Rapport d'Analyse Situationnelle du PNLS, Decembre 2000 I I I I I I I I INTRODUCTION I SO#2 I Increased use of essential FP/MCH and STII AIDS prevention services and practices Vision By the year 2002, Guinean families and individuals will have good access to high quality I services and information that meet their reproductive health needs. I This report covers PRISM activities and performance from 1 October to 31 December 2000. Like previous reports it is structured around PRISM's four implementing strategies to fulfill USAID's strategic objective # 2. The implementing strategies are designed to achieve the four I intermediate results (IR) linked to the SO#2: (1) Increased access to RH services, (2) Improved quality of services, (3) Increased demand for services and (4) Improved coordination between I the MOH, donors and other partners. Two important events had a significant impact on PRISM implementation during the quarter: I Security situation The security situation in most of Guinee Forestiere has deteriorated since early September 2000 when rebel invasions from Liberia and Sierra Leone started. Many prefectures have I been and continue to be subjected to fighting between the Guinean army and rebel groups. Other prefectures of the forest region, while not directly attacked, have become hard to reach. This situation concerns two out of the three administrative regions included in PRISM's I focus zone - the Region Administrative (RA) of Faranah and the RA ofN'Zerekore'. The situation has had a significant impact on PRISM's implementation of project activities during this past quarter. PRISM has a regional office in both RA. Technical activities that I were planned to occur during the last qUalier of2000 in GF, such as CBD support activities and STI training, were put on hold. At the same time, substantial time and attention were diverted from regular technical and managerial activities to deal with this crisis. In response I to the security situation at the border with Sierra Leone and Liberia, PRISM had to take the appropriate measures and make arrangements in order to ensure the safety of the project staff and assets. Also, PRISM prepared for the quick resumption of technical activities in GF in I case the situation was to improve in the near future. With hindsight, we now know that technical activities in Guinee Forestiere will not resume due to the current security situation. I Numerous measures were taken to ensure the safety of both the project staff and the project's material assets, while maintaining technical activities on schedule in undisturbed areas. For I instance, PRISM: • Withdrew professional expatriate staff from the concerned regions. This concerned Dr. Eleonore Rabelahasa, our Child Survival Specialist and Regional Representative I for the Faranah RA, and Dr. Michael Derosena, our Health Management Systems Specialist and Regional Representative for N'Zerekore RA. I • Temporarily assigned the technical staff from the N' Zerekore regional office to the I 1 The RA de Kankan is not within the defined risk zone. These are Dr. Sekou Conde and Mr. Michel Haba respectively Regional Coordinator for Health Management Systems and lEe. Dr Kalifa Mara had just joined the UN Volunteers in Rwanda at the outbreak of violence I in GF. • Transferred computer equipment and vehicles from the N'Zerekore office to Kankan I and secured the remaining equipment and supplies. • Assured that all communication systems and procedures were fully operational in all project locations. I • Evacuated Faranah at the end of December when rebels attacked Kissidougou and 2 were progressing north toward Faranah . I At the end of this quarter, PRISM's position in Faranah and N'Zerekore RA could be summarized as follows: The Faranah regional office was evacuated, its staff on vacation to deal with their I personal situations when necessary, or temporarily assigned to safe locations while participating in technical activities or supporting other PRISM offices. The N'Zerekore regional office was being attended to by essential support staff only. I Technical activities were on hold. Technical staff was on temporary assignment to the Kankan regional office. Well performing support staff were reassigned to same or I similar positions at other project locations were personnel changes were required. In light of the events it became very clear during this period that PRISM would have to suspend its technical activities for a long period of time in GF and that the N'Zerekore Office, and I possibly the Faranah Office would have to be closed down. PRISM proceeded with the review of its workplan and targets to address the concentration of activities in Haute Guinee during Y 4 and YS. We organized with the Kankan IRS a coordination meeting to address the new 3 I situation • This was done via a joint session of the GRIEC and ERCOSAR (see Annex for report). I Continuation Application for Year 4 and Year S The continuation application plan for year 4 and year S of the project was submitted to USAID I on October 10,2000. On November 14,2000, the S02 Team Leader presented PRISM with the Mission's concerns about the proposed continuation plan in a meeting held at the USAID Mission. In a letter dated I November 27, 2000, PRISM was notified that its proposed continuation plan for years 4 and S was not approved. The S02 team's main concern was in the area of maternal and child health planned interventions. In the same letter, we were requested to present our plans to address the I current security situation. Before the Christmas holidays, PRISM presented through a series of three meetings, with S02 and the MOH participating, information addressing related technical questions and concerns, as well as the revised workplan and targets for its MCH interventions. I The communications to the Mission concerning the security situation were also dealt with in I December. I 2 The Faranah regional office was reopened in January 200 I although with a reduced staff. 3 The collaborative work planning meeting for the Faranah RA occurred in January 2001 during a 2 days I extension of the CTRS PRISM: Quarterly Progress Report, December 2000 -3- I I I I SUMMARY OF PRINCIPAL ACTIVITIES & RESULTS (October-November-December 2000) I IRl: Increased Access • On the job training of 2nd ED&C managers was performed at 6 HCs in the Faranah RA I • 1M AT was performed in 33 facilities resulting in a marked performance at those sites in the N'Zerekore RA where the managers were trained with the curriculum developed by PRISM. • 15 trainers received basic ToT and were certified; 8 received advance ToT and were certified I • ED&C Logistics tools and forms for Hospitals are harmonised • USAID/SO#2 team was supported by PRISM with regards to determining the contraceptive needs for the target population, the management of products reception, port/custom clearance I and arranging for the contraceptive products storage in Conakry • PMA for the Guinean primary health care system was validated by the MOH • FP services integrated in one HC in Daboia prefecture (Konso) via adequate personnel I management by the DPS and allocation of initial stock of contraceptive products. • First six-month period results available and positive concerning the re-mobilised CBD/CBS I communities/agents in the 4
