A BRIEF ANALYSIS OF DATA ON REGISTERED BIRTHS, DEATHS AND INFANT DEATHS

3.1 Births and deaths are the two most important vital events in the life of an individual and for the society as a whole. Based on the data collected under the Civil Registration System on registered births and deaths, this chapter presents the trend in the level of registration and other related indicators, at the State and National level.

3.2 In 2018, the number of registration units along with the level of reporting at the national level and the States/UTs with rural/urban details may be seen at Statement 6. It may be noted here that level of reporting indicates the status of returns of registered births and deaths by the concerned Registrar within a State / UT during the year. It provides the deficiency at a glance and helps in reviewing the registration system and taking measures that would be necessary to improve the registration levels, wherever required.

3.3 At the national level, 94.1% of the registration units reported monthly returns of registered births and deaths to the District Registrar’s or Chief Registrar's Office during 2018. For rural areas, the reporting level are at 94.1% and for urban areas, it is 97.3%. However, these figures do not include Arunachal Pradesh and Delhi for which rural-urban breakup has not been made available by the State/UT while in case of Jharkhand, level of reporting data is not available.

3.4 Cent percent level of reporting have been achieved by 22 States/UTs viz. Andhra Pradesh, Assam, Chhattisgarh, Goa, Gujarat, Haryana, Karnataka, Kerala, Mizoram, Odisha, Punjab, Rajasthan, Sikkim, Tamil Nadu, Tripura, A & N Islands, Chandigarh, Daman &Diu, Dadra & Nagar Haveli, Delhi, Lakshadweep and Puducherry.

3.5 Statement 7 presents overall number of registered births and deaths at the national level for the years 1987 to 2018.

3.6 The above data provides details of the number of registered births and deaths over the years. The number of registered births has increased to 2.33 Crores in 2018 from 2.21 crores in 2017 – an increase of about 5.3%. In case of registered births, there has been fall in the year 2018 in respect of registered birth reported by Andhra Pradesh, Arunachal Pradesh, Chhattisgarh, Goa, Himachal Pradesh, Jharkhand, Karnataka, Kerala, Maharashtra, Manipur, Nagaland, Odisha, Punjab, Sikkim, Tamil Nadu, Tripura, A& N Islands, Dadra & Nagar Haveli, Daman & Diu, Delhi and Lakshadweep as compared to 2017. While in case of Assam, Bihar, Gujarat, Haryana, Jammu & Kashmir, Madhya Pradesh, Meghalaya, Rajasthan, Telangana, Uttarakhand, Uttar Pradesh, West Bengal, Chandigarh and Puducherry there has been increase in the number of registered Birth events. Three major States namely Uttar Pradesh, Bihar and West Bengal has contributed significantly in the increase in 2018 over 2017. 31

3.7 On the other hand, in case of registered deaths, number of registered events has gone up from 64.6 lakhs in 2017 to 69.5 lakhs in 2018, i.e. an increase of 7.5%. This Increase in numbers is despite the fact that Bihar, Jharkhand, Kerala, Nagaland, Tamil Nadu, Telangana, Lakshadweep and Puducherry have reported fall in numbers of death registered during the said period. While some States namely Uttar Pradesh, Madhya Pradesh, West Bengal, Gujarat, Maharashtra and Andhra Pradesh have contributed significantly in the increased number of Deaths Registered from 2017 to 2018.

3.9 Registration of Birth & Death within prescribed time limit of 21 days

3.9.1 Twenty Nine (29) States/UTs have provided data on Birth and Death registration with respects to prescribed time period (i.e within 21 days). The registered births and deaths have been classified in four categories depending upon their period of registration. The four time periods considered for the purpose are: i) within prescribed time limit ( up to 21 days), ii) After 21 days but within 30 days, iii) After 30 days but within 1 year, iv) More than 1 year. In respect of Time Gap Birth, 2 States Bihar and Jharkhand has not submitted information. While in case of Time Gap Death Registration, 3 States namely Bihar, Haryana and Jharkhand has not provided data. Data of two States namely, Haryana (in case of birth) and Uttarakhand (in case of both birth and death) has not been included as they have not submitted within prescribed time limit of 21 days separately. Further, in case of Maharashtra, Uttar Pradesh and Sikkim due to availability of only partial data, has not been taken into account while consolidating the data for Time gap tables.

