 Anjuman Islamia Medical and Social Service Society, Ranchi – East Central Zone – Jamshedpur DO Anjuman Islamia Medical and Social Service Society caters to all sections of people at affordable cost. Hospital has facility of general medicine, general surgery, orthopedic, pediatric, gynecology, skin, ENT, physiotherapy and dental health. Hospital has 40 doctors including 5 duty doctors and 66 other staff for taking care of the . On an average 250 to 350 patients per day are being treated at the hospital. 15 to 35 patients are treated free of cost per month. For poor patients the governing body helps with “Jakat”maximum Rs 10,000/- per . The project sought financial assistance for Cardio- Ambulance to bring patients and to refer critical patients to higher centre. Cardio – Ambulance will act as an extra ICU movable bed for the hospital. Financial assistance of ` 25,00,000/- has been provided for purchase of Ambulance.  SDA Diamond Hospital & Medical Research Centre -Western Zone –Surat DO SDA – Diamond Hospital & Medical Research Centre, Surat founded in February 2014, is a multispecialty charitable trust hospital with 150 beds, 4 operation theatres including 2 modular OTs and 15 beds ICU and 16 beds NICU. Hospital’s high quality services attract patients from Surat and its periphery. On an average there is more than 70% occupancy in the hospital as inpatients, about 350-400 people come through the O.P.D daily for medical relief.They are offering free normal delivery if baby girl is born and for second baby girl they handover a Bond of Rs. 10,000/- in favour of baby girl to support “save girl” campaign. Till now, they have handed over 750 Bonds. Every year, around 2200-3000 deliveries are done. The Hospital sought financial assistance for purchase of an Ambulance equipped with life saving equipments to provide quality health care to economically poor and needy sections of society. Financial assistance of `18,86,565/- has been provided for purchase of ambulance with life saving equipments.  Seth Tarachand Ramnath Charitable Ayurvedic Hospital Trust – Western Zone – Pune DO I Seth Tarachand Ramnath Charitable Ayurvedic Hospital Trust is working for poor and needy patients and provides training, education and awareness in the field of health and hygiene.On an average 500 out patients and in-patients are treated every day at subsidized cost of ` 20/- for case paper &`50 for Ayurvedic medicine for one week. Total 88 doctors and 40 employees are working in the hospital. They have been running 11 bedded dialysis wards from Oct 2013 with 13 dialysis machines. The project sought financial assistance for purchase of 4 dialysis machines. Financial assistance of `24,41,600/- has been provided for purchase of 4 dialysis machines.  Ratan Jyoti Charitable Foundation – Central Zone – Gwalior DO Ratan Jyoti Charitable Foundation (Trust) is providing comprehensive health care services including eye care services for more than 10 years to weaker sections of society. Trust organized health camps, eye camps and has extended its free services to more than 10 million people in and around Gwalior district ranging up to 150 kms. Hospital has performed 14126 free cataract surgeries in the year 2018-19 and treated around 50,000 patients for other ocular disorders. The Hospital sought financial assistance for purchase of 1

new ambulance which will provide urgent medical care to those in need and arranging for timely transportation of the patients to nearby hospital. Financial assistance of ` 24,51,000/- has been provided for purchase of Ambulance.  Virat Hospice run by Brahamarishi Mission Samiti - Central Zone – Jabalpur DO Virat Hospis run by Brahmrishi Mission Samiti Jabalpur is providing free of cost treatment, medication & emotional support to terminally ill cancer patients. Their sole purpose is to add life to remaining days of terminally ill cancer patients. Hospital has 30 Doctors, Oncologist, medical specialist, pain specialist and trained staff who are visiting regularly to take care of all patients. On an average 1280 patients are visiting hospital every day. The Hospital sought financial assistance for purchase of Brivo XR 115 Mobile X-ray Machine and Digital upgrade kit which will be helpful for serving patients in a better way and problems can be detected beforehand and treated properly. Financial assistance of `20, 00,000/- has been provided for purchase of Mobile X- ray machine and related Digital Upgrade Kit.  Vision Academy - The Socio Medical Society – Central Zone – Bhopal DO Vision Academy- The Socio Medical Society was established in 2006 and provides medical care specially eye care in remote areas of Bhopal Division. Hospital, is organizing regular activities by conducting free awareness/ screening camps for diabetic retinopathy, glaucoma and cataract in rural areas of Bhopal and surroundings districts since 2006. Hospital is treating on an average 13787 patients in OPD and 749 patients in OT per year. Out of which 4517 patients in OPD and 96 patients in OT were treated free of cost. The Socio Medical Society Vision Academy is planning to start a new project Tele Eye Care Programme for which they require a well equipped mobile Van for treating rural patients who require further investigation or operation and for referring and shifting of patients to the hospital. It is estimated that 60 patients will be utilizing services every day, 25 days in a month total 18000 patients per year through 300 screening camps. Financial assistance of ` 24,25,590/- has been provided for purchase of mobile van and equipments.  Jaipur Calgary Charitable Eye Hospital and Research Centre – North Zone – Jaipur DO Jaipur Calgary Charitable Eye Hospital and Research Centre Trust has three operation theatres equipped with microscopes, operation tables, ECG machines and all necessary equipments and provides all services, including procedure and surgeries at subsidize rates. Hospital conducts camps almost every alternate day. Patients suffering from cataract are identified, brought in the hospital, operated upon and dropped back. All services are rendered free of cost including spectacles and meals. The project sought financial assistance for purchase of two machines – one for Lab (I Count 3cp) and another for eye surgery (Auto Refractometer Machine) which will be helpful for providing better service to eye patients and will also serve poor patients during free camps. Financial assistance of ` 5,75,000/- has been provided for purchase of two machines.  Ramakrishna Mission Ashram, Kanpur - North Central Zone –Kanpur DO Ramakrishna Mission Ashram, Kanpur is 90 years old Ashram. RKM, Kanpur has been running a Higher Secondary School since 1947 and provide education to 600 students. There are 20 teachers and 7 non teaching staff. The Mission also runs a Charitable allopathic – cum homeopathic dispensary since 1939 which treats approximately 18,600 cases every year. Specialists in Orthopedic, Eye, ENT. Gynaecology, dental, paediatric and general physicians offer services regularly. The project sought financial assistance for purchase of vehicle for conducting health camps in and around the backward areas of Kanpur and for all transport facilities of the hospital like purchase of medicines, for 2

movement of doctors, staff and to carry the medicines and other hospital related commodities while conducting the medical camps. They have also requested funds for purchase of ECG machine, as earlier ECG machine is damaged and they are unable to attend the patients. Financial assistance of `15,14,000/- has been provided for purchase of Scorpio and ECG Machine.  Needy Heart Foundation - South Central Zone –Bangalore DO -II Needy Heart Foundation is a registered charitable institution working to give every person suffering from heart disease a chance to live a healthy life. They have tied up with prominent and reputed in Bangalore which have the required infrastructure and eminent surgeons to perform heart operations successfully. Patients treated free of cost /subsidized rates are more than 10,000 during the last 18 years. The institution has facilitated heart surgeries of the needy poor at highly subsidized rates. The project sought financial assistance for purchase of Ambulance to give new lease of life to many more patients suffering from heart ailment, by being able to transport them quickly and with para- medical help for cardiac patients with help of the ambulance. Financial assistance of `25,00,000/- has been provided for purchase of Critical Care Ambulance.  Lepra Society - South Central Zone –Hyderabad DO LEPRA was established in Hyderabad in 1989, as a partner of Lepra UK, to serve the needs of people affected by leprosy. They are operating in 10 states with 27 referral centers with the help of more than 500 staff members. Lepra Society is working for early detection of leprosy, lymphatic filariasis, HIV and Tuberculosis and treating needy people on wheels. LEPRA focuses health improvement activities in the community that are marginalized or poor, especially women and children, young people, slum population and migrants affected by the above mentioned diseases and tries to bring about positive changes in their life- style. The project sought financial assistance for purchase of mobile van which will be used for vaccinating people with new vaccine including people affected by Leprosy. Financial assistance of `12,14,978/- has been provided for purchase of Mobile Vehicle.  M/s Vijay Marie Hospital , Khiratabad - South Central Zone – Hyderabad DO Vijay Marie Hospital, Khiratabad, was started as a small dispensary in 1948, and now developed as a 200 beded full fledged General Hospital especially well known for its obstetric care. Hospital is fully equipped with all facilities like ICU, NICU and Dialysis units etc. The Hospital is extending free service to the people from Old Age Homes and Orphanages etc. Hospital has total 226 staff to take care of all patients. Hospital is charging very nominal amount ie. ` 400/- per patient. The hospital sought financial assistance for purchase of Ambulance which will be used for taking the sick persons to other Hospital and also to use the vehicle for the aged inmates who they take care. Financial assistance of `23,61,500/- has been provided for purchase of Ambulance.  Grace Cancer Foundation (Global Research and Cancer Education) – South Central Zone – Hyderabad DO Grace Cancer Foundation is service oriented organization working for early detection of cancer and it has been rewarded with many awards like Guinness Book of world records and Limca book of world record. Foundation has been conducting screening tests in the entire AP and Telangana regions for early detection of cancer. Trust is treating on an average 150 to 200 patients per day and charging a nominal fee of ` 500 per patient. The project sought financial assistance for purchase of Mobile Women Cancer screening project which will be helpful in raising awareness and also early detection of common cancer i.e of breast and cervix. This saves lives. 3

Financial assistance of `25,00,000/- has been provided for purchase of mobile chassis.  Thalassemics India –North Zone – Delhi DO I Thalassemics India was formed by a few Thalassemia parents in 1986. Thalassemics India (T.I) are a dedicated group of Thalassemia parents and patients working countrywide since 1987. T.I undertakes project for creating awareness about Thalassemia.Bone marrow transplant, disability and blood collection in collaboration with Indian Red Cross Society, Ministry of Health & Family Welfare, Ministry of Social Welfare and Government of NCT Delhi. At the Thalassemics India is counselling of 10-20 patients is done daily and treatment given at subsidized rates. Annually approximately 66 patients are treated free of cost. The basic objective of this project is for treatment of children with thalassemia which requires repeated blood transfusions at monthly intervals. Red Cross is providing more than 50% of its collection to Thalasemics India. Red Cross sought financial assistance for A Tempo Traveler kind of Vehicle filled with cooling equipment for transporting personnel and blood units collected at blood donation camp in Delhi & NCR. Financial assistance of Rs 20,65,861/- has been provided for purchase of traveler type vehicle.  Tata Memorial Centre – Homi Bhabha Cancer Hospital & Research Center at Vizag, Visakhapatnam – South Central Zone – Visakhapatnam Homi Bhabha Cancer Hospital & Research Center at Vizag, Visakhapatnam hospital was started with the objective of treating various cancer patients with different modalities of treatments like chemotherapy, radiotherapy and surgery. Hospital is located in 77 acres of land for a noble cause with a mandate for service, education and research at Visakhapatnam, on similar lines of Tata Memorial Centre, tertiary care center for cancer prevention, cure rehabilitation and palliation. The project sought financial assistancefor purchase of electric buggy for patient service and ease of mobility within the vast campus. Financial assistance of Rs 4,24,000/- has been provided for purchase of electric buggy .  Indian Cancer Society – Western Zone –Mumbai DO II The project from Indian Cancer Society is for providing financial support to underprivileged patients who are unable to afford the initial investigation/ diagnostics costs for cancer detection. Cancer if detected at early stage is curable. ICS has the onerous responsibility of managing the cancer spectrum to cover the end to end activities of prevention i.e Awareness and Screening, Cure (treatment), Survivorship and Rehabilitation (post treatment). The initial investigation/diagnosis cost for cancer runs into thousands of rupees and the underprivileged family who is barely able to meet the routine needs of the family does not want to go ahead with investigation, leave aside the treatment. Financial support will help the patients in getting diagnosed at an early stage and simultaneously getting treated rather than going around sourcing for fund for initial diagnosis. The project sough financial assistance for initial investigation/diagnosis cost for cancer treatment. Financial assistance of Rs 52,50,000/- has been provided to underprivileged patients who are unable to afford the initial investigation/ diagnostics costs. Approximately 200 patients will be benefited.  Marian Society - Asha Niketan Higher Secondary School for the Deaf –Central Zone- Bhopal Asha Niketan School is providing education and hostel facility to the hearing impaired children from nursery to class XII. The strength of the school is 231 children of which 113 are girls and 118 are boys. Children come from different social background and belong to various categories of castes i.e Dalits, OBCs and other minorities. Asha Niketan School has a Diagnostic Centre which caters to the needs of the hearing impaired children and their guardians. Since inception of the centre in 1991 no major repairs were done. Now they are facing the problem of leakages

