. She holds a Bachelor's Degree in Health and Wellness and a Master's Degree in Public Administration from SUNY Buffalo State. Prior to joining CPWNY, she led Health and Wellness outreach initiatives for Erie County Department of Senior Services and was responsible for the implementation of the Chronic Disease and Diabetes Self-Management programs. She serves as a board member of The Colorful Minds Foundation, which seeks to end the stigma associated with mental illness.

KELLY DARROW, LCSW Chief Program Officer Human Development Services of Westchester Kelly Darrow, LCSW, Chief Program Officer for Human Development Services of Westchester, has over 25 years’ experience in community-based behavioral health service provision with children, families and adults. She has experience with a wide range of service systems. She has worked as a direct care staff, supervisor and administrator. She is a seasoned trainer with an understanding of the challenges and rewards of today’s work in an evolving system of care. In 2017, Kelly was fortunate to be trained and certified in The Foundations of Motivational Interviewing by The Centre for Collaboration, Motivation & Innovation via training offered through the Montefiore Hudson Valley Collaborative (MHVC)

JAMES DAVID Director- Patient Relations and Engagement NYU Langone Brooklyn PPS James David serves as the Director of Patient Relations and Engagement for the Family Health Centers at NYU Langone and concurrently as the Assistant Director of DSRIP Partner Relations and Engagement for the NYU Langone Brooklyn PPS. He previously worked as the Director for Institutional Advancement at a number of independent schools in New York and California. In all his roles, he focuses on building strong, sustainable relationships that support both individual and common goals. He holds a BA from James Madison University and a MS from the Graduate School of Journalism at Columbia University.

JAIMEE DAVIS, MS Program Manager, Community Health Workers Programs New York-Presbyterian Hospital Jaimee Davis, MS is the Program Manager for Community Health Worker Programs at NewYork-Presbyterian.

MATTHEW DEMASI Program Manager, Community and Population Health NewYork-Presbyterian Hospital Matt DeMasi, MA, has worked in hospital settings for the past four years being on both the clinical and program development sides of the aisle. He spent time as a mental health worker at Jersey City Medical Center, working with patients in their communities to connect them to resources to help improve their health and support systems. At New York-Presbyterian Hospital, he has had the opportunity to bring to life a Behavioral Health Crisis Program and a Community Substance Abuse Workgroup to help patients transition from hospital care to the community and address their social determinants of health.

DONNA DEMETRI FRIEDMAN, PHD, LCSW Executive Director Mosaic Mental Health Donna Demetri Friedman, LCSW, MA, PhD., Adjunct Associate Professor, is an adult, child, and mother-infant therapist. She is the Executive Director of Mosaic Mental Health in the Bronx where she has worked for the past 25 years. She began working at Mosaic Mental Health in 1992 as a second-year intern from NYU. She received her PhD in 2015 from NYU’s Silver School of Social Work and her dissertation was on infant research with Dr. Beatrice Beebe at NYSPI at Columbia University. Dr. Demetri Friedman has taught at the NYU Silver School of Social Work since 1999, teaching Theories of Attachment and Early Childhood and Child Development. She is a therapist in the Mothers, Infants and Young Children of September 11, 2001: A Primary Prevention Project. She co-edited a special issue on attachment for The Journal of Clinical Social Work (JCSW) as well as a book, Attachment-based Clinical Practice with Children and Adolescents. She is on the editorial board of JCSW. She received the Outstanding Alumna Award for her work in infant mental health in 2009. Dr. Demetri Friedman completed the Three-year Anni Bergman Parent-Infant Program of the Contemporary Freudian and IPTAR analytic institutes in October 2015. ERIC D'ENTRONE Associate Director of Regional Services Arms Acres and Conifer Park Eric D’Entrone has been a professional in the field of addiction treatment for 20 years, and with Arms Acres and Conifer Park for 12 years. As part of the Regional Services team for Arms Acres and Conifer Park, Eric has created partnerships with many organizations in the community. Eric is also the point person for all DSRIP projects for Arms Acres, managing several initiatives including the Innovation Fund Pilot Project with Montefiore Hudson Valley Collaborative.

MAUREEN DORAN Vice President, Network Development Westchester Medical Center, a Member of WMCHealth Network Maureen Doran is responsible for working with key network partners of WMCHealth, including its Medicaid Re-design, or Delivery System Reform Incentive Partners (DSRIP) in the Hudson Valley. This network will collaborate to achieve DSRIP goals while expanding to integrate care for the WMCHealth Network. Maureen brings over 25 years of experience in strategic business planning, network development, physician-hospital integration, and the management of medical groups. Her background also includes experience in consulting, ambulatory care and managed care in NYC hospital systems and academic medical centers throughout New York, New Jersey and Connecticut.

ALLIE DUBOIS Chief Operating Officer HRHCare, Community Health As COO, Allison Dubois is responsible for all HRHCare administration, including contracting, grants management, outreach implementation, special populations, community relationships with special service providers, as well as hospitals and other community health centers, and legislative advocacy. As the former Director of Government Affairs and Special Projects for the Community Health Care Association of New York State, Ms. Dubois has broad experience in advocacy and policy- making on behalf of community health centers. She holds an MPH from the Mailman School of Public Health at Columbia University, and received her Bachelor’s degree from Cornell University, with a concentration in Gerontology. She was the 2010 National Emerging Leader Award winner from the Geiger Gibson Center for Community Health at George Washington University. In 2011, Ms. Dubois completed the Johnson & Johnson Health Care Executive Program at the UCLA-Anderson School of Management.

SHELBI DUBORD Regional Performing Unit Lead Care Compass Network Shelbi DuBord, MPA is a Regional Performing Unit Lead at Care Compass Network with a background in non-profit leadership, grant writing, strategic planning, emergency management, marketing communications, and volunteer management. Prior to working at Care Compass Network, Shelbi was the Director of Community Initiatives & Planning with United Way of Broome County and has also been the Executive Director of Broome County Community Organizations Active in Disaster. In her current role, Shelbi leads a team of Network Facilitators to cultivate networks of care around cohorts of Medicaid members through Care Compass Network’s Cohort Management Program. REBEKAH EPSTEIN Project Manager Bronx Partners for Healthy Communities Rebekah Epstein is a Project Manager with Bronx Partners for Healthy Communities. She has spent her career working with non-profit organizations, including a freelancer-focused benefits organization, a residential treatment facility for teens, and an emergency domestic violence and sexual assault shelter for women and children. TIMOTHY EGAN Chief Information Officer Rockland Paramedic Services Tim Egan began his EMS career in Orange County as an EMT for Horton Memorial Hospital in 1986, later upgrading to paramedic in 1988. He has been with Rockland Paramedic Services for over 28 years, and currently serves as Chief Information Officer. He is responsible for all aspects of information technology for RPS as well as Communications and Compliance. He also serves as a board member for both the United NY Ambulance Network and the NY Mobile Integrated Healthcare Association.

REBECCA EVANSKY Outreach Specialist Health Home Adirondack Health Institute Rebecca currently holds a position as a Project Manager with AHI, supporting initiatives focused on treatment and prevention of substance use disorders, working to strengthen infrastructure across systems and cultivate strong partnerships across the region. Her professional career includes over 15 years dedicated to behavioral health initiatives and providing direct client services, working with the Washington County District Attorney’s Office and prior to that, with Sexual Trauma and Recovery Services (STARS). He oversees the pediatric service line’s network of 17 community-based pediatric primary care practices. His responsibilities include achieving Patient Centered Medical Home recognition for each site, as well as focusing on performance improvement opportunities. Prior to joining Northwell, Dr. Fagan was the Chairman and Program Director of the Department of Pediatrics at Nassau University Medical Center. VERA FEUER Director, Pediatric Emergency Psychiatry and Behavioral Health Urgent Care Cohen Children's Medical Center Dr. Feuer completed residency training at Drexel University and a Child and Adolescent Psychiatry Fellowship at Hofstra-Northwell Health. She stayed on as faculty at Hofstra-Northwell Health after her training and participated in opening the Emergency Psychiatry Services in Adult and Child Psychiatry, developing the Northwell Behavioral Telehealth Service and led the opening of the Pediatric Behavioral Health Urgent Care Center. She is involved with local and national workgroups and committees developing care standards for Pediatric Emergency Psychiatry. ANNIKA GINSBERG Director of Engagement and Collaboration OneCity Health Annika Ginsberg, MPA, is the Director of Engagement and Collaboration at OneCity Health who co-developed the capacity building and technical assistance strategy for community based organizations in the Performing Provider System's (PPS) network. She is also responsible for the implementation and management of OneCity Health’s Innovation Fund. She previously worked at NYU Medical Center’s Rusk Rehabilitation and Mount Sinai School of Medicine. She attended NYU’s Wagner School of Public Service and the College of the Atlantic in Bar Harbor, Maine.

BRUCE GOLDBERG, MD Senior Associate Director Oregon Rural Practice Based Research Network Bruce Goldberg, MD, has devoted his professional career to improving population health and the organization, delivery and financing of health and human services. He is currently a Professor at the OHSU-PSU School of Public Health and Senior Associate Director of the Oregon Rural Practice Based Research Network at OHSU. Prior to that, Bruce served two Oregon Governors, as the Director of the Oregon Office for Health Policy and Research from 2003-2005, Director of the Oregon Department of Human Services from November 2005 through February 2011, and then as the founding Director of the Oregon Health Authority from February 2011 through 2013.As Director of the Oregon Health Authority, Bruce led Oregon's nationally recognized health reforms transforming Oregon’s Medicaid system to one based on a model of coordinated care and population health improvement. In addition, he established Oregon’s Healthy Kids program providing health coverage expansion for children, modernized Oregon’s antiquated mental health system and transformed the delivery of public human services to enhance efficiency and effectiveness. His experiences span time as an administrator of large complex organizations, a practicing clinician, teacher/academician, a county health officer, medical director for a Medicaid managed care organization, Clinical Director for the US Public Health Services in Zuni, New Mexico. Bruce continues to focus on health reform and community health system change. He regularly consults with numerous state and community health organizations.

