1. INTRODUCTION TO DEPARTMENT/SPECIALITY

The Dudley Group NHS Foundation Trust:

The department is housed in dedicated accommodation at the Corbett Hospital. It consists of 5 consulting rooms, phototherapy suite (PUVA and two narrowband UVB), treatment rooms and biopsy room. Medical, nursing and secretarial offices are on site. A further minor operation room is situated within the day surgery unit at the Corbett Hospital. Currently most outpatient activity is based at the Corbett Hospital with the exception of the Rapid Access skin cancer/parallel clinic with Plastic Surgery and paediatric dermatology which are at Russell’s Hall Hospital and a general dermatology clinic that takes place in outpatient clinics at the Guest Hospital.

Phlebotomy, pharmacy and X-ray services are provided at the Corbett Hospital

A range of nurse led services have been developed. This includes CNS led second line therapy-monitoring clinics, paediatric eczema clinics, PDT, community dermatology clinics, BADBIR research clinics. Other nurse-led services include regular biopsy clinics, as well as the long established phototherapy, dressing/day treatment and patch test clinics. and skin cancer CNS service. The post holder will be expected to contribute to medical supervision of these activities and contribute to the development of new initiatives.

Other Relevant Services

A full range of laboratory and imaging services are based at Russell’s Hall Hospital including theand Guest Hospitals. The histopathology service is provided by Pathology Service Team.

Medical photography is based at Russell’s Hall Hospital with the plans to expand this service to Corbett Hospital and the community.

The skin oncology multidisciplinary team (MDT) meets weekly on Tuesdays at Russell’s Hall Hospital. Core members of the MDT include dermatologists, plastic surgeons, skin oncology Clinical Nurse Specialists, pathologist, clinical and medical oncologists and radiologist. Dudley is one of the two centres, along with Wolverhampton, which forms the Black Country Specialist Skin MDT.

A dermatology audit/journal club, dermatopathology review and difficult cases dermatology MDT clinic happens once per month.

A combined Dermatology / Rheumatology clinic takes place every two months.

The Rheumatology day-case unit provides a service for patients requiring intravenous biological therapy.

Junior Staff

The department has a full-time SHO on a 4-month rotational basis as part of Dudley Group Hospitals CMT programme, a full time clinical/research fellow and a specialist registrar (trainee) as part of the Deanery Dermatology training programme. Trainees rotate every 12 months throughout the West Midlands region.

Medical Students

4th year medical students from the University of Birmingham Medical School are placed with the department rotating on a weekly basis. During their placement they attend consultant, nurse-led, phototherapy and biopsy clinics, in addition to departmental educational meetings.

The post holder will be expected to contribute to clinical teaching of medical students on a pro-rata basis.

Postgraduate teaching

A regular programme of teaching for FY1/2, CMTs, and MRCP candidates is in place and contributed to by consultants within the department. Teaching for primary care practitioners is also undertaken on an intermittent basis. The post holder will be expected to contribute to postgraduate teaching on a pro-rata basis.

Audit

Regular audit is undertaken within the department and as part of the skin oncology MDT. The post holder will be expected to contribute to clinical audit.

Dermatology Community clinics

The department is involved in running dermatology community clinics at 2 different locations in the Borough. The clinics are consultant led and include 2 GP Dermatology SAS, a dermatology CNS and a junior doctor. There is secretarial support.

Dermatology Research clinics

We have a very active dermatology research portfolio. Regular dermatology research clinics take place at Russell’s Hall Hospital by a dedicated dermatology research team. The team recruit patients for academic and industry sponsored trials that are adapted in the NIHR portfolio. The team was awarded “PI of the year” by NIHR/BBC CLRN in 2014.

Other activities

There is regular participation at hospital grand rounds and Journal club. The following are held once per month within the department:

SECRETARIAL AND OTHER SUPPORT / OFFICE ACCOMMODATION

The dermatology secretariat will support this post. Band 3 & 4 secretarial support is available. Arrangements are in place for leave cover for Consultant secretaries. The post will also be supported by the Multidisciplinary Team.

A suitably furnished office will be available for the successful applicant. Access to a personal computer will be provided, and training in IT will be provided through existing Trust-based courses, if required.

2. JOB SUMMARY

This post is in general adult and paediatric dermatology. The post holder will be a core member of the skin oncology MDT. There is a requirement to join the weekly on- call rota for ward referrals and urgent A&E/GP queries, which operates 0900-1700 Monday-Friday. There is no out of hours on-call commitment.

This new post will have secretarial and multidisciplinary team support.

The new post will have a commitment to 4.5 outpatient clinics (including MDT skin oncology meeting), 1 minor operation session, 1 session for ward round / hospital in patient referrals, 1 session for clinical administration and 1 session for development of a special interest clinic/ service lead for particular area of interest, to be decided dependent on the postholder’s interests and experience and to be mutually agreed as part of the departments overall strategy. 1.5 sessions are provided for supporting activities, including continuous professional development, audit, educational activities and research.

The appointee will be expected to participate in all departmental activities, including audit, teaching, sharing administrative responsibilities and contributing to the further development of the department are essential. The unit has undergone a period of service redesign to introduce improved models of care and looks to build upon these initial achievements. The post holder will agree to work within the models of care agreed within the department.