and white ants that have made it impossible to function. The project sough financial assistance to make a sound proof room with latest equipments that is required for this purpose. Financial assistance of Rs 19,75,000/- for renovation of diagnostic centre and equipments.  Sanjeevani…Life Beyond Cancer – Western Zone – Mumbai DO I Sanjeevani has been set up for prevention and early detection of cancer and providing care, counselling and rehabilitation to persons fighting cancer with focus on navigating them towards wellness through various activities. Sanjeevani runs Sanjeevani wellness centre and provides Psychotherapy, Nutrition, Emotional freedom techniques, Yoga, Meditation, Music therapy, Dance therapy, etc by trained faculty, leading to a better quality of life post treatment and reduced chance of recurrence. Sanjeevani is conducting awareness camps in schools, colleges and communities to create awareness of why and how of cancer, need for prevention and early detection and healthy life habits & wellness. They are not charging any fees toward holding camps. The project sough financial assistance for purchase of two vans for conducting camps in Goa and Ahmadabad. They have also mentioned that they will conduct approximately 60 camps in and around Ahmadabad & Goa covering over 6000 beneficiaries in 2020-21. Financial assistance of Rs 20,11,200/- has been provided for purchase of two vans .  Navajeevan Blind Relief Centre, Triupathi – South Central Zone – Nellore Navajeevan Blind Relief Center" (NBRC) was established in Kothapeta village near Tirupati in 1979. NBRC has now grown into a major charitable chain with 25 institutions spread over Andhra Pradesh and Odisha including blind schools, orphan homes and old age homes etc. Center is providing residential facilities for blind, destitute, aged and the orphan women and children and free accommodation with food for patients and their attendants. The organization has established the Navajeevan Charitable Eye Hospital in the year 1998, which is only charitable eye hospital in Chittoor District, AP. Free Eye Camps and surgeries are organized in remote villages of Andhra Pradesh and Odisha state. Average 100-120 patients are treated every day. 40% of the patients are treated free of cost and 60% at subsidized rate. The project sough financial assistance forr purchase of medical equipment Ophthalmic Phaco Emulsifier Model ‘Galaxy Orbit’ which provides modern treatment to the patients. Financial assistance of Rs 15,68,000/- has been provided for purchase of medical equipment Ophthalmic Phaco Emulsifier Model ‘Galaxy Orbit 2019-20

 Maharashtra Mandal -Construction of Building for Physiotherapy Centre Maharashtra Mandal Raipur is established in 1935 and registered under Society Registration Act 1860 on 26 January 1944. For the last 83 years, it has been relentlessly working for providing priority services in area of “Education and Health Care.” They are running two Higher Secondary Schools. Sant Dnyaneshwar Hr. Sec School (English Medium) & Sou Kusumati Dabke Hr Sec School (Hindi Medium).Another project is house for physically challenged girls. It is totally free of cost residential program for girls from age 4 years and above. They are also running Divyang Balika Vikas Grih, Hostel for working women and students, community mess, cultural activities, social services in Ramkund Area (Slum Area). They have requested funds for construction of Unique Modern Physiotherapy Centre with all facilities under one roof in their own complex. The Centre would provide all the physiotherapy services at a very low cost which are otherwise unaffordable to common man. Thus they would serve the underprivileged population of Chhattisgarh. Financial Assistance of Rs 25,00,000/- for construction of physiotherapy centre. 5

 Lions Eye Hospital Jaora- Construction of Modular OT for Eye Care-Central Zone- Indore DO Lions Eye Hospital Jaora,(LEHJ) is operated by Lions’ Public Charitable Trust, Jaora which is a non-profit organization established in 1989. Lions Eye Hospital Jaora is one of the hospitals in Ratlam district of Madhya Pradesh, which are providing free eye care and doing eye operations free of cost for poor and needy patients. Currently they are operating three vision centers at Riywan, Dalot and Shivgarh. LEHJ conducts eye screening camps in rural and tribal belt for diagnosing blindness due to cataract, diabetic retinopathy, uncorrected refractive error (URE), glaucoma etc. They also provide free food to the patients as well as free pick up and drop facility.Project Authority has requested funding for construction of operation theatre including equipments and construction of patient ward with total cost of Rs 181.46 lakhs. Financial Assistance of Rs 25,00,000/- has been provided for construction of Operation Theater.  Global Ability in Disability-Global Aid- Purchase of Ambulance with equipments South Central Zone-Visakhapatnam DO Global Aid deals with issues relating to accessible environment for the disabled people, like accessible workplace, public place and to conduct access audits. It addresses issues at large like carrying out community based rehabilitation programs and projects. Global Aid Offices are located in Vizianagaram and Gajapathinagaram. In Gajapathinagaram, Local Lions Club has allocated a premises of around 2000 sqft in the first floor of their club free of cost to the Global Aid, where it is providing hostel facilities to around 80 students who are under- privileged. Global Aid regularly conducts medical camps in the villages for the advantage and awareness of the villagers with the help of qualified Doctors. The Organisation is having the medical experts association, basically an ambulance with equipment would be of great help to the organization during the medical camps. Also it is useful to carry needy people to the nearest Primary Health centre. The organization maintain an office in Visakhapatnam also and from there they are providing basic medical facilities to the fishermen of Bheemili a sea shore town and surrounding villages. The Organisation is helping and supporting the downtrodden and below poverty line people. Financial assistance of Rs.19,56,570/- has been provided for purchase of an Ambulance with equipments.  Ramkrishan Mission Ashrama- Charitable Dispensary- Eastern Zonal Office- Guwahati DO The Ramakrishna Mission is a well known philanthropic organization having its Headquarters at Belur Math, Howrah, West Bengal and has 207 number of branch centres through out the world. This is voluntary NGO wholly managed by qualified monks and is registered under the Society registration Act, 1980 in West Bengal.The Mission is engaged in various charitable and philanthropic activities among needy and downtrodden people of the society in the filed of education, Health Care, Vocational Training for self-employment and also extends its welfare services during natural calamities viz. flood, drought and fire, etc. The Ramkrishna Mission Charitable Dispensary, Chatribari, Guwahati is a highly respectable institution not only for the people of Guwahati but also prominent for the people of North East. Every day around 200 patients are treated free of cost. The poor patients are getting pathological and diagnostic investigation reports at a very minimal cost/free of cost. As such the project will fetch high visibility and publicity mileage. The Project is for part construction and other infrastructure development of Ramkrishna Mission Ashrama Charitable Dispensary at Chatribari, Guwahati. Financial assistance of Rs 22,38,000/- has been provided for construction of Charitable Dispensary subject to naming the dispensary building as “ LIC Charitable Dispensary run by Ramkrishna Mission Ashrama”.  Latika Roy Foundation- Purchase of Four (4) Body Weight Supported Treadmill. 6

Latika Roy Memorial Foundation was registered in 1995 under the Society Act 1860 to provide specialized, localized services to children with disabilities and their families, and help others do the same. Latika Roy Memorial Foundation is working for physically & mentally challenged children from the age group 1-21 years, helping them to develop and grow further to live normal life by winning disability including cerebral palsy. There are eight senior staff members representing operations, awareness, research, rehabilitation, monitoring and evaluation, human resources, follow up, projects and rehabilitation for the organization. They have 114 professionals and para-professionals, each one dedicated to the cause of full inclusion for all. They are providing services at subsidized rate. Financial assistance of Rs.4,25,000/- has been provided for purchase of Un-Weight Mobility Trainer with Treadmill  Cancer Institute (WIA) - Purchase of Liquid Nitrogen Tank - South Zone- Chennai DO I The Cancer Institute (WIA), Chennai was established more than 50 years ago by the Women’s Indian Association Cancer Relief Fund under the leadership of Dr. Muthulakshmi Reddy with a mission to provide scientific treatment and promote health education amongst all sections of the people. The stem cells are required for Chemotherapy treatment. The cells have to be frozen in freezer at -20 degree Celsius or in liquid nitrogen at -190 degree Celsius. The institute sought financial assistance for installation of Liquid Nitrogen Tank. 30% of patients are treated free of cost and another 30% of patients are treated at a nominal cost. Financial assistance of Rs25,00,000/- has been provided for installation of Liquid Nitrogen Tank in the blood bank centre to store the collected stem cells at minus 190 degree Celsius.  Astha The Centre for Geriatric Medicine Palliative care hospital – Purchase of Ambulance - North Central Zone – Lucknow DO Aastha, the centre for Geriatric Medicine, Palliative Care Hospital focuses on improving the quality of lives of elderly people. The organization established in 2003 is a 70 bedded charitable hospital along with 200 to 250 patients in their OPD & Home Care Nursing facility at the residence of elderly patients. Aastha organizes free medical camps, free medication, investigation, counseling, medical helpline & ambulance facility for the poor and the terminally ill patients. Aastha has launched 24 X 7 free medical helpline in 2004 for senior citizens where they can get round the clock medical assistance and free ambulance facility. They sought funds for purchase of Ambulance /Hearse Van Financial Assistance of 24,90,333/-for purchase of Traveler Ambulance/Hearse Van (with Refrigeration unit).  Tata Memorial Hospital – Homi Bhabha Hospital – Purchase of 2 Mobile Blood Donation Van - North Central Zone - Varanasi DO Tata Memorial Hospital is a premier cancer centre in the country providing cancer treatment. Every year around 70,000 new cancer cases are registered. Patients from far and wide as well as neighboring countries avail specialized services. Homi Bhabha Cancer Hospital, Varanasi is an approximately 180 bedded multi disciplinary cancer hospital with a proposed Bone Marrow Transplant (BMI) facility. The hospital is started with objective of treating various cancer patients with different modalities of treatment like chemotherapy, radiotherapy and surgery. During these treatments, the patients continuously require the support of blood transfusion. Only in-house donations of blood cannot meet the daily blood requirements of these patients; Hence, outdoor blood donation camps need to be conducted for they sought funds for purchase of two mobile blood donation van. Financial assistance of Rs 42,19,321/- has been provided for purchase of one mobile blood donation van.