MAGDALENA GORDON, LMSW Director of Behavioral Health Care Management Maimonides Medical Center Department of Population Health Magdalena Gordon, LMSW is the Director of Behavioral Health Care Management for the Maimonides Medical Center Department of Population Health, which develops programs to transform the healthcare delivery system in Brooklyn, NY; implements interventions, and conducts research to better understand the populations whose health it seeks to improve. Ms. Gordon has a Master of Social Work from Columbia University and has worked with the justice-involved population through other NYC social service organizations and re-entry programs since 2008.

MARK GRAHAM, LCSW Vice President, Program Services Coordinated Behavioral Care Since joining CBC in 2014, Mark has played a critical role in managing CBC’s program expansion. He was instrumental in the launch of CBC’s innovative Pathway Home program, which uses the evidence-based Critical Time Intervention (CTI) to engage and effectively transition individuals from intensive behavioral health settings to the community. Under his leadership, this pilot project has now grown to five teams covering four boroughs and is funded through the NY State Office of Mental Health. He followed the success of Pathway Home with the development of a variety of programs, including the Staten Island CARES Health Home at Risk project which served over 1,700 clients. In addition to his work at CBC, Mark regularly teaches a class on Critical Time Intervention at Hunter College and has taught at University College Dublin in Ireland. Throughout his career, Mark has strived to improve the quality of care and outcomes for individuals experiencing social, economic and health-related cries. Prior to CBC, Mark provided direct clinical care and managed programs in NYC, Manchester, England and Dublin, Ireland with a focus on the homeless, people with serious mental illness and those with chronic health conditions.

ROGER GREEN Professor City University of New Yok Law School From 1981 until 2005, Mr. Green served as a member of the New York State Assembly representing several neighborhoods in central Brooklyn. During his tenure in the Assembly, Mr. Green was widely acknowledged as an expert on educational reform as well as on issues related to children and family policies. He served as the chairman of the Assembly’s Committee on Science and Technology, the Committee on Children and Families and the Joint Budget Conference Committee on Human Resources. A longstanding advocate of civil and human rights, Mr. Green worked within the legislative process to enact numerous laws that reflected his commitment to these principles. He also served as Executive Director of the DuBois Bunche Center for Public Policy at Medgar Evers College. He is currently a professor at CUNY Law School.

CAROLINE GREENE Senior Vice President, Chief Administrative and Financial Officer Maimonides Medical Center Caroline D. Greene is Senior VP and Chief Administrative and Financial Officer for the Maimonides Department of Population Health. Caroline plays a key role in managing the Maimonides PPS, known as Community Care of Brooklyn, and spearheaded the establishment of Community Care of Brooklyn IPA, an entity that supports value-based contracting on behalf of a network of hospital, physicians, and other partners across Brooklyn. Caroline has served as CFO and in other executive roles at hospitals and health systems, at the ACLU, and at Planned Parenthood of NYC. Caroline has a BA from Wellesley College and an MBA in Finance from NYU.

DAMARA GUTNICK, MD Medical Director Montefiore Hudson Valley Collaborative Dr. Damara Gutnick is the medical director of the Montefiore Hudson Valley Collaborative, an Associate Professor of Epidemiology and Population Health at Einstein School of Medicine, a member of the Motivational Interviewing Network of Trainers and a certified ProsciTM change practitioner. An internist by training, Dr. Gutnick is keenly aware of the challenges associated with providing quality care, including the impact of provider burnout. She is also passionate about incorporating Motivational Interviewing and “patient voice” into delivery system redesign to improve patient engagement, quality, and “joy-in-work.” Dr. Gutnick has presented at national and international forums including IHI, ACP and the MINT Forum.

LAUREN HAJJAR, PHD, MPA Assistant Professor of Public Management at the Sawyer Business School Suffolk University In addition to her primary role as Assistant Professor of Public Management at the Sawyer Business School at Suffolk University, Lauren is also the Research Advisor at the Relational Coordination Research Collaborative at Brandeis University, an international learning community fostering the development of relational coordination theory, research and practice. Her research is focused on how organizations build strategic partnerships and encourage enhanced coordination, communication, and collaboration to achieve their goals. Lauren began her career as a direct care worker and manager in the non-profit sector, providing services to adults with mental health and substance use disorders, disabilities and other chronic conditions. During that time, she was recognized by leaders in the field for her keen clinical skills, application of evidence-based practice and ability to lead high performing teams. Since then she has become a researcher, working on various government, and foundation-funded research projects focused on improving community safety initiatives, building relational capacity and understanding the role of diversity in community healthcare organizations. Her research has provided important implications for how community organizations leverage human, social and organizational capital to improve outcomes. Lauren received her MA and Ph.D. from the Heller School for Social Policy and Management at Brandeis University and holds an MPA from Suffolk University.

MARY HAN Director of Continuing Care and Quality Management Staten Island Performing Provider System Mary Han is implementing three DSRIP projects with skilled a nursing facility and home care partners. Prior to DSRIP, she was at working on initiatives for Collaborative Care, Accountable Care Organizations, and panel management. Her interest in quality management developed from working on an NIH funded community-based research project. She presented at the American Public Health Association on the “Quality of life and physical activity among foreign-born and U.S.-born older black men with hypertension residing in New York City”. She studied Physiology at University of Minnesota and has a passion for health and exercise.

MARY HANRAHAN Administrative Manager, Community and Population Health NewYork-Presbyterian Hospital Mary Hanrahan, MSW, LCSW is a licensed clinical social worker with 30 plus years of experience in diverse behavioral health settings. She has worked in network development, government relations and operations in a variety of inpatient and outpatient settings. Mary was part of the original DSRIP planning team for the behavioral health proposals for New York Presbyterian Hospital PPS.

TINA HANSEN PICKETT Senior Director, Sustainability and Strategic Initiatives Maimonides Medical Center Tina Hansen Pickett is Senior Director of Sustainability and Strategic Initiatives for the Department of Population Health at Maimonides Medical Center (Brooklyn, NY), which leads a network of health and social service organizations as part of the NYS DSRIP program—working together to improve population health and reduce avoidable hospital use. Tina holds a Master’s in Public Health and a BA in Biology, and has spent ten years developing and analyzing health care policy and innovative payment reforms at the federal and state level.

MARLON HAY, MPA DSRIP, PMO Director New York Presbyterian, Queens Marlon Hay is the NYP Queens PPS, PMO Director, who collaborates with executives, committee members, staff and providers across the PPS network to improve patient care through population health initiatives and process improvement projects. After serving in the roles of DSRIP IT Project Manager and PMO Director, Marlon knows what truly drives DSRIP success. Success in DSRIP is how well we collaborate with our networks partners to brainstorm solutions and implement the right tools and processes that drive performance outcomes that ultimately improve patient care. In addition, Marlon has been an integral part of the NYP Queens PPS initiative to implement RHIO connectivity, secured messaging and other HIT tools across the network. Marlon holds an MPA in Healthcare Administration from Pace University and an MS. in Health Informatics from the College of St. Scholastica.

ARIEL HAYES Senior Director, DSRIP Northwell Health Ariel Hayes is Director of Program Operations at Northwell Health Solutions: the care management organization of Northwell Health (formerly North Shore-LIJ). Northwell Health Solutions implements and stewards the health system's value based care programs: focused on the implementation of New York State's Delivery System Reform Incentive Payments (DSRIP) program for Northwell's participating hospitals and practices, particularly in Queens and Long Island.

Hirano conducts modeling and segmenting analyses of patient populations to identify opportunities for Community Care of Brooklyn to improve its network’s performance on quality measures and recommend ways to improve existing processes; and has a MPH from San Diego State University and BS from Bucknell University.

MARIANNE HOWARD-SIEWERS, RN, MED Clinical Consultant Staten Island Performing Provider System As the Staten Island PPS Clinical Consultant, Marianne provides input for the clinical components of various programs supported by the SIPPS. She has helped partners with integration by presenting evidence-based practices that are achievable within these organizations along with strategies to increase knowledge base and expertise of existing clinicians to support these changes in practice and the change in workflow. Marianne has over 36 years of experience in behavioral health, both in the hospital and the community. She is a former NYC Health and Hospital employee. She holds a Bachelor’s degree in Nursing and a Master’s degree in Education.

JASON HYDE Director, Patient Navigation Center NYU Langone Brooklyn PPS Jason Hyde, LMSW, MEd, joined NYU in 2011 after spending 10 years at the Vera Institute of Justice. Over the past two decades Jason has developed and implemented care coordination programming across a variety of systems and populations. Jason is currently the Director of NYU Langone’s Patient Navigation Center, which focuses on population health management of vulnerable populations with the goal of improving health outcomes by addressing social determinants of health (SDOH) and connectivity to care. In addition, the Patient Navigation Center deploys and evaluates innovative care coordination interventions designed to improve health outcomes in under-served communities.

MAUREEN ITALIANO, LCSW-R Administrative Director St. Joseph's Medical Center/St. Vincent's Hospital Westchester Maureen has served since 2017 as the Administrative Director for Incident Management, Quality Assurance and Regulatory Matters for Saint Joseph's Medical Center. In that role, Italiano provides leadership in program development including the innovative Bridge Home program, in partnership with the Staten Island Performing Provider System, and the NYC Human Resources Administration/Department of Homeless Services. She brings more than three decades of experience at NYC nonprofits serving adults living with behavioral health challenges, trauma histories and homelessness. Italiano is a graduate of Georgetown University, and of Catholic University's National Catholic School of Social Services and holds a Certificate in Executive Leadership in Non-Profits from the Marxe School of Public and International Affairs at Baruch College.

ARILYN JACOB Senior Director The Jewish Board Marilyn Jacob is the Senior Director of 100 Schools Project with The Jewish Board. She has over 20 years of clinical and administrative experience working in a number of different settings with a wide variety of populations. Marilyn received her PhD from Rutgers University, completing dissertation research in effective identification and treatment of delinquent girls. She also has a Master’s of Social Work and a Master’s of Clinical Psychology. Marilyn has worked as a director in outpatient clinics, court and juvenile justice setting, schools, hospitals, and residential. She specializes in crisis intervention and treatment of traumatized children and their families.

