Psychogenic amnesia is a complex disorder characterised by a wide variety of symptoms. Consequently, in a number of cases it is difficult distinguish it from organic memory impairment. The present study reports a new case of global psychogenic amnesia compared with two patients with amnesia underlain by organic brain damage. Our aim was to identify features useful for distinguishing between psychogenic and organic forms of memory impairment. The findings show the usefulness of a multidimensional evaluation of clinical, neuroradiological, neuropsychological and psychopathological aspects, to provide convergent findings useful for differentiating the two forms of memory disorder. But how much do we do? So far, science has provided only a very partial answer to this riddle. The magical number seven, plus or minus two, seems to constrain the capacity of our immediate memory (Miller 1956). But surely its constraints dissipate when memories settle in long-term stores. Yet how big are these stores? If we combine all of our factual knowledge and personal reminiscence, childhood scenes and memories of the past day, intimate experiences and professional expertisemhow many items are there, that, combined together, mold us into unique individuals? The answer is not simple, and neither is the question. For example, what is an item in long-term memory? And how can we measure it, being sure that we unveil memory capacity and not merely the occasional ability to tap it? Such theoretical and practical difficulties, no doubt, have contributed to the fact that the capacity of human memory is still an enigma. Yet, despite the inherent and undeniable complexities, the issue deserves to be retrieved, once in a while, from the oblivions of the collective memory of the scientific community. (For a selection of earlier discussions of the size of human long-term memory, see Galton 1879; Landauer 1986; Crovitz et al. 1991.) What is it like to be Confabulating? Sahba Besharati, Aikaterini Fotopoulou and Michael D. Kopelman Kings College London, Institute of Psychiatry, London UK Different kinds of confabulations may arise in neurological and psychiatric disorders. This chapter first offers conceptual distinctions between spontaneous and momentary (“provoked”) confabulations, as well as between these types of confabulation and other kinds of false memories. The chapter then reviews current explanatory theories, emphasizing that both neurocognitive and motivational factors account for the content of confabulations. We place particular emphasis on a general model of confabulation that considers cognitive dysfunctions in memory and executive functioning in parallel with social and emotional factors. It is argued that all these dimensions need to be taken into account for a phenomenologically rich description of confabulation. The role of the motivated content of confabulation and the subjective experience of the patient are particularly relevant in effective management and rehabilitation strategies. Finally, we discuss a case example in order to illustrate how seemingly meaningless false memories are actually meaningful if placed in the context of the patient’s own perspective and autobiographical memory. Key words: Confabulation; False memory; Motivation; Self; Rehabilitation. 1 Memory is often subject to errors of omission and commission such that recollection includes instances of forgetting, or distorting past experience. The study of pathological forms of exaggerated memory distortion has provided useful insights into the mechanisms of normal reconstructive remembering (Johnson, 1991; Kopelman, 1999; Schacter, Norman & Kotstall, 1998). An extreme form of pathological memory distortion is confabulation. Different variants of confabulation are found to arise in neurological and psychiatric disorders. -

It was founded by Sigmund Freud between 1885 and 1939 and continues to be developed by psychoanalysts all over the world. Psychoanalysis has four major areas of application: 1) as a theory of how the mind works 2) as a treatment method for psychic problems 3) as a method of research, and 4) as a way of viewing cultural and social phenomena like literature, art, movies, performances, politics and groups. E-Mail: [email protected] ORCID id: 0000-0001-6544-5738 2 Abstract This article profiles the scientific lives of six women on the staff of the Clinic of Neurology and Psychiatry at the University of Vienna in 1927, the year when its director, Julius Wagner-Jauregg (1857–1940), was awarded the Nobel Prize in Physiology or Medicine. They were all of Jewish descent and had to leave Austria in the 1930s to escape from the National Socialist regime. With a solid background in brain science and mental disorders, Alexandra Adler (1901–2001), Edith Klemperer (1898–1987), Annie Reich (1902–1971), Lydia Sicher (1890– 1962) and Edith Vincze (1900–1940) pursued academic careers in the United States, while Fanny Halpern (1899–1952) spent 18 years in Shanghai, where she laid the foundations of modern Chinese psychiatry, before going to Canada. At the dawn of their medical career, they were among the first women to practice neurology and psychiatry both in Austria and overseas. Joel Whitebook 139 JOEL WHITEBOOK Jacob’s Ambivalent Legacy This paper elucidates Freud’s ambivalent and complex relation to Judaism, tracing it to the “break with tradition” that ran through the center of his father’s life. Jacob Freud was a conflicted and tran- sitional figure, with one foot planted in the parochial world of Jewish traditionalism and the other in the cosmopolitan world of European modernity. And he wanted his son to partake of these two legacies in equal degree. But Sigmund found the conflict untenable and chose in favor of science, atheism, and the Enlightenment. The point to be appreci- ated, however, is that he did not see his choice of the Aufklärung as a simple rejection of the Jewish tradition. Rather, by construing Moses as an Aufklärer, he was able he was able to define his project as an inner transformation of the Jewish tradition so that he could thereby to be “a godless Jew,” thereby confirming both his atheism and his cultural heritage. 1 What are we to make of Freud’s provocative idea of a “godless Jew”? Is it simply a contradiction in terms that he uses to patch over an irresolvable and fundamental rift in his character and outlook? This is the position of those champions of Enlightenment universalism who want to inoculate Freud’s science against any taint of ethnic particularity. They argue that the godlessness ought to be retained and the Judaism discarded. This is also the position of certain Jewish thinkers when they want to claim Freud as a prodigal son who ultimately returned to the tribal fold.1 In this case, the idea is to drop the godlessness—or at least radically attenuate it—while holding onto the Judaism. -