 Tata Cancer Hospital for providing support to cancer patients - Western Zone – Mumbai DO II LIC GJF has provided support to children suffering from cancer and undergoing treatment at Tata Memorial Hospital, Mumbai. GJF had sanctioned Rs. 50 lacs each in the years of 2013, 2017 and 2018. Till now, 146 beneficiaries across the country have been serviced under this scheme. Now, Tata Memorial Hospital has sought funds for the same project for Rs. 50 lacs. Financial assistance of Rs 50,00,000/- has been provided to continue support to the same scheme for the cancer patients.  Sri AVMPSJC Medical Foundation -Sharada Dhanvanthari Charitable Hospital – Purchase of Tempo Traveller Ambulance (9+D) seater with equipment - South Central Zone – Udupi DO Sri AVMPSJC Medical Foundation -Sharada Dhanvanthari Charitable Hospital is rendering medical service mainly to the people of rural area in and around Sringeri in Chikamagalur District of Karnataka State. It is 150 bedded- hospital and has well equipped operation theaters consisting of modern clinical laboratories, blood bank, casualty etc. Every day on an average they give treatment to around 100 patients in OPD which is free of cost and food is also provided free of cost to in-patients. They sought funds for purchase of Ambulance for transporting the patients in deep rural areas and downtrodden to the hospital whenever medical help is required. Financial assistance of Rs. 21,06,074/- has been provided for purchase of Ambulance.  Help Age India –Hyderabad Unit – Purchase of T1Delivery Van 3350WB FM 2.6 CR PS SLD AIR Blower Model Vehicle – for Modification of Tempo Traveler - South Central Zone – Hyderabad DO Help Age India is a leading charitable organization in India working with and for disadvantaged elderly for nearly 4 decades. It was set up in 1978 and is registered under the Societies’ Registration Act of 1860. There are an estimated 106 million elderly persons in India. Help Age advocates for their needs such as for Universal Pension, quality healthcare, action against Elder Abuse and many more at a national, state and societal level with Central and State governments. Hyderabad branch has sought funds for purchase of new mobile van as the present van is not Road worthy and gets breakdown frequently causing inconvenience to both medical team as well as the patients. Financial assistance of Rs14,33,603/- has been provided for purchase of Mobile Van.  Mount Rosary Hospital – Purchase of Eco Ambulance - South Central Zone – Udupi DO Mount Rosary Hospital was established in 1937 in Alangar Village of Moodbidriby Very Rev Msgr.Francis Elias D’Souza. The Society has its own building where they are running Eye Hospital. The Hospital has a team of three doctors, 25 beds facility and has all the facility to treat and conduct surgery of the eye. Hospital has organized eye camps in remote areas of western Rajasthan where patients were being checked for various eye diseases. Patients were operated for cataract, glaucoma, pterygium etc. Project authority has 8

Financial assistance of Rs14,75,000/- has been provided for purchase of Phaco Emulsification System .  Vivekananda Netralaya -Ramakrishna Mission - North Central Zone – Dehradun DO On completion of 100 years, Ramakrishna Mission Ashrama Dehradun, inaugurated “Vivekananda Netralaya”, a multi specialty eye hospital on 18th April, 2018. The Netralaya provides a wide spectrum of services in the field of eye care and eye surgeries. Treatments include complete eye checkups, cataract surgery, trauma and eye injuries. Vivekananda Netralaya has 50 beds facility for hospitalization. Hospital has rendered completely free surgical treatment including travel, food and hospitalization to over 3500 underprivileged patients and served free medical treatment and spectacles to over 12000 patients in the year 2018-19. Hospital realized that demand from poor patients for eye treatment has been continuously increasing and they have limited number of equipment, it becomes difficult at times to check all the patients on the same day. The project sought financial assistance for purchase of one Keratometer which will enable them to check more number of patients on the same day. Financial assistance of Rs 8,50,000/- has been provided for purchase of Nidek Hand Held Auto- Ref/Keratometer with standard accessories.  Simhapuri Vydya Seva Samithi –Jayabharat Hospital –South Central Zone –Nellore DO Simhapuri Vydya Seva Samithi is a voluntary organization working for the development of poor and the welfare of tribal communities in Nellore District for the past 35 years. The Samiti started the Jayabharath Hospital with 10 beds in 1982 and now they have facility of 200 beds. Hospital is having full equipped I.C.U. with four ventilators, two X –Ray plants one being mobile, ultra sound scan, clinical laboratory, ambulance, four operation theatre, dental unit, physiotherapy unit, endoscope, C-ARM , dialysis unit etc. Total 1,89,056 tribal patients were treated so far. LIC GJF has already sanctioned a digital X –Ray Machine in 2013 and with that they are giving free digital X-Ray facility to poor and needy at nominal charges in general and free of cost for Tribals. Since the installation 39,165, X-Ray photos were taken. They have also requested that they have 4 operation theatres in Hospital and only one is equipped with C-ARM from 2007, which is useful in Ortho operations for precision and success. Now that C-ARM equipment became outdated and there is a dire need to replace it. The project sought financial assistance for purchase of new C-ARM machine Financial assistance of Rs 25,00,000/- has been provided for purchase of new C-ARM machine  Dr P.V. Ramachandra Reddy’s Peoples Poly Clinic –South Central Zone –Nellore DO Dr P.V. Ramachandra Reddy’s Peoples Poly Clinic was registered under Societies Registration Act on 28th July, 1953. It is a non-governmental and non-profitable hospital with daily outpatient turnout of 1000. Hospital has 22 doctors as Trustees and is a 200 bedded hospital. About 416 staff members including doctors with 7 fully equipped Operation Theatres are rendering services to the poor. Hospital is providing all treatment at affordable rate. The project sought financial assistance for purchase of 15 (quantity) Make N-600 Model refurbished pulse ox meter & another equipment from Olympus CF H170, Optera Colono Video Scope which will be helpful to detect early stage of cancer in the lower Gastro intestinal system and to monitor the seriously ill patients in ICU, multi para monitors are required. Financial assistance of Rs 5,04,000/- has been provided for purchase of 15 (quantity) Make N- 600 Model refurbished pulse oxymeter and Rs 20,34,876/- for another equipment from Olympus CF H170, Optera Colono Video Scope.  Lions Blood Bank, Hubballi - South Central Zone – Darwad DO Lions Blood Bank, Hubballi is part of International Association of Lions Clubs. This NGO is having 48,000 clubs with 1.4 million members all over the world. Lion blood bank of Hubballi 9

has taken initiative to provide dialysis to diabetic patients through KIMS Hospital (Government Hospital) Hubballi . Diabetes is a growing challenge in India with estimated 8.7% diabetic population the age group of 20 and 70 years. According to world Health Organization, India has 70 million people living with diabetes. The project sought financial assistance for purchase of two dialysis machine for usage of KIMS Hospital Hubballi. At present charges for dialysis is 6,000/- for first sitting & Rs 1,200 to Rs 1,400 for subsequent sitting treatment as per the Doctor advice and also as per the patient’s medical condition. Generally, dialysis is needed 3 times a week. This high price is totally out of reach of poor & especially rural area patients who suffer the most. Hospital is charging Rs 800/- per sitting. Financial assistance of Rs 13,30,000/- has been provided for purchase of two dialysis machine.  Tata Cancer Hospital for providing support to cancer patients - Western Zone – Mumbai DO II under scheme 2019 (A) LIC GJF has provided support to children suffering from cancer and undergoing treatment at Tata Memorial Hospital, Mumbai. GJF had sanctioned Rs. 50 lacs each in the years 2013, 2017 and 2018, 2019 & 2019 A. In toto, 191 beneficiaries across the country have benefited from this Scheme (68 beneficiaries from 2013 scheme, 42 beneficiaries from 2017 scheme, 36 beneficiaries from 2018 scheme, 34 beneficiaries from 2019 scheme and 11 beneficiaries from 2019 A Schemes). The Scheme 2019 A is still in progress. Also support provided for purchase of 51 seater Bus for transportation of patients from TMH, Mumbai hospital to their Research centre at Kharghar, Navi Mumbai in the year 2017. 2018-19

It started its operation in India in 1874 and providing leprosy services to the persons affected from leprosy. PMLH, apart from offering service to leprosy affected persons, it caters to the community for eye care since its inception in 1987. Hospital is empanelled with RSBY(Rashtriya Swasthya Bima Yojana) scheme under which patients from below poverty line have been covered for cataract Surgery by Phacoemulsification. A good cataract workstation comprising of optical biometer enabling to calculate the power of intraocular lens to be implanted and the auto refract meter to give the accurate post operative correction is essential to cater to the needs of the people seeking intervention for visual rehabilitation. Through this project 500 patients would be benefited from cataract surgery, 2000 patients will be given glasses. Financial assistance of Rs 18,00,000/- has been provided for purchase of IOL Master/Optical Biometric.  Ambulance - Tsurphu Labrang- Eastern Zone - Jalpaiguri Tsurphu Labrang, meaning administrative body of the Lama /organization is traditional administrative office of Dharma Chakra Centre (popularly known as Rumtek Monastery) located at Rumtek village in East Sikkim.The Labrang is running a free regular school up to grade 8 for destitute and orphaned children and young monks. There are 220 students registered in the school. A free medical clinic is also run by the Labrang, which provides basic medical care to all institutes under the Dharma Chakra Centre as well as lay community within the Centre and local population of Rumtek. Financial assistance of Rs 6,92,894/ - has been provided for purchase of an Ambulance which will be dedicated more for people who are financially deprived and shall be used for carrying sick and injured persons to and from places of medical treatment, hospitals in Sikkim and West Bengal.

The Hospital started with 40 beds and now they have 120 beds in different categories. JPM hospital has modern equipment and technology i.e operating microscopes, YAG III technology, slit lamp, humphrey field analyser, optical coherence tomography, optical biometry etc. The hospital is providing treatment for cataract, ulcerative keratitis, retina disorder, squint, Glaucoma and operating around 7000 patients every year and examined more than 10,00,000 patients free of cost since inception at Cuttack District. Most of the beneficiaries of this project are a combination of scheduled caste and scheduled tribes. Financial assistance of Rs 4,75,664/- has been provided for purchase of Eeco Ambulance for transportation of patients from rural areas.  Tata Winger Vehicle (School Bus) - Aadhar Gyan Dhatree Samiti- Central Zone – Bhopal DO Arushi Society registered in1995 and working for the empowerment of disabled for last seventeen year at National and state level. The Organization has recording studio to record books & study material in audio for visually & print impaired children, ten classrooms and computers centre, vocational training center etc. They have 38 staff members looking after all the activities. Apart from that centre is equipped with an advanced therapy unit where advanced physiotherapy, speech therapy, sensory integration, audio- verbal therapy are provided by trained and experienced professionals, The children with orthopedic problem, developmental delay, polio and cerebral palsy and other disabilities are also examined and provided therapy. Financial assistance of Rs 11,91,000/- has been provided for purchase of Tata Winger Vehicle for children coming from all over the territory of Bhopal to this centre for therapy.  Hematology Analyzer (Make- UBM Model, FX 19t & blood bag tube sealer - Dr Pattabhi Red Cross Blood Bank - South Central Zone -Machilipatnam DO Dr Pattabhi Red Cross Blood Bank Machilipatnam was established in 2004 by Indian Red Cross Society Krishna District. This is the second blood bank of Krishna district and first branch is in Vijayawada which is 70 km away from this branch. Dr Pattabhi Red Cross Blood Bank Machilipatnam run its services on 24/7 basis and serve to the 23 surrounding mandals. The blood bank has facility to collect, store the blood and dispense the same after thoroughly screening for transmissible infection. They have doctors and other technical staff who are qualified and experienced in sterile techniques of blood collection, screening, storing and dispensing of safe blood. The blood is dispensed on written request of qualified doctors in the requisite pro-forma. Medical officer and administrative staff are maintaining the record properly to prevent misuse of blood. Financial assistance of Rs 3,40,000/- has been providing for ‘Hematology Analyser & Blood Bag Tube sealer” which will be useful for supply of fresh blood to the hospitals.  Funds camera , Non- Contact Tonometer , B- Scan and vehicle )- Parvathi Mahabala Shetty Charitable Trust- South Central Zone -Udupi DO Parvathi Mahabla Shetty Charitable Trust was registered on 22.06.2012. They set up a Parvathi Mahabala Shetty Memorial Eye Hospital at Shrioor Village about 35 Kms north of Udupi. This is the one and only eye hospital in that area. Paravathi Mahabala Shetty trust is working towards prevention of diabetic retinopathy. Their strategy is to do village wise screening for diabetes. They identify the place for retinopathy screening using the primary health centre of villages. They arrange for retinopathy screening using Trinetra Fundus camera and then refer the patients identified with retinopathy to the . They organize Fundus Fluoresce in Angiography examination and treatment with Green Laser, Intra 11