JOHN JAVIS Director of Behavioral Health Nassau Queens PPS John Javis is the Director of Behavioral Health for the Nassau-Queens PPS. He has more than 23 years of leadership experience in the public mental health sector with rehabilitation and recovery programs, crisis services, employment programs, peer support and PROS. He also has extensive experience in developing and implementing programs serving veterans. Prior to coming to NQP John was the Operations Director of the Advanced Health Network, which represented over 20 behavioral health agencies in the Long Island and New York City area.

LILLIAN JIMÉNEZ Sr. Manager, Community Engagement WMCHealth Performing Provider System For 10 years, Lillian Jimenez taught thousands of health care workers how to incorporate independent media into their AIDS education, counseling and prevention work in NYC. At Hudson Health Plan (now MVP Health Plan) as Sr. Director for Outreach and Community Relations, she consolidated several regions into one department; created leadership opportunities for staff and exceeded market share. As the Sr. Program Manager for Community Engagement at Westchester Medical Center Health PPS & Network, she is responsible for Community Engagement, and training on Cultural Competency and Health Literacy. She is currently spearheading Community ID events in Rockland county.

KHAALIDA JONES Student Researcher Medgar Evers College of the City University of New York Khaalida Jones is currently a senior at Medgar Evers College of the City University of New York, majoring in Biological Sciences. She has been involved in all three iterations of PAR, for which she received a Congressional Citation. From starting as a student researcher to most recently taking the lead in building the training curriculum for other student researchers, Khaalida’s academic evolution parallels her civic engagement with her community—having once been a self-described “casual observer” and now being heavily invested in creating actionable plans to create healthier Brooklyn communities.

SARA KAPLAN-LEVENSON Executive Director, Brooklyn Health Home Maimonides Medical Center Sara Kaplan Levenson is Executive Director of Brooklyn Health Home (BHH) and a member of the Department of Population Health at Maimonides Medical Center (Brooklyn, NY), which leads BHH, the Community Care of Brooklyn PPS and other population health initiatives. Sara has designed and implemented a number of innovative care management programs focusing on the delivery of care management services to high need high risk populations. Sara has a Master’s degrees in Public Health and Social Work from Columbia University, which have informed her work developing population level interventions to address health, behavioral health, and social determinants of health.

ERFAN KARIM Director of Innovation OneCity Health PPS Erfan Karim is Director at New York City Health + Hospitals OneCity Health PPS, responsible for implementation of strategic initiative to introduce new care models. As a healthcare management professional with extensive strategic operations experience, he is currently leading the implementation of ExpressCare. Erfan holds a Master’s in Public Health from New York University’s College of Global Public Health and Bachelor of Science from CUNY Medical School.

DAVID KARPF Provider Relations Manager Suffolk Care Collaborative David Karpf is a Provider Relations Manager at the Suffolk Care Collaborative (SCC). A member of the Provider Relations team, David manages relations and overall progress of contracted partners. In this role, David is responsible for providing ongoing support and education and facilitates an action planning process focused on improving performance metrics. Prior to joining the SCC, David interned at a large multi-specialty medical practice building custom reports to support business needs and developed curricula to support electronic health record training for staff. David is a graduate of Stony Brook University with a Master of Science degree in Healthcare Informatics.

BRONTE KASTENBERG Project Manager Bronx Partners for Healthy Communities Bronte Kastenberg is a Project Manager at Bronx Partners for Healthy Communities (BPHC) PPS. Working with the Senior Director of System Integration, she oversees the DSRIP Cardiovascular Disease, Diabetes and Asthma projects and supports the development and management of BPHC’s Innovation Fund initiative. Prior to working at BPHC, she engaged in programmatic work at Malaria No More and the Voss Foundation. Ms. Kastenberg holds a Bachelor of Arts Degree from Kenyon College.

HARRY KATZ Professor of Collective Bargaining & Director of the Scheinman Institute on Conflict Resolution Cornell University Harry C. Katz is the Jack Sheinkman Professor of Collective Bargaining and Director of the Scheinman Institute on Conflict Resolution at the ILR School, Cornell University. He is the President-elect of the International Labor and Employment Relations Association (ILERA) and Past- President of the Labor and Employment Relations Association (LERA). He served as the Dean of the ILR School from 2005-14 and Interim Provost at Cornell University from 2014-15. He received his PhD in Economics from the University of California at Berkeley in 1977. IRENE KAUFMANN Executive Director Bronx Partners for Healthy Communities PPS: St. Barnabas Health System Irene Kaufmann, MA, MPA, is Senior Vice President at SBH Health System and Executive Director of Bronx Partners for Healthy Communities, an emerging performing provider system created to implement and manage DSRIP, the New York State Medicaid redesign program and serving 375,000 individuals in the borough. Ms. Kaufmann specializes in strategic care delivery transformation and integration of hospital and community-based services. Previously, she was Sr. Assistant Vice President, Office of Ambulatory Care Transformation at NYC Health and Hospitals Corporation. In this role, she led projects in ambulatory care redesign, implemented patient-centered medical homes, and system-wide referral and care coordination systems.

ROBERT KLEPPANG Director of Community Services Hamilton County Community Services Robert Kleppang has been the Director of Community Services in rural Hamilton County for just over 19 years. He is the chief administrator of the county department responsible for assuring the availability of mental hygiene services to all county residents, exercising general direction in the planning, coordination, and operation of the county’s mental health services. Kleppang is a NYS Licensed Social Worker and sits on the NYS Conference of Local Mental Hygiene Directors, the ARC New York Board of Governors, and numerous other agencies and memberships. He has a Master of Social Work from SUNY Albany.

LINDSAY KNOWLTON NCI Deputy Director North Country Initiative As NCI Deputy Director, Lindsay coordinates and implements aspects of the Delivery System Reform Incentive Payment program, the Accountable Care Organization (ACO), and the North Country Independent Practice Association (IPA) in their northern New York region. Dr Kolbasovsky is a NYC licensed psychologist and holds an MBA. He has published numerous articles and two books on the topics of reducing healthcare costs and improving quality of care.

ALEXANDRA KRANIDIS, MPH, CPH, CHES Project Manager Suffolk Care Collaborative Alexandra Kranidis earned a Master’s degree in Public Health from Stony Brook University and a BA in Public Health from Franklin and Marshall College. She is a certified Health Education Specialist and Tobacco Treatment Specialist. Prior to working at the Suffolk Care Collaborative, Alexandra served as a Community Health Worker with the Keeping Families Healthy Program at Stony Brook Children’s Hospital and took part in Cognitive Aging research at the World Trade Clinic in Suffolk County. Alexandra currently serves as a Project Manager at the SCC and oversees the Tobacco Cessation Coalition, as well as the Asthma Self-Management Project (3dii).

CHRISTINA KRAUSE CEO British Columbia Patient Safety & Quality Council Christina Krause brings a variety of health care experience to her role as CEO with the BC Patient Safety & Quality Council. Her passion and interests include the use of social change models and network theory in efforts to engage and mobilize stakeholders to improve quality of care. More recently, this has expanded to include social media to create enhanced connections and shared learning. Christina was an author on the Canadian Disclosure Guidelines and serves as Master Faculty with the Institute for Healthcare Communication’s Disclosing Unanticipated Medical Outcomes. ALYSE LARUE Project Manager for Community Engagement Nassau Queens PPS Alyse LaRue is the Project Manager for Community Engagement at the Nassau Queens PPS, managing projects including behavioral health, Tier 1 CBOs, and Patient Activation. She believes in creating a system that provides hope and dignity to the individuals with the most complex needs. Alyse has nearly a decade of experience working in the community based behavioral health space in both clinical and administrative roles. She began her health care career working as a psychotherapist for Medicaid recipients in central Brooklyn and quickly learned that addressing medical, behavioral, and social needs in a person centered way, gets the best outcomes. Alyse is a Licensed Mental Health Counselor and received her Master’s degree from Fordham University.

CHRISTINE LASHER Vice President, Customer Success CHESS Health Christine is the Vice President of Customer Success for CHESS Health. Christine joined the CHESS Health team in 2018 bringing more than 26 years of technology services experience to the implementation of CHESS Health’s innovative A- CHESS platform across various providers addressing substance use disorder (SUD). OKENFE LEBARTY Senior Community Engagement Specialist Maimonides Medical Center Okenfe Lebarty is Senior Community Engagement specialist in the Department of Population Health at Maimonides Medical Center (Brooklyn, NY), which leads a network of health and social service organizations as part of NYS’s Delivery System Reform Incentive Payment program—working together to improve population health and reduce avoidable hospital use. Okenfe works closely with CBOs and other stakeholders. Okenfe holds a Master’s in Urban Policy and Bachelor’s in Political Science from the CUNY Brooklyn College.

RACHEL LEEP Manager of Health and Social Services Analysis Maimonides Medical Center Rachel Leep is Manager of Health and Social Services Analysis in the Maimonides Medical Center Department of Population Health, which leads a network of health and social service organizations as part of the NYS DSRIP program. As part of her role at Maimonides, Rachel manages the ongoing evaluations of programs supported by the Department of Population Health, including the CCB Legal Health Clinic. Rachel has an MPH from Columbia University and BA from Brandeis University.

PATRICIA LEMP, LCSW Assistant Executive Director, Clinic-Based Mental Health Services Westchester Jewish Community Services Patricia Lemp serves on the Executive Staff of WJCS as the Assistant Executive Director of Clinic Based Services, overseeing four licensed mental health clinics, nine satellite clinics, clinical programs for assessment and treatment of inter-personal trauma, and a treatment program for individuals with intellectual and developmental disabilities. Combined, these programs serve over 4,000 individuals annually. She represents WJCS as a founding member of CBHS, a seven county multi- agency IPA, aggressively preparing for value based contracting in the Hudson Valley. Lemp has been recognized by the Westchester Chapter of the National Association of Social Workers for both Distinguished Service (2015) and Social Worker of the Year (2004).

