t is the 1st day of March. Each minute sweeter than before. There is a blessing in Ithe air - William. Wordsworth For those of us who love the warmer more than the colder climate, March is the month when we know that the tide will turn. We may get snow and it still is cold but April and then May are around the corner. March also means our Annual “Besties” for People and Places followed soon by the annual “Besties” for Food and Beverage in April. This is our celebration of the local busi- ness owners and professionals. We recognize their effort as the leaders of our communities and the employers of our families. Our annual poll encourages you, our read- ers, to vote for your favorites in over 200 cate- gories. We learn each and every year about new businesses and admire once again the businesses that have thrived for generations. Our medical column is all about endometriosis. While I never have had to deal with this condition myself, I have witnessed the heartbreak with friends and family. We hope this will be an eye-opener for everyone and offers help to all seeking answers. Here is a wonderful travel idea this month in Vermont for the entire family to enjoy while learning and appreciating maple syrup—where it is from and its many uses. Visit the Vermont sugar houses for maple open house weekend. Then head to the Inn in Manchester to unwind. Real maple syrup is just amazing and the uses are endless. Please enjoy this March issue. Dow 20,000: What next?

s we watched the Dow Jones Industrial Average close above the 20,000 mark Afor the first time ever on January 25th, we could only wonder whether or not that fig- ure would be used as an imaginary spring- board to further gains or as a magnet, as some large round numbers often do. A common theory is that menstrual blood and endometrial cells flow back from the uterus through the fallopian tubes into the pelvis during the menstrual period. The cells then grow where they land in the pelvis. This theory is called retrograde menstruation. There are other theories that try to explain endometriosis. Endometriosis diagnosis Endometriosis is suspected when women have pelvic pain or painful menstrual periods. However, the only way to know for sure if there is endometriosis is to have surgery to visualize the lesions. Endometriosis cannot be diagnosed by ultrasound, x-ray or other non- invasive methods. It is considered mild, mod- erate or severe depending on what is found during surgery. Women with mild disease can have severe symptoms, and women with severe disease can have mild symptoms. Medicines such as NSAIDs (ibuprofen) or hor- monal birth control can treat the symptoms of endometriosis. Even though surgery would diagnose endometriosis, in many cases surgery is reserved for women for whom medical treatment does not improve the pain. Endometriosis and fertility Almost 40% of women with infertility have endometriosis. Inflammation from endometriosis may damage the sperm or egg or interfere with their movement through the fallopian tubes and uterus. In severe cases of endometriosis, the fallopian tubes may be blocked by adhesions or scar tissue. While endometriosis can cause infertility, many women who have this condition don't have any difficulty getting pregnant. In addition, sur- gery itself can cause adhesions, which may impair fertility. Therefore, the decision to do sur- gery is complex and needs to be individualized. So what did Sarah decide to do? Sarah wanted to review the different methods of contraception. We discussed that any of the hormonal contraception options can help with the symptoms of endometriosis. Since she used birth control pills before, she wanted to start them again until she decides to get pregnant. She had a pelvic ultrasound that was normal. Even though, based on her symp- toms and family history, she was more likely to have endometriosis, she wanted to avoid sur- gery at this time. hen I was 11 years old, the number one song on the radio was “Please Wcome to Boston” by David Loggins. The catchy tune opens with the line “Please come to Boston for the springtime.” Since this month’s magazine features “spring getaways” you’ll understand why that particular song has been bouncing around my brain. That’s because when it comes to spring trips, my favorite, by far, was a weekend that started in Boston. And when I tell you where I went from there, all in the span of 24 hours, you’ll think I’m fibbing but the truth is these fabulous des- tinations are all just a stone’s throw from the Capital Region and quite doable on a modest budget. So grab a friend, pack some snacks and let’s go for the ride.