vitreal injections for the patients identified with retinopathy. In the past 3 ½ years the hospital arranged for 3245 cataract operation and out of which 2611 cases were on free basis. They attended 632 school children for screening. Financial assistance of Rs9,55,825/- has been provided for purchase of vehicle for bringing patients from the eye camp site to the hospital for eye operation and after operation taking back to the camp.  Construction of New Dispensary Building -Ramakrishna Mission Ashram, ManasdwipEastern Zone - KMDO-2 Ramakrishna Mission, Manasdwip was established in 1928 at South 24 Parganas of West Bengal. This place is inaccessible by road where cyclonic storm and huge tidal waves take a heavy toll several times a year, situated as it is, bordering the Bay of Bengal. They have been rendering various welfare services in the field of education, health care and natural calamities to the local people belonging to poor and backward communities, unremittingly for more than nine decades. Most of the beneficiaries are SC, ST and OBC.The Ashram is running Homeopathic Charitable dispensary where patients are treated free of cost. Total beneficiaries are 1842 people. This building is very old and declared unsafe by the engineers and experts. The building has to be demolished immediately and a new construction is required.Financial assistance of Rs 25,00,000/- has been provided for construction of new building for Dispensary in place of old one.  Eeco 5 star AC Van -Cancer & Aids Shelter Society - South Zone - Kottayam/ DO Cancer and AIDS Shelter Society (CASS) is promoted by a group of committed women of Kottayam under the leadership of Sr. Dolores Kannampuzha, Medical Mission Sisters, Kottayam. It was registered in 1999 to offer care, financial support, consolation and shelter to needy and poor Cancer and AIDS patients. CASS is running Snehatheeram where more than 100 families of HIV affected are being taken care by the society. They are providing facility of Anti- Retroviral Therapy (ART) medication, financial help for the education of children, sanitation facilities at home, funds for poultry farms and goat rearing, training to women for making paper carry bags, soap, candles, medical check- up facility etc. CASS encourages and insists that the patients should stay with their own families. They are visiting the houses of their patients regularly and monitoring the status of their medication, blood check- up and health condition in general. Presently 125 children are studying with the support of CASS. Financial assistance of Rs 4,49,534/- has been provided for purchase of Eeco 5 star AC Van which will be for travelling to Patients’ homes in many districts of Kerala to provide aids like nutritious food, educational facility, medical and financial support.  Construction of Hospital building- Vivekananda Kendra ArunJyoti, Laipuli, Assam – Eastern Zone / Jorhat DO Vivekananda Rock Memorial and Vivekananda Kendra, Kanyakumari, a society registered in 1963, is a spiritually oriented service organisation with the motto of Man Making and Nation Building as envisaged by Swami Vivekananda. The ArunJyoti Project of Vivekananda Kendra is running two Kasturba Gandhi BalikaVidyalaya at Barap in Tirap district and Wessang in East Kameng district. The school provides formal education, vocational training, computer education and residential facility to the students. They are running 3 vocational training centers, where 40 girls are being trained in tailoring, weaving, pickle making etc. ArunJyoti Project is providing health services, which includes cataract operation, cochlear implant operation for rural population free of cost. They are constructing a hospital in Laipuli, Tinsukia Assam for creating a centre for providing health services. Financial assistance of Rs 25,00,000/- has been provided for construction of 1st floor of the hospitali.echaritable OPD & pathological laboratory.

 C-Arm X-ray machine – Dwarkanath Kotnis Memorial Trust & Medical Research Center, 24 south Parganas, West Bengal - Eastern Zone / KMDO-II DwaraknathKotnis Memorial Trust &Medical Research Centre was established in the year 1996. The Trust was known as ‘DakshinanchalSwasthyaSadan’ registered under West Bengal Clinical establishment Act 1954. It was formed by some eminent doctors and social workers with a view to set up a non-commercial socio-medical organisation. Trust has been rendering medical services to the vast area of South 24 Parganas, particularly to the poor people of the society at the subsidized rate. Trust has started critical care ward with 2 CCU beds, 1 PCU bed and emergency care, now they have 15 bedded hospital with emergency unit. Trust rendered services to around 1300 indoor patients and 130 surgeries have been done in F.Y 2016-17. They also arrange medical camps and provide free pathological test, medicine, check- up facility and also organize disaster management camps in remote areas like cyclone affected, flood affected areas and others. Financial assistance of Rs 10,80,000/- has been provided for purchase of C-arm machine which is useful for converting x –rays into a visible image and also helpful in surgery cases. It will be helpful for providing better services to the patients.  Ambulance - Sabuj Sangha (Nandakumarpur), 24 South Parganas – Eastern Zone / KMDO-II Sabuj Sangha was established in 1975 and has developed into a vibrant organization active in multiple sectors for the inhabitants of the Sunderban region of West Bengal. Presently they are running many projects like residential education, sheltering children from exploitation, save village programme, rehabilitation of deprived children through education, water sanitation and hygiene, environment and disaster response, livelihood, women’s empowerment, computer education etc. They are running KishalyaShishu Shiksha Niketan School at Nandakumarpur and a library cum resource centre at SolgohaliaNimnaBuniyadiVidayalya, Sabuj Sangha has started a health and training centre‘SwarnalataSubujSevaSadan (SSSS) at Nandkumarpur in 2007 to provide comprehensive range of health care needs to the poor people at affordable cost. In 2008, SSSS was empanelled under the Government of India’s Ayushmati Scheme under the National Rural Health Mission for promoting Institutional delivery in under-served areas. SSSS has many facilities like outpatient consultation, 20 bedded maternity ward, 30 bedded eye care, and outreach resource centre with 5 bedded maternity unit, fully operational operation (OT) unit, emergency unit, pathology laboratory, radiology & ECG, 24*7 pharmacy service, ophthalmology unit, referral services, ward nursing training etc. Presently 1,41,914 people are benefited with this programme. Financial assistance of Rs 12,30,741/- has been provided for purchase of Ambulance to provide health service to patients under serious condition, aged persons and pregnant ladies. It is also helpful for carrying patients from residence to health center.  Constructing the first floor comprises of one big hall, two rooms and one library- Sri Sarada Math – North Central Zone – Haldwani DO Sri Sarada Math, Almora, a branch of Sri Sarada Math &RamkrishnaSarda Mission, Dakshinewhwar, Kolkata was established in 1998 and has been serving in the field of education and culture, health and nutrition. Sri Sarada Math, AlmoraBranch is running Homeopathy Dispensary, free camps, health and nutrition programme for women & children. They are providing free treatment and medicine, nutritious food, health drink, milk and multi –vitamins and health tonic to poor patients. They are conducting BalMandir classes for village children, Vocational training of Knitting to rural girls and women to become self- reliant financially. Presently they are running all activities in a small room, which is not sufficient for all participants and guests. They are planning to construct a separate two storied Vocation Training- cum- kitchen Block. Financial assistance of Rs 20,74,613/- has been provided for 13

constructing the first floor comprising of one big hall, two rooms and one library which will be utilized for conducting various educational/vocational/ culture welfare activities for village girls and women.  Advanced life support Ambulance- Jagadguru Sri Shivarathreeswara Medical Service Trust, Mysore - South Central Zone - Mysore DO Jagadguru Sri Shivarathreeswara (JSS) Medical Service Trust, Mysore was established in 1986. Trust is serving society in the field of Education through Primary School, High School, PU College, First Grade College, Polytechnic Engineering College and also Medical Education through Pharmacy, Medical, Para Medical and Dental Colleges throughout Karnataka and a few centers outside the State. JSS Medical College was established in 1984 and the primary health center was upgraded to a 60 bedded hospital in 1986 as a requirement for . Later, it was upgraded into a 1,200 bedded hospital to fulfill the demand for healthcare services for needy people in and around Mysore region. The new JSS Hospital was established in 2008 and provides a facility of 1,800 beds under one roof. It also has one of the largest critical and emergency care facilities with 260 beds. The motto of the hospital is to uphold the value of human life and provide advanced and affordable healthcare to all sections of the society. Financial assistance of Rs. 25,00,000/- has been provided for purchase of Ambulance with advanced life-saving equipment which is essential in case of emergency on account of illness and also accidents.  Five Dialysis machines – Kedari Redekar Shikshan Sansthan, Gadhinglaj - Western Zone -Kolhapur DO KedariRedekarShikshanSansthaGadhinglaj, Kolhapur was established in 1998. An Ayurvedic Medical College and Hospital was established by them in 1999 to educate the youth& to develop Medical Health Services for the people. A 272 bed major hospital complex is also situated in the college campus. The hospital also provides eight Out Patient sessions daily. It is equipped with modern diagnostic and clinical facilities, including operation theatres, modern bio-chemical laboratory, an X-ray unit, ECG, Sonography (USG) as also pathological investigations including H.I.V test. The hospital has conducted 2300 medical camps in the hospital complex and 5000 medical camps in the nearby villages. The hospital is planning to start a new unit for dialysis to cater to the needs of the patients requiring dialysis who have to go either to Kolhapur or to Belgaum which is very far from Gadhinglaj. SDM has opened medical colleges and ayurvedic hospitals in Dharwar, Udupi, Hassan Mangalore and Ujire. SDM Hospital Ujire, opened on 31.12.1980 to help TB patients and other general patients. Now the hospital has 30 doctors and 134 staff members, capacity of 200 beds, 4 OT, 9 ICU beds and 70,188 OPD patients were served in 2017-18.LIC GJF has sanctioned an amount of Rs 25,00,000/- in the year 2012-13 for construction of Obstetrics and Gynecology Unit. They are already running a Mobile Medical Hospital since 45 years to serve the poor in remote villages of Udupi and Dakshina Kannada Districts. LIC GJF has sanctioned an amount of Rs 9,31,631/- in the year 2008-09 towards purchase of mobile medical van. The Trust also runs an eye Hospital at Lalbagh, Mangalore which is equipped with all modern facilities for eye testing and treatment. In 2017-18 total 24,055 patients were benefited. Treatments are done at subsidized cost. There are 4 doctors and 10 staff in the 14