MATT LEWIS Senior Strategic Consultant Primary Care Progress Matt Lewis is an expert on the use of narrative as a tool for connection, teamwork, and leadership development. With more than a decade of experience in classrooms and clinics, Matt has taught on topics ranging from narrative, leadership, trauma, war, American culture, and personal identity at Harvard University (as a Teaching Fellow), Syracuse University (as a visiting researcher), and at Emory University (as a professor) where he completed his doctorate (an ethnography focused on narrative, war, and trauma). Matt also holds an M.T.S. from Harvard Divinity School and a B.A from Georgetown University. While completing his masters, Matt worked at the Kennedy School of Government with Dr. Marshall Ganz teaching Public Narrative. Upon graduating, he accepted a Teaching Fellowship through the Carr Center for Human Rights to continue his work on Public Narrative. And while completing his PhD, Matt was awarded the Dean’s Teaching Fellow in the Graduate Institute for the Liberal Arts at Emory University. An advocate for the use of storytelling to break down barriers and cultivate interpersonal communication, Matt serves as a Senior Strategic Consultant for Primary Care Progress, a national nonprofit working to strengthen primary care teams. He also frequently writes, speaks, and advises organizations and individuals on ways to leverage their personal stories, and the stories of others, for social good and systems change.

LEILANI LOCKETT, C-TTS Tobacco Program Manager, Center for a Tobacco-Free Hudson Valley American Lung Association Leilani, with a background in community healthcare, began her work at the American Lung Association in 2011. She has gained the knowledge and skills to train hundreds of clinicians – from nurses to cardiac specialists utilizing the Public Health Service Guidelines for tobacco cessation. She completed the UMass Medical School Tobacco Treatment Specialist training in 2015 becoming certified in 2018. Leilani took on the role of presenting to high-level administrators to implement sustainable Tobacco Dependence Treatment systems change across Federally Qualified and Mental/Behavioral health organizations. Her successful collaboration with the MHA of Rockland County is our case study for this presentation.

JOANNA LOOMIS Director of Provider Strategy and Transformation North Country Initiative As the Director of Provider Strategy and Transformation at NCI, Joanna is responsible for leading efforts to strengthen the regional clinically integrated network through growth of affiliated IPA, ACO, and MSO. Joanna has a M.A. in social and public policy from SUNY Empire State College and a B.A. in philosophy from St. Lawrence University.

JUAN CARLOS LOPEZ Executive Associate Cooperative Home Care Associates (CHCA) Juan Carlos Lopez has been serving as Executive Associate at Cooperative Home Care Associates (CHCA) since January 2018. As the Executive Associate, Juan Carlos is responsible for providing support to the Executive Team and Board of Directors. Among his responsibilities are ensuring compliance with contractual agreements; reviewing policies and legislation that may impact the home care industry; professional development of elected home care workers serving as members on the Board of Directors; and cultivating and developing internal and external relationships. Juan has prior experience working at CHCA as the Assistant Director of Field Operations for CHCA’s dba Cooperative Home Attendant Program between 2008 and 2012 before relocating to California. Among his responsibilities as the Assistant Director of Field Operations was ensuring the delivery of high-quality services to members in the community living with disabilities. While living in Los Angeles, California, Juan worked at the American Red Cross as the Community Programs Manager. He managed daily operations, strategic planning and volunteer recruitment for four community-based programs in support of a four-year preparedness campaign called PrepareSoCal. As a result, he established community relationships and increased brand visibility in Los Angeles. Juan Carlos was among the first to respond to St Thomas, U.S.V.I. after Hurricane Irma and during Maria where he spent three weeks managing mass care and bulk distribution in Saint Thomas. Juan Carlos holds a BS in Political Science from Williams College.

HANNAH MADDOX, LMSW Program Manager, HEALTHi Project Hospitality Hannah Maddox received her MSW in 2016 from Hunter College and is licensed as a LMSW in NYS. She has been working in the social work field since 2011 in different settings including a half-way house, an alternative girl’s high school program, and a case management agency. Hannah has been involved in multiple DSRIP projects on Staten Island.

NADEEN MAHKLOUF, PHARMD, MPH Manager, Clinical Performance Improvement Staten Island Partnership for Community Wellness Inspired by medicine and helping people, Nadeen always knew she would pursue a career in healthcare. She currently serves as Manager of Clinical Performance Improvement for the Behavioral Health Infrastructure Project. Her knowledge as a Doctor of Pharmacy combined with her passion for helping people led to continuing her education and pursuing a Master’s in Public Health Degree. Previously Nadeen worked at the New York City Department of Health and Mental Hygiene as an infectious disease epidemiologist and a Zika testing call center supervisor.

KISHOR MALAVADE Associate Medical Director Maimonides Medical Center Department of Population Health Kishor Malavade, MD is currently the Associate Medical Director for the Central Services Organization that manages the Maimonides-led PPS, Community Care of Brooklyn. He was previously the Vice Chair in the Department of Psychiatry. Kishor was Chief Medical Officer for a CMMI Health Care Innovation Award to Maimonides in 2012 that developed a model to coordinate care for individuals with serious mental illness through community-based care management. He completed his medical degree at Cornell University Medical College, and his residency training in psychiatry. Kishor was also a fellowship in forensic psychiatry at NYU School of Medicine.

XENIA MALDONADO Assistant Commissioner NYC Department of Homeless Services (DSS/HRA) Xenia is the Assistant Commissioner for the Department of Homeless Services in the Division of Families with Children. She has extensive experience with managing homeless service programs, with a proven track record of program implementation, evaluation and development. Xenia is successful in providing technical support to providers and meeting and exceeding placement target goals. Ms. Maldonado is also skilled at building effective and productive working relationships with organizations that focus on providing quality services to homeless clients. She holds a Master’s Degree in Human Services from Lincoln University and has received an International Honor Society Social Science Certificate.

STACEY MALLIN Director, Program Operations Northwell Health Stacey Mallin is Director of Program Operations at Northwell Health. In this role, she leads a team working with community practices on transformation, evidence-based protocols, integration of behavioral health and leveraging health information technology. Earlier, she served in roles at HANYS, Long Island College Hospital, Visiting Nurse Service of New York and VA New York Harbor Healthcare System. Mallin has a Master of Public Administration from NYU’s Wagner School of Public Service. DANIEL MAUGHAN, FN, BSN, MBA, MSN, FNP-C Senior Vice President Transformation Dan brings over 25 years of clinical and leadership experience to St. Luke's Cornwall Hospital. Dan attained his Associates Degree in Nursing from Capitol Community Technical College in Hartford, CT, both a Bachelor's and Master's degree in nursing, and a Master's Degree in Business Administration, from Mount Saint Mary College, Newburgh, NY. Dan is also nationally certified as a Family Nurse Practitioner through the American Academy of Nurse Practitioners. Dan's experiences span both clinical and administrative roles including staff ED RN at the Detroit Receiving Hospital Level I Trauma Center, locally as a hospitalist Nurse Practitioner, and additionally holding varied healthcare leadership positions. Today, as Senior Vice President of Transformation, Dan's role requires integrating population health models of care not only locally, but throughout the system and regionally. In addition to his role in system transformation, Dan oversees the Emergency Department, Trauma Services, Cancer Services, Dialysis, Wound Care, Pain Management, Cardiac and Pulmonary Rehabilitation, Physical Medicine, Marketing and Public Relations.

JON MARELLI, PSYD Program Manager, BH and Primary Care Integration NYU Langone Brooklyn PPS Jon Marelli, PsyD is Program Manager of Behavioral Health and Primary Care Integration at NYU-Langone- Brooklyn. His clinical training at Yale University focused on providing evidence based treatment using a recovery-model for those with severe mental illness (SMI). Jon’s passion is in advocating for and integrating behavioral health and primary care services, whether it be on improving the physical health of those with SMI, or integrating BH services within primary care sites. His focus now expands to BH/PC integration goals aligned with the NYS-DSRIP program, particularly improving physical health of those with SMI, and designing projects to assist those with SMI improve adherence to outpatient care.

ALYSE MAROTTA Administrative Manager, Behavioral Health Suffolk Care Collaborative Alyse Marotta is currently the Administrative Manager for Behavioral Health for the Suffolk Care Collaborative, overseeing the 3ai and 4aii projects. She has earned a Master of Public Health degree and an Advanced Certificate in Health Communications from Stony Brook University. She also has a BA in Health and Human Kinetics from Ohio Wesleyan University. Prior to her position with the Suffolk Care Collaborative, Alyse served as a Community Health Educator with Calvert Memorial Hospital in Maryland, and as a Project Staff Assistant and Grant Coordinator for the Department of Family, Population, and preventive Medicine at Stony Brook Medicine.

ANNETTE MARZAN Director of Community Outreach Projects Planned Parenthood Mid-Hudson Valley (PPMHV) Annette Marzan is the Director of Community Outreach Projects for Planned Parenthood Mid-Hudson Valley (PPMHV) where she has been employed for 23 years. She oversees several asset based projects at PPMHV. She served as Chair of the Board for the Newburgh Girl Power Program at St. George’s Episcopal Church from 2010 through 2015 and continues to be a member of the Advisory Board. Annette advocates for the Latino Community and along with another PPMHV colleague, oversees the Poughkeepsie and Newburgh Healthy Black and Latino Coalitions, which were formed through a Diversity Initiative by Planned Parenthood Mid-Hudson Valley in May 2014.

ADRIANA MATIZ, MD Associate Professor of Pediatrics, Medical Director of the Center for Community Health Navigation Columbia University Medical Center Adriana Matiz, MD, FAAP, is an Associate Professor of Pediatrics at Columbia University Irving Medical Center and the Medical Director for the Center for Community Health Navigation at NewYork-Presbyterian.