3.9.2 On the basis of data received from States/UTs the percentage of registration achieved out of total registration undertaken in 2018 within prescribed time limit (21 days) is as under:

3.9.3 The above Statement 11 shows the percentage of births registered within the prescribed time period of 21 days to the total births registered in the state/UT. 13 States/UTs have achieved more than 90% registration of births within the prescribed time limit of 21 days. Further, 4 States/UTs are in the category of more than 80 to less than or equal to 90 percent, 10 States are in the category of more than 50 to less than or equal to 80 percent and remaining 2 States are under the category of less than or equal to 50 percent in completing the birth registration within time limit of 21 days. In Statement 12, the State- wise percentage of birth registered within prescribed time period has been arranged in descending order.

3.9.4 The Statement 11 also shows the percentage of deaths registered within the prescribed time period of 21 days to the total deaths registered in the state and arranged in descending order. 11 States/UTs have achieved more than 90% registration of death within prescribed time limit of 21 days. 5 States/UTs are in the category of more than 80 to less than or equal to 90 percent, 10 States are in the category of more than 50 to less than or equal to 80 percent and remaining 3 States are under the category of less than or equal to 50 percent in death registration within prescribed time limit of 21 days. In Statement 13, the State-wise percentage of death registered within prescribed time period has been arranged in descending order. Chart 3 & 4 shows percentage of Births and Deaths registered within prescribed time period respectively.

3.10.1 Chart 5 on sex wise distribution of registered births reveal that share of registered female births is about 4.1 per cent short of registered male births.

3.10.2 On the other hand, in case of death registration, the share of male registration is about 19.2 per cent more than female. (Statement 14). Sex wise distribution of registered deaths is presented in Chart 6.

3.11. Sex-wise Distribution of Registered Births and Deaths of Major States

3.11.1 Chart 7 and 8 respectively shows the sex-wise distribution of registered births and deaths for the major States (population of more than 10 million as per Census 2011). The data on birth registration reveals that the share of male registration is more than the female in almost all these major States. This gap is much wider for Jharkhand, Punjab, Gujarat, Rajasthan, Madhya Pradesh, Haryana and Assam. In case of Death Registration, the gap is wider almost in all major States viz. Rajasthan, Haryana, Assam,

3.12 Distribution of registered births and deaths vis-à-vis estimated births and deaths for the period 2009-2018

3.12.1 The time series data on registered births and deaths for the years 2009 to 2018 reveals a significant growth in coverage of registration. Share of birth registration has crossed the level of 80 percent and has reached to 89.3% during 2018. In case of death registration, it has reached 86.0% in 2018 from 79.6% in 2017.Chart 9 and 10 shows estimated & registered births and deaths respectively.

3.13.1 While comparing estimated and registered births, a significant gap is found in estimated and registered births in Bihar, Madhya Pradesh, Jammu & Kashmir, Jharkhand, Uttar Pradesh, Tamil Nadu and Odisha. The States of West Bengal, Telangana,, Assam and Uttarakhand have more number of registered births than estimated. Chart 11 shows the distribution of estimated and registered births respectively in major States.

3.14 Medical attention received at the time of birth and death

3.14.1 The data on birth by type of medical attention received at the time of delivery has been reported from 33 States/UTs and out of them three States namely Maharashtra, Uttar Pradesh and Sikkim have submitted only partial data and therefore their data has not been taken into account while consolidating the data. While another three States namely Bihar, Jharkhand and Uttarakhand have not submitted

medical attention at the time of Birth. The distribution of medical attention received at the time of birth for these States/UTs has been illustrated in Chart 13. While the data shows that majority of births have been occurred in an institution, either in Government or private hospitals. Institutional births occurred in about 83.1% of cases, 7.4% of the cases were attended by the Physicians/Nurse/Midwife. However, about 3.5% of the events are still attended by untrained Mid-wife/Dai and another 2.8% by others.