Financial assistance of Rs 13,65,952/- has been provided for purchase of well equipped ambulance for SDM hospital Ujire which will provide transport facility to the patients from their home to hospital in times of emergency.  Tata winger AC Ambulance - Riya Kalyan Evam Shiksha Samiti – Satyam- Central Zone - Jabalpur DO Riya Kalyan Evam Shiksha Samiti is working since 2002 for providing free medical aid and education for poor people and tribal’s of surrounding areas of Chindwada district. The organization is running a charitable hospital catering to the needs of villages which are within 50 Kms reach. The hospital provides pre and post delivery care, dental care, conducts eye check up camps especially for the tribal illiterate population. The organization organizes health camps, eye check up camps in schools and free health camps for senior citizens and arranging for pathological tests. Trust has requested funds for Ambulance which will be useful for transportation of poor patients from the villages to district hospital or any other big hospital. Financial assistance of Rs 11,17,000/- has been provided for purchase of Tata winger AC Ambulance  Ambulance with medical equipment and 3 Dialysis Machine - Maa Sharda Charitable Trust- North Central Zone – Allahabad Division Maa Sharda Hospital has advanced multi specialty center for General & Laparoscopic Surgery, Orthopedic surgery, ENT surgery, Gynecology surgery, Urological surgery, Neurology surgery, Plastic surgery and Pediatric surgery and is the only Multi Specialty Centre with state-of-the-art facility in the city. Maa Sharda Hospital has been established to provide diagnostic facilities related to all type of surgeries under one roof. Hospital treats 100 patients per day out of which 40 patients are treated free of cost and remaining 60 are treated at subsidized rate i.e. 200 per patient. Financial assistance of Rs 13,20,000/- for AC Ambulance with medical equipment and Rs 17,50,000/- for 5 Dialysis Machines has been provided to provide subsidized treatment to society.  Force Tempo Traveller EURO IV 3350 mm WB Ambulance - Saint Hardyal Educational and Orphans Welfare Society (SHEOWS) - Northern Zone - Delhi Division -II Saint Hardyal Educational and Orphans Welfare Society (SHEOWS) was registered in 1994. They established Guru Vishram VridhAahram, New Delhi in 2003 to cater to the needs of old age patients who are in the terminal stages of life and who have no one to support them in their homes. In 2015, a new block of old age home was started which presently facilitates 150 people. Presently, 210 elderly residents are being taken care in the Ashram. They provide nutritious diet, hygienic environment, comprehensive medical care as well as leisure activities. The ashram has experienced doctors and a physiotherapist attending to the patients regularly. Financial assistance of Rs 16,20,780/- has been provided for purchase of an Ambulance to enable them to reach the streets to attend poor ailing people with medical team and also to carry patients if necessary to the Ashram for better treatment. The basic condition for eligibility for the beneficiaries are (1) age of the child be 15

less than 18 years and (2) Annual Income ceiling of the parents should be less than or equal to Rs.1.5 Lakhs per annumn. Funds of Rs 50,00,000/- has been provided to the children who are suffering from cancer. Full cost of treatment is given to the hospital for the treatment of children.

AMB Phoropter and examination Chair - Tarabai Desai Eye Hospital & Research Centre, Jodhpur DO, NZ Tarabai Desai Eye Hospital & Research Centre is a public Charitable Trust registered in the year 1984. Trust established a hospital in 1986 and charges only Rs.50/Rs100 for registration of the patient. The Trust is providing eye care services in the state particularly in the western Thar Desert region.The Hospital has a team of three doctors who treat 100 patients daily and perform 2000 surgeries annually. 110-115 eye camps are arranged in a year.The mobile outreach unit of the hospital visits pre-selected sites and organizes an outpatient clinic on the first day of the camp. The mobile unit also organizes eye camps where patients are screened at a rural location and then brought by us to the base hospital at Jodhpur for surgery. Funding of Rs 24,70,000/- has been provided for purchase of vehicle, Phoropter and examination chair which will be useful for eye checkup and transport to and fro from the site of the eye camp. . Construction of Pathology Lab and Female patient ward - Social Work and Research Centre (The Barefoot College), Tilonia – Ajmer DO /NZ Social Work and Research Centre (The Barefoot College), Tilonia ( SWRC) was founded in 1972 by a group of young professionals invited by Shri Sanjit (Bunker) Roy, after conducting a survey of water supplies in 100 drought prone areas. The SWRC has been addressing rural problems in areas of water, solar electrification, education, communication, women empowerment, livelihood for nearly four decades. Since 1973, SWRC health care programme has provided basic health services to more than 150 villages in 5 blocks of Rajasthan, through a team of doctors, health workers, midwives, pathologists and dentists. Till 2014-15, 8302 patient had been treated in OPD unit. The Tilonia health centre also has a 10 bed facility. They are taken to Kishanghar Govt Hospital or Harmada PHC for delivery. The centre has started sanitary napkin manufacturing unit in 2006-07 which has benefited 5000 rural women. SWRC established an acupuncture unit in 2011 with 4 villagers trained by doctors from England. They have treated patients from 42 villages and have organized 4 camps To enhance health facilities in the Rural Community Health Centre, Funding of Rs 9,98,252/- has been provided fund for construction of female ward, pathology lab and five toilets. . X ray Machine Medilux 300mA and Digital Machine Regins Sigma 2.- Vivekananda Hospital & Research Institute - KMDO II /EZ Vivekananda Hospital & Research Institute is running the hospital since last 50 years with motto to serve the poor people at a minimum cost and runs absolutely at no profit no loss basis. The Trust is providing low cost treatment to the lower income group people at Parnashree and adjoining areas of Rabindranagar, Sarkarpool, Gopalpur, Maheshtala, Garden reach, Bata erc. The Hospital has a team of thirty eight Doctors and 44 employees associated with the Institute. There are 3 operation theatre, for Phacosurgery, Microsurgery and major operations like Cholecysectomy, Appendectomy, Hysterectomy, Laparoscopic surgery and Orthopedic surgery, who treat 100 patients daily and perform 2000 surgeries annually. It is a non-political, socio- religious, cultural organization that has branched out in many directions. It maintains a charitable dispensary for both homeopathic and allopathic treatment, a free physiotherapy unit, conducts a regular night school, a Scouts and Rovers unit and maintains a free vocational training centre. It maintains a free residential student’s home for poor and destitute boys, where 100 boys are benefited. The boys are trained in different vocational courses after regular school. HSRS runs Vivekananda Silpayan, which is a technical training centre where 164 students of downtrodden families are imparted vocational/technical education including computer training in software and hardware, T.V repairing, moped/scooter repairing, motor mechanics, carpentry, tailoring, and stenography. To extend facilities for free medical services to economically and socially weaker sections of society, funding of Rs Rs16,45,818/- has been provided for the 3rd floor of the hospital building particularly for pathology and radiology unit. . Amader Hospital is a not-for-profit organization which was started by FHA in 2008 and is run in collaboration with the local Teghari Gram Panchayat in the tribal village of Phoolberia, West Bengal. They have been providing rational drug therapy to the villagers of Bankura District at lower cost. It has been serving the eye care needs of the community and is a beacome of hope for thousands of people suffering from avoidable blindness and who have no access or cannot afford quality eye care. The hospital has its own building with the facilities like retinopathy, cataract hospital, cornea specialties. The trust is hosting eye donation rallies persistently during National Eye Donation Fortnight between 25th August to 08 September every year’, by which it is spreading the awareness about eye care. The trust has established an Eye Bank named “Dada Virendraji Eye Bank and encourages people to donate their eyes. In India, the percentage of eye donation is negligible, where as in Sri Lanka , this percentage is about 100%. The trust has aimed to make our country also at par in this area. With the instrument the light focuses into the single intense beam that shows the eye’s structure in great detail and it will be serving the needy eye patients at a very nominal cost.  Eeco Ambulance Van - and 25 Wheel Chairs -Hari Om Sewa Dal, Rohtak - North Zone - Rohtak Division Hari Om Sewa Dal, Rohtak (HOSD) is a society registered in the year 1999 with charitable objective of providing food, shelter and relief from distress to the helpless and orphans, education to poor children. HOSD has 800 monthly donors who contribute amount from Rs 20/- to Rs 1000/- monthly. They also receive donations on festivals and special occasion sponsoring meals, clothing, food articles as well as medicines, wheel chairs etc. HOSD has a staff of 20 and 25 volunteers. Their aim is to provide world class cardiac care at a nominal fee and to reach the last man. On an average 273 patients are treated per day and 20% are treated free or at subsidized rate. They have 271 staff and 46 doctors to take care of patients. Financial assistance of total Rs 25,00,000/- has been provided for purchase of Life saving and life supporting new Intra Aortic Balloon Pump equipment for providing cardiac treatment to the downtrodden people of the society under one roof at a reasonable cost.  Ambulance -VVM De-Addiction Research Centre –Eastern Zone – Howrah. The VVM DE-Addiction Research Centre is formed on 9th November, 2003, with the motto of serving the drug/alcohol addicted people and bringing them back to the main stream of the society. The Drug and Alcohol Rehabilitation Centre is a non-profit organization dedicated to substance abuse rehabilitation and restoring personal dignity to afflicted individuals through an intensive healing programme in conjunction with sober living facilities and a stable loving and caring environment. At present they are having 48 beds (all paid beds with a monthly charge of Rs.10,500/- per head and 3 paid doctors. Recently they have opened one centre for Street Children Project (for children at risk) and Psychiatric Patient Centre at “Rupayan” (Biman Nagar, Kaikhali, North 24 Parganas, Kolkata. Financial assistance of total Rs 13, 97,270/- has been provided for purchase of AC Ambulance Van for rehabilitation of Drug/Alcohol Addicted patients.  Adopting poorest village in the poorest district of the country – Purchase of Ambulance and funding for literacy campaign to Banwasi Seva Ashram ,Govindpur - North Central Zone- Varanasi Division. Adopting poorest village in the poorest district of the country – was suggested in the 19th Board Meeting held on 27.02.2016. Accordingly, Govindpur village from Sonbhadra Distraict UP was adopted for providing medical relief and education purpose. The Banwasi Seva Ashram Trust, Govindpur, was established in 1956. Banwasi Seva Asharam (BSA) is a voluntary organization believing in Gandhian principles of life. BSA came in to existence in 1954 when its primary work area –South Sonbhadra was in crises, due to a severe famine. The Ashram has undertaken a variety of development activities in the South Sonbhadra area such as Water and soil conservation, land development, employment oriented apprentice training, rural health education, women’s development and empowerment, education, public interest litigation in Supreme Court for land rights 19

Financial assistance of Rs 1604358 has been provided for two things i.e for Rs 1,91,000/- for literacy campaign and Rs 14,13,358 /-for Force Travelers 3350 Ambulance.  Mobile Ophthalmic Van - Shri Guru Mahipatiraj Eye Bank & Research Foundation Trust Hubli – South Central Zone - Dharwad Division Shri Guru Mahipatiraj Eye Bank and Research Foundation Trust is a unit of well known M M Joshi Eye Institute and established in the year 1979.Trust runs an active Eye Bank, the only one of its kind in North Karnataka and it is affiliated to Eye Bank association of India. It collects about 500 eyes per year. This helps corneal blind persons, some of whom are completely visually disabled. Normally the sight restoring corneal grafting operation costs Rs 20,000/ but they are not charging any fees as the beneficiaries are usually from below poverty line. SGM had conducted 862 free eye camp in the rural area of Karnataka where 12,50,000 patients have been examined & operated for cataract cases through Micro Surgical IOL implants and in last year total 2000 people were benefited. Financial assistance of Rs 25,00,000/- has been provided for purchase of LP 712/42 Tata B S4 Bus and Fabrication & Mounting Mobile ophthalmic van body for providing to & fro transportation facility to the patients from their vision centers located in the slums of Karnataka and neighboring rural areas. The bus will enhance their outreach work and will make eye care accessible to the poor patients.  Five Cochlear Implant System for 5 patients - KEM Hospital, Pune - Western Zone - Pune DO 1 K.E.M. Hospital is a charitable hospital that caters to low socio economic strata of society, many of whom cannot afford this very expensive technology. The Hospital had committed itself to the care of challenged children in 1979, with the inception of the TDH Rehabilitation Centre which is the first of its kind in India and was started in collaboration with a West German Social Service Organization – Terre des Hommes. The main aim of the Centre is to provide diagnostic and therapeutic services for all kinds of disabilities under one roof. Financial assistance of Rs 25,00,000/- has been provided for five cochlear implant surgeries to five children who needs urgent surgery.  Cost of Cancer Treatment – TATA Memorial Hospital, MDO II Cost of cancer treatment is provided to the children who are suffering from cancer. Full cost of treatment is given to the hospital for the treatment of children.