KATHLEEN MCAULIFF, PHD Project Director Albert Einstein College of Medicine Kathleen McAuliff is a community psychologist and researcher who is interested in how social determinants of health (in particular, poverty) impact patient outcomes and contribute to population health disparities. Prior to working in New York, she lived in Chicago, where she evaluated the impact of local and state-wide initiatives and policies on marginalized communities (e.g., Medicaid recipients, people of color, and people with disabilities). Her master’s thesis focused on undergraduate attitudes towards poverty, and her doctoral dissertation focused on predictors of health- related quality of life among youth with spinal cord injury.

SOPHIA MCINTYRE, MD, MPH, MBA, FAAFP, CPE Chief Medical Officer Hudson River Healthcare, Inc Dr. Sophia McIntyre serves as Chief Medical Officer for Hudson River Healthcare, a network of 29 federally qualified health centers in the Mid-Hudson Valley and Long Island of New York. In her role, Dr. McIntyre develops and oversees the clinical quality strategic direction of the organization. Dr. McIntyre works collaboratively across disciplines to cultivate a culture of high performance. Dr. McIntyre serves as a change agent bolstering team member participation and highlighting the inherent value of each team member. She is passionate about leadership and consistently strives to develop innovative strategies to engage, empower and educate patients, clinicians and staff with the overarching goal of improved clinical outcomes and enhanced patient experience. Dr. McIntyre’s enthusiasm to learn and grow coupled with her desire to respond to the rapidly changing health care landscape served as the impetus for her to purse and complete her Master’s of Business Administration and her year-long fellowship as a Baldridge Executive Fellow in 2016.

NATALIE MCGARRY Senior Analyst Maimonides Medical Center Natalie McGarry is a Senior Analyst for the Maimonides Medical Center Department of Population Health, which leads a network of health and social service organizations as part of the NYS DSRIP program. As part of her role at Maimonides, Natalie conducts data analysis primarily using claims data to monitor the utilization and performance quality of DSRIP and other programs.

ASHLEE MCGLONE Provider Relations Manager Suffolk Care Collaborative Ashlee McGlone is a Provider Relations Manager at the Suffolk Care Collaborative (SCC) and is responsible for building and cultivating relationships with providers to ensure successful participation in the DSRIP program. In this role, Ashlee develops educational materials and programs to support partners and facilitates an action planning process focused on improving performance metrics. MARY MCLAUGHLIN Executive Director Adirondack Health Institute (AHI) Mary is currently the Executive Director at AHI responsible for assuring success with all aspects of DSRIP transformation for the Glens Falls Population Health Network, one of the five focused areas in our PPS. In addition, the AHI Health Home Lead organization is one of her key responsibilities. Previous experience includes various director and management positions at and an RN position at NYU Medical Center. Additional roles included Nursing Unit Management, Director Clinical Informatics, Quality Specialist, Case Management Director, and Patient Care Service Director. Mary earned her BSN from Adelphi University and MBA from SUNY Albany, both with honors. Mary has also presented at several ACMA Case Management Annual Conference, and published articles and book chapters on various subjects.

EILEEN MCMANUS VP, Operations HRHCare, Community Health

MAGGIE MEEHAN Director of Pilot Projects BronxWorks Maggie Meehan, MA, MPH, RDN is the Program Director for Pilot Projects at BronxWorks. Maggie oversees programs addressing high utilizers in ED that are homeless, a trauma-informed program to guide shelter residents’ transition to permanent housing, and a peer educator program for People Living with HIV/AIDS. Her work led to a desire to respond to the impact of the social determinants of health on low-income populations. Maggie completed her MPH/RD from Hunter College and spent 10 years as a nutrition educator at City Harvest, Inc. a food rescue organization in New York City, most recently as the Director of Nutrition Education.

KAREN MEJIA Workforce Development Consultant 1199SEIU Training and Employment Funds Karen Mejia is the DSRIP workforce consultant at the 1199SEIU Training and Employment Funds. Her professional experience includes the non-profit sector, community development, and governmental policy. Before assuming her current role, she worked extensively in the Hudson Valley region in various public health education campaigns. Prior to this role, Marissa has ten years of experience as a primary therapist in the Mount Sinai Hospital Continuing Day Treatment Program working with patients diagnosed with serious mental illnesses and co-occurring substance use disorders. During her tenure, Marissa also served as the Quality Assurance and Performance Improvement Coordinator and participated in the development and implementation of the Partial Hospital Program. DODI MEYER, MD Professor of Pediatrics at Columbia University Medical Center, Director, Community Pediatrics Columbia University Medical Center Dr. Meyer’s main areas of work are health disparities, community health and cultural competency. She has expertise in developing and sustaining community-academic partnerships for the purpose of training, service and research. As director of Community Pediatrics, she oversees several population health initiatives that address major challenges to children’s health through the interlinking contexts of biology, family and community (such as obesity, mental health, and early literacy). She oversees the Community Pediatrics training for pediatric residents using Service Learning as the main educational methodology therefore embedding the training within established community-academic partnerships. As a member of the Division of Child and Adolescent Health, she sees patients and teaches residents and medical students at a community-based, hospital-affiliated practice in Washington Heights.

MARY MORRIS Director, Workforce Innovation Bronx Partners for Healthy Communities Mary Morris began her career teaching school in Australia. She also worked as a guidance counselor, human relations director and an actress, before beginning her career in health care at Montefiore Medical Center, where she was the Director of Human Resources Development. Mary also worked as the Director of Organizational Development at St Vincent’s in and became the Vice President for Human Resources in 2007. Returning to the Bronx as the Director of Workforce Innovation for Bronx Partners for Healthy Communities, Mary views this job as a gift, having the potential to be the most impactful work of her career. Mary has an EdM and an MA in Applied Human Development from Teachers College, Columbia University.

SCOTT MORTON Director of Clinical Services Endeavor Health Service Scott Morton has been with Endeavor for 19 years and is currently responsible for the oversight of outpatient behavioral health clinic services across Erie, Niagara, Orleans, and Monroe counties. He is a graduate of the State University of New York at Buffalo’s Graduate Program in Social Work and currently serves as the chair and on the board of the Treatment Committee of the Western New York Chemical Dependency Consortium. KRISTEN MOUTON Senior Administrative Director, DSRIP Staten Island University Hospital - Northwell Health Kristen M. Mouton, MBA is the Senior Director for DSRIP at Staten Island University Hospital. Kristen received her bachelor degree in psychology from Monmouth University (2007) and her MBA with a concentration in Health Care Administration from Monmouth University (2010). She is currently pursuing a PhD in Health and Medical Sciences at Seaton Hall University. Kristen is currently responsible for the implementation and oversight of all DSRIP initiatives at Staten Island University Hospital.

JEFFREY MUSMACHER, MBA Senior Director Northwell Health Jeffrey Musmacher is a Senior Administrative Director in the Department of Medicine. Musmacher has an MBA with 25 years of healthcare experience with 18 of those working in Radiation Medicine with advanced technologies, information systems and quality programs. He has authored papers and presented locally, nationally and internationally and brought his experience to the Northwell Health Medicine Service Line to lead the Patient Centered Medical Home (PCMH) initiative. In just under two years, his team has successfully completed the recognition process at nearly 40 practices and continues to transform practices at Northwell Health. RACHEL NAIUKOW, MS, MPH Program Coordinator NewYork-Presbyterian Hospital As a Program Coordinator for the Division of Community and Population Health at NYP, Rachel manages both Workforce Development and Training as well as Cultural Competency and Health Literacy efforts for the NYP Performing Provider System. In this role she has worked to support PPS projects and operations through training and advocacy/support for a person-centered, cross-cultural approach. With a background in public health, her main professional interests involve the intersection between social determinants of health, social justice, prevention, community and system capacity building and engaging in meaningful community collaboration. Rachel received her Master of Science in Interdisciplinary Studies in Biological and Physical Sciences at Touro College of Osteopathic Medicine in Middletown, New York, and is a graduate of Tulane University School of Public Health and Tropical Medicine.

KAREN NELSON, MD Senior Vice President for Integrated Delivery Systems Maimonides Medical Center Karen Nelson, MD, is Senior Vice President for Integrated Delivery Systems at Maimonides Medical Center and Chief Medical Officer of its Department of Population Health. She is responsible for the development, implementation, and ongoing oversight of the clinical and care management programs that will ensure the achievement of CCB’s DSRIP goals. Previously, Dr. Nelson served as the CEO and Medical Director of the Union Health Center, Vice Chair of Medicine at Maimonides, and Medical Director of Dorchester House Community Health Center, a multi-service center in one of Boston’s poorest neighborhoods. Dr. Nelson graduated from Mount Sinai School of Medicine, completed her residency in internal medicine at Boston City Hospital, and received an MPH degree from Boston University School of Public Health.

JEANNE NISSEN Senior Project Manager Northwell Health Jeanne Nissen has been a Senior Project Manager at Northwell Health since December 2017. She works with the voluntary community physicians who are active members of the clinically integrated network IPA. Together with the providers and staff, Jeanne transforms practices into Patient Centered Medical Homes while integrating the initiatives of the Delivery System Reform Incentive Payment Program. Jeanne came to Northwell with over 18 years of Medical Practice operations experience. She is a certified in Cultural Competency & Health literacy. VICTORIA NJOKU-ANOKAM, MPH Director of Behavioral Health and Care Management Initiatives Staten Island Performing Provider System Victoria is a health management professional with over 10 years of experience implementing large-scale population health management transformations. JASON NOXON Director of Communications Alliance for Better Health Jason is at Alliance to tell the story of their organization, partners, and ultimately their Medicaid members. His background is in information technology, but he always had a desire to work in communications (and honed his skills while still in IT by being the person who could disseminate technical information to non-technical people), and, luckily, he had the opportunity at the Alliance to become part of the Communications Team. Jason currently lives in Saratoga Springs with his family, and he enjoys doing Voice Over, writing, and making music. He currently hosts the podcast for Alliance’s Storytelling Collaborative, as well as his own podcast called WriteBoom, where he interviews writers about their challenges, and their process.