3.14.2 Medical Attention received by deceased at the time of death has been received from 32 States/UTs out of them, three States namely Maharashtra, Sikkim and Uttar Pradesh have submitted only partial data and therefore their data has not been used to while consolidating the data. Four States namely Bihar, Haryana, Jharkhand and Uttarakhand have not submitted the data. Chart 14 represents the distribution of medical attention received by the deceased at the time of death. It is evident from Chart that 33.8% of the total registered deaths have occurred in institutions and is on a higher side than that of other places from where the deceased had received medical attention. About 1.3% of the total registered deaths during 2018 have received medical attention from qualified allopathic professional and practitioner of other system and 35.7% of deceased did not receive any medical attention at the time of death.

3.15 Infant Deaths

3.15.1 The infant deaths registered by place of occurrence (Rural /Urban) of the event for the year 2018 has been given in Statement 17. It may be observed that the share of rural area is only 28.0% while that of urban area is 72% in total registered infant deaths during the year. Non registration of infant deaths in rural area is a cause of concern which may be due to non-reporting of infant deaths to the Registrars especially in case of domiciliary events. This figure is exclusive of returns of Jharkhand and Uttarakhand due to non-receipt of information from the State.

3.16 Still Births

3.16.1 The World Health Organisation (WHO) defines a still birth as a baby born with no signs of life at or after 28 weeks' gestation. Still birth, also known as foetal death, has been defined as ‘death prior to the complete expulsion or extraction from its mother of a product of human conception, irrespective of the duration of pregnancy and which is not an induced termination of pregnancy. The death is indicated by the fact that after such expulsion or extraction, the fetus does not breathe or show any other evidence of life, such as beating of the heart, pulsation of the umbilical cord, or definite movement of voluntary muscles. Heartbeats are to be distinguished from transient cardiac contractions; respirations are to be distinguished from fleeting respiratory efforts or gasps.’

3.16.2 Statement 19 presents still births registered by place of occurrence (Rural /Urban) of the event for the year 2018 and it may be observed that in total registered still births, share of rural registered still births is only 37.5% which may be due to poor netting of still births in rural areas. In urban areas, this share is about 59.7% which may be due to better netting of still births from medical institutions. About 2.7 percent of still births could not be classified due to non-availability of rural-urban breakup. This is based on registered events from 35 States/UTs. The data from one State namely Manipur is not available. State/UT-wise registered still births has been depicted in Table 4. 3.17 Analysis and Findings

Level of Registration (LOR)

3.17.1 The level of registration, defined as the percentage of registered births/deaths to the births/deaths estimated through SRS, determines the performance of a State / Union Territory in registration of births and deaths. In the absence of reliable estimates of number of births and deaths on a year to year basis from any other source, the number of events as revealed from SRS is used to calculate the level of registration. The level of registration so calculated is very high for some of the smaller States/Union Territories either due to the lower estimates of vital events than the actual and due to the fact that SRS estimates of vital events are based on the place of usual residence while events are registered at the place of occurrence irrespective of the place of residence. 3.17.2 The levels of registration of births and deaths for India and States/UTs for the period 2009 to 2018 are shown in Statements 20 and 21 respectively. The level of registration of births has increased to 89.3 in 2018 from 81.3% in 2009 i.e. in last 10 years an increase of about 8.0%. Similarly, the level of registration of deaths has also increased to 86.0% in 2018 from 66.9% in 2009, showing an increase of 19.1% over the last 10 years. Chart 15 depicts the trend in level of registration of births and deaths from 2009 to 2018. Though the level of registration for births has recorded consistent improvement over the years, gap of about 10.7% in case of registration of births and about 14.0% for registration of deaths at all India level remains a cause of concern. It is also pertinent to mention here that the gap between Levels of Birth and Death Registration is narrowing down.

There are 13 States/UTs namely Arunachal Pradesh, Assam, Manipur, Meghalaya, Mizoram, Nagaland, Telangana, Tripura, Uttarakhand, West Bengal, Chandigarh, Delhi and Puducherry which have recorded cent percent registration of births in 2018.