 Construction of Health Training Centre- Sreema Mahila Samity, Nadia– KSDO/Eastern Zone Sreema Mahila Samity, Nadia was started in year 1972. The Mission of the Organization is holistic development of rural community of West Bengal through empowerment of poor and marginalized women by enabling them to access social and economic opportunities. It has been 20

recognized as District Training Centre by Health & Family Welfare department of Govt of West Bengal. The NGO had received funds from the Government for implementing two Govt projects viz. Child line and Accredited Social Health Activists (ASHA). About 200 trainees are imparted residential training free of cost, particularly to ASHA with the support of Govt of India under the National Rural Heath Mission. Sreema Mahila Samity is a NGO and it is engaged in various governments, non-government as well as self-funded projects with the support of 299 staff and over 100 volunteers. Funding of Rs 25,00,000/- has been provided for Construction of Health Training Centre to provide training and awareness programs for upliftment of the marginalized people, particularly women.  Construction of first floor on existing building for the extension of health services- Mamudpur Unnayan Parishad, Mamudpur, Hooghly Distt /Howrah – Howrah DO/EZ Mamudpur Unnayan Parisad (MUP) was registered as Society in March 1981. It is a NGO funded and managed by limited financial contributions by the members and organizations. The aim of the establishment is to provide wider range of medical help and care through medical health programme at affordable cost. The organization receives grant of Rs 42,382/- every year from West Bengal Social Welfare Board to run a creche Unit. They are providing Health care programme i.e. Creche, Eye Care, Mental Health, Physiotherapy to the people who are living in rural area of Hooghly Distt, West Bengal. MUP provides need based education as well as aids and appliances to 552 disabled children. During the last year 68 cataract operations and 52 mental patients were treated free of cost by them. On an average, they are treating 22 patients per day out of which 20% patients are treated free of cost. Majority of the people of this area depend on the agricultural income which is very meager and it is very difficult for them to spend money for the treatment and education of their children. Funding of Rs 12,30,875/-has been provided for Construction of first floor on existing building for the extension of health services to people living in the rural area of Hoogly district.  Construction of building for Physiotherapy and Acupressure Centre - Sewa Samiti, Pali – Jodhpur DO/NZ The Vriddhashram was established in 1990 and is being run by Sewa Samiti in the area of Marwar, Rajasthan for old and elderly needy persons. They are running Ayurvedic Hospital in the premises. This hospital provides free treatment to old age home people and people living nearby the hospital. The Vriddhashram takes care of old age patients who are in the terminal stages of life and who have no one to support them in their homes. Presently 70 persons are living in the old age home which provides accommodation of one room to each person and in case of old age couples, rooms accommodating two persons are provided. There are 51 rooms, 6 halls, a mess and a store room. Medical facilities are available for elderly people. The elderly persons usually suffer from knee pain, back ache, joints pain, other old age problems and physiotherapy and acupressure is the best solution for them. Funding of Rs 25,00,000/- has been provided for construction of building for physiotherapy and acupressure centre for destitute old and needy people.  Medical Equipment for Spinal Injury Centre - Jyoti Institute of Medical and Rehabilitation Sciences (JIMARS), Bundelkhand- Allahabad DO/NCZ Jyoti Institute for Physical Medicine and Rehabilitation Sciences (JIMARS) previously called Viklang Kendra was established in 1974 at Allahabad. JIMARS is taking care of the marginalized sections of people who are suffering from disability especially those in agony, infirm, infants, senior citizen and youth affected by crippling disorders. Institute has been functioning effectively since last 38 years. More than 6 lakhs disabled persons were benefited till 2015. They are running special education centre for mentally and physically challenged

children. They provide special education, speech therapy, physiotherapy, occupational therapy, play therapy and activities of daily living etc. Funding of Rs 5,37,378/- has been provided for Purchase of three Medical Equipments for Spinal Injury Centre at Allahabad to provide facility to spinal cord injured patient in the area of Bundelkhand and save the lives with kidney infection/failure, bedsore and consequent infection or septicemia.

 X-Ray Machine- Ramakrishna Mission Ashram, Kanpur (Branch of Ramakrishna Mission, Belur Math, Howrah) –Kanpur DO/NCZ Ramakrishna Mission Ashrama, Kanpur is a branch centre of Ramakrishna Mission, Belur Math, which was registered under Societies Act of 1860. The Ashram extended its services through “Palli Unnayan Prakalpa” which is an Integrated Rural Development Programme since 1988 to uplift poor, backward people through education, health, self-employment training, community development etc. The Mission runs a Charitable Hospital since 1939 where they offer both Allopathic and Homeopathic treatment. The hospital is equipped with a diagnostic facilities like X-Ray, laboratory for pathological test, ECG and radio–sonogram. During the year 2014-15, the hospital has treated total 1, 02,065 patients free of cost. Due to increasing number of patients per day, existing machines are not sufficient for testing. Hence, funding of Rs 10,10,000/-has been provided for additional X-Ray machine which will be helpful for timely treatment of poor people living in the remote areas of Kanpur.  X-Ray Machine and Mobile Medical Van - Ramakrishna Mission Math, Nattarampalli, Vellore (Branch of Ramakrishna Mission, Belur Math, West Bengal) – Vellore DO/SZ Ramakrishna Math, Natrampalli was established in 1978 and is a branch of Ramakrishna Mission, Belur Math. The Ashram does philanthropic activities in the field of education, medical, relief operations, preaching, spreading ethical and moral values and publication of Ramakrishna-Vivekananda literature and Holy Scriptures. It runs a free Students Home (orphanage) for boys. 80 students from economically poor background and orphans from tribal areas are benefitted. They run a Rural Charitable Dispensary which is equipped with X-Ray machine and a Lab. They are serving about 200 patients daily with the help of 3 doctors who are working on honorarium. People are coming for treatment from about 104 surrounding villages. The mobile dispensary caters to about 87 villages treating on an average 100 patients per day for six days in a week. They are providing free medical service to the patients. Ashram runs a physiotherapy unit for people belonging to economically backward class on no profit no loss basis which is serving 50-60 patients daily. Funding of Rs 12,15,000/- has been provided for purchase of Digital X-Ray Machine to replace a 30 years old machine having frequent problems.  Four Fresenius Dialysis Machine - Guru Shri Gorakshnath Chikitsalay, Gorakhpur- Gorakhpur DO/NCZ Object of the Trust is to facilitate the health of people of Eastern UP, Western Bihar and Tarai. The deficiency of health care and treatment facilities in these areas is being taken care by Gorakshpeeth through their institution viz Mahant Digvijainath Ayurved Chikitsalaya and Guru Shri Gorakshnath Chiktsalaya. The main objective of the Hospital is to treat ailing patientsand to provide services at reasonable cost by offering concessions in fee to those who cannot afford. Funding of Rs 25,00,000/- has been provided for purchase of four Haemodialysis Machines for the treatment of patients from poor community.  Blood Donation Mobile Van - Mahavir Cancer Sansthan - Patna / Patna DO-II / ECZ MahavirMandir Trust started the Mahavir Cancer Sansthan and Research Centre (MCSRC) at Patna, realising the huge burden of the cancer patients and lack of adequate treatment facilities in Bihar and Jharkhand. With a population of about 13 Crores in the two states, approx. 22

80,000 new cancer patients are added every year. MCSRC is a comprehensive state-of-the art charitable cancer institute with all the treatment facilities under one roof. Within a short span of time it has become the Centre of Healing for cancer patients and has met the requirements of economically weaker sections of the society. On an average 350-400 cancer patients are treated per day in the hospital. Mahavir Mandir helps by giving Rs 1.25 Cr. per year for poor patients and the Hospital also gives almost equal amount of concession to the needy per year. The Hospital requires more than 45,000 units of blood every year for cancer patients. It has the Mission of ensuring timely access to quality leprosy services that result in improved outcome for leprosy affected people.It is working on OPD basis especially in Ulcer care, medication, sustainability of leprosy affected person through providing education to their children, providing low cost housing and for general dermatology cases, provide them medication.They are also running 6 vocational training centres and train the most deprived and vulnerable young people affected by leprosy. TLMTI’s runs several community development projects like Comprehensive Health, Advocacy, Networking, Empowering Communites Project, etc thus contributing substantially toward restoring dignity of communities they worked with. Ms Anju Chaurasia from TLM Hospital Barabanki, received the National Award for Empowerment of Persons with Disabilities 2014, for Best Employee (leprosy cured) at the hands of Shri Pranab Mukherjee, Hon’ble President of India. An Ambulance (TATA Winger – non AC) has been provided for mobility of patients to medical centre for treatment.  Slit Lamp Imaging Model (for diabetic retinotherapy) and DGH 550 Pachette (for corneal thickness measurement - Navjeevan Blind Relief Centre- Nellore DO /SCZ "Navajeevan Blind Relief Center" (NBRC) was established in Kothapeta village near Tirupati with a motto of rehabilitation of the blind, orphan children, destitute aged people and for prevention of blindness. The Centre was registered under Societies Registration Act in the year 1982. NBRC has now grown into a major charitable chain with 25 institutions spread over Andhra Pradesh and Orissa including blind schools, orphan homes and old age homes etc. Navjeevan Charitable Eye Hospital established by NBRC is only charitable eye hospital in Chittoor District, AP and has achieved a rare distinction in the field of service by bagging the ISO 9001:2008 certification for the betterment of lives of visually challenged. Navjeevan Charitable Eye Hospital is providing free medical services to eye patients including IOL/ICCE cataract operations and Yag laser treatment. Table tennis, Cricket coaching, Carom training, Chess tuitions etc was started at Prabodhan which has been giving great careers for many students with a sole aim of making more and more candidates capable of participating at higher level and getting success in the same. Any disabled can visit for artificial limb and register his name without any appointment. For fitment of a limb or caliper they have expert medical professionals. If any correction surgery is required, they contact Raipur Institute of Medical Science.RKB foundation is running a project namely “Smile Train” under which they are providing cleft lip and palate correction surgeries. They are doing cataract surgeries free of cost and have granted financial assistance to the poor and needy persons.Hospital also provides facility of dialysis every day on cost to cost basis and also provides medicine at subsidized rate to the general public irrespective of caste, creed, sex and religion. RKB was established as a well-equipped Artificial Limb and calipers manufacturing centre in association with Raichur Institute of Medical Science by providing free premises with water and electricity supply. One pick up van has been provided which will be useful in carrying the staff, patients and also carrying the artificial limbs to the camps and also disabled persons from their centre to the Hospital to regain their mobility. It is basically a philanthropic organization run under the ideals of Swami Vivekanand. It works mainly for downtrodden and poorer sections of the Society. This Ashrama has been serving poor and needy people in the field of education, health care, social upliftment and cultural programmes. Bal Bhawan (Anathashram) - Karnal/NZ : Mission to the Desperate and Destitute of India was established with objective to provide care, shelter, training and capacity to the destitute and underprivileged, marginalized who live in streets pavement, slums, villages irrespective of religion, caste, creed, language and community. They are providing shelter for orphans, widows, handicaps and uncared old aged with love and care, food, clothing, education, medical care and other human necessities. M.D.D Charitable Dispensary run at the premises of MDD Bal Bhawan Anathashram is providing medical care to the inmates of Anathashram and also to the residents of nearby villagers free of cost. Visual Field Testing Machine is to test field of vision of patient to diagnose and monitor the progress of Glaucoma, a controllable cause of blindness.  Humphrey HFA Visual Field Analyser – Sonamukhi Pragati Netralaya, Asansol Division Sonamukhi Pragati Sangha is a non-political, non-religious, Non-Government Organisation registered under Foreign Contribution (Regulation) Act 1976 (FCRA). It is a Philanthropic Organization at Sonamukhi under Bankura District, West Bengal, India. They are providing treatment to eye patients in the area of Bankura and neighboring area. They are also arranging 3-4 free eye camps in a month. They have started Village Survey for screening of ocular patients in the area and in neighbouring Blocks. After screening, poor & distressed ocular patients are brought to the Netralaya for treatment and operation. The Sangha will be able to render more sophisticated and advanced treatment to blind patients to restore eye sight.  C-arm Machine and Digital X-Ray – Rehabilitation Centre for Children, KMDO II Rehabilitation Centre for Children has been providing surgical interventions for the last 41 years to the orthopedically challenged children belonging to the poorer sections of society. Apart from surgery, the Centre provides mobility aid appliances to ensure that children can lead near normal lives. The Centre endeavours to provide holistic treatment that goes much beyond surgical intervention. Once the child is medically evaluated, the surgery is performed. Post surgery the child is kept in the hospital ward for post operative care. Later the child is shifted to the residential block where the rehabilitation process begins for which the duration is 2-10 months depending upon the severity of the deformity. Beneficiaries will have advantage of undergoing precision surgery thereby ensuring lesser time in the OT and faster recovery period.  Construction of building for 60 bedded Female Patients Ward – Seva Bharati Chikitsalay, Udaipur Division Seva Bharti Chikitsalaya is run by Seva Bharti Samiti, a Non Government Organization. It provides free and quality medical services to the poor and needy people of slum areas.It imparts training to the poor and is engaged in various programmes of education, health care and social awareness. Total 1,68,000 patients have been treated and 5700 patients suffering from serious diseases have been benefited from the Chikitsalaya. Construction of Ward will be beneficial for the poor and needy female who will be getting treatment free of cost in good environment conditions.  Construction of two additional floors for providing free lodging to poor patients – Ashraya Charitable Trust, Kottayam Asraya Centre is run by Asraya Charitable Trust which is established for the social development, development of youth, helping poor patients admitted in Medical College Hospital, Kottayam by providing them free lunch, lodging, shelter for the homeless orphans and aged and providing security for women and children. Asraya Centre is providing free lunch to the patients and their bystanders and also free medicines and clothes to them. Social workers along with members of the Trust visit patients in the Hospital daily and counsel them and help in prayers.Members of the Pain and Palliative Care Unit under the centre visit homes of those who are seriously ill and provide help and consolation.  Laser equipment – Sri Sukabrahma Ashramam, Nellore Division Sri Sukhabrahma Ashramam was established by Sri Vidyaprakashananada Giri Swamiji in the year 1950 which is rendering the best services in the area of free eye operations to the poor people. Free Medical Camps are also conducted every month in the neighbouring villages in which six specialist doctors provide free medical aid. They are having modern Operation Theatre, Computer Machinery and necessary instruments for the treatment of eye diseases. They are providing treatment to 23 patients per day and all the services provided are free of cost. So far 18062 eye patients were operated and lenses were set in their eyes. Also 197263 27

