We are seeking to recruit an enthusiastic and motivated Consultant Psychiatrist in Addictions to fill a post which has arisen following retirement of the existing post holder.

The post is one of four consultant posts within NHS Tayside substance misuse services. This position is based within the Dundee Integrated Substance Misuse Service and you will have responsibility for one of two emerging Dundee locality teams. There will be significant opportunities to lead service development activity both within Dundee and in the wider Tayside area, as well as provide clinical leadership around governance and quality improvement. With strong links to the University of Dundee Medical School and the University`s Division of Neuroscience research facilities, there are opportunities for both under and post graduate teaching as well involvement in addiction related research activity.

You will be joining a well-established clinical service consisting of experienced substance misuse nurses, an expanding Non-Medical Prescribing nurse workforce, dedicated pharmacist input, substance misuse psychology, a Specialty Doctor, and General Practitioners with Special Interests. Higher Specialty Trainees are regularly placed with the service. There is access to a Substance Misuse Inpatient facility for patients requiring Tier 4 interventions. You will be working alongside Social Work colleagues and this partnership working will be enhanced with the further development of integrated teams within the Dundee Health and Social Care Partnership.

Dundee is the fourth largest city in with a population of approx 148,270. Currently undergoing a large scale riverside re-development, this vibrant university city has a well established art scene, boasting the internationally renowned Dundee Contemporary Arts Centre and the recently opened Victoria & Albert Design Museum. There are excellent transport links both north and south, as well as easy access to the rugged north east coastline of Fife and Angus, and the mountains of Highland Scotland.

Applicants must have full GMC registration, a licence to practise and be eligible for inclusion in the GMC Specialist Register. Those trained in the UK should have evidence of higher specialist training leading to a CCT in General Adult Psychiatry or eligibility for specialist registration (CESR) or be within 6 months of confirmed entry from the date of interview.

Consultant Psychiatrist Consultant in General Adult Psychiatry (sub-specialty: substance misuse psychiatry)

Background This post, based within the Dundee Integrated Substance Misuse Service has arisen following the planned retirement of the current post holder. This is an exciting opportunity to help develop and lead one of two emerging locality substance misuse teams within the Dundee area.

Tayside is a mixed urban and rural area with a population of approximately 400,000. It includes the cities of Dundee and Perth, a range of medium and small towns alongside extensive rural areas. The NHS board area includes the three local Health and Social Care Partnerships of Angus, Dundee and Perth & Kinross.

Tayside Substance Misuse Services Dundee Integrated Substance Misuse Service is a substantial and well established joint health and social care service which works with a range of partners to provide a comprehensive range of clinical and social care services to the population of Dundee. It works with, and alongside, two other community substance misuse services in Perth & Kinross and Angus. Each substance misuse service is operationally managed by their respective Health and Social Care Partnership. Medical staff are medically line managed within the Perth & Kinross Health and Social Care Partnership.

There is currently one consultant providing input to Perth & Kinross substance misuse service, one consultant for the Angus substance misuse service, and two consultants allocated to the Dundee substance misuse service. All consultants are expected to take a lead role in setting the direction for future service delivery and to take responsibility for clinical governance within their own locality. Additionally there are Tayside wide clinical, service development and teaching responsibilities that are distributed amongst the consultant body.

Dundee Integrated Substance Misuse Service This integrated health and social care team covers the geographical area coterminous with the local authority boundaries of Dundee City Council. This team is large and is currently based in two team bases – reflecting historic management arrangements, where alcohol and drug services were managed separately. The drug service element provides the core drugs assessment and treatment service for the city, providing a range of prescribing and psychological therapies. The drug service element is delivered from Constitution House – a dedicated base in the City centre.

The alcohol service element is delivered from the Wishart centre – an administration base with most clinical work delivered from GP surgeries or health centres. The drug and alcohol service offers clinics across Dundee and a community detoxification and relapse prevention programme, with inpatient detoxification being delivered by the

specialist NHS Tayside Addictions Unit in Kinclaven Unit based at Murray Royal , Perth.

There is a Dundee Health and Social Care Locality Manager with responsibility for Substance Misuse and who directly manages two Senior Integrated Managers (Grade 8a). There are three Non-Medical Prescribers ,sessional GPwSi’s and a Psychologist in the service, as well as a team of nursing, social work and support staff and dedicated team administration. There are also NHS Tayside specialist substance misuse pharmacists.

In line with services in Angus and Perth, Dundee ISMS intends to deliver the alcohol and drug elements as integrated drug and alcohol services, along with further integration of health and social care provision. This process will be developed in the coming year, alongside the operational delivery of services in Dundee as two localities. Each Consultant Psychiatrist will have responsibility for a single Dundee locality and will be expected to take a lead role in these service developments.

Tayside Substance Misuse In-Patient Services There is an 8 bedded inpatient unit based within Murray Royal Hospital which provides alcohol and opiate detoxification, as well as other complex interventions such as simultaneous alcohol detoxification and stabilisation on to opiate replacement therapy.

Third Sector Organisations The service works closely with both commissioned and non commissioned third sector services across Dundee and Tayside

Other Clinical Staff The number of junior doctors in the service varies according to training needs. At present a key element of service for the in-patient unit is provided by a junior doctor, typically a psychiatric trainee in the second or third year of training. Medical staffing is planned on this post continuing.