JENNIFER OH DSRIP Pharmacy Liaison NYU Langone Brooklyn PPS Jennifer Oh is a Certified Pharmacy Technician who graduated from St. John’s University with a Bachelor’s in Health and Human Services with a concentration in healthcare. As a marketing representative for MetroPlus Health plan, she enrolled and educated families in three languages (English, Spanish, and Korean) on the benefits of having health insurance. She is currently the Pharmacy Liaison for the NYU Langone Brooklyn PPS where her goal is to improve medication management measures in the Medicaid population.

JENNIFER O'REILLY, MD Norwich Health Center Medical Director Bassett Healthcare Network Dr. O'Reilly is a Family Medicine physician with a special interest in treatment of addiction. She received her MD from SUNY Upstate and completed her residency at the University of Massachusetts Medical School in 2011. She has been a physician at Bassett since she completed her residency, and was named Medical Director of the Norwich Health Center in 2014. Dr. O'Reilly received her DATA waiver in 2016 and has since treated dozens of patients struggling with opioid addiction. She is currently working towards Board Certification in addiction medicine, which she aims to complete by the end of 2019.

KESHANA OWENS-CODY Director, Community Relations Alliance for Better Health Keshana is the point of contact for all CBO partners and provides support to them as they target their efforts to serve patients holistically in their communities. Prior to her appointment at Alliance for Better Health, Keshana resided in Western, NY and provided support to youth and families in both OMH Residential Treatment Facilities (RTF) and the OCFS Home and Community Based Medicaid Waiver: Bridges to Health. Keshana provided support to families who were receiving mental health services and learned very quickly that a majority of her focus would be making sure that basic needs were met. Keshana led a team of community health workers that served six counties in Western, NY. Using the knowledge gained from residential she sought to bridge the gaps between providers, and community-based organizations. Keshana developed professional development opportunities for the region that would focus on assessing needs, care planning, and community resource sharing. This work led her to the Capital Region where she a managed statewide training contract for the Bridges to Health Program, and her most recent work at Healthy Capital District Initiative. At HCDI, Keshana led workgroups that focused on the NYS Prevention Agenda (Diabetes-Obesity, Behavioral Health, and Prevention Agenda Workgroup) and the Care Coordination Task Force. Each of these facilitated workgroups included both the development and execution of a workplan, product development and both evidence based practice and community resource sharing. Keshana holds a bachelor’s degree in Criminal Justice and is currently pursuing her Master’s in Human Resource Management at the University of Connecticut. Keshana has been certified as a CANS-NY (Child and Adolescent Needs and Strength Assessment) Trainer, Getting Ahead Facilitator, and continues to provide professional development surrounding care planning, and community navigation in the Capital Region.

KATHY PANDEKAKES Chief Executive Officer Human Development Services of Westchester Kathy Pandekakes has been involved in the evolution of community-based direct-care services for vulnerable populations in New York for over 30 years. As Chief Executive Officer (CEO) of Human Development Services of Westchester, she is integral in moving the agency toward value-based care, with the Quadruple Aim of improving an individual’s experience of care, improving their health, reducing the cost of providing care and focusing on Care Team Wellness to foster "Joy In Work". Pandekakes also represents CBHS on the MHVC Leadership Steering Committee.

EMILY PAPE Director of Population Health Care Compass Network Emily Pape, PhD, is the Director of Population Health at Care Compass Network. Emily has a background in health care policy, health services research, and health informatics. She holds a PhD from the University of Michigan School of Public Health and a Master’s in Economics. Emily’s team at CCN is responsible for supporting the PPS with analytics and data warehousing, managing a performance incentive program for CCN Partners, and overseeing a program for innovative pilots to improve DSRIP performance metric results. Emily is a lead designer of the Cohort Management Program. In her role at CCN, Emily is also developing population health analytic services and tools for CCN partners outside of DSRIP and helping to cultivate evidence-based approaches to improve patient outcomes.

MARK PAPISH, MD Medical Director, Emergency Department Mid-Hudson Regional Hospital Mark Papish, MD, is the Medical Director of the Emergency Department at Mid-Hudson Regional Hospital – a Member of WMCHealth Network – in Poughkeepsie, NY. Dr. Papish has worked as an attending physician throughout the Hudson Valley and in Southern California. He is a member of the American Academy of Emergency Medicine and a Diplomate of the American Board of Emergency Medicine. Dr. Papish completed his training in Emergency Medicine at Harbor UCLA Medical Center in 2005, and earned his medical degree at Stony Brook University School of Medicine.

NEIL PASCO, MD Residency Program Director NYU Langone Brooklyn PPS Neil Pasco, MD, is the program Director for the Family Health Center at NYU Langone Primary Care Internal Medicine Residency Program and Associate Medical Director of Quality. Dr. Pasco focuses on resident education, health care equity, challenging healthcare disparities and utilizing the resources of the FHC network to deliver high quality care in a culturally sensitive context. Additionally, Dr. Pasco serves as the clinical lead for the NYU Langone Brooklyn PPS DSRIP Asthma and Diabetes initiatives. Prior to joining the FHC, Dr. Pasco was the Associate Program Director in Internal Medicine, and Director of Ambulatory Care services at Kingsbrook Jewish Medical Center. Previously, he served as a hospitalist and clinical instructor for NYU Langone Medical Center's Division of General Internal Medicine and has also worked in the Montefiore Medical System as a Hospitalist at Jack Weiler Hospital/ Albert Einstein College of Medicine. Dr. Pasco earned his medical degree from the State University of New York, Downstate College of Medicine in Brooklyn. He did his training in Internal Medicine at the University Of Miami Miller School of Medicine at Jackson Memorial Hospital. Dr. Pasco has presented on several research studies focused on healthcare disparities and inequity in asthma, diabetes, Hepatitis C, HIV screenings and on quality improvement. He is also published in the American Journal of Medical Quality on the proceedings of medical quality and Journal of Clinical Oncology for his study on cancer screenings in an urban community.

NEIL PATEL Director, Performance Improvement Mount Sinai PPS Neil Patel serves as the Director of Performance Improvement for the Mount Sinai PPS. He is responsible for leading efforts with PPS partners to achieve pay for performance (P4P) measures and improve overall patient outcomes. He also oversees deliverables across the ten MSPPS DSRIP projects and manages both the MSPPS’ Clinical Quality Committee and Chief Medical Officer Council. Prior to his role with MSPPS, Neil managed the DSRIP Program at Memorial Hermann Health System in Houston, Texas, valued at $211M in incentive payments. Jessi then transitioned into her current role as Nutrition Coordinator at Island Harvest Food Bank managing the Food as Health Program at Long Island Jewish (LIJ) Valley Stream Hospital. KIMBERLY PETERS, PT, DPT Vice President of Operations, Home and Community Based Care Catholic Health System In her current role, Kim is responsible for the day to day operations of Catholic Health’s Niagara County Home Care Agency and manages the Authorization and Utilization department for Catholic Health’s home health care division, including their Palliative Care/Telemonitoring Pilot program. She began her career as a staff physical therapist in outpatient orthopedics and later transitioned to the home health care industry where she served as a field clinician, rehab team trainer and orthopedic rehab manager. Kim is a graduate of the State University of New York at Buffalo with a Bachelor of Science Degree in and Exercise Science. She attended Daemen College and received a Doctorate in Physical Therapy and later attended Medaille College, where she received an Integrated Health Care Leadership Certificate toward completion of her MBA. DIEGO PONIEMAN Chief Medical Director SOMOS Community Care As Chief Medical Officer at SOMOS Community Care, Dr. Diego Ponieman is responsible for SOMOS’s clinical projects, which are focused on reducing avoidable hospitalizations through proactive, coordinated care and evidence-based interventions with Medicaid members, who are at risk for or are experiencing chronic conditions such as diabetes, hypertension and asthma. SHAUNA PORTEUS Community Services Librarian Yonkers Public Library Shauna Porteus is the Community Services Librarian for the Yonkers Public Library and brings over 10 years of experience working in public libraries throughout Massachusetts, Illinois, and New York. In her current role, she helps increase the library’s community engagement and outreach to new and underserved audiences and builds strategic partnerships to create programs that better serve library patrons. She is an advocate for introducing public health programs in libraries and helped spearhead the pilot project to bring case management services onsite at YPL. She is an active volunteer and currently serves as a mentor for Children’s Village in Dobbs Ferry.

CELINA RAMSEY Director of Health Literacy, Diversity and Outreach Staten Island Performing Provider System Celina Ramsey is the Director of Health Literacy, Diversity and Outreach at Staten Island Performing Provider System (SI PPS). She is passionate about promoting health equity for all of Staten Island’s diverse communities. Celina completed her Master of Science in Health Communication from Boston University and recently celebrated her 10 year anniversary as an ESL and Health Literacy teacher. She is thrilled to be working with SI PPS and their extensive partner network during this transformational time in healthcare and wishes everyone well on their personal journey towards wellness.

DIDI RAXWORTHY, TTS Director, Center for a Tobacco-Free Hudson Valley American Lung Association Didi has been the Director of two NYS DOH Bureau of Tobacco Control Grants; first, as a Technical and Training Specialist for POW’R Cessation Center becoming Director after three years. The Cessation Center grant ended and the team responded to the Health Systems for a Tobacco-Free New York RFA, and initiated work as the Center for a Tobacco-Free Hudson Valley in July of 2015. This team’s work plan is dedicated to partnering with providers concentrated on improving health outcomes with disparate populations in the seven counties of the Hudson Valley.

MARLENE RIPA Senior Director of Network Integration Montefiore Hudson Valley Collaborative Marlene has worked in health care for 15 years specializing in network development and management, quality improvement, process redesign, and technology implementation. In her role of Senior Director of Network Integration for Montefiore Hudson Valley Collaborative, Marlene is responsible for the design and implementation of a comprehensive network-wide engagement strategy. Most recently Marlene has led the standing up of data informed regional risk hubs referred to as Communities of Care across the MHVC Network. These forums, designed to support provider engagement and performance improvement, have utilized both qualitative and quantitative data to inform the development and deployment of supportive resources (including funding, workflow re-design and training) to meet the unique needs of each community; and the unique needs of each patient.