3.17.6 Statement 21 shows that the level of registration of deaths in the country and the States/UTs. In 2018, there were 16 States/UTs namely Andhra Pradesh, Goa, Gujarat, Haryana, Karnataka, Kerala, Mizoram, Odisha, Punjab, Sikkim, Tamil Nadu, Tripura, Chandigarh, D & N Haveli, Delhi and Puducherry which have recorded cent percent registration of deaths followed by the Rajasthan, Maharashtra and West Bengal which is the only three states reporting more than 90 percent registration of deaths. The Union Territories of Chandigarh, Delhi and Puducherry have maintained cent percent level of registration of deaths during last 10 years. 20 States/UTs have recorded their level of registration of deaths higher than the National level of 86.0% while remaining 16 States/ UTs have their level of registration of deaths below the national level. 9 major States namely Bihar, Jharkhand, Telangana, Jammu & Kashmir, Uttar Pradesh, Assam, Uttarakhand, Madhya Pradesh and Chhattisgarh features among States that have level of death registration below the National Level.

3.17. 8 Andhra Pradesh, Gujarat, Haryana, Karnataka, Kerala, Punjab, Maharashtra and Rajasthan are the only 8 major States recording more than 90 percent of registered births as well as deaths both in 2017 and 2018. Out of three States, namely, Bihar & Uttarakhand and Uttar Pradesh which were placed in the 25 - 50 % category in terms of death registration in 2017, both, Uttarakhand and Uttar Pradesh has moved up 1 category in 2018. No State is under the category of less than 25 percent of registration of births & deaths during 2017 as well as 2018. 2 States namely Tamil Nadu and Jharkhand have come to lower level of 80-90 % during 2018 in terms of Level of Birth Registration. While in terms of level of Death registration, 1 States namely West Bengal improved from 80-90 % in 2017 to highest category i.e above 90% in 2018. During 2018, Bihar and Uttar Pradesh improved their position by 1 category up and moved to 80-90 % from 50-80% in 2017 in terms of level of registration of Births.

3.17.9 The level of registration of births and deaths for the year 2018 for India and States/UTs has together been mapped in the Chart 18. The rectangle on the top right corner is the expected target towards achieving the cent percent registration and status of each States is depicted in the Chart 18.

3.18.1 The Sex Ratio (Number of females per thousand males) at birth of registered events is an important indicator to map the sex differential of the population at the beginning of their life. 3.18.2 The requisite information from 6 States namely Bihar, Jharkhand, Maharashtra, Sikkim, Uttar Pradesh, West Bengal Madhya Pradesh, Maharashtra, Manipur, Sikkim, Uttar Pradesh and West Bengal is not available. The sex ratio at birth based on registered births have been worked out after deducting the respective time delay in birth registration of more than 1 year. These figures of Sex Ratio at Birth (SRB) based on Registered events from only 30 States/UTs are presented in the Statement 23. The figures varies from State to State. Arunachal Pradesh and Nagaland registered top two slots respectively followed by Mizoram and Kerala while Manipur and Lakshadweep have shown the lowest sex ratio at birth. Status of States according to Sex Ratio at birth in respect of registered births is shown in chart 19.

3.19 Time Gap in Registration of Birth & Death

3.19.1 On the basis of data made available by States/UTs, the national data table on time gap of birth and death registrations i.e. within time limit (<21 days), 21-30 days, 30 days to less than 1 year and After one year has been prepared and may be seen in Statement 24 and 25. The 2 States namely Bihar and Jharkhand have not provided data on time gap of birth and death while Haryana has not submitted time gap of death table. The reasons given by these States for not supplying the data are lack of computerization and Governance issues. Further, Maharashtra, Uttar Pradesh and Sikkim, due to availability of only partial data, has not been taken into account while consolidating the data and has not been published. In case if Haryana and Uttarakhand data lowest available category is less than 30 days and hence due to non-availability with-in Time limit (21 days) separately, two categories have been merged for these States
