The hair shaft consists of a cortex and cuticle cells, and a medulla for some types of hairs. Hair follicle has a continuous growth and rest sequence named hair cycle. The duration of growth and rest cycles is coordinated by many endocrine, vascular and neural stimuli and depends not only on localization of the hair but also on various factors, like age and nutritional habits. Distinctive anatomy and physiology of hair follicle are presented in this chapter. Menopause and Hair loss Written by Consultant Dermatologist, Dr Sajjad Rajpar Many women will experience concerns with their hair during or after the menopause. This can come as quite a shock and can be deeply worrying because hair is such an important aspect of selfidentity. Hair loss may not be obvious to anyone else at ﬁrst, and convincing others (including relatives and family doctors) that there is a problem can be a hurdle in itself. The signs may be an increasing loss of hair when brushing or in the shower, general thinning, the hair line moving backwards, and dry or brittle hair. The parting may widen, and more scalp may show through the hair, especially when the hair is wet. There can be anxiety with brushing or washing the hair, or even looking in the mirror. Stress from hair loss is worsened by not knowing whether the condition will deteriorate and whether HRT will help or not. It can be confusing trying to make sense of the sea of supplements and tonics on the internet or recommended by trichologists and hairdressers. Who gets hair loss? Some 40% of women will experience hair loss after the menopause. In two thirds of women this will be all over the scalp. For the remainder, the hair loss is over the front or temples only. Hair loss may start during the perimenopause or occur many years after. Why does hair loss occur with the menopause? While a hair follicle is only tiny, it is an extremely complex structure that is still not well understood, especially when it comes to hormonal changes.

Treatment of Eyebrow Hypotrichosis Using Bimatoprost: a Randomized, Double-Blind, Vehicle-Controlled Pilot Study
ORIGINAL ARTICLE Treatment of Eyebrow Hypotrichosis Using Bimatoprost: A Randomized, Double-Blind, Vehicle-Controlled Pilot Study † KENNETH R. BEER, MD,* HILLARY JULIUS, PA,* MONICA DUNN, RN,* AND FRED WILSON,MS BACKGROUND The Food and Drug Administration has approved bimatoprost ophthalmic solution (0.03%) for the treatment of eyelash hypotrichosis. Previous reports of its efficacy in eyebrow hypotrichosis are anecdotal. OBJECTIVE To assess the efficacy and safety of bimatoprost 0.03% ophthalmic solution applied to the eyebrows in a randomized, double-blind, vehicle-controlled study. METHODS Subjects (n = 20) with mild to moderate eyebrow hypotrichosis enrolled in the study. One group (Bim) applied bimatoprost to each eyebrow daily for 9 months, and another applied vehicle nightly to each eyebrow for 5 months. Subjects in the latter group were re-randomized to apply bimatoprost (Veh-Bim Group) or vehicle (Veh Group) daily to each eyebrow for 4 months. The primary end point was investigator-assessed eyebrow appearance; secondary end points were subject-reported outcomes. RESULTS Investigator assessments showed significant improvements from baseline to 6 (p = .002) and 7 (p = .005) months for the eyebrows treated with bimatoprost. p-Values for the Veh-Bim and Veh groups were not significant at any time point. End-of-study subject satisfaction with eyebrow fullness or thickness and darkness or color was greater in the Bim group than in the Veh group. Adverse effects were not observed. CONCLUSION Bimatoprost 0.03% ophthalmic solution applied daily for 9 months improves the appearance of eyebrows noticeably more than vehicle, without side effects. This study of 76 males observed that skin reflectance measuring skin colouration and melanin pig- mentation correlated with hair size and follicle density. Individuals with a greater concentration of melanin within the superficial layer of the skin had a lower follicle density and smaller sizes of hairs. In contrast, individuals with a lower melanin concentration and lighter skin colour had a full range of hairiness. This leads to the suggestion that over the course of human evolution, high concentrations of melanin in consist- ently exposed to ultraviolet radiation areas developed first and that hair loss was a consequence of compe- tition in the skin between melanin production and hair growth. Darker pigmented skin and lower follicle density are significantly correlated (R2=0.283; p<0.05). Individuals with darker skin had a mean of 4.91 follicles per cm2 whereas those with lighter skin reflectance had 11.20 follicles per cm2. This suggests that increased concentrations of melanin in the basal layer of the epidermis may limit hairiness by negatively influencing the skin’s ability to produce hair.