ELISABETH RYDEN BENJAMIN, MSC, JD Vice President of Health Initiatives Community Service Society Elisabeth Benjamin is the Vice President of Health Initiatives at the Community Service Society, where she supervises health policy, health advocacy, and consumer health assistance programs that annually help over 130,000 consumers and small businesses enroll in and use health insurance or access low-cost health care through a network of 50 community based organizations and live answer helplines. Benjamin has received many honors, including: Crain’s Notable Women in Healthcare (2018); the New York State Health Foundation Luminary Award (2016), the Progressive Leadership Award (2014), the Families USA State Health Advocate of the Year Award (2011), and the Public Health Association of NYC Special Merit Award (2009). VICTORIA SALE Independent Consultant, VSA, Inc. Population Health Victoria Sale operates as an independent consultant, focused on population health initiatives. Victoria grew and lead the Camden Coalition of Healthcare Providers, a community based non-profit focused on improving the lives of individuals with complex health and social needs. A nurse and teacher by training, Victoria developed COACH, the Coalition’s nationally recognized approach to Healthcare Hotspotting. COACH is a training program designed to engage individuals and communities in authentic healing relationships. The demand for COACH quickly grew and is now being taught to clinicians at more than six health systems and over 2000 health-professions students across the country. Over the last five years, Victoria has mentored several health systems to design interventions that serve vulnerable populations. In each of these systems, the scope of work has been diverse, including: launching health system or state-wide Medicaid Accountable Care Organizations, growing cross-sector community collaboratives, and directly mentoring staff in face-to-face care coordination interventions. Victoria holds a Master of Science in Nursing from the University of Pennsylvania with a focus in Health Leadership. Her experience at the Coalition has grounded her in the belief that cross-sector collaborations are essential to bolster the resilience of communities. Recognizing the similarities between healthcare and education, she is now using lessons from healthcare Hotspotting to design cross- sector interventions between school districts, health systems, students, and families who fall outside traditionally defined school norms of performance, engagement, and behavior.

ABRAHAM SARAYA Director of Data Strategy Northwell Health Dr. Abraham Saraya is the Northwell Health Director of Data Strategy. He has over 10 years of experience in health Information Technology, with an emphasis on Medicare, Medicaid, and commercial regulations and reimbursement. He oversees a full range of analytical services that support Clinical Quality, Care Management, Utilization Management, and Population Health. His areas of focus include Data Management, Data Analysis, Reporting and Predictive Analysis. Dr. Saraya is a graduate of Cairo University Medical School. He also earned a Master of Science degree in Medical Informatics at SUNY Downstate Medical Center, and is currently pursuing his Doctorate in Public Health with a focus on Epidemiology.

MARK SASVARY, LCSW Chief Clinical Officer Coordinated Behavioral Health Services, Inc. Mark Sasvary has close to 20 years of experience working in behavioral health. Sasvary has worked extensively in inpatient psychiatry and community mental health, as well as research, policy, and advocacy in the field of behavioral health. He has training and experience in Cognitive Behavioral Therapy including intensive training from the Beck Institute, the Albert Ellis Institute, the Linehan Institute, and the International OCD Foundation. Sasvary is the Chief Clinical Officer for Coordinated Behavioral Health Services, Inc. (CBHS), which is a partnership of progressive community behavioral health providers serving seven counties in the Hudson River Region of New York. CBHS was formed to help behavioral health providers adapt to a changing healthcare landscape and to play an active role in the transformation of the system of care. CBHS is working to help their providers operate more efficiently, to meet the challenges of sustaining their services and workforce, to improve the experience of clients, and to achieve better overall health and quality of life outcomes for the population that they serve. Sasvary also has a small private practice in Rhinebeck where he specializes in treating clients with obsessive compulsive disorder and post-traumatic stress disorder.

KRISTEN SCHOLL VP of Population Health Alliance for Better Health Kristen chose to work for the Alliance for Better Health because DSRIP is a phenomenal, possibly once in a lifetime opportunity, to make a difference for the Medicaid population by removing barriers to health equity using federal and state funds. In her role, success means facilitating the provision of technology solutions, funding or guidance to Alliance partners to assist with collaboration and innovation. Sometimes, just bringing the right people together in a room (often traditional healthcare providers and community based organizations) can result in discussion and workflow redesign that fosters meaningful change in the community. Prior to joining Alliance in January 2016, Kristen was introduced to Medicaid Redesign and DSRIP from the perspective of a Medicaid Managed Care Organization (MCO). Most recently, she was the Medicaid Operations Director at MVP Health Care. Kristen has more than 25 years of experience in various roles with MCOs which has provided me with the expertise to contribute to the Alliance goal of improving the health of our community and preparing partners for success in a value based payment environment.

ERIC SCOTT Case Manager CLUSTER Community Services Eric Scott has been with CLUSTER Community Services for the past 18 years. During this time, he has built a relationship of trust with his peers, vendors, and most importantly with the people he serves. He began as a Mental Health Worker and has dedicated himself to honing his craft through ongoing trainings and support from his supervisors. He embraces the core values of independence, a person-centered approach and growth. He consistently provides service to assist individuals to become productive citizens in their community. Eric is proud to be one of the Outreach Case Managers for the Yonkers Community.

RONA SHAPIRO Executive Vice President 1199 SEIU Home Care Division Rona Shapiro has been active in the labor movement and the fight for social justice for over 40 years. She became active in 1199SEIU in 1976, while working as a paralegal for the Legal Aid Society. In 1986, she was elected an 1199 Vice-President working in the Home Care Division, which now represents over 75,000 homecare workers in NYC, Westchester and Long Island. In January 2011, Rona became the Executive Vice-President of the Home Care Division. She is a Trustee of the Home Care Benefit Fund, the Home Care Pension Fund and the Home Care Education Fund, all of which she helped to establish. She also helped establish the Basil Patterson Scholarship Fund so that homecare workers could attend college and go on to be nurses, social workers and other healthcare professionals. Together with the union leadership, Rona has fought to better the lives of homecare workers, both economically and fighting for them to be recognized for the important work they do in the community. Today, she is working with managed care companies, St Barnabas PPS, hospitals and 1199 home care agencies to develop new roles for the home care workers, and use technology to become a more valued part of the healthcare team.

MARIEL SHULL Pharmacy Utilization Management Coordinator NYU Langone Brooklyn PPS Mariel Shull earned her Doctor of Pharmacy degree from St John’s University, completed PGY-1 Pharmacy Practice Residency at Einstein Medical Center Philadelphia, is a board certified Ambulatory Care Pharmacy and certified Asthma Educator. As Transition of Care Pharmacist, she developed and implemented a readmission reduction program at Einstein Medical Center in Philadelphia focusing on Medicare patients at high risk for re-admissions. As Clinical Outcomes and Therapeutic Optimization Manager at Amerihealth Caritas, she oversaw disease state management programs in government insured patients. Currently, as Pharmacy Utilization Management Coordinator at NYU Langone Health, she focuses on improving quality measures in the Medicaid population.

BONNIE SLOMA Senior Vice President People Inc. As Senior Vice President, Sloma is responsible for the agency’s healthcare business development in addition to healthcare quality standards as the agency continues to position itself for success in a changing managed care environment. Previously, Sloma was the Vice President of Ambulatory and Diagnostic Services at Erie County Medical Center. In this role, she was responsible for the development of policies, procedures, new services, programs and quality oversight for the organization’s outpatient ambulatory and diagnostic clinics. She held the position of Vice President of Heritage Health Care Group, LLC & Vice President of Clinical Services/DON at Our Lady of Peace Nursing Care Residence. Bonnie graduated from Niagara University with a Bachelor of Science in Nursing and has served on the Niagara County Community College Board of Trustees since 2008.

KAYLA SPENCE Manager, Program Implementation & Partner Engagement Maimonides Medical Center Kayla Spence is Manager of Program Implementation and Partner Engagement for the Department of Population Health at Maimonides Medical Center in Brooklyn, NY, which leads a network of health and social service organizations as part of the NYS DSRIP program. As part of her role at Maimonides, Kayla works with stakeholders at partner organizations to design and implement integrated models of care for high risk high need populations with a particular focus on transitions of care. Kayla holds a BS in Healthcare Administration from the St. John’s University, and is currently working on her MHA at Seton Hall University.

ELANA SPIRA, PHD Director of Research Westchester Jewish Community Services Elana G. Spira, PhD, is Director of Research and Evaluation at Westchester Jewish Community Services, overseeing evaluation and integration of evidence-based practice across WJCS programs. In addition, she is Adjunct Lecturer at the NYU Silver School of Social Work and Clinical Assistant Professor of Child and Adolescent Psychiatry at the Child Study Center of Hassenfeld Children’s Hospital at New York University (NYU) Langone Health. She is coauthor of several peer-reviewed articles and two books: Organizational Skills Training for Children with ADHD: An Empirically Supported Treatment and The Organized Child: An Effective Program to Maximize Your Kid's Potential—in School and in Life.

ZOË STOPAK-BEHR, MPH, MIA Director, Care Coordination Systems SBH Health System Zoë Stopak-Behr oversees ambulatory care coordination and hospital-based care transitions for SBH Health System. From 2015-2018 Zoë led care coordination programming for Bronx Partners for Healthy Communities (BPHC), the PPS for which SBH Health System acts as fiduciary. As Director of BPHC’s DSRIP Care Coordination Initiative, Zoë led the implementation of the Critical Time Intervention (CTI) program with four community-based CTI providers and 19 unique referral sources across the Bronx. Zoë also oversaw various other DSRIP initiatives to address social determinants of health among Medicaid patients in the Bronx—with pilots focused on housing, food insecurity, and non-emergent medical transportation—as well as efforts to improve access to needed care through the development of a PPS-wide referral management system. Zoë has experience in both domestic and international program implementation and research. She worked with the United Nations International Organization for Migration (IOM) in Washington, D.C. and the Dominican Republic on migration health and policy, and participated in the United Nation’s disaster response immediately following the 2010 earthquake in Haiti. Zoë has conducted research on behalf of the NYC Center for Innovation through Data Intelligence, Columbia’s Averting Maternal Death & Disability Program, the National Center for Disaster Preparedness, the Center for Migration Studies, and the Tomás Rivera Policy Institute. She holds a Master’s in Public Health and a Masters in International Affairs from Columbia University.

