This can come as quite a shock and can be deeply worrying because hair is such an important aspect of self­identity. Hair loss may not be obvious to anyone else at first, and convincing others (including relatives and family doctors) that there is a problem can be a hurdle in itself. The signs may be an increasing loss of hair when brushing or in the shower, general thinning, the hair line moving backwards, and dry or brittle hair. The parting may widen, and more scalp may show through the hair, especially when the hair is wet. There can be anxiety with brushing or washing the hair, or even looking in the mirror. Stress from hair loss is worsened by not knowing whether the condition will deteriorate and whether HRT will help or not. It can be confusing trying to make sense of the sea of supplements and tonics on the internet or recommended by trichologists and hairdressers. Who gets hair loss? Some 40% of women will experience hair loss after the menopause. In two thirds of women this will be all over the scalp. For the remainder, the hair loss is over the front or temples only. Hair loss may start during the perimenopause or occur many years after. Why does hair loss occur with the menopause? While a hair follicle is only tiny, it is an extremely complex structure that is still not well understood, especially when it comes to hormonal changes.

This study of 76 males observed that skin reflectance measuring skin colouration and melanin pig- mentation correlated with hair size and follicle density. Individuals with a greater concentration of melanin within the superficial layer of the skin had a lower follicle density and smaller sizes of hairs. In contrast, individuals with a lower melanin concentration and lighter skin colour had a full range of hairiness. This leads to the suggestion that over the course of human evolution, high concentrations of melanin in consist- ently exposed to ultraviolet radiation areas developed first and that hair loss was a consequence of compe- tition in the skin between melanin production and hair growth. Darker pigmented skin and lower follicle density are significantly correlated (R2=0.283; p<0.05). Individuals with darker skin had a mean of 4.91 follicles per cm2 whereas those with lighter skin reflectance had 11.20 follicles per cm2. This suggests that increased concentrations of melanin in the basal layer of the epidermis may limit hairiness by negatively influencing the skin’s ability to produce hair. INTRODUCTION Hirsutism is the excessive growth of facial or body hair on women. Hirsutism can be seen as coarse, dark hair that may appear on the face, chest, abdomen, back, upper arms, or upper legs. Hirsutism is a symptom of medical disorders associated with the hormones called androgens. Polycystic ovary syndrome (PCOS), in which the ovaries produce excessive amounts of androgens, is the most common cause of hirsutism and may affect up to 10% of women. Hirsutism is very common and often improves with medical management. Prompt medical attention is important because delaying treatment makes the treatment more difficult and may have long-term health consequences. OVERVIEW OF NORMAL HAIR GROWTH Understanding the process of normal hair growth will help you understand hirsutism. Each hair grows from a follicle deep in your skin. As long as these follicles are not completely destroyed, hair will continue to grow even if the shaft, which is the part of the hair that appears above the skin, is plucked or removed.