PSYCHODYNAMIC PERSPECTIVES ON PERSONALITY This educational CAPPE module is part i in section III: Theories of Human Functioning and Spirituality Written by Peter L. VanKatwyk, Ph.D. Introduction Psychodynamic theory goes back more than 100 years and has been a principal influence in the early history of clinical pastoral education (CPE). It is a way of thinking about personality dynamics in interpreting and understanding both the spiritual care-provider and care-receiver. This module will briefly summarize the basic theory and punctuate psychodynamic concepts that have been significant in the study of psychology of religion and theological reflection in the practice of spiritual care and counselling. Psychodynamic theories presently practiced include in historical sequence the following three schools that will be covered in this module: 1. Ego Psychology, following and extending the classic psychoanalytic theory of Freud, with major representatives in Anna Freud, Heinz Hartmann and Erik Erikson. 2. Object Relations Theory, derived from the work of Melanie Klein and members of the “British School,” including those who are prominent in religious studies and the practice of spiritual care: Ronald Fairbairn, Harry Guntrip, and D.W. Winnicott. 3. Self Psychology, modifying psychoanalytic theory with an interpersonal relations focus, originating in Heinz Kohut, systematized and applied for social work and counselling practice by Miriam Elson. In conjunction these psychodynamic theories offer three main perspectives on personality: 1. the human mind harbors conflict – with powerful unconscious forces that are continually thwarted in expressing themselves by a broad range of counteracting psychological processes and defense mechanisms. 2. each person carries an unconscious internalized world of personal relationships – with mental representations that reflect earlier experiences of self and others which often surface as patterns in current relationships and interpersonal problems. -

WHY FREUD? Sigmund Freud’s impact on how we think, and how we think about how we think, has been enormous. The twentieth century has been called the Freudian century, and whatever the twenty-first century chooses to believe about the workings of the human mind, it will be, on some level, indebted to Freud (of course, this may be a debt that involves reacting against his ideas as much as it involves subscribing to them). Freud’s theory, psychoanalysis, suggested new ways of under- standing, amongst other things, love, hate, childhood, family relations, civilisation, religion, sexuality, fantasy and the conflicting emotions that make up our daily lives.Today we all live in the shadow of Freud’s innovative and controversial concepts. In their scope and subsequent impact Freud’s writings embody a core of ideas that amount to more than the beliefs of a single thinker. Rather they function like myths for our culture; taken together, they present a way of looking at the world that has been powerfully transformative. The poet W.H.Auden prob- ably put it best when he wrote of Freud:‘if often he was wrong and, at times, absurd,/to us he is no more a person/now but a whole climate of opinion/under which we conduct our different lives’ (‘In Memory of Sigmund Freud’,Auden 1976: 275). But what is this strange ‘climate of opinion’, psychoanalysis? How did a turn-of-the-century Viennese doctor, who may now seem to us often wrong and sometimes absurd, become so central to our vision of 2 WHY FREUD? ourselves as thinking, feeling beings in the twentieth century? And if psychoanalysis really is ‘often wrong and sometimes absurd’, why read it at all? While providing a compact introduction to Freud’s life, impor- tant concepts and key texts, this study also aims to offer some answers to these wider questions. -

From Neuroscience which, in accordance with his thinking at the time, Freud never disregarded, the concepts of neuronal plasticity, enriched environment and the neurobiological aspects of the attachment process. From Psychoanalysis, the theory of transference, M. Mahler’s psychological evolution model, the concept of the regulating function of the self-objects and Winnicott’s holding environment concept. Together these provide a useful bridge toward the understanding of the neurobiological changes resulting from psychoanalytical psychotherapy. One concludes that psychoanalytical psychotherapy, through transference, acts as a new model of object relation and learning which furthers the development of certain brain areas, specifically, the right hemisphere, and the prefrontal and limbic cortices, which have a regulating function on affects.
