Sussner, Juhee Kim, and Steven Gortmaker Summary As researchers continue to analyze the role of parenting both in the development of childhood overweight and in obesity prevention, studies of child nutrition and growth are detailing the ways in which parents affect their children’s development of food- and activity-related behav- iors. Ana Lindsay, Katarina Sussner, Juhee Kim, and Steven Gortmaker argue that interven- tions aimed at preventing childhood overweight and obesity should involve parents as impor- tant forces for change in their children’s behaviors. The authors begin by reviewing evidence on how parents can help their children develop and maintain healthful eating and physical activity habits, thereby ultimately helping prevent child- hood overweight and obesity. They show how important it is for parents to understand how their roles in preventing obesity change as their children move through critical developmental peri- ods, from before birth and through adolescence. They point out that researchers, policymakers, and practitioners should also make use of such information to develop more effective interven- tions and educational programs that address childhood obesity right where it starts—at home. The authors review research evaluating school-based obesity-prevention interventions that in- clude components targeted at parents. Although much research has been done on how parents shape their children’s eating and physical activity habits, surprisingly few high-quality data exist on the effectiveness of such programs. The authors call for more programs and cost-effective- ness studies aimed at improving parents’ ability to shape healthful eating and physical activity behaviors in their children.

    Special Theme – Global Public Health and International Law A global response to a global problem: the epidemic of overnutrition Mickey Chopra,1 Sarah Galbraith,2 & Ian Darnton-Hill3 Abstract It is estimated that by 2020 two-thirds of the global burden of disease will be attributable to chronic noncommunicable diseases, most of them strongly associated with diet. The nutrition transition towards refined foods, foods of animal origin, and increased fats plays a major role in the current global epidemics of obesity, diabetes and cardiovascular diseases, among other noncommunicable conditions. Sedentary lifestyles and the use of tobacco are also significant risk factors. The epidemics cannot be ended simply by encouraging people to reduce their risk factors and adopt healthier lifestyles, although such encouragement is undoubtedly beneficial if the targeted people can respond. Unfortunately, increasingly obesogenic environments, reinforced by many of the cultural changes associated with globalization, make even the adoption of healthy lifestyles, especially by children and adolescents, more and more difficult. The present paper examines some possible mechanisms for, and WHO’s role in, the development of a coordinated global strategy on diet, physical activity and health. The situation presents many countries with unmanageable costs. At the same time there are often continuing problems of undernutrition. Undernutrition can lead to health problems across generations, particularly among the most vulnerable populations. Overnutrition leading to overweight and obesity is increasing rapidly among low and middle-income countries, increasing the prevalence of chronic noncommunicable diseases and associated healthcare costs. Rapid progress in improving food ’ S and nutrition security is needed to reach health and development goals beyond 2015. HEALTH Evidence-based, cost-effective, and relatively simple solutions to reduce malnutrition, together with nutrition-sensitive development strategies, need to be scaled up. Overnutrition results in obesity; therefore, obesity is a form of malnutrition. The incidence of obesity is increasing at an alarming rate. Worldwide, an estimated 1.7 billion people are overweight, with an excess of 97 million obese adults in the United States [2,3]. Obesity is also a growing problem in children, and its prevalence parallels that of adult obesity [4,5]. Weight loss in obese individuals followed by long-term maintenance of an ideal weight is difficult. Increased physical activity and caloric reduction with low-calorie or low-carbohydrate diets can reduce weight by 5% to 10% over a 6-month period, but weight regain is common [6–9]. Currently available pharmacologic therapies can be combined with dietary measures for a greater weight loss effect but dose-dependent side effects limit their use, and long-term safety and efficacy beyond 2 years have not been fully evaluated [10–12]. Surgery has been demonstrated to be the most effective treatment for weight loss and improvement in some comorbid conditions in patients with morbid obesity [13,14]. Surgical approaches to induce weight loss include the use of restriction, in which food intake is limited by a small gastric pouch or reservoir, the use of malabsorption, in which the length of intestine available for nutrient absorption is reduced, or by a com- bination of both techniques (Box 1).
