Insights and Innovations in Community Mental Health The Erich Lindemann Memorial Lectures organized and edited by The Erich Lindemann Memorial Lecture Committee hosted by William James College Foreward The Erich Lindemann Memorial Lecture is a forum in which to address issues of community mental health, public health, and social policy. It is also a place to give a hearing to those working in these fields, and to encourage students and workers to pursue this perspective, even in times that do not emphasize the social and humane perspective. It’s important that social and community psychiatry continue to be presented and encouraged to an audience increasingly unfamiliar with its origins and with Dr. Lindemann as a person. The lecturers and discussants have presented a wide range of clinical, policy, and historical topics that continue to have much to teach. Here we make available lectures that were presented since 1988. They are still live issues that have not been solved or become less important. This teaches us the historical lesson that societal needs and problems are an existential part of the ongoing life of people, communities, and society. We adapt ways of coping with them that are more effective and more appropriate to changed circumstances—values, technology, and populations. The inisghts and suggested approaches are still appropriate and inspiring. Another value of the Lectures is the process of addressing problems that they exemplify: A group agrees on the importance of an issue, seeks out those with experience, enthusiasm, and creativity, and brings them together to share their approaches and open themselves to cross-fertilization. -

Atkinson, M.D. 2003 Oregon Health & Science University SUMMARY The interview begins in mid-stream, with Dr. George Saslow recounting his earliest employment as a teacher, a career he began in college as a way to help defray the cost of tuition. He has been teaching ever since and continues to teach even now, beyond his ninety-seventh birthday, in the psychiatry residency program at OHSU. He even teaches fellow members of the Psychiatric Security Review Board, a unique institution that determines the treatment and supervision of criminals who have used the insanity defense. Saslow talks about the Board and its successful record of reintegrating the mentally ill into the community. The conversation then turns to a consideration of the failings of psychoanalysis. Saslow recounts early experiences which led him away from the work of Freud and fueled his interest in psychotherapy. One of these experiences was as co-therapist, with Erich Lindemann, of the survivors of the horrific Cocoanut Grove fire in Boston in 1942. Saslow goes on to describe the strengths of psychotherapy and its interview techniques. He shares his opinions of several post-Freudian analysts as well as nonanalysts, and notes that the current teaching of psychiatry focuses perhaps too narrowly on the physiological basis of mental diseases. One of the early leaders in group therapy, Saslow has had many opportunities to use group therapy techniques. As psychiatrist for the scientists at Los Alamos Scientific Laboratory during the years of the Manhattan Project, Saslow was often called upon to provide group counseling to the small community; he talks about a few of the incidents that occurred during his time there and the challenges he faced. -

The present essay tells the subsequent story, beginning in 1892, when full-time pathologists begin to staff the medical schools and hospitals of Boston. Three individuals from this era deserve special mention: William T Councilman, Frank Burr Mallory and James Homer Wright, with Councilman remembered primarily as a visionary and teacher, Mallory as a trainer of many pathologists, and Wright as a scientist. Together with S Burt Wolbach in the early-to-mid-twentieth century, these pathologists went on to train the next generation of pathologists—a generation that then populated the various hospitals that were developed in Boston in the early 1900s. This group of seminal pathologists in turn formed the diagnostically strong, academically productive, pathology departments that grew in Boston over the remainder of the twentieth century. The Eclipse of Community Mental Health and Erich Lindemann These volumes make new contributions to the history of psychiatry and society in three ways: First, they propose a theory of values and ideology influencing the evolution of psychiatry and society in recurring cycles and survey the history of psychiatry in recent centuries in light of this theory. Second, they review the waxing, prominence, and waning of community mental health as an example of a segment of this cyclical history of psychiatry. Third, they provide the first biography of Erich Lindemann, one of the founders of social and community psychiatry, and explore the interaction of the prominent contributor with the historical environment and the influence this has on both. We return to the issue of values and ideologies as influences on psychiatry, whether or not it is accepted as professionally proper. This is intended to stimulate self-reflection and the acceptance of the values sources of ideology, their effect on professional practice, and the effect of values-based ideology on the community in which psychiatry practices. The books will be of interest to psychiatric teachers and practitioners, health planners, and socially responsible citizens. David G. Satin is a board-certified psychiatrist who has trained at the Massachusetts General and McLean Hospitals, has been Assistant Professor of Psychiatry at Harvard Medical School, where he also obtained his MD and taught gerontology and the history of psychiatry, and has had a clinical practice in adult and geriatric psychiatry. He was unsuccessful, however, in his attempt to enter Harvard Medical School (HMS). Having a prosperous relative in New Haven with whom he could live, he applied to and was accepted by Yale Medical School, where he excelled. His idol and champion there was Dr. Milton C. Winternitz, Chairman of the Pathology Department and later Dean of the medical school. Eager to return to the Boston area to practice medicine, Dr. Castleman volunteered to spend the summers of 1929 and 1930 as an unpaid assistant in the Pathology department at the Massachu- setts General Hospital (MGH). -

For psychiatrist Erich Lindemann, the West End offered a unique opportunity to study how acute stress and loss affected populations, thus contributing to social psychiatry, which sought to prevent mental illness by addressing factors in the social and physical environment. While Lindemann’s project provided a sophisticated response to the often simplistic arguments about the cities and mental health, it also highlighted the challenges of applying social psychiatric theory in practice. Introduction In 1958, the Boston Redevelopment Authority began its demolition of a 48-acre portion of Boston’s West End, displacing 2,700 lower-working- class families. For Erich Lindemann, chief of psychiatry at the nearby Massachusetts General Hospital (MGH), this urban renewal programme offered a unique opportunity.
