Centennial High School Ski/Snowboard Club 2020-2021 Dear Parents and Students, Centennial High School Ski/Snowboard Club is gearing up for the slopes, and we are excited for another fun, snow-filled winter. Participants and parents please read through the enclosed registration rules and regulations carefully. If you participated in the ski/snowboard club last year, this year will look different because of COVID. First, participants will be responsible for their own transportation to and from the hills. Members must complete the travel waiver to participate. The cost of participating will, therefore, be lowered. Second, we will only be traveling to Wild Mountain and Trollhaugen this year because the hills are nearer and many other hills have created restrictions due to COVID. The following trips have been scheduled for the 2020-2021 season: Saturday, December 19 Wild Mountain Saturday, January 9 Trollhaugen Saturday, January 23 Wild Mountain Saturday, February 6 Trollhaugen Saturday, February 20 Wild Mountain *If a trip is cancelled due to weather, make-up date is March 13. Registration: Registration form and payment are due by December 12, 2020 Register at https://www.isd12.org/schools/high-school/centennial-high-school/athletics-activities-clubs/ac tivities-and-clubs-1 If unable to access the Google form, submit paper forms and checks/cash to Mr. Haag (Room 104). Fees: Registration fee for CHS Ski/Snowboard Club is - $150.00 for 5 trips. Discounts will not be given for possession of a season pass or inability to attend an event. Refunds will not be issued for inability to attend trips. We want as many students as possible to enjoy this fun activity. The fee includes discounted group lift tickets, chaperones and insurance (with RDM Agency). Membership is not transferable. Students needing rental equipment will be responsible for paying for their own rental fees at the ski resort. Group discounted ski and snowboard rental packages are $22 at Wild Mountain and $21 at Trollhaugen. Members may hand chaperones a check or cash for rental equipment. Schedule: Chaperones will be at Wild Mountain and Trollhaugen and ready to hand out lift tickets and rental slips to members at 3:00 PM. At 3:15, the Chaperones will stop handing out tickets and begin to ski themselves. If you arrive after 3:15, please request that Wild Mountain or Trollhaugen make an announcement for a Centennial chaperone to come to the front desk. At 5:00, any unclaimed tickets will be returned to Wild Mountain or Trollhaugen. All members should arrange for a ride to pick them up at 9:00 PM or earlier. Chaperones will also be leaving the ski hill at 9:00 PM and will not be available to assist members after 9:00 PM. Guests: In years past, we accepted guests to join us on our trips to ski hill; however, COVID restrictions will not allow us to accommodate guests this year. Behavior Expectations: Ski/snowboard Club is a school-sponsored activity. Club members are expected to maintain the same conduct code that applies at school while on the slopes. This means that members are to be respectful to supervisors, chaperones, ski patrol, and ski hill employees. Centennial High School Ski/Snowboard Club 2020-2021 Inappropriate conduct (using vulgar language; listening to wireless speakers (bring headphones); viewing inappropriate content on electronic devices; possessing tobacco, vaping products, drugs and/or alcohol; harassing students and/or chaperones; and inappropriately touching students and/or chaperones) will result in loss of a lift ticket for that night or for the entire season. Depending on the severity, students might not receive a warning. Violations will result in the loss of skiing/snowboarding privileges, and no money will be refunded. Masks and Bags: Because of COVID, ski hills are not allowing skiers to leave bags in the chalet. Therefore, you should bring a backpack with you and wear it while skiing/snowboarding. You will also need to wear a mask on while skiing/snowboarding and while in the chalet. You may remove your mask briefly to place food in your mouth or to take a drink. Failure to follow these rules will result in a loss of lift ticket(s). Refunds: The fee is based on a group rate; therefore, there will be no refunds for any missed trips. If there is an injury that would cause a student to miss the remaining trips, a partial refund can be processed with a letter from a physician. Enclosed is my registration for $150.00, payable to CHS. th *Please note that full refunds will not be given after December 12 , 2020. Any student dropped from the program for disciplinary reasons will lose their right to participate and will not receive a refund for trips missed. Partial refunds can be given only in cases of season-ending injury with doctor’s orders and Mr. Haag’s approval. I agree to follow all of these rules, as well as the established code of conduct of Centennial, including those forbidding the possession of any controlled substance. I understand that these rules have been established to ensure a safe, harmonious environment for all of us and that I will lose my privilege to Ski/Snowboard Club if I violate them. Centennial High School Ski/Snowboard Club 2020-2021 Signature of Student Date Permission of Parent/Guardian: I give my child permission to participate in the CHS Ski/Snowboard Club Program. I have discussed the rules with my child and the importance of proper conduct, both on the bus and at the ski resort. I support the chaperones’ authority in facilitation a safe and harmonious environment, and give authorized personnel permission to look in my child’s gear while my child is present. I understand that my child skis at his or her own risk, and that neither the school nor the chaperones are responsible for personal injury or theft of equipment. I will make arrangements to have transportation ready promptly at 10:00 p.m. at CHS. Signature of Parent/Guardian Date Permission Form, Waiver, and Release of Liability for Alternative Student Transportation to and from Off-Campus Activities This form certifies that my child has permission to use alternative transportation to and from the off-campus activity sponsored or organized by Centennial School District (“District”) as listed below, rather than using District-provided transportation. My child has my permission to use alternative transportation to and from the off-campus activity listed above and will not be riding District-provided transportation for this off-campus activity. Such alternative transportation may be provided by my child in her/his own vehicle or by another student or adult who is willing to transport my child. 2. I understand that the District offers transportation to and from off-campus activities and the decision not to utilize District-provided transportation is voluntary. 3. I understand that my child and I are solely responsible for arranging alternative transportation to and from the off-campus activity and that I am responsible for ensuring that my child utilizes such transportation. I understand that I am solely responsible for the supervision of my child’s alternative transportation and the District will not provide supervision for my child while my child is utilizing any transportation that is not provided by the District. 5. I understand that the District does not have any involvement, control, or supervision over transportation provided by anyone who is not an employee or representative of the District, nor does it make any statements or assurances regarding such transportation. 6. I recognize and understand that allowing my child to utilize alternative transportation involves inherent risks, including but not limited to the risk of contracting covid-19 and the risk of physical injury or death, and I voluntarily assume all such risks. 7. I have discussed with my child the dangers associated with distracted driving, including the significant risk of injury or death associated with using portable electronic devices in a motor vehicle, driving while impaired, and driving while being distracted by other passengers. My child and I understand that these dangers exist whether my child is the driver or a passenger. 8. I have discussed with my child the importance of wearing a seat belt, whether my child is the driver or a passenger, and of requiring all passengers in my child’s car to do so. I have discussed with my child the importance of obeying posted speed limits and driving at an appropriate speed based on road conditions. I have discussed with my child the need to only ride with other drivers who operate their vehicles in a safe, lawful, and reasonable manner. 9. I have discussed with my child the requirements of Minnesota’s graduated driver’s license law. I will not allow my child to transport any other individuals if doing so would violate the conditions placed on an instructional permit or provisional driver’s license.