out patients were examined and treated for eye ailments. There are so many patients who are operated for cataract in the Ashramam and again suffer from it. For providing them proper treatment, they need laser machine to provide free laser operations.  Ambulance with life saving equipments – Swami Vivekanand Medical Mission The Mission was formed 35 years ago by a team of dedicated persons to provide medical aid to villages in the outskirt of Nagpur. It initially provided primary treatment to patients of various villages adjoining Khapri. Now the Mission is capable of providing secondary level treatment of various disciplines to patients. The Mission is organizing Eye Diagnostic Camps, Blood Donation Camps, Dental and Oral Health Camps, Ultra Sonography of poor patients and General Diagnostic Health Camps in villages every year. With advanced life saving equipments like transport ventilator, cardiac monitor, defibrillator etc fitted with Ambulance, the accident patients and other critical patients like snake bite, insecticide poisoning, heart attack patients will receive effective and timely treatment thus saving their lives.

The medical services rendered by the Ashrama have been of immense help to poor local people to obtain proper treatment for various diseases and suffering. They are providing medicines of homeopathy or allopathy, consultancy, free medicine distribution, ECG at subsidized rates, 24 hours ambulance service with oxygen, X-ray, USG, CT Scan etc. Air conditioned Ambulance will be used to carry the patients to nearby hospital especially for senior patients who need ICCU medical attention.  Construction of Charitable Health Centre – Chatra Kalyan Samiti, KSDO Chhatra Kalyan Samiti is related to social welfare, educational service, health service, nursery school, training centre and other welfare activities for students. They have implemented various programmes such as pre school crèche centres, condensed course for adult women, nursery and primary school. They are also running an Anganwadi workers training centre which has been sponsored by social welfare department and co-ordinated by Ramakrishna Mission Lokeshiksha Parishad. 28

They also provide small income project i.e. weaving and tailoring for the rural poor women. They have organized Legal Awareness Camps, Health Awareness Camps and General Awareness Camps keeping in mind the urgency of the needs of poor people.  TATA Winger Ambulance – Lok Bikash, Bhubneswar Division Lok Bikash was formed in the year 2005 which provides proper health care facilities for slum dwellers and making provision for health education to women and children to enable them for employment through the support unit “Padma Hospital”. The prime activities of the organization are to provide free treatment to the slum dwellers of the urban as well as rural area. Special health awareness camps are conducted by the institution in different localities to prevent them from drug addiction and educate them for better healthy life. They are treating 20-25 patients at Padma Hospital per day and out of them 5 patients are treated free. The total number of inmates in the Institution is 145 from whom no fee has been charged.  Two Ambulance - Cancer Institute (WIA), Chennai I Division Cancer Institute (WIA) Womens India Association was established in 1954 by Dr. MUTHULAKSHMI REDDY who was the First woman in India to graduate in medicine – 1912. It is registered under Socieities Registeration Act 1860. The Institute is having two centers, one at Gandhinagar and other one is at Sardar Patel Road. The aim of the Institute is early detection and prevention of cancer through proper planned treatment and work towards eliminating cancer. They are providing treatment to 487 patients per day out of which treatment is provided free of cost to 292 patients. Zaveri Mangalji Vamalshi Dispensary is a Charitable Trust situated at Palanpur. The Trust was started to serve the poor without any discrimination on the basis of creed, Tribe or religion. The Trust is providing concessional & free treatment for General Surgery, Dental Surgery, Children & Gynae treatment. They also provide outdoor treatment by General Physician to the poor and needy persons. The average number of patients treated per day is 300. Some patients are treated at subsidized cost and some free of cost. Laparoscopy (Telecam SL II with xenon) with Laparoscopy hand instruments will help them to cope with surgery treatment for the poor patients coming from the remote areas of Banaskantha and Rajasthan.  Repairs & Renovation of ground floor of the hospital - Ramakrishna Mission, MDO III Division Ramakrishna Mission Khar Mumbai is a branch of Ramakrishna Mission, Belur Math. Ramakrishna Math and Ramakrishna Mission are worldwide, non-political, non-sectarian spiritual organizations which have been engaged in various forms of humanitarian, social service activities for more than a century. The Hospital was started in 1924 with the aim to provide quality medical services at affordable prices. The patients are treated without any distinction of caste, creed or community. The average number of patients treated per day is 400. Approx. 2000 patients are treated free of cost and 800 patients are treated at a subsidized cost per year.

 LIC Medical Equipments for setting up of operation theatre - Ramakrishna Math, Antpur, Howrah Ramakrishna Math Antpur is an affiliated branch centre of Ramakrishna Math, Belur Math founded by Swami Vivekanand .Ramakrishna Math is providing education services, medical services, general welfare services, drinking water supply etc. to the poor people. The Math runs Allopathic and Homoeopathy clinics through its two branches for patients who get treatment and medicine free of cost. The doctors of general medicine, eye, dental, gynecology, pediatrics attend the medical units and four other doctors are in homoeopathy department. LIC Medical equipment together will form a complete operation theatre unit for the Organisation.  LIC Pathological and Radiological Instruments and Ambulance - Dr. Prabhat Memorial Society, KMDO II The Organisation was started with a social commitment for the development of the rural poor and marginalized sections of society by Ms Maya Mondal teacher Krishnachandrapur High school, in the memory of her late brother Shri Prabhat Mandal ex professor Allahabad University. The Institute is running free health clinic (OPD) for the last ten years for the poor people particularly women and children of AILA affected Sunderban areas who do not have enough money for private treatment. LIC Pathological & Radiological equipments and an ambulance will provide medical services to the economically backward people of the surrounding areas. The Ambulance will also be used for carrying patients from remote areas of Sundarban to nearest Hospitals and to Kolkata.  LIC Medical Van with Equipments - Gandhi Mission Trust, Kharagpur The Trust is working in the Paschim Medinipur district which has four sub divions Kharagpur, Medinipur sadar, Ghatal, Jhargam and 29 blocks which are predominantly rural and difficult to reach. The Trust is involved in different social activities meant for rural people in the backward region of the district. Nimta Dr. Amitava Basu Smriti Raksha Committee was formed in the year 2003. This organization caters to people below the poverty line around Nimta area by providing medical support and pathological tests on concessional rates. They have arranged Blood Donation Camps, Free Medical Checkup Camps. Distribution of free clothes to the poor people and issue of BPL cards to the poor people so that they can get the medical support on the concessional rates. The Jayabharath Hospital was started with 10 beds in 1982 and at present the bed strength is 150. More than 50 consultants of various specializations visit the hospital and 15 doctors are available all through in the Hospital. They have 185 Paramedical and other staff in the Hospital. They have conducted medical camps at Marripadu mandal and have given Hepatitis ‘B’ vaccine for nearly 3000 children aged less than 15 years. They provide free cleft lip surgeries to thousands of poor children and treat the Child with comprehensive and rehabilitative care including Speech therapy, general dentistry and orthodontics. The instrument will be helpful in treatment of patients with high myopia and infants with retinopathy of prematurity.  Construction of Rehabilitation Centre – Amar Jyoti Charitable Trust, Gwalior Division Amar Jyoti Charitable Trust is providing inclusive education in a barrier free environment to a group of over 650 students with and without disability in Delhi and Gwalior. Vocational skills and cultural activities are a part of the curriculum. The Trust is also providing mobility aids, appliances, corrective surgery, therapeutic services, X ray unit and pathological lab, OPD and immunization programme in the premises. Seven free coaching centres are being conducted for poor boys and girls from lower income group of this locality. Free eye operation camps are also organized every year for treatment of poor. More than 15000 patients have been treated this year. Free medicines are provided to the patients and free eye camps are also organized. They are treating 100 patients per day free of cost. The medical van will be equipped with a few sophisticated equipments for on the spot diagnosis.  LIC Medical Van - Oju Welfare Association, Jorhat Division The Association is one of the pioneering and philanthropic organizations of the state of Arunachal Pradesh. Since its inception in 1979 the organization emphasized more on the significance of education and thereafter registered to promote multi-dimensional welfare activities to the cause of the people-in-need in both rural and urban areas. The vehicle is to be utilized in case of medical emergency, since there is no hospital nearby. The vehicle will also be used for medical awareness programmes.  LIC Ambulance – Ramakrishna Mission, Jorhat Division Ramakrishna Mission, Narottam Nagar Branch was started in 1971 to serve the tribal people of Arunachal Pradesh at the request of the then Prime Minister Smt. Indira Gandhi. The Institute runs a Higher Secondary School affiliated to C.B.S.E. for the tribal children. At present there are 526 students studying in the school from class I to XII. This School ranks 1st in the state, and it is in great demand by the students community. A 30 bedded hospital and a mobile dispensary is also run for students, staff and nearby villagers. LIC Ambulance will enable them to serve poor patients in nearby villages.  Construction of Hospital for weaker sections – Belur Sramajibi Swasthya Prakalpa Samity, Howrah Division Belur Sramajibi Swasthya Prakalpa Samiti has been relentlessly offering health care services to the distressed suffering millions from all walks of life since last three decades through Belur Sramajibi Hospital at the most affordable prices. The Organization is working sincerely for the socio economic upliftment of the weaker section in different ways. They conduct programs on Health and Social awareness, early Cancer detection and awareness, camps on Health, Eye, Child care, Family Planning, Regular Blood Donation etc. The Organization 34

