  • Alopecia Areata Part 1: Pathogenesis, Diagnosis, and Prognosis
Clinical Review Alopecia areata Part 1: pathogenesis, diagnosis, and prognosis Frank Spano MD CCFP Jeff C. Donovan MD PhD FRCPC Abstract Objective To provide family physicians with a background understanding of the epidemiology, pathogenesis, histology, and clinical approach to the diagnosis of alopecia areata (AA). Sources of information PubMed was searched for relevant articles regarding the pathogenesis, diagnosis, and prognosis of AA. Main message Alopecia areata is a form of autoimmune hair loss with a lifetime prevalence of approximately 2%. A personal or family history of concomitant autoimmune disorders, such as vitiligo or thyroid disease, might be noted in a small subset of patients. Diagnosis can often be made clinically, based on the characteristic nonscarring, circular areas of hair loss, with small “exclamation mark” hairs at the periphery in those with early stages of the condition. The diagnosis of more complex cases or unusual presentations can be facilitated by biopsy and histologic examination. The prognosis varies widely, and poor outcomes are associated with an early age of onset, extensive loss, the ophiasis variant, nail changes, a family history, or comorbid autoimmune disorders. Conclusion Alopecia areata is an autoimmune form of hair loss seen regularly in primary care. Family physicians are well placed to identify AA, characterize the severity of disease, and form an appropriate differential diagnosis. Further, they are able educate their patients about the clinical course of AA, as well as the overall prognosis, depending on the patient subtype. Case A 25-year-old man was getting his regular haircut when his EDITor’s KEY POINTS • Alopecia areata is an autoimmune form of barber pointed out several areas of hair loss.

Case 5:12-cv-00026-WLS-CHW Document 243 Filed 08/07/19 Page 1 of 18 IN THE UNITED STATES DISTRICT COURT FOR THE MIDDLE DISTRICT OF GEORGIA MACON DIVISION LESTER JAMES SMITH, : : Plaintiff, : : CASE NO.: 5:12-CV-26 (WLS) v. : : GREGORY DOZIER, : : Defendant. : : BENCH OPINION I. Procedural Background On January 24, 2012, Plaintiff filed a pro se Complaint alleging that the Georgia Department of Corrections’ (“GDOC’s”) grooming policy violates the Religious Land Use and Institutionalized Persons Act (“RLUIPA”), 42 U.S.C. § 2000cc et seq. (Doc. 1; see Doc. 183-22 at 6-7.) Because the policy forbids inmates from growing facial hair in excess of one half-inch in length (Doc. 183-22 at 7), Plaintiff pleaded that the grooming policy substantially burdens the exercise of his sincerely held religious beliefs because Islam prohibits him from shaving his beard. (Doc. 1.) Plaintiff sought nominal damages and an injunction to allow him to grow a beard for religious reasons. (Doc. 1 at 5.) After the Court granted Defendant’s first Motion for Summary Judgment, Plaintiff appealed to the Eleventh Circuit on March 6, 2014. (See Docs. 125 & 129.) On February 17, 2017, the Eleventh Circuit vacated and remanded this case, instructing this Court to analyze Plaintiff’s RLUIPA claim in a manner consistent with Holt v. Hobbes, 135 S. Ct. 853 (2015). Smith v. Owens, 848 F.3d 975, 981 (11th Cir. 2017). The Eleventh Circuit also appointed Plaintiff counsel, and Plaintiff’s appointed counsel continues to represent him in this case. The system recommends hairstyles that suit the individual’s characteristics. This is based on a neural network learning algorithm, which is trained with features, extracted from 1060 images of people, relating to 53 different hairstyles. The trained network reaches an accuracy of 28.10% when validated with images that were not used for training. This can be improved by trying different combinations of input variables, or using a different conversion for the values that were gained from the APIs. It is also possible that the APIs are not completely accurate. A third possibility for improvement would be to use a different learning algorithm, such as k-Nearest Neighbors or naive Bayes.
