To get detailed information about your health care coverage, please see your Evidence of Coverage. This directory lists the providers in your area that are part of the network for your plan. When you go to network providers, you are responsible only for any deductible and copayment or coinsurance amounts required by your plan. The list of providers in this directory was reviewed at the time this list was developed. Participation in the network can change at any time, so you might want to check to ensure that the provider is still part of the network before you receive care. Either call the providers office, or call the Customer Service number on your membership ID card. Primary Care Physician Although it is not required, we recommend that you choose a Primary Care Physician (PCP). A Primary Care Physician will provide most of your care and will help you arrange or coordinate most other care you need. If you currently have a Primary Care Physician who provides most of your care, please check to be sure that the physician is part of the network for your plan. If not, you may want to choose another physician who is in the network. The “network providers” listed in this directory have agreed to provide you with your health care, vision and dental coverage. You may go to any of our network providers listed in this directory. If you have been going to one network provider, you are not required to continue going to that same provider. Your PCP may need to get prior authorization (prior approval) from us before you receive certain care. See your Summary of Benefits or your Evidence of Coverage to find out what services need prior authorization. Out-of-Network Care You may choose to get covered services from non-network providers. You don’t need to get a referral or prior authorization when you get care from the out-of network providers. However, if you use an out-of-network provider – your share of costs for covered service may be higher – except for emergency care, urgently needed care when a network provider is not available and out-of-area dialysis services. Emergency care can always be obtained in or out of the service area from the nearest available provider. You may receive covered emergency medical care whenever you need it, anywhere in the United States. After your emergency is over you are entitled to follow-up care as well as post stabilization care. i If you have an urgent need for care outside your plan’s service area and are still in the United States, but are unable to get care from a network provider – urgently needed care will be available from providers at the lower, in-network cost-sharing amount. If you go to an out-of-network provider, ask the provider to send us the bill. We recommend that you wait to pay the provider until you know what we have paid and the amount you owe. If the provider sends you the bill, please contact Customer Service about sending the bill to us. We will pay the provider your benefits for covered services, unless we know that you paid the bill. Then we will pay your benefits to you. You won’t have to pay an out-of-network provider any more than what the provider would have been paid if you had been covered with the Original Medicare Plan. We cannot pay a provider that has decided not to participate in Medicare. You will be responsible for the full cost of services you receive from a non-Medicare provider. Please confirm that the provider participates in Medicare before you receive care.