LIC Medical Van will enable them to cater better services to the poor people.  LIC TATA Winger Ambulance – Ramakrishna Math Ghatshila, Jamshedpur Division Rama Krishna Math Ghatshila is a branch of Ramakrishna Math, Belur Math Howrah and is engaged in several welfare activities in remote tribal areas of Ghatshila. Ram Krishna Math is working continuously towards providing healthcare services in Ghatshila area. The Math also has a mobile unit touring remote areas to provide medical aid. The Math has a local Dispensary which is regularly visited by poor people who are given free treatment. Apart from this, the organization goes to different villages on different days of the week with their medical team and sets up a mobile medical camp in the villages where people come from adjacent villages for free medical treatment. LIC Ambulance van will enable them to take poor primitive tribal people who are seriously ill to Jamshedpur as only primary medical facilities are available in Ghatshila.  Construction of second floor of hospital building - National Society for the Prevention of Blindness, New Delhi Division The organization is working towards prevention of blindness particulary avoidable blindness in India focusing on persons who are poor, living in remote areas, illiterate and destitute and engage in advocacy, education and surgical intervention, wherever necessary through its dedicated team of volunteers and doctors in its various hospitals and branches across India. The society runs three eye hospitals directly under its control at Motinagar New Delhi, Modinagar UP and Raison (HP) to provide the gift of sight to as many beneficiaries as possible. LIC Multipurpose critical care ambulance along with ECG machine will enable them to provide better services.  LIC Traveller fully equipped Ambulance – Karnataka Health Institute, Belgaum Division Over the past 78 years, Karnataka Health Institute has evolved into a bustling Hospital Complex(on 212 acre plot) The Hospital complex has a general hospital, a 150 bed TB sanatorium, a 60 bed maternity hospital , nurses training school and a primary school in addition to other facilities for the inmates. There are professionals looking after the general hospital and sanatorium, projects in basic health services, health education to rural masses, social welfare, primary and vocational education. At present 250 to 300 patients are treated per day. 95 % patients are from the poor sections of the society and about 200 patients are treated free of cost per month. Veerendra Heggade ‘Karnataka Ratna’. Shri Kshethra Dharmasthala has opened medical colleges and Ayurvedic hospitals in Dharwar, Udupi, Hassan Mangalore and Ujire. Free boarding and lodging facilities are provided by the temple to the devotees visiting Dharmasthala. Nearly 3000 persons are treated as in and out patients every month. Construction of Obstetrics and Gynaec unit will provide free/subsidized medical aid to the rural population of the area.  Construction of building for HIV infected children – St. John Health Services, Thiruvananthapuram Division St. John’s Health Services is a non-government organization and it is the official health organ for the community hospital of the Syro-Malankara Catholic Major Archiepiscopal Church of Trivandrum, Kerala State. Over a period of 49 years the centre reached out to 10463 leprosy affected people by providing them care and medical treatment. For the past nine years organization has extended its services to those suffering with Tuberculosis. In 2011, 47721 suspects were examined by them. Kerala AIDS Control Society has approved St. John’s Health Services as the Community Care Centre for HIV/AIDS patients in 2007 to provide care and support to the HIV patients. The new building will enable them to provide accommodation for the HIV infected children for better education and skills to manage their lives successfully.  LIC Sonography machine - Tribhuvandas Foundation, Nadiad Division The organization was founded by late ShriTribhuvandas Patel, Founder of AMUL & winner of Magsaysay Award. It is a Community health organisation, working mainly on reproductive and child health. The Foundation covers around 682 villages, working through the village milk cooperatives of AMUL, in 18 talukas in the districts of Anand and Kheda in Gujarat. The organization caters to mentally challenged adults aged between 18 to 70 years. Their aim is to provide complete relief to their parents from their worry of caring for their children after their demise. Construction of residential facility for mentally challenged students who will settle in the institution, will relieve the parents from the worries and make the life of people therein happy and meaningful.  LIC hemodialysis Machine– Rayat Aarogya Mandal, Nanded Division Rayat Rugnalaya is located in the heart of the city and working since the last 7 years. This is a 30 bed Hospital with basic specialty services. In the last 4 years the Hospital has done more than 1000 major and about the same number of minor surgeries at very reasonable charges. 10% free beds and 10% half concession beds as per the rule of Public charity trust Act are reserved for BPL patients. Organisation is providing “Rational Medical Care at affordable cost” as per its motto. The charges are heavily subsidized as the centre is created out of donations. LIC Medical equipment for diabetic retinopathy will enable them to provide the treatment of eye related retina and Glaucoma affected persons at lower rates.  LIC Mobile Medical Van for Blood Donation – Late Kakasaheb Chitale Smruti Kendra, Thane Division The Jankalyan Raktapedhi is the blood bank managed by Late KakasahebSmruti Kendra, which is a Trust founded in 1981, to cater to the needs of people from Panvel to Ratnagiri. The blood bank is located in a mofussil area wherein no blood transfusion services are availableand provides free blood to thalassemia patients of villages from Panvel to Chiplunas thalassemia is a disease affecting children who require frequent blood transfusions for their survival. They conduct around 60 blood donation camps per year and they have collected around 2000 blood bottles per year.The Trust caters service to about hundred Hospitals and Nursing homes in the area. A well equipped Mobile Van will enable them to cater to the needs of society to a large geographical area.  LIC Mobile Medical Van – Divya Jivan Sangh (Divine Life Society), Bhavnagar Division The Trust aims to provide doorstep services to patients for general diagnostic and medical aid, eye camps for cataract operations, distribution of spectacles, medical camps for children and women and also publicize health awareness in rural areas. The Trust is running a dispensary for the last 32 years for diagnosing and treating patients at nominal cost. The average patients treated per day are 75 to 100. The Trust was giving free medical treatment. LIC Mobile Medical Van will enable them to cater to the poor and needy patients in the backward areas/slum areas of Bhavnagar City and surrounding villages.

They have treated nearly 48,000 patients till today. Average number of patients treated per day is 150 and the total number of patients treated free till December 2010 is 1404. 1396 patients were treated on subsidized rate. Apart from these patients, hospital provides various investigations and treatment facilities free of cost to the Gas Victim cancer patients as per the arrangements with the State Govt. LIC medical equipments will enable them to provide better services.  LIC Audiometry lab - Samanvay Pariwar Trust under Jabalpur Division The organization was started on 21.11.1995 with the objective of providing free rehabilitation and development services to physically handicapped children. The trust is supporting through artificial limb, audiometric equipment, calipers, tri cycle, sticks for blinds, walking sticks, sewing machine, brail slat, artificial hand and wheel chair. LIC audiometry lab will enable approximately 1000 patients per year to take proper treatment.  LIC ICU Medical Van - Ramkrishna Mission Vidyapith under Bhagalpur Division The organization has started a Mobile Medical Unit to serve the poor patients of remote villages around Deoghar since the year 2008. LIC Ambulance with Life saving equipments with the capacity to carry 135 passengers per day will be helpful for timely help to poor patients in the ambulance itself.  LIC Ambulance - Community Health Implement &Literacy Development Society, Chennai DO Since June 2004, this organization have been conducting free eye and general health camps and have been meeting the demands of the needy by providing the necessary medical aids. Every month they are conducting free general health and eye camps twice. In the general health camp, diabetic screening and diabetic care has been provided for the people at free of cost which also includes medicine and supplements. This programme is based on innovative concept of “Community Centric Sustainable Health Delivery Model” which provides wide range of promotive, preventive and curative health services to the beneficiaries. This is a 25 year old organization and has undertaken multifold activities viz., Educational Programmes, Medical Camps, Vikalang Sahayata, Sanagra Grama Vikas wherein they have fully developed a village Kumbarkoplu near K.R. It is a reputed Eye Hospital and is well equipped with all the facilities. Their aim is to play a major role in reduction of preventable blindness in the state of Maharashtra. Foundation has contributed to some of such families which help them in rehabilitating the children into the mainstream of society. 5 Cochlear implants were sanctioned to children residing at Pune, Nashik, Aurangabad, Srinagar and Baramati, belonging to weaker sections of society.  LIC mobile medical camps - Ramkrishna Mission Tribal Health &Welfare centre, Thane DO Ramkrishna Mission is working since 1972 for providing medical help to weaker sections of society in rural area. LIC Day Care Centre is a comprehensive rehabilitation centre for mentally retarded and Leprosy Affected persons where 400 patients can be taken.  LIC Clinic - Family Planning Association of India, Gwalior under Gwalior Division The Association provide comprehensive educational, clinical and non clinical services in the area of Reproductive health, family Planning, safe motherhood and child health including infertility management. They treat average number of 200-250 patients per day at subsidized rate and approximately at least 20% free of cost. This hospital provides affordable cost dialysis @ Rs. 450/- per dialysis which is perhaps one of the lowest in the country.  LIC Laboratory - Swami Shri Nirdoshanandji Manav Seva Trust, Timbi under Bhavnagar Division Poor and needy patients in surrounding area of 30 to 40 kms will be benefited by high-tech LIC Laboratory as the hospital will provide medical aid to 200 to 300 patients daily at free / nominal cost  LIC Hostel - Maharogi Sewa Samiti, Warora under Nagpur Division LIC Hostel will provide home for 120 Blind and physically challenged students of IT Training Centre and English Language School at Anandwan, and enable them to become socially independent.  LIC Thalassemia Patients Adoption Programme – Ahmedabad Indian Red Cross Society under Ahmedabad Division Under LIC Thalassemia Patients Adoption Programme, we have adopted 100 children belonging to economically weaker section, for the treatment at the cost of Rs. 6000/- per child for one year.  LIC Mobile Van - Peoples Welfare Society, under Allahabad Division Fully equipped LIC Mobile Van dispensary is donated for providing medical check-up facilities at door steps of needy people in far flung areas of Chaka Block Vikas khand and surrounding block of Allahabad.  LIC Dispensary - Ramkrishan Math, Vyttila, Kochi under Ernakulam Division Foundation has constructed Ground Floor of Dispensary. LIC Block is providing medical services to the poor people from the nearby villages where the patients are treated free of cost and free medicines.  LIC Sponsorship for Cochlear Implant Surgeries - KEM Hospital, Pune Division Cochlear implant is not just about surgery, it is a lifelong commitment on the part of the family and the programme for the habilitation into mainstream of society. Foundation has contributed to some of such families which help them in rehabilitating the children into the mainstream of society. 5 Cochlear implants were sanctioned to children residing at Pune, Nashik, Aurangabad, Srinagar and Baramati, belonging to weaker sections of society.