ST4/5/6 trainees are commonly placed within the Dundee Substance Misuse Service.

GP with Special Interests provide sessional input into the three community substance misuse service. There are also Non-Medical Prescribers working within all three areas with plans to expand capacity over the next five years.

Within Tayside there is one Lead Consultant Psychologist for addictions as well as Clinical Psychologists working within each of the three localities.

The post The post is for 1 full time Consultant Psychiatrist and the contract will be for 10 programmed activities. The details of duties are negotiable, but the key clinical elements will be to provide the Consultant Psychiatric input to the Dundee Integrated Substance Misuse Service, including clinical service delivery and leadership, staff supervision, and providing clinical leadership for governance and quality improvement. Treatment areas will include both alcohol and drug misuse. In line with proposed service developments, this role will become aligned with one of the two Dundee locality areas. The post holder may take a strategic lead role in a Tayside wide priority area that requires considerable development and leadership – but must be delivered in partnership with other services/organisations. The priority areas are: general hospital liaison work; mental health co-morbidity; criminal justice services. There will also be opportunities to negotiate additional Tayside wide duties relating to quality improvement, teaching, training etc. The posts will be based within the Dundee Integrated Substance Misuse Service and community clinical duties will be within that service. Cross cover arrangements exist between the Consultant Addiction Psychiatrists working in Tayside for the purposes of annual leave and study leave. The applicant is required to participate in this arrangement.

Line management Professional line management for this post is via the Tayside Substance Misuse Services Lead Clinician and the Associate Medical Director for Mental Health. The general management of the post is through the Locality Manager – Dundee Health & Social Care Partnership (Dundee HSCP).

Duties of the Post The duties of the post will include the specialist assessment and treatment of adults with Substance Misuse problems for the agreed elements of service.

The successful applicant will be expected to take on a significant clinical leadership role in the design and provision of services by NHS Tayside and our partner organisations – including having a lead role in linking with the local area Alcohol & Drug partnership (ADP). Contributions to national strategy and operational guidance

for Substance Misuse will be encouraged. In particular, the post holder will be expected to take a strategic lead for TSMS in the design and development of services for TSMS priority areas as listed above.

The successful applicant will be expected to contribute to Training and Staff Development and to be an active part of the cohort of Consultant Psychiatrists in their locality.

On-call responsibilities for Consultant Psychiatrists in Tayside are under review. The current arrangement is a three tier Tayside Wide On Call system, with Junior Doctor, Specialty Trainee and Consultant Tiers. The on-call frequency is 1 in 8. There is an expectation that the successful candidate will participate in the on call rota.

CT1-3s and StR4-6s from the Tayside scheme are placed within Tayside substance misuse services as part of their training and the post holder is expected to take an active role in this. The level and configuration of all medical training grade staff is subject to change.

Provisional job plan The final job plan will be agreed between the post holder and Lead Clinician and signed off by the Associate Medical Director for Mental Health. An indicative timetable is shown below. The DCC/SPA split is 7.5/2.5.

The detailed job plan will reflect the broad range of duties. The job plan will be discussed with the appointee to ensure that the final agreed timetable permits delivery of the key objectives of the post. The job plan will be reviewed on an annual basis in line with NHS Tayside policy and there will also be an interim review 6 months after commencement of post.

Continuing education and teaching NHS Tayside Mental Health Services are approved for Core Trainees in Psychiatry and General Practice. There are approved Specialty Registrar placements in Substance Misuse and the full range of other Psychiatric specialties. The successful candidate would be encouraged to apply for approval for Higher Training. Students from the University of Dundee enjoy regular placements with Tayside Substance Misuse Services for a range of teaching and project activities and consultants are expected to take educational responsibility for such students. Successful applicants will be offered the opportunity to participate in CPD activities throughout Tayside – including an addictions consultant CPD group.

Academic activities are conducted in collaboration with, and supported by, The University of Dundee Division of Neuroscience (DoN), within the School of Medicine at and Medical School. Addiction and pain represent two of the priority themes for activities within the Division.

The DoN has several molecular/cellular neuroscience labs undertaking projects related or relevant to substance dependence. At the clinical end of the research spectrum, several recent projects have studied opioid dependence in humans, including the characterisation of large cohorts of methadone-dependent patients within Tayside and Fife using neuropsychological testing and behavioural imaging. There is also a portfolio of health informatics based research on substance misuse (opioid dependence and alcohol misuse). There is marked co-morbidity of substance dependence with mood disorders (e.g. depression and anxiety) and with pain. There is project level support both at the level of molecular/cellular and clinical neuroscience for research in each of of these areas. The DoN and the University of Dundee hosted the International Society for Addiction Medicine (ISAM) 2015 Conference in Dundee. With drug dependence and addiction as a thematic research priority for the School, the DoN are keen to provide support for NHS staff to develop their academic interests and activities, including training in teaching and research skills and to pursue quality improvement initiatives. The School of Medicine has supported job planned Academic Programmed Activities for some NHS Consultant staff where appropriate. Honorary status as a Clinical Senior Lecturer may be conferred at the discretion of the School of Medicine.