3. KEY RESPONSIBILITIES

3.1 Activities • Provision of a Dermatology Service for outpatients, day-cases, in-patients, ward referrals, and relevant surgical procedures. • Education, training and supervision of medical students, junior medical staff, nurses, paramedics, general practitioners. • Administrative duties to ensure the proper functioning of the department. • Provision of cover for colleagues' periods of leave. • Participation in medical audit, clinical governance, research and continuing professional development relating to dermatology. • Management and supervision of medical, nursing and other professional staff within the Dermatology multidisciplinary care team. • Attendance at Directorate, District and Regional meetings, as required, to represent the interests of the speciality and the department. • Participation in the Dermatology on-call rota and prospective cross cover with colleagues for such. • Contribute to the development of dermatology services, treatment guidelines The Dudley Group NHS Foundation Trust

and the promotion of greater knowledge of the management of dermatological conditions in primary care. • Provide a consultation and advisory service to medical colleagues in other specialties in the Trust and to general practitioners • Develop a special interest to complement those of the existing Consultants in the Dermatology Service accordance with the priorities of the Trust. • Develop appropriate services and techniques required to fulfil clinical needs, within available resources. • Collaborate and work to maintain dermatology services in line with strategic requirements. • Liaise with medical staff in other specialties and participate in clinical meetings and postgraduate activities in the Trust.

3.2 On Call Requirements There is a requirement to join the weekly on-call rota for ward referrals and urgent A&E/GP queries, which operates 0900-1700 Monday-Friday.

3.3 Productivity Clinic and theatre time is allocated in sessions (blocks of time) and consultants are expected to ensure optimum utilisation of the sessions allocated to them, in terms of time and productivity.

4. WORK SCHEDULE

Proposed Work Programme

The proposed programme of work comprises of 10 programmed activities (PAs) each of which has a nominal timetable value of four hours.

Direct clinical care (DCC) (work relating directly to the prevention, diagnosis or treatment of illness) includes emergency work (including whilst on-call), outpatient activities, multi-disciplinary meetings about direct patient care. Activities (PA’s) that take place between 7pm to 7am Monday to Friday, weekends or on public holidays will have a timetable value of three hours.

Supporting professional activities (that underpin direct clinical care), include participation in training, medical education, continuous professional development, formal teaching, audit, clinical management and local clinical governance activities. (For further information see Section 4.2)

4.2 SPA details

Activities contained within Supporting Professional Activities (SPAs) are activities that underpin Direct Clinical care (DCC). This includes participation in training, medical education, continuing professional development, formal teaching, audit, job planning, appraisal, research, clinical management and local clinical governance activities.

Following discussion in the Trust Joint Local Negotiating Committee, the Trust has clarified the total number SPA’s will not exceed 2.5 PAs per week for a Consultant on a full time (10 PA) contract. 1.5 SPA’s as a minimum will be allocated to cover revalidation activities including CPD. Adjusting the balance The Dudley Group NHS Foundation Trust

between your DCC’s and SPA’s will not in itself affect your total PA’s which should be agreed as part of the job planning process.

SPA time for teaching / training, research, audit lead or clinical lead roles etc. should be carried out on Trust or NHS premises. These should be agreed and specified in the job plan with the allocation of time above the 1.5 SPA’s for CPD / revalidation, but up to the maximum of 2.5 SPA’s in total.

A maximum of 1 SPA in relation to CPD / revalidation per week can be worked flexibly offsite (with the approval of the Medical Service Head / Clinical Director), however, the expectation is that this offsite work should not be regularly timetabled. The Dudley Group NHS Foundation Trust

5. GENERAL RESPONSIBILITIES

5.1 Performance Management The Trust operates a structured performance management approach for all newly appointed consultants to ensure that there is an appropriate fit between the individual and the organisation. This includes:

7.1 Dudley Group

The Dudley Group NHS Foundation Trust is the main provider of Hospital services to the populations of Dudley, significant parts of the Sandwell Borough and smaller, but growing, communities in South Staffordshire and Wyre Forest. Currently we serve a population of around 400,000 people from three sites at Russells Hall, Guest Outpatient Centre in Dudley and Corbett Outpatient Centre in Stourbridge, providing the full range of secondary care services and some specialist services for the wider populations of the Black Country and West Midlands regions. The Trust was authorised by Monitor, the independent regulator of NHS foundation trusts, to commence operation as an NHS Foundation Trust from 1st October 2008. The Trust’s hospitals form part of a Private Finance Initiative (PFI) with Summit Healthcare and its service providers: Interserve Facilities Management and Siemens Healthcare. With effect from 1st April 2011 the adult community services section of the Dudley PCT Provider Services function has transferred under the management of the Foundation Trust. With the addition of Adult Community Services the Trust employs over 5,000 members of staff providing over a million patient contacts per year across all Trust sites. The Trust has invested heavily in medical equipment over recent years and during 2010 commissioned a £7m new multi-tiered staff car park at Russells Hall Hospital. Local state schools and universities (Birmingham, Aston and Wolverhampton) are excellent and have high success rates for students continuously.

Teaching Hospital Status

As of 2002 Dudley Group NHS Foundation Trust achieved teaching hospital status as part of the Birmingham Medical School. Third, fourth and fifth year medical students are taught within the Trust. Since then Dudley has begun the introduction of an Undergraduate Teaching Academy status under the leadership of Mr Atiq Rehman (Head of Teaching Academy). A dedicated Undergraduate Educational Block supports learning of undergraduate medical students. Locum Consultant staff, Registrars and other junior medical staff are expected to participate in the teaching of medical students.

Medical Education

A modern and fully equipped Postgraduate Medical Education Centre, is situated at Russells Hall Hospital. There is a modern and fully equipped library, which is linked with the Barnes Library at the Medical School in Birmingham together with the national lending library networks.
