Quality of Inter-Hospital Transportation in 431 Transport Survivor Patients

Graf1, Christian Karagiannidis3, Christian Apfelbacher2, Thomas Bein1 and For the DACAPO Study Group Abstract Background: The acute respiratory distress syndrome (ARDS) is a life-threatening condition. In special situations, these critically ill patients must be transferred to specialized centers for escalating treatment. The aim of this study was to evaluate the quality of inter-hospital transport (IHT) of ARDS patients. Methods: We evaluated medical and organizational aspects of structural and procedural quality relating to IHT of patients with ARDS in a prospective nationwide ARDS study. The qualifcation of emergency staf, the organizational aspects and the occurrence of critical events during transport were analyzed. Results: Out of 1234 ARDS patients, 431 (34.9%) were transported, and 52 of these (12.1%) treated with extracorpor‑ eal membrane oxygenation. 63.1% of transferred patients were male, median age was 54 years, and 26.8% of patients were obese. All patients were mechanically ventilated during IHT. Pressure-controlled ventilation was the preferred mode (92.1%). Median duration to organize the IHT was 165 min. Median distance for IHT was 58 km, and median duration of IHT 60 min. Forty-two patient-related and 8 technology-related critical events (11.6%, 50 of 431 patients) were observed. POLICY POSITION ON TRAFFIC EDUCATION & DRIVER TRAINING: A LIFELONG JOURNEY SAFETY Executive Summary A safe road transport system requires tailored education and training interventions at every step of one’s life. Every traffic participant should be encouraged to undertake a process of continuous personal development. FIA recommends: • The financing and conducting of awareness campaigns for parents on proper use of Child Restraint Systems • That traffic education in schools be made mandatory across the EU 28 • A revision of the driving licence systems to mandate a multi-phase retraining system whereby novice drivers have to undertake further training to validate their licence • A revision of the European Directive 2003/59/EC on periodic training of professional drivers extending the minimum list of topics to improve the effectiveness of the trainings. For instance, training on higher order skills such as self-assessment and the impact of attitudes on road risk should be addressed • A revision of the European Directive 2003/59/EC on periodic training of professional drivers to include practical training (in-vehicle, or at the very least in simulators) as a requirement • The promotion and provision of voluntary refresher and assessment driving courses tailored to senior citizens; and the provision of mobility alternatives and information to enable seniors to remain mobile while guaranteeing their safety The FIA welcomes the European Commission’s plan to continuously develop its strategy for road user education and training, as stated in the interim evaluation of the European road safety policy orientations 2011-20201. -

PARAMEDICS Emily Rowland & Madison Brydges Introduction to the Health Workforce in Canada | Paramedics 1 Paramedics INTRODUCTION The core role of paramedics is to provide emergency- based care and connect individuals to more specialized care. Paramedics are tasked with a significant responsi- bility and a large scope of practice that has steadily increased in recent years: today, the role of paramedics across Canada often includes many other forms of community-based care. There are more than 30,000 licensed paramedics in Canada; however, demand for their services has been increasing, leading to shortages in many regions (Brown, 2018; National Occupational Competency Profile for Paramedics, 2011; Paramedic Association of Canada [PAC], 2015). As paramedicine is an evolving profession, and as advancement has occurred at different stages across Canada, there are several different classifications of paramedics and terms used to refer to them. “Paramedic” is the most common professional title; however, some provinces have other levels of para- medics with different education requirements and scopes of practice called “emergency medical assistants” (EMAs) or “emergency medical techni- cians” (EMTs). In general, the term “paramedic” encapsulates three levels of practice (primary care, and during transfers to receiving medical facilities, advanced care and critical care) that are distinct from such as hospitals (PAC, 2015). Paramedics are trained EMAs, EMTs and “emergency medical responders” clinicians who provide a range of advanced life support (EMRs). 2018 Arkansas Minimum Pre-hospital Clinical Guidelines These guidelines were developed by the Medical Directors Council of the National Association of State EMS Officials (NASEMSO). These guidelines were then reviewed by the Arkansas Core Medical Directors Committee to address Arkansas specific guidelines. These guidelines will be maintained by both NASEMSO and the Arkansas Core Medical Directors to address updates in clinical guidelines, protocols or operating procedures. Arkansas medical directors and encouraged to use the guidelines at their discretion. These guidelines are considered the minimum guidelines that all EMS Agencies should adopt. EMS Agencies clinical guidelines, protocols or operating procedures should at a minimum meet these guidelines. These guidelines are either evidence-based or consensus-based and have been formatted for use by field EMS professionals.