Dr. Stroupe is Board Certified in Psychiatry and Addiction Medicine with a particular passion for taking care of young adults with opioid use disorders. Dr. Stroupe completed psychiatry residency training at the University of North Carolina - Chapel Hill, has been working in Brooklyn for the past 14 years, and has 20+ years’ experience treating general psychiatric and substance use disorders.

SUE ELLEN STUART Executive Director Visiting Nurse Service of Ithaca Sue Ellen Stuart is the Executive Director of the Visiting Nurse Service of Ithaca and Tompkins County (VNS). In addition to her role leading her organization, Sue Ellen has played a pivotal role in supporting the Care Compass Network PPS in the design and implementation of the Care Transitions Program and is a co-chair of the Project Advisory Council (PAC) Executive Committee. VNS is a key partner in CCN's Care Transitions project, the Community- based Navigation project, and has received a CCN Innovation Fund grant for their work in public housing complexes to improve outcomes for residents with chronic conditions.

SHARI SUCHOFF Vice President for Population Health Policy & Strategy Maimonides Medical Center Shari Suchoff is the Vice President for Population Health Policy and Strategy in the Maimonides Medical Center Department of Population Health, overseeing the implementation of key aspects of DSRIP projects and programs. Previously, Ms. Suchoff served as Legislative Assistant for Health and Labor in the Office of Senator Kirsten E. Gillibrand, advising on health care issues during the consideration of comprehensive health care reform, and as Senator Charles E. Schumer’s Legislative Aide for Health and Welfare. Ms. Suchoff has both a B.A. in Government and Politics and an M.P.P. with a specialization in Social Policy from the University of Maryland.

WAYNE TERIS, MD, FAAFP Chief Medical Officer Care Compass Network Wayne Teris, MD, FAAFP, joined Care Compass Network in September 2017, as the Chief Medical Officer. He earned his MD from the University of Pittsburgh School of Medicine in 1979, and a Graduate Certificate in Biomedical Informatics from Oregon Health and Sciences University in 2011. Dr. Teris completed a Family Medicine residency at The Washington Hospital and is Board Certified in Family Medicine. He has been named as a Fellow of the American Academy of Family Physicians. He practiced Family Medicine for 32 years, then was a full-time hospital Physician Advisor and Chief Medical Information Officer for three years prior to joining CCN.

MARCY THOMPSON Chief Strategy Officer Alliance for Positive Change Marcy has worked diligently on behalf of diverse communities impacted by the HIV/AIDS epidemic for nearly 20 years and advocated on behalf of people living with HIV/AIDS in New York City through involvement in community planning in partnership with government agencies, medical institutions, Managed Care Organizations/Special Needs Plans, Health Home/PPS Networks and a multitude of service organizations. Marcy is a long-term voting member of the NYC HIV Health and Human Services Planning Council and serves as Co-Chair of the Needs Assessment Committee. She currently participates in several state and city-wide forums including but not limited to: DOH MRT Health Home- Managed Care Workgroup, AIDS Institute Technical Assistance Group, CCMP HH Governance & Operations Committee, NYP PPS IT/Financial Committee, MSPPS Clinical Quality Committee and OCHPPS Strategic Advisory Workgroup. Previously, Marcy participated in the Health Home Learning Collaborative, Greater NY Hospital Association (GNYHA) DSRIP Care Plan-RHIO Collaboration and served on the FPWA AIDS Leadership Coalition Steering Committee.

PRAJAKTA VAGAL Senior Director, Office of Behavioral Health NYC Health + Hospitals Praj (Prajakta) Vagal is the Sr. Director in the Office of Behavioral Health of New York City Health + Hospitals focused on implementing individual centered, recovery oriented and integrated service delivery models in all NYC H+H’s behavioral health services. Praj began her career at NYC H+H/Elmhurst as a NYU fellow in 2004, assisting the ambulatory care division in developing chronic disease management initiatives and patient registries. She was instrumental in implementing a Care/Case Management department at two H+H Hospitals, providing patient centered care management over continuum. Praj is an International Medical Graduate and holds Master of Public Administration in Health Policy and Management.

SUE-ANN VILLANO Senior Director of Client Services, Marketing & Outreach NY Care Information Gateway Sue-Ann Villano has been with NYCIG since 2014. As the current Senior Director of Client Engagement, she has more than 20 years’ experience in healthcare operations and marketing. DORELLA WALTERS Sr. Director of External Program Affairs God's Love We Deliver Dorella Walters is the Senior Director of External Program Affairs at God’s Love We Deliver, an agency that in 2018, delivered 1.8M nutritious, medically-tailored, high-quality meals to men, women and children living with HIV/AIDS, cancer and other life-altering illnesses in New York City and Hudson County, New Jersey. Dorella is responsible for representing the agency in both healthcare and community settings to establish and maintain contracts with managed care organizations, hospital systems and community-based organizations. She previously oversaw client enrollment and care management, contract management and outreach efforts for the agency. She is a graduate of Long Island University and received her Master’s in Public Administration from Baruch College, and has worked in the non-profit arena of social and nutrition services for over fifteen years. She also serves on the HIV Health and Human Services Planning Council of New York City and currently serves as co-chair of its Integration of Care Committee.

AMY WHELAN, LMHC Program Coordinator, CORE The Bridge, Inc. Amy Whelan received an MA in Mental Health Counseling at the University of Central Florida and a BS in Family & Child Sciences at Florida State University. As a Licensed Mental Health Counselor, she has worked in clinical and community-based settings to provide easier access to quality behavioral health services for vulnerable New Yorkers of all ages. She is most passionate about working with young adults who lack support systems and are navigating the turbulent transition of adolescence to adulthood. Amy is an advocate and ally to the mental health field who aims to reduce the stigma surrounding mental illness by defying one stereotype at a time.

MONICA WILDER Director of Network Development Bronx Partners for Healthy Communities Monica Wilder is Director of Network Development at Bronx Partners for Health Communities where she leads multiple initiatives related to practice transformation, network operations, and quality improvement. She is also an Adjunct Lecturer in the Health Sciences Administration program at Lehman College, where she teaches health research and evaluation. Monica received an MPA from Baruch College as a National Urban Fellow and completed the Leadership Education in Adolescent Health Fellowship at the University of Rochester. She has a BA in Social Thought and Political Economy from the University of Massachusetts, Amherst.

LANA WILLIAMS-SCOTT Director of Diversity, Inclusion and Community Engagement Planned Parenthood Mid-Hudson Valley Lana, Director of Diversity, Inclusion and Community Engagement for Planned Parenthood Mid-Hudson Valley (PPMHV), is the co-chair of PPMHV’s Poughkeepsie Healthy Black and Latino Coalition (PHBLC). Lana launched the Community Health Promoter (CHP) in 2013. The CHP project is an African American outreach program in Orange County that expanded to the City of Poughkeepsie in 2014. Under Lana’s co-leadership, the Newburgh Healthy Black and Latino Coalition (NHBLC) formed with MVP Health Plan and Congressman Sean Patrick Maloney’s Newburgh District office. In less than a year, the coalition brought 25 members together to develop community-wide solutions.

KRISTIN WOODLOCK Chief Executive Officer Woodlock & Associates LLC Montefiore Hudson Valley Collaborative Kristin Woodlock, MPA, BSN, RN, is the CEO of Woodlock & Associates, a national consulting firm working with health and human services organizations. Kristin has over 30 years of experience in state and local government. She has served in a variety of senior executive roles, including the Acting Commissioner of the New York Office of Mental Health.

SYLVIA WRIGHT Director of Adult Treatment & Rehabilitation Services Mental Health Association of Rockland County Sylvia is a licensed Clinical Social Worker practicing in NYS. She obtained her Master’s Degree from Hunter College School of Social Work. She began her professional career in mental health while working for the Community Mental Health Division of the Visiting Nurse Service of NY, where she worked with various programs. Over the past several years Sylvia has served as the Director, Adult Treatment & Rehabilitation Services for the MHA of Rockland. In this capacity Sylvia led her team to launch a successful smoking cessation program, and was instrumental in implementing Tobacco Dependence Treatment into their counseling system for the PROS (Personalized Recovery Oriented Program) service offering.

CHAOFAN YUAN, MA, BA Medical Student Albany Medical College/Better Health for Northeast New York (BHNNY PPS) Chaofan Yuan is a 4th year medical student at AMC. He completed his undergraduate degree in Neurobiology at Cornell University and a Master’s degree in Medical Sciences at Boston University. He is pursuing a residency in Internal Medicine with a strong interest in Geriatrics medicine. Yuan believes there remains major shortfalls in the care of the elderly population and seeks to better their care through research and innovation.

CHRISTINA ZARCADOOLAS, PHD Founder Health Literacy Lab Christina Zarcadoolas is a sociolinguist and internationally recognized expert in health literacy and public understanding of health and science. She focuses on analyzing and closing the gaps between expert and lay knowledge and understanding, paying particular attention to how people make meaning out of complex things. The New England Journal of Medicine called her critically acclaimed book, Advancing Health Literacy: A Framework for Understanding and Action, “required reading” for public health communication professionals. Chris has over 40 years of rewarding academic work in with highly applied contexts. Among her academic positions she served most recently as the primary linguist in the Anthropology Dept. of Hunter College. Prior to that she was a Professor at City University of New York (CUNY) School of Public Health; spent five years on the faculty at Mount Sinai School of Medicine, New York and a long tenure at Brown University’s Center for Environmental Studies.
